교통사고상해증후군 한의표준임상진료지침 개발을 위한 환자의 의료이용 경험과 의료체계 인식도 설문조사 연구 (A Survey on Patient's Experience on Health Care Utilization and Perception of Health Care System for Developing of a Korean Medicine Clinical Practice Guideline for Traffic Injuries)

  • Objectives To explore the traffic injury patients' experience on health care utilization of Korean Medicine (KM) practice and perception of health care system for developing a Korean medicine clinical practice guideline (CPG) of traffic injuries by survey method. Methods Two hundred ten patients in suffering from traffic injuries were surveyed from September 1st, 2019 to January 31th, 2020 at 2 University Hospitals (Pusan National Korean Medicine Hospital and Kyung Hee Korean Medicine Hospital at Gangdong) and 1 Spine Specialty Hospital (Jaseng Hospital of Korean Medicine). A structured questionnaire of experience on health care utilization of KM practice and perception of health care system of was distributed to responders by visits and all data were statistically analysed. Results Survey results showed high satisfaction of patients with the experience of KM treatments in order of daoyin exercise (7.8±2.3), chuna manual therapy (7.7±4.0), pharmacoacupuncture (7.4±3.0) etc. Safety concerns were reported in 9.1% subjects and 205 (97.6%) patients answered that collaboration with KM and western medicine is necessary for patients with traffic injuries. For the patients' requirement for extending insurance coverage, the most required therapy was chuna manual therapy (57.5%) and pharmacoacupuncture (42.0%). Conclusions This study presented the realistic patient-centered perception of KM practice and health care system in Korea. These results will provide basic data to be reflected in the process of adaptation for the revision of Korean Medicine CPG for traffic injuries.

동결견 환자에 대한 한의복합치료 효과: 후향적 차트 리뷰 (Effect of Complex Korean Medical Treatment in Patients with Frozen Shoulder: A Retrospective Chart Review)

  • Objectives Through this study, we investigated the effects of Korean medicine treatments on range of motion recovery and pain reduction in patients with Frozen shoulder. Methods In this study, the medical records of 26 patients diagnosed with frozen shoulder were retrospectively observed and analyzed through the IBM SPSS Statistics 25 program. We used range of motion to observe the patient's motion recovery and numeric rating scale to evaluate pain reduction. Results The shoulder range of motion increased statistically significantly from 160.0 (123.5~170.0) to 170.0 (160.0~180.0) (flexion), 115.0 (90.0~135.0) to 167.5 (130.0~178.8) (abduction), 40.8±22.1 to 58.3±16.0 (external rotation) (p<0.001). The average of numeric rating scale reduced statistically significantly from 6.7±2.0 to 3.2±1.4 (p<0.001). Depending on the period, the range of motion increased statistically significantly when visited within 3 months, also the pain score decreased 7.0 (5.4~8.0) to 3.0 (1.8~3.6) statistically significantly when visited within 3 months. In the treatment intervention, the increase in the range of motion and the decrease in pain were statistically significant, especially when the bee venom acupuncture and chuna manaul therapy were used together. Conclusions Korean medicine treatments was effective in improving the range of motion and reducing pain in frozen shoulders. In particular, when treated within 3 months of onset and when bee venom acupuncture and chuna manual therapy were used together, it was effective in recovering the range of motion and reducing pain.

상지의 단일신경병증에 대한 수기치료의 국내외 동향 (Literature Review of Manual Therapy for Mononeuropathies of Upper Limb)

  • Objectives This study aimed to review manual therapies for mononeuropathies of upper limb through domestic and foreign studies designed for human body. Methods We searched databases (KMbase, OASIS, RISS, NDSL, KISS, KoreaMed, MEDLINE/Pubmed, CENTRAL, EMBASE) on the 1st to 31th of July 2017 to find related literatures that published after 2000. Results Twenty-eight studies were finally included. Of these, 13 articles were published after 2010. Twenty-two studies were clinical trials and 6 were observational studies. Carpal tunnel syndrome were the most researched type of diseases (85.7%). Most frequently used method of manual therapies was neurodynamic mobilization (35.7%). Pain scales and questionnaires were generally employed for evaluation. Significantly effective studies were 72.2% in controlled trials and 90% in the studies without control group. Conclusions In this study, we reviewed literatures concerning manual therapies on mononeuropathies of upper limb. Further studies are needed on the various diseases of mononeuropathies of upper limb to retain the evidence for the effectiveness of manual therapies.

The Research Trends of Pharmacopuncture Therapy on Cervical Pain Caused by Traffic Accidents

  • Objectives: The purpose of this study is to comprehensively review Korean domestic studies and investigate the research trends of pharmacopuncture therapy on cervical pain caused by traffic accidents. Methods: Domestic studies between February 1999 and May 2020 from four Korean databases were searched with combinations of keywords 'cervical pain', 'traffic accident', 'whiplash injury', and 'pharmacopuncture'. Results: 17 studies were selected for review, including 7 randomized controlled trials, 5 retrospective observational studies, 3 case reports, and 2 non-randomized controlled trials. Each study was reviewed by published year, study type, types of pharmacopuncture solutions, selected acupuncture points, dosage of pharmacopuncture solutions, frequency of treatment, concurrent treatments, outcome measurements, and the effectiveness of pharmacopuncture therapy. The results are as follows: (1) It showed that the total number of published studies had increased slightly in the last 10 years compared to the previous decade. (2) The pharmacopuncture solutions used in the studies were in the following order: Bee-Venom (蜂毒), Jungsongouhyul (中性瘀血), Hwangryunhaedoktang (黃連解毒湯) and Soyeom (消炎). Frequently used acupuncture points were GB20, GV21, A-shi point, GV16, EX-B2, and SI15 in order. (3) The most commonly used total injection dosage was 1.0 cc at a time, and the frequency of treatment was twice a week. (4) Concurrent treatments such as acupuncture, herbal medication, physical therapy and Chuna manual therapy were performed in all 17 studies. (5) Pharmacopuncture therapy showed positive effects on cervical pain caused by traffic accidents in all 17 studies reviewed. Conclusion: Pharmacopuncture therapy was effective in cervical pain caused by traffic accidents in all 17 studies selected. Further studies will be needed using more larger scales and more objective data to confirm the effectiveness of pharmacopuncture therapy and to generalize its application.

성인의 발달성 고관절 이형성증에 대한 한방치료: 증례보고 (Korean Medicine Treatment for Developmental Dysplasia of the Hip in Adults: A Case Report)

  • The purpose of this study is to report the effect of korean medicine treatment for developmental dysplasia of the hip (DDH) in adults. A patient diagnosed with DDH had been treated with acupuncture, electroacupuncture, cupping therapy and chuna manual therapy for 8 weeks. The patient was evaluated by using range of motion (ROM) of hip joint, muscle strength of lower extremity, leg length, numeric rating scale (NRS) and Korean version of hip disability and osteoarthritis outcome score (K-HOOS). After the treatment, the patient had an improvement in the symptoms, pain, and activities of daily living of K-HOOS, especially the quality of life. In addition, NRS decreased from 7 to 4 points, and ROM and muscle strength also improved. The results of this study show that korean medicine treatment is effective and meaningful as one of the conservative treatment for DDH in adults.

Bankart 병변의 관절경적 복원술 후 한방 재활치료 프로토콜을 적용하여 호전된 증례 보고 (A Rehabilitation protocol for Arthroscopic Bankart repair in Korean Medicine : A Case Report)

  • Objectives: The aim of this study was to report the clinical improvement of a patient who underwent arthroscopic Bankart repair using a rehabilitation protocol involving Korean medicine. Methods: The patient was treated with acupuncture, cupping, Korean Medicine, Chuna therapy, and Doin exercise therapy during the admission period. To estimate the patient's status, we used the Numeric Rating Scale (NRS), Pain Disability Index (PDI), assessments of the range of motion (ROM) and Muscle Manual Test (MMT) for the shoulder joint. Results: After administering the abovementioned treatments, we found that the patient's NRS, PDI, and ROM and MMT for the shoulder joint were improved. Conclusions: The rehabilitation protocol involving Korean medicine can be applied to and produce good results for patients who undergo arthroscopic repair for Bankart lesions. The limitation of the study was that the number of cases we studied was insufficient to prove the effectiveness. Hence, further studies are needed for designing the rehabilitation protocol involving Korean medicine.

양방치료를 받고 있는 신경계질환아동에서 한방치료 및 기타 병행치료 이용실태에 관한 조사 (A Study on the Utilization of Korean Medicine & Other Parallel Treatments for Neurological Disease Children & Adolescents Treated with Western Medicine)

  • Objectives The purpose of this study is to investigate prevalence and the frequency of using Korean medicine, Complementary Alternative Medicine (CAM), and other parallel treatments in children and young adolescence with neurological diseases. Methods From April to July 2011, parents of the children and adolescents patients suffering from neurological disease completed questionnaires. Results 578 parents answered the questionnaires. 310 cases (53.5%) were using Parallel Treatments. Types of the Parallel Treatments being used are as follows. Rehabilitation 166 cases (27.4%), Speech Therapy 169 cases (27.9%), Education (Art, Music, Play) 109 cases (18.0%), Health supplement 72 cases (11.9%), Counselling, Cognitive & Behavioral Therapy 45 cases (7.4%), Herbal Medicine 24 cases (4.0%), Acupuncture, Moxibustion 13 cases (2.1%), Neurofeedback Therapy 1 case (0.2%), Etc 7 cases (1.2%). 257 cases (44.5%) have used Parallel Treatment before. Parallel Treatments being used in the past are as follows. Herbal Medicine 146 cases (35.4%), Acupuncture, Moxibustion 64 cases (15.5%), Education (Art, Music, Play) 54 cases (13.1%), Rehabilitation 47 cases (11.4%), Speech Therapy 46 cases (11.1%), Health supplement 30 cases (7.3%), Counselling? Cognitive & Behavioral Therapy 15 cases (3.6%), Neurofeedback Therapy 4 case (1.0%), Chuna, Manual Massage 2 case (0.5%), Etc 5 cases (1.2%). Conclusions Although patients were on both western medicine and Korean medicine, CAM, and other parallel treatment, the ratio of Korean medicine treatment was not as high as expected. Further studies are required to develop the model of integrative medicine.

한방부인과질환에 사용되는 치료방법에 대한 조사연구 (A Survey on the Treatments used in Oriental Obsterics & Gynecology)

  • Purpose: In order to know therapies used in the field of Obsterics & Gynecology of Oriental medicine. Methods: This survey investigated papers about clinical study, literature investigation for therapeutic methods and treatment reports published from 1999. 2${\sim}$2008. 11. Results: In the clinical studies, acupuncture is more frequently used than herbal medicine. But in the treatment reports, herbal medicine is most frequently used except some cases. Besides acupuncture and moxibustion was used frequently. In the treatment reports various acupuncture methods were used except vaginal bleeding or abdominal pain cases. Whole body acupuncture was used mainly. Pharmacopuncture, Sa-am Acupuncture, auricular acupuncture was also used frequently. And Dong's acupuncture, Hwa acupuncture was used sometimes. Moxibustion was tend to be used for dysfunctional uterine bleeding, menopausal disorder, urinary incontinence, vaginal bleeding or abdominal pain during pregnancy period, hyperemesis, recurrent miscarriage, postpartum disease, lochiorrhea. Also other treatment methods was used such as external therapy, aromatherapy, herbal retention enema, fumigation, electric lipolysis acupuncture, Chuna manual medicine, obesity management, acupuncture at uterus cervix, magnetic innervation therapy, exercise, cupping and physical therapy. Conclusion: These results suggests that acupuncture, moxibustion and herbal medicine are most frequent therapy in the field of Obsterics & Gynecology of Oriental medicine. And other therapy are used such as external therapy, aromatherapy, herbal retention enema, fumigation and so on.

회전근개 석회화 건염의 한의 치료에 대한 국내외 임상 동향 분석 (Review of Studies on Domestic and International Clinical Trends for Korean Medicine Treatment of Rotator Cuff Calcific Tendinitis)

  • Objectives This study aimed to investigate trends in domestic and international clinical approaches to the Korean medicine treatment of rotator cuff calcific tendinitis and to suggest future research directions. Methods We utilized nine electronic databases to identify domestic and international studies on Korean medicine treatment for rotator cuff calcific tendinitis. By analyzing the title, abstract, and main text, studies concerning Korean medicine treatment of rotator cuff tendinitis were included. Results Among the searched for studies, fourteen studies met our criteria. Eight of these were case reports and six were randomized controlled trials (RCTs). Across the studies, five treatment methods were employed, including acupuncture, herbal medicine, physical therapy, cupping therapy, and moxibustion. Acupuncture, used in all fourteen studies, was the most frequently utilized intervention. Thirteen evaluation tools were used to measure the effectiveness of Korean medicine treatment, and all studies reported positive outcomes. Conclusions We identified current trends in clinical approaches of Korean medicine treatment for rotator cuff calcific tendinitis and assessed the effectiveness of such treatment. However, further studies are warranted to establish a basis for Korean medicine treatment for rotator cuff calcific tendinitis.

한방 복합치료로 호전된 경추성 현훈 환자의 증례보고 1례 (A Case Report on a Patient with Cervical Vertigo Who Improved with a Combination of Korean Medicine Treatments)

  • Objective: To report the case of a patient suffering from cervical vertigo who was treated with a combination of Korean medicine treatments (including acupuncture, pharmaco-acupuncture, and Chuna manual therapy). Method: An outpatient with cervical vertigo was given a combination of Korean medicine treatments five times, from December 5 to 16, 2016. All of the treatments, acupuncture with electric stimulation and pharmaco-acupuncture, were applied on the trigger points (TP) of the cervical muscles and the upper trapezoid muscles. Chuna manual therapy was then applied on the cervical area. The Numeric Rating Scale (NRS) - according to outpatient's statement on vertigo, neck pain and headache - and the Korean Dizziness Handicap Inventory (DHI), which is a self-evaluating scale of dizziness in daily life, were used to evaluate the effects of this treatment. Results: At the end of the treatment, the vertigo and the headache disappeared. The NRS decreased from 9 to 0 for vertigo and from 5 to 0 for headache. Although the neck pain also decreased, it did not disappear; this NRS decreased from 7 to 1. The DHI score also decreased significantly, from 64 to 4. Conclusion: Since the symptoms of the patient with cervical vertigo improved after the very first treatment and the improvement of the symptoms was observed as the treatment progressed, this case report is worth supporting the efficacy of combined Korean medical treatments. However, because there was only one subject, it is hard to say that this efficacy can be generalized. Moreover, this report cannot prove the individual efficacy since various treatments were combined. While there are many researches related to cervical vertigo in Western medicine, the studies in Korean medicine are comparably insufficient; therefore, follow-up research on more subjects is needed.
