중년여성의 요통에 따른 골밀도 측정 (Measurement of Bone mineral density According to Middle aged Women with Low Back Pain)

  • 본 연구는 2000년 7월 20일부터 2001년 1월 12일까지 대구광역시에 소재한 가톨릭병원 건강검진센타에서 골다공증 검사를 시행한 여성 50명을 대상으로 실시하였다. 연령은 50~59세가 66.0%로 가장 많았고, 평균연령은 56세이고, 49세 이하는 정상군이 18.7%, 50~59세는 골감소군 83.4%, 60세 이상은 골다공증군이 36.4%로 각각 가장 많았으며, 요추부의 평균골밀도는 연령의 증가에 따라 감소되었지만 유의하지 않았다. 요통유무는 요통이 있다가 76.0%로 많았고, 요통이 있다는 골다공증군 90.9%, 요통이 없다는 정상군이 43.7%로 각각 유의하게 많았으며 (p<0.05), 골밀도는 요통이 있다가 1.00g/$cm^2$로 유의하게 낮았다(p<0.05). 좋아하는 식품은 채소류가 62.0%로 많았고, 생선류는 요통군과 비요통군에서 정상군이 각각 33.3%, 57.1%, 채소류는 요통군과 비요통군에서 골다공증군이 각각 85.0%, 100.0%로 각각 가장 많았고, 골밀도는 요통군에서 채소류가 0.71g/$cm^2$로 각각 유의하게 낮았으며 (p<0.05), 비요통군에서도 채소류가 0.82g/$cm^2$로 낮았지만 유의하지 않았다. 혈액형은 B형은 요통군과 비요통군에서 골다공증군이 각각 50.0%, O형은 요통군과 비요통군에서 정상군이 각각 44.4%, 42.9%로 요통군이 많았고, 골밀도는 요통군과 비요통군에서 O형이 각각 가장 유의하게 높았다(p<0.05). 골밀도(BMD)는 평균골밀도(BMD)는 0.79g/$cm^2$, 요통군에서 0.90~0.99g/$cm^2$는 정상군이 77.8%, 0.60~0.69g/$cm^2$는 골다공증군이 50.0%로 각각 가장 많았고, 골밀도는 요통군과 비요통군에서 골밀도 수치가 증가할수록 유의하게 높았다(p<0.05) 다중회귀분석을 이용한 요추골밀도의 관련성 있는 요인은 연령, 결혼상태, 운동시간, 좋아하는 식품, 칼슘(Calcium), 골밀도의 표준편차인 T값이다(P<0.05). 요통과 골밀도의 관련변수별 상관관계에서 연령과 요통유무, 알카리포스파타제(Alkaline phosphatase), 몸무게와 체질량지수는 유의한 상관관계를 나타냈고 (p<0.01), 몸무게와 신장, 알카리포스파타제(Alkaline phosphatase)와 칼슘(Calcium), 골밀도와 폐경유무는 유의한 상관관계를 나타냈으며(p<0.05), 연령과 폐경유무, 골밀도, 골밀도의 표준편차인 T값, 골밀도와 좋아하는 식품, 알카리포스파타제(Alkaline phosphatase)는 유의한 역상관관계를 나타냈다(p<0.01). 이상의 결과를 요약하면, 요추골밀도의 관련성있는 요인은 연령, 운동시간, 좋아하는 식품, 칼슘(Calcium), 골밀도의 표준편차인 T값이고, 본 연구의 연구대상자의 50대가 66.0%로 가장 많아 폐경유무에서 폐경이다는 골감소군이 83.3%로 가장 많았으므로 폐경 후 기간이 길수록 요추골밀도가 낮아지는 현상을 본 연구에서는 의미 있는 설명 변량이 되지 못했다. 요통과 골밀도의 관련변수별 상관관계에서 연령, 요통유무, 폐경유무, 칼슘(Calcium), 인(Phosphorus), 알카리포스파타제(Alkaline phosphatase)가 중요한 요인으로 생각된다.

식이단백질량이 성장기 흰쥐가 골밀도에 대한 칼슘효율에 미치는 영향 (Effect of Dietary Protein Level on Ca Efficiency in Bone Mineral Density in Growing Rats)

  • This study was performed to evaluate the effect of dietary protein level on Ca efficiency in bone mineral density in growing male rats. Twenty male rate were divided into two groups. The rats in one group were fed on casein 20% diet as control group and the others were fed on casein 40% diet as protein group. All rats were fed on experimental diet and deionized water ad libitum for 9 weeks. The total body, spine and femur bone mineral density and bone mineral content were measured using dual energy-x ray absorptiometry. Urinary calcium, phosphate, pyridinoline and creatinine, serum calcium, phosphate, total protein, albumin, alkaline phosphatase(ALP) and osteocalcin were measured. Urinary Ca excretion, pyridinoline and crosslinks value and serum ALP content seem to be increased in high protein group. It appears that the growing rats in high protein group had a higher bone resprption and bone formation than those in control group. Animal fad a high protein diet had a siginficantly higher Ca efficiency in BMD, BMC of total body, spine and femur. The results of this show that increasing of dietary protein level (40%) is beneficial of improvement of Ca efficiency during growing period.

고유수용성신경근촉진법 운동이 골다공증 환자의 골밀도, 균형 및 하지 근력에 미치는 효과 (The Effects of Proprioceptive Neuromuscular Facilitation Exercise on the BMD, Balance and Lower Muscular Strength in Patients with Osteoporosis)

  • Purpose: This study examined the effects of an exercise program involving the Proprioceptive Neuromuscular Facilitation (PNF) lower-extremity patterns on the Bone Mineral Density (BMD), balance and lower muscular strength in patients with osteoporosis. Methods: The participants were allocated randomly to 2 groups: experimental group and control group. To evaluate the effects of exercise, the subjects were evaluated using the BMD and Time Up & Go Test and One Leg Stance Test for balance, and the Sit To Stand for lower muscular strength. Results: After the 8 week study, the experimental group showed a significant difference in all items compared to the control groups. Conclusion: These results suggest that a PNF exercise program is effective in patients with osteoporosis. Furthermore, an exercise program involving these patterns can be used in a follow-up program for patients with osteoporosis. Overall, more study will be needed to develop and apply more efficient exercise programs for such patients.

폐경 전 성인여성에서 영양섭취 상태와 운동이 골밀도 및 골무기질 함량에 미치는 영향 (Effects of Nutrient Intake and Exercise on Bone Mineral Density and Bone Mineral Density in Premenopausal Women)

  • 본 연구에서는 골다공증의 예방 차원에서 골밀도와 영양소 섭취 상태 및 운동과의 상관성을 조사하기 위하여 대구 지역 성인 여성 74명을 대상으로 간이식이 조사법과 면접 및 설문지를 이용하여 영양소 섭취 량과 운동량 등을 조사하였으며 이중에너지 방사선 골밀도 측정기 (DEXA)를 이용하여 척추 골밀도를 측정한 연구 결과를 요약하면 아래와 같다. 1) 운동군과 비운동군의 각각 에너지 섭취량은 1,921 kcal와 1,733 kcal였고, 평균 일일 칼슘 섭취 량은 각각 652 mg과 635 mg으로 두 군간에 유의적인 차이가 없었다. 2) 운동군과 비운동군은 각각 골밀도가 1.16 g $\pm$0.11/$\textrm{cm}^2$, 1.14 g = 0.14$\textrm{cm}^2$이었고, 골무기질 함량 은 각각 49.6 g $\pm$ 7.8, 47.4 g $\pm$ 9.3으로 두 군간에 유의적 인 차이가 없었다. 3) Alkaline phosphatese (ALP)는 운동군이, 비운동군 보다 유의적으로 낮았다 (p < .05) 비운동군에서는 연령과 ALP가 양의 상관관계가 보였으나 운동군에서는 상관성이 없었다. 4) 칼슘 섭취수준에 따라 낮은 25%군과 높은 25%군을 비교하였을 때 높은 25%군의 골밀도는 낮은 25%군 보다 높은 경향을 보였고 골함량은 유의적으로 높았다. 5) ALP는 칼슘섭취가 낮은 25%군이 칼슘섭취가 높은 25%군보다 유의적으로 높게 나타났다.

Incidentally detected abnormal finding of femoral bone density image due to treatment of postoperative lymphatic leakage

  • Bone Mineral Densitometry (BMD) values is determined numerically with T or Z-score while the image is not considered in the interpretation. Abnormal finding was observed in the left femoral bone density image of a 64-year-old female patient who underwent a left kidney transplantation. This was due to the ethiodized oil used for embolization in the treatment of lymphatic leakage. Radiologic technologists should not only monitor changes of BMD values, but keep in mind to assess the changes in the image to provide accurate information to the patient.

채식과 일반식 폐경 후 여성의 식물성 에스트로겐 섭취와 골밀도와의 관련성 (The Study of Pytoestrogen Intake and Bone Mineral Density of Vegetarian and Nonvegetarian Postmenopausal Women)

  • There is some evidence that phytostrogen plays an important role in bone metabolism in postmenopausal women. In this study, we investigated the phytoestrogen intake levels and the relation between dietary phytoestrogens intake and bone mineral density of vegetarian (n = 77) and omnivore postmenopausal women (n = 122 . Vegetarian women, all of them were seven day adventists, who had been on vegetarian diet (almost lacto-ovo vegetarians;a few vegans) oyer 20 yrs. The average age of vegetarians and omnivores were 62.3 yrs and 60.2 yrs, respectively and, there was no significant difference. However, body weight (p < 0.001), body mass index (p < 0.001) of vegetarians were significantly lower than those of omnivores. The mean daily energy intake of vegetarians and omnivores were 1386.1 kcal (76.3% of RDA) and 1424.5 kcal (76.9% of RDA), respectively. The mean calcium intake of vegetarians (456.7 mg, 66.3 % of RDA) was not significantly different from that of omnivores (453.5 mg, 65.2 % of RDA). The mean daily isoflavones (daidzein +genistein) intake of vegetarians and omnivore were 33.9 mg and 23.9 mg, respectively. The vegetarians consumed significantly greater quantities of isoflavones (p < 0.05) and lignans precursor (p < 0.05). In the vegetarians, intake of isoflavones was significantly positively correlated with BMD of femoral neck, after adjusted for age and BMI. Also lignan precursor intake of vegetarians was significantly positively correlated with BMD of spine, after adjusted for age and BMI. In conclusion, in omnivore post menopausal women, intake of phytoestrogen such as isoflavones and lignans was little low, and it is not clear that positive association with bone mineral density. But in vegetarian postmenopausal women, phytoestrogen intakes be important factors related to bone mineral density.

경기 지역 폐경 전.후 여성들의 골밀도와 신체 계측, 모성요인 및 운동과의 상관성 연구 (A Study on Bone Mineral Density, Anthropometric Measurements, Maternal Factors, and Exercise and Their Correlation in Premenopausal and Postmenopausal Women in Gyeonggi-do Korea)

  • This study was performed to assess the relationships among bone mineral density, anthropometric measurements, maternal factors and exercise in premenopausal and postmenopausal women. Anthropometric measurements were taken by a trained practitioner and the maternal factors of the 78 subjects in Gyeonggi-do were acquired by an interview questionnare. The BMD of the lumbar spine(L2~L4), femoral neck, Ward's triangles and trochanters were measured by dual energy X-ray absorptiometry. The mean age of the premenopausal women was 45.23 years and that of the postmenopausal women whose ase of menopause was 49.37 years was 61.27 years. The age, waist and waist to hip ratio of postmenopausal women were significantly higher than those of premenopausal women. BMD of the lumbar spine(L2~L4) in postmenopausal women was significantly higher than that of premenopausal women. Number of children and age at last delivery were significantly higher in postmenopausal women than those in premenopausal women. The BMDs of the lumbar spine and femoral neck of premenopausal women was positively correlated with weight, body mass index, waist and WHR and the BMDs of these two sites in postmenopausal women were positively correlated with height, and weight. In postmenopausal women, BMD of the lumbar spine was negatively correlated with duration time after menopause and BMD of the femoral neck was positively correlated with age at last delivery. These results suggest that it is necessary to maintain adequate body weight. Health management and education about performing more exercise are recommended for postmenopausal women to prevent osteoporosis.

The Correlation of Metabolic Syndrome Factors and Bone Mineral Density on Postmenopausal Osteoporosis Patients with Low Back Pain under Korean Medicine Treatment

  • Objectives The relationship between metabolic syndrome causes and bone mineral density (BMD) was explored by taking 60 female chronic low back pain patients with age 61 years old or elder having metabolic syndrome and osteoporosis as study subjects. Methods Fasting blood glucose, serum total-cholesterol, triglyceride and HDL were measured by biochemical tests. Anthropometric elements and blood pressure were measured. Results Average BMD and T-score of part number 1 to 3 of lumbar vertebra were estimated by Quantitative Computed Tomography (QCT). In order to find the relationship between clinical factors and osteoporosis, correlation analysis was done on T-score. Age (r=0.679, p<0.01) had significant negative correlation and weight (r=0.342, p<0.01) and height (r=0.475, p<0.01) had significant positive correlation. Blood glucose, blood pressure, total cholesterol, triglyceride, HDL and body mass index did not have significant correlation. BMD had negative correlation with age (r=0.317, p<0.05). Regression analysis was done by taking T-score as independent variables and taking other factors as dependent variables. It was possible to know that age ($\beta$=-0.471, t=-7.050) with p<0.001, height ($\beta$=0.277, t=4.120) and weight ($\beta$=2.856, t=2.780) with p<0.05 have significant impact on osteoporosis. Conclusions Therefore, it was possible to know that T-score and BMD decrease as one gets older and T-score and BMD increase as one is taller and heavier.

칼슘보충 섭취와 운동이 중년 여성의 골밀도에 미치는 영향 (Effects of Calcium Supplementation and Exercise on Bone Mineral Density in Middle-Aged Women)

  • This study examined the effect of calcium supplementation and exercise on bone mineral density (BMD) and general characteristics, dietary intake and biochemical measurements for 45 healthy middle-aged women (40-57 years). Subjects were divided in to 4 groups; control group (G1), exercise group (G2), calcium supplementation group (G3), and calcium supplementation with exercise group (G4). The subjects were 45.8$\pm$ 0.66 years old. BMI(kg/$m^2$) was 23.31 $\pm$ 0.63 in Gl group, 22.92 $\pm$ 0.51 in G2 group, 23.64 $\pm$ 0.61 in G3 group, 23.40 $\pm$ 0.37 in G4 group, and BMI of all groups were in normal value. Energy intake was 1332.28㎉/day, 60% of RDA, so that may be unbalance of micronutrients. Especially, calcium intake was 62.8% of RDA that was very low level. Osteocalcine was not statistically significant but calcium supplementation group (G3) be showed increase. BMD was usually decreased by Aging, but in this study was increased in the all group, except control group. It showed increase of BMD for calcium supplementation and/or exercise. Overall results indicate that calcium supplementation and/or exercise increased BMD of middle-aged women, but long-term calcium supplementation and exercise will be able to more effect.(Korean J Nutrition 35(9) : 962~969, 2002)

폐금속광산 지역주민의 요중 카드뮴 수준 및 골밀도와의 관련성 (Relationship between Urine Cadmium and Bone Mineral Density of Residents Around Abandoned Metal Mines)

  • Objectives: The objective of this study was to investigate the possible effects of environmental cadmium (Cd) exposure on of bone mineral density (BMD) levels. Methods: A total of 171 residents around abandoned mines in Chungcheongnam-do were surveyed in 2008-2011. Urinary Cd and BMD were analyzed by atomic absorption spectrometry and Dual-Energy X-ray absorptionmetry, respectively. Osteoporosis and osteopenia were defined by T-scores set by the WHO; Tscore ${\geq}$ -1.0, normal; -1.0 > T-score > -2.5, osteopenia; and T-score ${\leq}$ -2.5, osteoporosis. Logistic and multiple linear regressions were applied to estimate the association between U-Cd levels and BMD. Results: The U-Cd geometric mean of 171 Koreans was 2.79 ${\mu}g/g{\cdot}cr$. The U-Cd concentration was significantly higher among women (2.98 ${\mu}g/g{\cdot}cr$) than men (2.39 ${\mu}g/g{\cdot}cr$). With the multiple regression model, the BMD was influenced by U-Cd, BMI, and monthly income. With the logistic regression model, osteoporosis was associated with U-Cd levels (OR = 3.239, 95% CI = 1.770-5.927). Conclusions: We conclude that exposure to cadmium is associated with an increased risk of osteoporosis.
