Effects on Glycemic Control by Continuous Self Monitoring of Blood Glucose in Diabetic Patients (당뇨병환자의 지속적 자가 혈당관리에 따른 혈당조절의 효과)

  • Purpose: To evaluate the effects of continuous self monitoring of blood glucose (SMBG) on glycemic control in diabetic patients. Methods: Among the diabetic patients who visited the department of internal medicine in H hospital from April 1 to October 27 2008, 76 patients were divided into the experimental group (n=39) and control group (n=36). The experimental group was required to measure blood glucose levels at least twice every other day using SMBG; their compliance was aided by weekly telephone reminders for 3 months after discharge. Glycated hemoglobin, fasting blood sugar and postprandial 2 hours glucose level, triglyceride and high density lipoprotein were measured before and after the study. Results: Baseline characteristics were not different between the experimental and control groups. Three months later, there was a significant difference in postprandial glucose between the two groups (t=-2.042, p=.045). Conclusion: There was significant improvement of glycemic control after the use of continuous SMBG, suggesting its value in strict glycemic control and the merit of an aggressive education program.

  • HbA1c test measures the amount of glycated hemoglobin in blood. HbA1c shows the average of blood glucose levels for the past three months, this is a better indicator of how overall diabetes is doing. HbA1c gives a much better idea of how the body is breaking down the glucose. Therefore, this HbA1c is very important tool for maintaining normal glucose levels for pre-and diabetic patients. Total 408 participants were tested HbA1c voluntarily from Chosunilbo Health Expo (8th~11th, July 2010). Through this small-scaled direct HbA1c, about 54.7% (207 out of 408) was shown glucose tolerance and diabetes. However, 61 from 157 participants who were shown under 6.9% HbA1c (normal and pre-diabetic stage) are taking only antidiabetic drugs to maintain a normal blood glucose. Regular HbA1c test can bring an important management and awareness about controlling blood sugar level and prevention of diabetic complications.

  • This study has examined the effect of music Rope-skipping on body composition and health related blood components of middle school students. The experiment has been performed for 12 weeks, varying the exercise intense depending on preliminary stage, elementary and intermediate stage and conclusion is as follows based on the change in body composition, blood components related to health before and after the experiment. All males and females showed decreased, but there were no statistically different in body fat mass and body fat percentage. Fat-free mass of males showed decreased and females had an increased, but there were no statistically different. Regarding body-mass index of males and females were on the decreased, but there were no statistically different. As for hematocrit of males and females statistically grew by about 4.53%(p<.05). The number of erythrocyte and leukocyte in males and females showed increased, but there were no statistically difference. Concerning hemoglobin of males and females showed a increased(p<.01). The number of thrombocyte of males and females were on the increased, but there were no statistically different. Cholesterol in males and females were on the decreased, but it was not statistically different. Blood glucose of males and females were on the decreased(p<.05).

  • This study was to investigate the protection of the extracts from four parts of deer antler in an anemia model induced by intravenous injection of phenylhydrazine·HCl (PHZ) at 10 mg/kg for 4 days. After PHZ injection, female Sprague-Dawley rats were administrated partial deer antler extract (200 mg/kg/day, p.o.) daily for 1 week. Results showed that sever hemolysis was induced by PHZ. For antler extract-treated groups, the concentration of hemoglobin, hematocrit and red blood cells number increased much more significantly than PHZ-treated group. Upper antler extract-treated group was more remarkable than other parts in suppressing the increase of reticulocyte in whole blood. Moreover, antler extract administration significantly improved serum erythropoietin concentration. The activity of $\delta$-aminolevulinic acid dehydrates (ALAD) in liver homogenate was increased in antler extract-treated groups, especially middle and base extract-treated groups showed statistical significance. These results could be concluded that the deer antler extract improved anemia induced by PHZ injection through improving hematological values, serum EPO value, ALAD enzyme activity.

Processing Suitability of Canned Ark Shell (새고막의 통조림 가공 적성)

  • This study was carried out to process canned ark shell with highly quality by management of proper processing conditions for prevention of oxidation and discoloration by heating. Ark shell has hemoglobin as blood pigment in red blood shell which same as other cockles. Hemoglobin is easy to come oxidation and browning reaction, and it has large contents of carotenoid as meat pigment. Proximate compositions in ark shell were 76.9% of moisture, 18.1% of crude protein, 1.8% of crude lipid, 1.3% of carbohydrate and 1.6% of crude ash. And contents of carotenoid and hemoglobin were 0.67~1.02mg% and 0.98~1.64g/dl, respectively. When the living ark shell was soaked in 2% NaCl solutions, about 89% of mud was removed after 10 hours soaking, and over 91% was removed when the pH was adjusted to 7.5. Carotenoid pigment were prepared that extracted from ark shell by using acetone. And determined visible spectrum were two peak at 452nm and 687nm, and λmax were 452nm. During thermal treatment at 95$^{\circ}C$, 111$^{\circ}C$, 116$^{\circ}C$ and 121$^{\circ}C$ for 60 minutes, retention ratio of carotenoid were 71.8%, 66.8%, 64.4% and 36.5%, and after 120 minutes retention ratio were 56.6%, 30.6%, 30.3% and 17.2%, respectively. When heated at 95$^{\circ}C$, 111$^{\circ}C$, 116$^{\circ}C$ and 121$^{\circ}C$, formation of browning material were increased at high temperature and long time treatment.

Does a Preoperative Temporary Discontinuation of Antiplatelet Medication before Surgery Increase the Allogenic Transfusion Rate and Blood Loss after Total Knee Arthroplasty? (항 혈소판 제제의 술 전, 일시적 중단은 슬관절 전치환술 이후의 실혈량 및 동종수혈의 필요성을 증가시키지 않는가?)

  • Purpose: The aim of this study was to determine if preoperative temporary discontinuation of antiplatelet medication (aspirin, clopidogrel, or cilostazol) is a safe procedure that does not increase early postoperative bleeding and allogenic blood transfusion after a total knee arthroplasty. Materials and Methods: A retrospective analysis was conducted among consecutive patients who underwent navigation assisted primary total knee arthroplasty performed by a single surgeon, from January 2013 to December 2016. A total of 369 patients enrolled in this study were divided into two groups, 271 patients with no history of antiplatelet therapy and 98 patients who underwent 7 days of temporary withdrawal of antiplatelet therapy. Comparative analysis between the two groups, on the variation of hemoglobin and hematocrit during the first and second postoperative days, was conducted to determine the amount of early postoperative bleeding and the frequency of allogenic blood transfusion during hospitalization. Results: The variation of hemoglobin, hematocrit during the first and second postoperative days and the frequency of allogenic blood transfusion between no history of antiplatelet medication and discontinuation antiplatelet medication before 7 days from surgery were similar in both groups. Of the 369 patients, 149 patients received a blood transfusion during their hospitalization. Compared to patients who did not receive a blood transfusion, those who did received blood transfusion were significantly older in age, smaller in height, lighter in weight, and showed significantly lower preoperative hemoglobin and hematocrit values. No statistically significant differences in sex, preoperative American Society of Anesthesiologists scores, and the history of antiplatelet medication until 7 days prior to surgery were observed between the two groups according to blood transfusion. Conclusion: Compared to patients with no history of antiplatelet medication, the temporary discontinuation of antiplatelet medication 7 days prior to surgery in patients undergoing antiplatelet medication did not increase the amount of postoperative bleeding or the need for allogenic blood transfusion.

  • The purpose of this study was to obtain reference data of parameters for hematological health diagnosis in cultured freshwater fish and also evaluate application of automatic dry-type chemistry analyzer (FUJI DRI-CHEM 3000) used to those blood tests. A blood profile of total 200 fish for rainbow trout (Onchorhynchus mykiss), israel carp (Cyprinus carpio), tilapia (Oreochromis niloticus) and eel (Anguilla japonica) cultured in Inland Fisheries Research Institute of NFRDI was determined by hemoglobin (Hb) and plasma chemistry tests: total protein (TP), albumin (ALB), alkaline phosphatase (ALP), blood urea nitrogen (BUN), lactate dehydrogenase (LDH), triglyceride (TG), total cholesterol (TCHO), creatinine (CRE), aspartate aminotransferase (AST), alanine aminotransferase (ALT), glucose (GLU). The values of ALT, TG, LDH, ALB, TCHO, AST and ALP were outside from the minimum and/or maximum of the established detectable range of the analyzer. ALT and TG were not detectable in the range of 67%~61.5%. LDH, ALB and TCHO were not detectable in the range of 36~17%. AST and ALP were not detectable in the range of 5.5~0.5%. However, the values of BUN, CRE, GLU, Hb and TP were below the detectable limits of the analyzer.

Hypoglycemic effects of Welsh onion in an animal model of diabetes mellitus

  • Tight control of blood glucose is the most important strategy for the treatment of diabetes mellitus. Here, we investigated the beneficial effects of Welsh onion on fasting and postprandial hyperglycemia. Inhibitory activities of hot water extracts from the green stalk and white bulb, which are the edible portions of the Welsh onion, and the fibrous root extract against yeast ${\alpha}$-glucosidase were measured in vitro. To study the effects of Welsh onion on postprandial hyperglycemia, a starch solution (1 g/kg) with and without Welsh onion fibrous root extract (500 mg/kg) or acarbose (50 mg/kg) was administered to streptozotocin-induced diabetic rats after an overnight fast. Postprandial plasma glucose levels were measured and incremental areas under the response curve were calculated. To study the hypoglycemic effects of chronic feeding of Welsh onion, five-week-old db/db mice were fed an AIN-93G diet or a diet containing either Welsh onion fibrous root extract at 0.5% or acarbose at 0.05% for 7 weeks after 1 week of adaptation. Fasting plasma glucose and blood glycated hemoglobin were measured. Compared to the extract from the edible portions of Welsh onion, the fibrous root extract showed stronger inhibition against yeast ${\alpha}$-glucosidase, with an $IC_{50}$ of 239 ${\mu}g/mL$. Oral administration of Welsh onion fibrous root extract (500 mg/kg) and acarbose (50 mg/kg) significantly decreased incremental plasma glucose levels 30-120 min after oral ingestion of starch as well as the area under the postprandial glucose response curve, compared to the control group (P < 0.01). The plasma glucose and blood glycated hemoglobin levels of the Welsh onion group were significantly lower than those of the control group (P < 0.01), and were not significantly different from those fed acarbose. Thus, we conclude that the fibrous root of Welsh onion is effective in controlling hyperglycemia in animal models of diabetes mellitus.

Operative Treatment of Gastric Carcinoid Tumor Presenting as Multiple Polyps: A Case Report (다발성 용종의 형태로 발현된 위유암종(Gastric Carcinoid Tumor)의 수술적 치료 1예)

  • Gastric carcinoid tumor is a neoplasm that arises from enterochromaffine-like (ECL) cells in the gastric fundus. It is a rare disease that comprises less than 2% of all gastric neoplasms; however its incidence has been recently increasing. We experienced one case of gastric carcinoid tumor that was revealed to be multiple polypoid lesions. A 29-year-old female patient visited a hospital three years ago due to syncope. The blood hemoglobin was measured as 6.0 g/dl. Gastroscopy revealed multiple polypoid lesions with bleeding; therefore endoscopic clipping was performed. The polyps were diagnosed as carcinoid tumor via endoscopic biopsy. She was transferred to our hospital because of persistent iron deficiency anemia that was caused by bleeding at the gastric polyps. Gastroscopy revealed more than twenty various-sized polypoid lesions from the mid-body to the antrum. The blood hemoglobin level was 9.0g/dl. Total gastrectomy was performed under the diagnosis of gastric carcinoid tumor with bleeding. All of the gastric polyps were diagnosed as carcinoid tumors, and any metastasis to the regional lymph nodes was not found. Eighteen months after operation, the blood hemoglobin was increased to 12.8g/dl with no evidence of recurrence. Surgical resection should be considered for treating gastric carcinoid tumor with continuous bleeding.

Obesity as associated factors for Insulin, Fasting blood sugar, and Hemoglobin A1c in Korean adults (한국 성인의 비만이 인슐린, 공복혈당 및 당화혈색소에 미치는 영향)

  • The purpose of this study was to investigate the effects of body mass index(BMI) on insulin. fasting blood sugar(FBS), and hemoglobin A1c(HbA1c). This study analyzed the data of 2015, The Sixth KNHANES, and it was included to 4554 subject over 20 years old. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and stepwise multiple regression analysis(SPSS 25.0). The major findings, BMI was positively correlated with insulin(r=0.397, p<0.001), FBS(r=0.241, p<0.001), and HbA1c(r=0.237, p<0.001). Factors affecting insulin are overweight(${\beta}=.130$, t=8.193, p<0.001), obesity(${\beta}=.390$, t=24.390, p<0.001) and age. the explanatory power is 13.1% (F=53.584, p<0.001). Factors affecting FBS were overweight(${\beta}=.072$, t=4.478, p<0.001), obesity(${\beta}=.186$, t= 11.485, p<0.001), age, the explanatory power was 10.6% (F=42.351, p<0.001). Factors affecting HbA1c were overweight(${\beta}=.059$, t=3.712, p<0.001), obesity(${\beta}=.190$, t=11.895, p<0.001) age and sex, the explanatory power is 14.1% (F=58.134, p<0.001). Based on this study result, diabetes mellitus prevention and management programs should include interventions related to obesity.
