THE STUDY OF BONE MINERAL DENSITY IN THE MANDIBLE OF STREPTOZOTOCIN-INDUCED DIABETIC RATS (흰쥐에서 당뇨 유도 후 하악골 골밀도 변화에 관한 연구)

  • DM is a systemic disease with many complications. One of them, diabetic osteopenia is important sequelae and many authors reported reduced bone mass in diabetic rats. However, in mandible, study has been rare because of its anatomical limits. So the objective of this study was to investigate bony change in mandible of diabetic rats. Thirty-two adult rats were used in this study. Half of them were male and female respectively. In sixteen rats, streptozotocin was injected intraperitoneally to induce DM and the serum glucose concentration was checked to ensure the induction of DM prior to the time of sacrifice. At 1, 2, 3, 4, 6, 8, 12, 16weeks, control group and diabetic group rats were sacrificed respectively. And then bone mineral density of mandibles and femurs of the rats was measured using dual energy X-ray absorptiometry(DEXA). In addition serum osteocalcin and urine deoxypyridinoline were measured as markers of bone formation and resoption respectively. Mandibular and femoral bone density in streptozotocin induced rats was decreased with significance statistically after 4 weeks from injection. In mandible, comparing with femur, bone density was moderately decreased. The alveolar bone in mandible was more decreased bone density than the whole body in the mandible From these results, bone mineral density decreased in uncontrolled diabetic group with time, and especially alveolar bone was more destructive in the mandible. So authors think that consideration of reduced bone mineral density is necessary in dental procedure.

  • An aneurysmal bone cyst (ABC) can occur in many parts of the human body, but a primary ABC of the talus is extremely rare. ABCs are benign, but aggressively growing tumors that usually occur in the first two decades of life. Patients mainly complain of pain, limited movement of the involved joint or a palpable mass. Pain may worsen suddenly because of pathological fractures. If not treated properly, ABC has a risk of local recurrence, followed by the destruction of the joint and a significant functional deficit. While the complete removal of the bone tumor is essential, it is also important to treat the resultant bone defect after removal. The talus has an important part to play in weight-bearing. Therefore, an appropriate bone graft is required for large bone defects that occur after an ABC removal from the talus. We report a primary ABC of the talus in a 28-year-old male that was treated by curettage and a bone pillar pattern graft of autologous tricortical iliac crest bone. The patient had an excellent functional outcome with early weight-bearing, and there was no recurrence at 16 months of follow-up.

Evaluation of health screening data for factors associated with peri-implant bone loss

  • Purpose: Systemic health has a profound effect on dental treatment. The aim of this study was to evaluate peri-implant bone loss and health screening data to discover factors that may influence peri-implant diseases. Methods: This study analyzed the panoramic X-rays of patients undergoing health screenings at the Health Promotion Center at Seoul St. Mary's Hospital in 2018, to investigate the relationship between laboratory test results and dental data. The patients' physical data, such as height, weight, blood pressure, hematological and urine analysis data, smoking habits, number of remaining teeth, alveolar bone level, number of implants, and degree of bone loss around the implant, were analyzed for correlations. Their associations with glycated hemoglobin, glucose, blood urea nitrogen (BUN), creatinine, and severity of periodontitis were evaluated using univariate and multivariate regression analysis. Results: In total, 2,264 patients opted in for dental health examinations, of whom 752 (33.2%) had undergone dental implant treatment. These 752 patients had a total of 2,658 implants, and 129 (17.1%) had 1 or more implants with peri-implant bone loss of 2 mm or more. The number of these implants was 204 (7%). Body mass index and smoking were not correlated with peri-implant bone loss. Stepwise multivariate regression analysis revealed that the severity of periodontal bone loss (moderate bone loss: odds ratio [OR], 3.154; 95% confidence interval [CI], 1.175-8.475 and severe bone loss: OR, 7.751; 95% CI, 3.003-20) and BUN (OR, 1.082; 95% CI, 1.027-1.141) showed statistically significant predictive value. The severity of periodontitis showed greater predictive value than the biochemical parameters of blood glucose, renal function, and liver function. Conclusions: The results of this study showed that periodontal bone loss was a predictor of peri-implant bone loss, suggesting that periodontal disease should be controlled before dental treatment. Diligent maintenance care is recommended for patients with moderate to severe periodontal bone loss.

  • Osteopotosis is a disease characterized by reduced amount of bone mass leading to enhanced bone flagility. The number of patients with osteoporotic vertebral fracture is increasing and it is one of the leading causes of morbidity in the elderly and postmenopausal women. It is a condition in which bone mass decrease, causing bones to be more susceptible to fracture. A trivial trauma can easily cause one or more bones to break in a person with severe osteoporosis. So it is a major health problem. Pysicians and patients are concerned with the optimum approach to the treatment and prevention of osteoporosis. Until a recent date, many oriental medicine studies were performed to find the preventive and curative efficacy on the osteoporosis, which is differ from therapeutics of Western-medicine. The proper use of Herb-med and role of Accupuncture are issues that have generated major research efforts. This study was carried out to investigate evaluation of clinical and experimental study on the osteoporosis. So, these are to be mentioned in this paper.

  • The author observed a case of osteogenic sarcoma in a 11-year-old female with complaint of painful swelling on face in right side. The observed results were as follows: 1. Large hematoma was obsered, and patient complanited painfull swelling on c/c site. 2. Predisposing factor of osteogenic sarcoma was not clear, but patient had history of extraction before patient visiting infirmary of our dental collage. 3. Serologic findings were not specific, and serum alkaline level was normal. 4. Radiographic findings were as follows: ① Diffuse faint radiopacity in the lesion ② Bony destruction and increased radiopaciy in right antrum ③ Displacement of multiple teeth on involved area(i. e ;#12, 15. 55, 16, 17, 18), ④, Increased periodontal space in singel tooth(#13) ⑤Destruction of bony crypt on involved teeth(#13, 14, 15, 17, 18) ⑥ Loss of lamina dura of three teeth in involved area(#11, 12, 16) 5. Computed tomographic findings were as follows: ① Large calcific and hetergenous component mass in the Rt. maxillary sinus, and this mass extending to Rt. maxilla, alveolar bone, ethmoid sinus. ②, Soft tissue bulging into Rt. side nasal cavity and oral cavity. ③ Bone destruction of maxillary sinus wall and Rt. alveolar bone.

Effects of Nutrients Intakes on Development of Osteoporosis in Korean Postmenopausal Women (폐경후 우리나라 여성의 영양 섭취 상태가 골다공증 발병에 미치는 영향)

  • The relationship of nutrients intake and bone mineral density (BMD) was investigated in 285 Korean postmenopausal women (age 40-70 y) consisted of 65 normal women and 159 osteopenia patients who don't have other diagnosed disease. BMD was measured at the spine (vertebrae L2-4) and femur (neck, Ward's triangle and trochanter). Height, weight, body mass index(BMI), lean body mass(LBM), body fat(Bfat) and dietary intakes of animal calcium(animal Ca), protein and phosphorus per 1,000kcaI intake were correlated with BMD of the spine and the femur positively(p<0.05). Women with an animal calcium intake < 315mg/d (mean % animal Ca/total Ca intake > 50%) exhibited significantly lower BMD of the spine and Ward's triangle than that of women with an intake > 315mg/d, p<0.OS. When subjects were grouped by diagnosis into 3 groups (normal, osteopenia and osteoporosis group), animal Ca intake of osteoporosis group (mean animal Ca intake 261mg) was significantly lower than those of the other two groups (mean animal Ca intake 306mg and 297mg, respectively), p < 0.05. These results suggest that balanced nutrients intake and increased animal Ca in the diet is likely to be beneficial in reducing bone loss in postmenopausal women.

  • Currently, x-ray is mostly used for the diagnosis of dental cavity and osteoporosis. The osteoporosis is broadly defined as a decrease in the amount of bone mass per unit volume of the bone. Clinically the manifestation of low bone mass presents a clinical problem to the general population as an increase in fracture risk and especially in aging population[1]. Although the amount of the irradiated x-ray to the human body for the clinical diagnosis is relatively small, the exposition of the x-ray to the human body should be minimized as much as possible, since the x-ray is an ionizing radiation. To investigate other possible systems replacing X-ray, ultrasonic imaging and MRI(Magneto-Resonance-Imaging) systems were studied. Unfortunately, an effective and safe diagnosis tool for detecting the dental cavity and the osteoporosis is currently lacking. (omitted)

  • Langerhans' cell histiocytosis is a rare disease showing proliferation of histiocytes in multiple organ system. Bone lesions are the most common radiolologic manifestation of Langerhans' cell hysticytosis. Extraskeletal sites of Langerhans' cell hystiocytosis involvement include the skin, lymph nodes, thymus, lungs, central nervous system, liver, pancrease, spleen, and bowel. The authors have experienced a case of Langerhans' cell histiocytosis which involved the skull. A 3-year-old female presented with soft tissue mass on right periorbital area. Plain skull X-ray showed punched out bone lesion. Computed tomography showed non-enhancing osteolytic lesion on right frontal skull. Histologic findings of an excised mass revealed pathologic features of Langerhans' cell histiocytosis.

  • Aneurysmal bone cyst(ABC) is a benign lesion and generally occurs in the long bones and vertebral column. ABC of the rib is an uncommon entity. We reported a case of ABC originating in the left first rib. The patient was a 19-year-old woman and presented with palpable mass on the shoulder. Chest X ray and MRI showed a large expansile mass, with multiple cystic areas, arising from the left first rib and bulging out into retroclavicular and paravertebral soft tissue, so the lesion was initially misdiagnosed as a soft tissue malignant tumor destructing adjacent rib. On histological examination, the mass, which was surrounded by a peripheral band of mature trabecular bone, consisted of multiple anastomosing cavernous blood channels separated by fibrous septa that containing osteoid, myxochondroid material and osteoclast-type giant cells.

Acidification of drinking water improved tibia mass of broilers through the alterations of intestinal barrier and microbiota

  • Objective: Diet acidification supplementation is known to influence intestinal morphology, gut microbiota, and on phosphorus (P) utilization of broilers. Alterations in intestinal barrier and microbiota have been associated with systemic inflammation and thus regulating bone turnover. Hence the effect of acidifier addition to drinking water on tibia mass and the linkages between intestinal integrity and bone were studied. Methods: One-d-old male broilers were randomly assigned to normal water (control) or continuous supply of acidified water (2% the blend of 2-hydroxy-4-methylthiobutyric acid, lactic, and phosphoric acid) group with 5 replicates of 10 chicks per replicate for 42 d. Results: Acidification of drinking water improved the ash percentage and calcium content of tibia at 42 d. Broilers receiving acidified water had increased serum P concentration compared to control birds. The acidified group showed improved intestinal barrier, evidenced by increased wall thickness, villus height, the villus height to crypt depth ratio, and upregulated mucin-2 expression in ileum. Broilers receiving drinking water containing mixed organic acids had a higher proportion of Firmicutes and the ratio of Firmicutes and Bacteroidetes, as well as a lower population of Proteobacteria. Meanwhile, the addition of acidifier to drinking water resulted in declined ileal and serum proinflammatory factors level and increased immunoglobulin concentrations in serum. Concerning bone remodeling, acidifier addition was linked to a decrease in serum C-terminal cross-linked telopeptide of type I collagen and tartrate-resistant acid phosphatase reflecting bone resorption, whereas it did not apparently change serum alkaline phosphatase activity that is a bone formation marker. Conclusion: Acidified drinking water increased tibia mineral deposition of broilers, which was probably linked with higher P utilization and decreased bone resorption through improved intestinal integrity and gut microbiota and through decreased systemic inflammation.
