Current tendency of oriental approach to the cerebral palsy (뇌성마비(腦性痲痺)의 한의학적(韓醫學的) 접근(接近)에 관한 최신(最新) 동향(動向))

  • Back ground : Cerebral palsy(CP) is a static encephalopathy caused by an insult to the brain during the prenatal, perinatal, or postnatal period (ie, up to 2 years). CP can lead to global dysfunction but always includes motor problems Objective : The purpose of this study is to investigate the current tendency of oriental approach to the CP and promote oriental treatment of CP in Korea Method : Investigation of current literature and clinical paper in Korea and Chinese Result and Conclusion : 1) CP fall under the category of wu-chi(five kinds of retardations : 五遲), wu-ruan(five kinds of flaccidity : 五軟), wu-ying(five kinds of Stiffness : 五硬) in oriental medicine and correspond to naoxing-tanhuan in current chinese medicine. 2) CP is mainly caused by weakness of the liver and kidney(肝腎不足), weakness of the spleen and stomach(脾胃虛弱), and the method of treatment is tonify the liver and kidney(補益肝腎), tonify the spleen and replenish qi(補脾益氣), but yu-chi(the faculty of speech : 語遲) is caused by deficiency of the heart(心虛) so that treated with method of invigorating the heart and nourishing blood(補心養血). Recently blood stagnancy the stagnation of qi(氣滯血瘀) is considered as the cause of CP, promoting qi circulation to invigorate blood(行氣活血) is mentioned the treatment of method. 3) In addtion to a herbal medication and acupuncture, the various treatments of scalp acupuncture(頭鍼), acupoint injection(穴位注射), catgut embedding therapy(埋鍼) etc. had been applicated to CP and for the objective evaluation of remedial value, TCD, EEG, BMD have been used.

Mechanism of Jaeumgenby-tang adding Aurantii FructusㆍGastrodae Rhizoma on the Improvement and Changes of Cerebral Hemodynamics (자음건비탕가지각ㆍ천마가 뇌혈류역학 변동에 미치는 작용기전)

  • Jaeumgenby-tang(JGT) have been used in oriental medicine for many centuries as a therapeutic agent of vertigo caused by deficiency of qi(氣) and blood(血). Effect of Aurantii Fructus(AF) take off the phlegm by promoting the circulation of qi, Gastrodae Rhizoma(GR) has effects treating for headache, vertigo by calming the liver and suppressing hyperactivity of the liver-yang(陽). And, We reported that JGT adding AFㆍGR extract(JGTAG) was significantly increased regional cerebral blood f1ow(rCBF) by dilating pial arterial diameter(PAD). Therefor we designed to investigate whether JGTAG is mediated by adrenergic β-receptor, cyclooxygenase or guanylate cyclase in normal rats. The changes of rCBF and mean arterial blood pressure(MABP) were determinated by laser-doppler flowmetry(LDF), and the change of PAD was determinated by video microscope and width analyzer. The results were as follows in normal rats; Pretreatment with propranolol(1mg/kg, i.v.) was significantly inhibited JGTAG induced increase of rCBF, PAD and MABP, and pretreatment with indomethacin(1 mg/kg, i.v.) was significantly inhibited too. But pretreatment with methylene blue(10μg/kg, i.v.) were accelerated JGTAG induced increase of rCBF and MABP, but pretreatment with methylene blue was inhibited JGTAG induced increase of PAD. This results suggest that the mechanism of JGTAG is mediated by adrenergic β-receptor and cyclooxygenase.

A Study on Oriental Medical Factors of Teenagers' Menstrual Disorders Based on DSOM (한방진단시스템(DSOM)을 이용한 청소년 월경통, 월경량 이상의 한의학적 인자(因子) 연구)

  • Purpose: In Oriental medicine, menstruation is a significant index of women's health. Especially high school girls often have various menstrual problems. We think that students who have menstrual problem have some symptoms in relation to an etiological cause. So we had carried out this study to investigate association of DSOM scores and an abnormality of menstruation in oriental medicine. Methods: First we have 2137 high school girls answer to menstrual questionnaire that we made and DSOM, next investigated the pathogenic factor of DSOM which effect dysmenorrhea and menorrhagia using regression model. Logistic regression analysis indicate as follows. Results: 1. In comparison with dysmenorrhea, Qi depression(氣滯), blood stasis(血瘀), dampness (濕), phlegm(痰) is associated positively and liver(肝) negatively. The sensitivity is 64.3%, and the specificity is 75.1%. 2. In comparison with menorrhagia, Blood deficiency(血虛), Qi depression(氣滯), blood stasis(血瘀) is associated positively and liver(肝) negatively. The sensitivity is 53.9%, and the specificity is 76.1%. Conclusion: In this study, that has been carried out to organize the diagnosis of dysmenorrhea and menorrhagia in Oriental medicine, we can see that the disease factors of them were correlated with our Medical theory.

The Study on the Books of Oriental Medicine Which Deal with Variation in Diagnosis on the Neck and Nuchal Pain (경항통(頸項痛)의 변증(辨證)에 관한 문헌고찰(文獻考察))

  • Objectives : The studies on variation in diagnosis on the neck and nuchal pain has not been done thoroughly as we can use it in clinical practice of these days. For this reason, I examined the variation in diagnosis mentioned in the classics of Oriental Medicine as the preceding study on standardization of variation in diagnosis on neck and nuchal pain. Methods : I gathered the twenty kinds of classics of Oriental Medicine that were computerized, the textbooks on Oriental Medicine which are being used these days, and the theses on current clinical research. After gathering these data, I analyzed these according to the variation in diagnosis. Results : The classics of Oriental Medicine on the neck and nuchal pain mentioned very much about the neck and nuchal pain occurred by the pathogenic factor of Wind, Cold, and Dampness, disharmony created by deficiency of Liver and Kidney, and pathogenic state of Meridians of Taiyang. According to the texts of these days, the differentiation of syndromes can be divided into four kinds of items such as Wind-Cold pathogen, Wind-Dampness pathogen, Phelgm-Heat, and disharmony between Qi and Blood. The theses of these days rarely mentioned about variation in diagnosis on the neck and nuchal pain. Conclusions : The differentiation of syndromes on the neck and nuchal pain can be divided into four kinds items as affection by exopathogen like Wind, Cold, Dampness, Heat, and so on, stagnation of Qi and the coagulation blood, deficient syndrome of Liver and Kidney, and deficient syndrome of Qi and Blood.

Oral Administration of Alcohol-Tolerant Lactic Acid Bacteria Alleviates Blood Alcohol Concentration and Ethanol-Induced Liver Damage in Rodents

  • Excessive alcohol consumption can have serious negative consequences on health, including addiction, liver damage, and other long-term effects. The causes of hangovers include dehydration, alcohol and alcohol metabolite toxicity, and nutrient deficiency due to absorption disorders. Additionally, alcohol consumption can slow reaction times, making it more difficult to rapidly respond to situations that require quick thinking. Exposure to a large amount of ethanol can also negatively affect a person's righting reflex and balance. In this study, we evaluated the potential of lactic acid bacteria (LAB) to alleviate alcohol-induced effects and behavioral responses. Two LAB strains isolated from kimchi, Levilactobacillus brevis WiKim0168 and Leuconostoc mesenteroides WiKim0172, were selected for their ethanol tolerance and potential to alleviate hangover symptoms. Enzyme activity assays for alcohol dehydrogenase (ADH) and acetaldehyde dehydrogenase (ALDH) were then conducted to evaluate the role of these bacteria in alcohol metabolism. Through in vitro and in vivo studies, these strains were assessed for their ability to reduce blood alcohol concentrations and protect against alcohol-induced liver damage. The results indicated that these LAB strains possess significant ethanol tolerance and elevate ADH and ALDH activities. LAB administration remarkably reduced blood alcohol levels in rats after excessive alcohol consumption. Moreover, the LAB strains showed hepatoprotective effects and enhanced behavioral outcomes, highlighting their potential as probiotics for counteracting the adverse effects of alcohol consumption. These findings support the development of functional foods incorporating LAB strains that can mediate behavioral improvements following alcohol intake.

  • Chronic fatigue syndrome is an agonizing illness that impedes the quality of life of people worldwide. Although this syndrome is reported to be increasing in industrialized countries, there are as yet no effective therapies. In the view of Oriental medicine, chronic fatigue syndrome is thought to result from a state of unbalanced inter-organ functions, or a condition of deficiency in the qi or blood characteristics. On the other hand, chronic fatigue-related symptoms are one of the most common complaints of patients using complementary and alternative medicine. This study aimed to theoretically analyze chronic fatigue syndrome from an Oriental medicine point of view, so to help Oriental doctors to design appropriate therapeutic models.

Mucopolysaccharidosis Type III: Overview and Future Therapeutic Approaches

  • Mucopolysaccharidosis (MPS) type III, or Sanfilippo syndrome is a rare autosomal recessive lysosomal storage disorder. It is caused by a deficiency of one of four enzymes involved in the degradation of the glycosaminoglycan (GAG) heparan sulfate. The resultant cellular accumulation of heparan sulfate causes various clinical manifestations. MPS III is divided into four subtypes depending on the deficient enzyme: MPS IIIA, MPS IIIB, MPS IIIC and MPS IIID. All the subtypes show similar clinical features and are characterized by progressive degeneration of the central nervous system (CNS). Main purpose of the treatment for MPS III is to prevent neurologic deterioration. However, conventional enzyme replacement therapy has a limitation due to inability to cross the blood-brain barrier. Several experimental treatment options for MPS III are being developed.

  • In adult patients, the corrections of their malocclusion may be more difficult and require longer treatment time due to thicker layer of cortical bone and reduced blood supply. Recently, various methods such as surgery, implant for anchorage and corticotomy have been tried to overcome these problems. Corticotomy is a surgical technique in which a fissure is made through the cortical bone that surrounds a tooth so that the tooth is embedded within a block of bone that is connected to adjacent blocks through only the medullary bone. Technique of corticotomy has been widely used for correction of maxillary transverse deficiency, but hasn't actively in other fields of orthodontics. We applied corticotomies in many types of orthodontic treatment and had satisfactory results. We suggested clinical application of corticotomy in adult orthodontics to reduce treatment period and to achieve better stability after orthodontic treatment.

A Clinical Report about the Effects of Dansambohyul-tang on a Stroke Patient with a Gastric Ulcer (뇌경색 환자로 위장관출혈 소인이 있는 위궤양환자의 단삼보혈탕을 이용한 치험 1례)

  • After stroke, there are many cases of gastro-intestinal bleeding in patients with dysphagia who are fed by Levin tubes. Ulcers in the stomach and duodenum are occasionally produced by psychiatric stress, irritation with the Levin tube and central nerve rusturbances. Because Dansambohyul-tang has been used to treat deficiency of blood, alleviate pain, tranquilize and to treat patients differentiated with an insufficiency of the heart and the spleen in oriental medicine, we treated a 62 year-old male patient who suffered from gastric ulcers with gastro-intestinal bleeding after stroke, and who was differentiated with an insufficiency of the heart and the spleen with Dansambohyul-tang, herb complex. After 20 days of treatment with Dansambohyul-tang, we observed improvement in the gastric ulcer through gastroscopy; serum hemoglobin increased by 5.1g/dl after 2 months; and symptoms (pale complexion.dysphagia.general weekness) were alleviated. So Dansambohyul-tang shows therapeutic effects on gastric ulcers with gastro-intestinal bleeding after stroke.

A Case Report on Cardiac Asthma due to Congestive Heart Failure (울혈성 심부전으로 인한 심장성 천식환자 치험 1례)

  • Congestive heart failure is a cardiac dysfunction that can't supply sufficient amount of blood. The disorder usually causes hypertension, ischemic cardiac disease and give rise to pulmonary congestion and pulmonary epileptic edema. The symptoms of the disease are dyspnea, palpitation, edema, etc. We treated a 82 year-old female patient who had severe dyspnea, nausea, and dizziness. At the point of differentiation of syndromes(辨證), the case was diagnosed as Su-Cheon(水喘) induced by Yang-deficiency of Both Spleen and Kidney(脾腎陽虛) and was administered Yerunpang(胃?湯). After two weeks of the treatment, we witnessed improvement in the symptoms of CHF and general depressed condition. This report describes the process and contents about the way the patient was treated.
