Biomarkers for the lung cancer diagnosis and their advances in proteomics

  • Over a last decade, intense interest has been focused on biomarker discovery and their clinical uses. This interest is accelerated by the completion of human genome project and the progress of techniques in proteomics. Especially, cancer biomarker discovery is eminent in this field due to its anticipated critical role in early diagnosis, therapy guidance, and prognosis monitoring of cancers. Among cancers, lung cancer, one of the top three major cancers, is the one showing the highest mortality because of failure in early diagnosis. Numerous potential DNA biomarkers such as hypermethylations of the promoters and mutations in K-ras, p53, and protein biomarkers; carcinoembryonic antigen (CEA), CYFRA21-1, plasma kallikrein B1 (KLKB1), Neuron-specific enolase, etc. have been discovered as lung cancer biomarkers. Despite extensive studies thus far, few are turned out to be useful in clinic. Even those used in clinic do not show enough sensitivity, specificity and reproducibility for general use. This review describes what the cancer biomarkers are for, various types of lung cancer biomarkers discovered at present and predicted future advance in lung cancer biomarker discovery with proteomics technology.

Songwriting as Therapy with Pediatric Patients Undergoing Cancer Treatment (소아암환자들을 위한 노래 만들기(Songwriting)의 음악치료적 적용)

  • The purpose of this study is to give an overview of the clinical process of songwriting with pediatric patients undergoing cancer treatment. Many literatures have proved that music therapy aids patients during their cancer treatment to relax, reduce anxiety and feelings of isolation, and promote self-esteem and quality of life and so on. However, among music therapy methods, I found songwriting proved to be very effective method. By using songwriting as therapy with pediatric patients undergoing cancer treatment, songwriting helped patients express difficulties of dealing emotionally with their illness and ongoing aggressive and invasive treatments. Also the process of songwriting established trust and relationship between therapist and patient which promoted normalization of hospital environment. In my case study, patient N has expressed her feelings and thoughts through writing songs about her current life which revolved around treatment of her brain tumor. Moreover, patient N found her creativity and developed ways of getting touch with her inner strength.

  • Objectives: This study was designed to evaluate the effects of symptom differentiated treatment on cancer patient. Methods: We retrospectively analyzed the medical record of a case of hepatoma patient with lungs and brain metastasis who had been treated with oriental medicines from 16 august 2001 through 5 september 2001. Results: For the 21 hospital days, he was treated with oriental medicines. Not only all most symptoms were disappeared but also hematological and radiological examinations were improved. According to the results, it could be suggested that symptom differentiated treatment has significant effects on improving symptoms and quality of life as a supportive or palliative therapy for cancer patients.

Development of Glioblastoma In Vivo Model for the Research of Brain Cancer Diagnosis and Therapy (뇌암 진단 및 치료 연구를 위한 교모세포종 동물모델 개발)

  • The research was carried out to develop a animal model of malignant brain tumor for the researches in glioblastoma multiform (GBM) diagnosis and therapy. C6 cells were transplanted into the right striatum of SD rat using stereotactic instrument for the development. The developed animal model was verified by MRI and H&E stain assay of anatomicohistological examination. The MRI observations showed that the tumor developed at the injection site at the 7 days after glioblastoma inoculation. At 14 days post inoculation, the tumor grew to a large volume occupying almost a half of the right cerebral hemisphere. It was confirmed that the expression of excessive mitosis and pleomorphism in anatomicohistological examination. The developed animal model must be necessary and useful tool for the in vivo level research in the development of the new modality for the diagnosis and therapy of brain cancer.

Exposure Dose of DIPS in Proton Therapy for Pediatric Cancer Patients (소아암 환자의 양성자치료 시 DIPS 촬영에 따른 피폭선량)

  • We investigated the radiation exposure caused by DIPS, which is used to identify accurate repositioning and tumor location in pediatric cancer patients proton therapy. To compare and analyze DIPS condition, 50 pediatric cancer patients who underwent proton therapy were selected in Ilsan K cancer-specialized hospital from March 2007 to October 2009. For DIP exposure, 0.09~1.57 mGy is measured in AP and lateral directions and 23.55 mGy is measured in CSI patients. In whole brain patient, the amount of a day DIP exposure dose was 1.13 mGy. During treatment period, who exposed the biggest DIP dose are whole brain patients, 632.71 mGy is exposed. It is 1.13% of prescribed dose, represented dose is adequate because it is not exceeded 2% of recommended dose. Even though the exposed dose is not exceeded more than 2% of prescribed in DIP exposure, we should recognize the radiation damage and genetic influences to pediatric cancer patients, who is much sensitive to radiation and has longer mean residual life time. Therefore, DIPS guideline for pediatric cancer patients should be indicated to minimize the radiation exposure.

False-positive I-131 Uptake in Meningioma (갑상선암 환자에서 관찰된 뇌수막종의 위양성 옥소 섭취)

  • We experienced a case with meningioma showing false positive I-131 uptake. A 55-years old female patient underwent high dose (150 mCi) radioactive iodine therapy to ablate remnant tissue after total thyroidectomy for papillary thyroid cancer. in addition to intense tracer uptake in thyroid bed, there was mild but focal abnormal uptake in left frontal lobe of the brain on post-therapy I-131 whole body scan. Subsequent brain MR imaging showed single mass lesion in left frontal lobe and the mass was resected under the impression of brain metastasis of thyroid carcinoma. Pathologic report confirmed meningioma from the surgical specimen.

  • The paper is based on the record of researching the patients with cancer in the Chun-nam National University Hospital from September 1985 to December 1988. The results are the as follows ; 1. Among the total O.P.D. 921, 028, the patients of Therapeutic Radiology (Opening the Therapeutic Radiology in September) are classified into 27,159(2.95%), (186 in 1985, 2,388 in 1986, 10,511 in 1987, and 14,074 in 1988) 2. Among the 4,925 cancer patients, cervix and uterus cancer patients are 1,138(23.10%), stomach cancer patients are 592(12.02%), brain and thyroid cancer patients are 565(11.47%), liver cancer patients are 400 (8.12%), lung cancer patients are 355 (7.20%) and sexual ratio appeared 1 : 1.13. Therefore, female patients are a slightly more than the male patients. 3. The age distribution of cancer was that of $45{\sim}54$ ages are 1,244(25.26%), $55{\sim}64$ ages are 1,119(22.72%) and $35{\sim}44$ ages are 773(15.70%) and the half of all the cancer patients are $45{\sim}64$ ages. 4. Among the 2,519 cancer patients, 742(29.46%) are in the uterus system, 620(24.62%) are in the brain and thyroid part, 402 (15.96%) are in the lungs. Therefore, these three kinds of cancer consist of 70%. 5. The occupational distribution of 3,067 cancer patients ($87{\sim}88\;year$) house wives are 636 (20.73%), orderly farmers are 622 (20.28%) public service personnells are 193 (6.29%), salarymen are 162(5.28%) and businessmen are 159(5.18%).

Radiation-Induced Glioblastoma Multiforme in a Remitted Acute Lymphocytic Leukemia Patient

  • Radiation therapy has been widely applied for cancer treatment. Childhood acute lymphocytic leukemia (ALL), characterized by frequent central nervous system involvement, is a well documented disease for the effect of prophylactic cranio-spinal irradiation. Irradiation, however, acts as an oncogenic factor as a delayed effect and it is rare that glioblastoma multiforme develops during the remission period of ALL. We experienced a pediatric radiation-induced GBM patient which developed during the remission period of ALL, who were primarily treated with chemotherapeutic agents and brain radiation therapy for the prevention of central nervous system (CNS) relapse. Additionally, we reviewed the related literature regarding on the effects of brain irradiation in childhood and on the prognosis of radiation induced GBM.

In vivo Trafficking of Liposomes Containing Boron Compounds for Boron Neutron Capture Therapy (BNCT)

  • For over 50 years, boron neutron capture therapy (BNCT) has been steadily developed for treating various cancers. This is a non-invasive, selective, and targeted radiotherapy wherein boron-rich molecules accumulate at the tumor site. Liposomal vesicles have become a popular and effective drug delivery system for BNCT, with strategies including surface decoration, bilayer integration, and hydrophilic core encapsulation. This review highlights the state-of-the-art uses of liposomes in BNCT and elucidates a new perspective where BNCT can be used with radiotracer guidance in all-in-one delivery systems.

Role of Radiation Therapy in Oncologic Emergencies (종양응급 상황에서의 방사선치료의 역할)

  • Purpose. Radiation therapy for oncologic emergencies is an important modality in the management of cancer patients. The aim of the present study was to review the roles of RT in oncologic emergencies based on literature review. Methods. The oncologic emergencies requiring rapid treatment include superior vena cava syndrome, spinal cord compression, brain metastasis, and bone metastasis. We reviewed the literatures on the pathophysiology, diagnosis, and treatment of oncologic emergencies. Results. In this literature review, radiation therapy among treatment modalities for oncologic emergencies has been shown to be fast and very effective treatment modality for oncologic emergencies. Conclusions. Based on this review, we conclude that the literature provides support for the role of radiation therapy in the situation of oncologic emergencies. As the number of cancer patients increase, the prevalence of oncologic emergencies will increase. In the future, the development of RT techniques will provide the improvement of not only patient's quality of life but also the survival.
