The Literary study on Flaccidity-syndrome (위증에 대한 문헌적 고찰)

  • We came to the conculsion after considering all of information from many kinds of books on the cause, pathogenesis and treatment of Flaccidity-syndrome. The results were as follows : 1. Flaccidity-syndrome means limb-relaxation due to muscle atony that isn't able to constraction. It's begun as mild degree from extremities, in some cases ended to quadriplegia or expire. 2. Cause factor and pathogenesis of Flaccidity-syndrome is various. After Lung fluid consuption caused by heat-evil was refered in The Yellow Emperor's Canon of Internal Medicine. They were refered as cause factor that Main channel asthenia, excess of sexual intercourse, wetness-evil, heat-evil gets into the interior, asthenia of the spleen and stomach. Since Ming Dynasty, It's classified to wetness-heat evil, wetness-phlegm, deficiency of vital energy, deficiency of blood, deficiency of yin, blood stasis and indygestion, etc. 3. In the view of treatment of Flaccidity-syndrome, Yangming was selected in The Yellow Emperor's Canon of Internal Medicine, and it's been mean to clear away wetness-heat evil located at Yangming. In the method of acupuncture it was same on the base, and many skills have been used that electronic acupuncture, point-injection theraphy, acupuncture point block, catgut implantaion at acupuncture point, cutaneous acupuncture, auriculo-acupuncture and head acupuncture by the through post generation. 4. Flaccidity-syndrome was defined to weak, disuse and non-pain. Beacause it was non-pain, so medicine to expel wind-evil was prevented to use. But through post generation Flaccidity-syndrome has been treated that is able to cause pain or numness as arthralgia-syndrome. Therefore there is tendency that medicine to expel wind-evil is capable within pathological basement of Flaccidity-syndrome in recent. 5. In the view of west-medicine, Flaccidity-syndrome is diplegia or quadriplegia with sensory disorder, muscle atropy in some cases. And there are spinal disease, peripheral nerve disease, muscular disease, nerve-muscle copula disease. The symptoms are able to amyotomia, numness, sensory disorder, pain.

  • Following conclusions are drawn from study about Yukmigiwhangwon through referring to literatures and reviewing in the point of Hyungsang medicine. Yukmigiwhangwon was prescribed by Jeon Eul in order to cure five kinds of flaccidity of infants, but after since then it has been basically applied to symptoms which was caused by deficiency of the kidney not only infants but also adult. The deficiency of kidney is the lack of vital essence and body fluid stored in kidney, it happens because infants are born in the conditions of lack of receiving in the womb. And it happens because adults infringe the rules of life, especially the sexual life. Main symptoms of the poverty of renal functions are emptiness of the brain marrow, dizziness, forgetfulness, tinnitus, deafness, vertigo and dim eyesight, insomnia, the low back and knees aching and limping, heat on the legs, tibia aching, nocturnal emission, dry mouth, reddened tongue and little coating or non coating of the 'tongue, weak blood meridians. the treating method is nourishing Eum and supplementing kidney. In Dongeuibogam Yukmigiwhangwon is mentioned in the parts of vital essence, blood, voice, kidney, urine, eye, ear, the low back, hair, fire, exhaustion syndrome, cough, polydipsia, infant, etc. The shapes which Yukmigiwhangwon is applied to are the Dam Body Doing deficiency of Eum due to excess of Yang, man, infant, Jung type, Fish group, reverse triangular shape, big headed man, big eared man, big mouthed man, man having prominent cheekbones, man having long waist, man having big hips, man with thick hair or white hair, man with bald on the head, etc. In color it is applied to darkish complexion, flushed tip of the nose, or flushed cheek bones. Through inquiring into the clinical cases on the points of Hyungsang medicine Yukmigiwhangwon is efficacious cure to lumbago, symptoms of tendons and bones, symptoms of eyes and ears, symptoms of respiratory organs, dermatopathia, headache, polydipsia, urinary disorders, etc.

Effect of Korean Red Ginseng on Psychological Functions Patients with Severe Climacteric Syndromes : A Comprehensive Study from the Viewpoint of Traditional KAMPO-medicine and Western Medicine

  • Objective; Antistress effect of Korean red ginseng (RG) on postmenopausal women with severe climacteric syndrome (CS) were evaluated from the viewpoint of traditional KAMPO-medicine and Western medicine. Methods; All patients with CS were treated with daily oral administration of 6g RG for 30 days. Nine patients with CS were evaluated with the use of diagnostic scores for KI-deficiency (deficiency of vital energy) and OKETSU (blood stagnation) syndrome from the viewpoint of KAMPO-medicine. In the same patients with CS, peripheral blood levels of ${\beta}$-endorphin and total plasminogen activator inhibitor-1 (t-PAI-1) were measured before and after treatment with RG. In another group, 12 patients with CS, psychological test using CMI, STAI and SDS were performed from the viewpoint of Western medicine. Stress related hormones, such as ACTH, cortisol and DHEA-S in those 12 patients with CS were also measured before and after treatment with RG. Results; KI-deficiency score and OKETSU score in patients with CS were significantly (p<0.001) higher than those in patients without CS. After treatment with RG, both scores were markedly (p<0.001) decreased compared to before treatment with RG. ${\beta}$-endorphin levels in patients with CS were significantly (p<0.05) higher than those in patients without CS. Total PAI-levels in patients with CS were increased before treatment with RG. No significant difference, however, were observed between patients with and without CS. After treatment with RG, both levels of ${\beta}$-endorphin and total PAI-1 in patients with CS were significantly (p<0.001 and p<0.05, respectively) decreased compared to before treatment with RG. CMI and STAI scores in patients with CS were significantly (p<0.05) higher than those in patients without CS. SDS scores in patients with CS were also markedly (p<0.00l) higher than in those without CS. After treatment with RG, all scores decreased within normal range. DHEA-S levels in patients with CS were about a half of those without CS. Consequently, cortisol/DHEA-S (C/D) ratio was significantly(p<0.001) higher in patients with CS than in those without CS. Although the decreased DHEA-S levels were not restored to the levels in patients without CS, the C/D ratio decreased significantly (p<0.05) after treatment with RG. Conclusion; Reinforcement of vital energy and improvement of stagnant blood circulations by oral administration of RG were elucidated from the viewpoint of traditional KAMPO-medicine. From the viewpoint of Western medicine, effect of RG on postmenopsusal women with CS seemed to be brought about in part by not only an improvement of psychoneuroendocrine dysfunctions but also an amelioration of blood coagulation systems.

A Clinical Study about Low Back Pain Patients Based on 10 Classes Lumbago in Dong-Eui-Bo-Kham (동의보감(東醫寶鑑)의 심종요통(十種腰痛)에 근거(根據)한 요통환자(腰痛患者)의 임상적(臨床的) 연구(硏究))

  • Objectives : The analysis of prognosis about lumbago classes is important for low back patients. This study was designed to show the clinical significance about lowback pain patients based on 10 classes lumbago ill Dong-Eui-Bo-Kham. Methods : To analyze classify 80 patients who had received the hospital treatment for low back pain, they were classified according to 10 classes lumbago in Dong-Eui-Bo-Kham. To estimate the efficacy of oriental medical treatment, we used Quardruple Visual Analog Scale (QVAS). Results & Conclusion :1. According To 10 classes lumbago in Dong-Eui-Bo-Kham, the largest group of lumbago was due to muscle strain in 30(37.5%) cases, the next was due to wind pathogen group in 18(22.5%) cases, due to blood stasis group in 16(20%), due to Kidney deficiency by 12(15.0%), due to retention of food group by 2(2.5%), the lumbago due to humidity pathogen group by 2(2.5%) in order. 2. By the results which puts out the statistics in lumbago classes on a objectivity treatment record, the Lumbago due to blood stasis, the lumbago due to kidney deficiency, the lumbago due to muscle strain, the lumbago due to wind pathogen, the lumbago due to retention of food, the lumbago due to humidity pathogen showed $3.6({\pm}0.68),\;2.83({\pm}0.84),\;3.13({\pm}0.78),\;2.67{\pm}(0.77),\;3.00,\;3.00$. 3. By the results which puts out the statistics in lumbago classes as a subjectivity treatment record, the Lumbago due to blood stasis, the lumbago due to Kidney deficiency, the lumbago due to muscle strain, the lumbago due to wind pathogen, the lumbago due to retention of food, the lumbago due to humidity pathogen changed from $8.19{{\pm}1.68),\;8.58{\pm}(1.08),\;7.80{\pm}(2.28),\;8.67({\pm}1.46),\;9.00({\pm}1.41),\;8.50({\pm}0.71)\;to\;2.81({\pm}1.91),\;4.17({\pm}2.52),\;2.43({\pm}1.63),\;3.00({\pm}1.85),\;2.00,\;3.00$.

A Pattern Identification Study on the Middle-Aged Women between Sasang Constitution using DSOM (한방진단시스템 DSOM을 활용한 사상체질에 따른 폐경(閉經)전후 중년여성(中年女性)의 변증연구(辨證硏究))

  • Objectives We carried out this study to know that there is any differences on the health condition of the middle aged women between Sasang coustitution. We used Diagnosis System of Oriental Medicine(DSOM) for diagnosis and determining pattern identification. Methods This research was approved by the Dongeui University Oriental Hospital Institutional Review Board (certificate no. 2011-06). From March 2012 to October 2012, we examined Kupperman's index, the MENQOL, DSOM, and Sasang Constitution of 291 women from the general population, with ages ranging from 40 to 60 years. And we compared the results statistically by the chi-square test and F-test. Results and Conclusions We excluded 4 Taeyangin to analyze because they were few, and analyzed 287 subjects which were 53 Soyangin, 131 Taeeumin, and 103 Soeumin. 1. The pathogenic factors that showed significant differences were deficiency of qi, insufficiency of Yin, heat on the frequency of the output and deficiency of qi, heat, phlegm on the mean of pathogenic factor score. Deficiency of qi was higher in Soeumin, insufficiency of Yin and heat were higher in Taeeumin, and phlegm was higher in Taeeumin and Soeumin. Overall, Soyangin tended to be lower than others on both the frequency of the output and the mean of pathogenic factor score. 2. The middle aged women with ages ranging from 40 to 60 years tended to be dryness, kidney, damp and became to be blood-deficiency, stagnation of qi, heart easily. Soyangin was more health than others, Taeeumin tended to be heat, phlegm, insufficiency of Yang, and Soeumin tended to be deficiency of qi.

Pain management in 『Treatise on Cold Damage and Miscellaneous Disease』 according to sufficiency-deficiency of fluid and humor (mainly with greater yang disease part) (진액(津液)의 유무(有無)에 따른 『상한잡병론(傷寒雜病論)』의 통증(痛症) 치법(治法)에 관하여(태양병편을 중심으로))

  • Human body fluid and humor include not only sweat, joint fluid but also every fluids, for example, blood, essence, kidney essence and marrow. Historically, in the oriental medicine, there are a lot of efforts in order to preserve fluid and humor. In "Treatise on Cold Damage and Miscellaneous Disease (傷寒雜病論)", when treating cold damage, preservation of fluid and humor is put first. I tried to find out the relation about fluid and humor and pain treatment in "Treatise on Cold Damage and Miscellaneous Disease". So, I investigated sentences related to pains in "Treatise on Cold Damage and Miscellaneous Disease" "greater yang disease part" and commentaries of several chinese medical doctors. And I divide pain treatments into four categories. (fluid and humor sufficiency-exterior pattern, fluid and humor sufficiency-interior pattern, fluid and humor deficiency-exterior pattern, fluid and humor deficiency-interior pattern) At first, when treating pains of cold damage, there are many considerations about deficiency and sufficiency of fluid and humor through pulse condition and symptoms. Second, in pain-cases of fluid and humor sufficiency, purge methods is chosen. And in pain-cases of fluid and humor deficiency, tonifying methods is chosen. Finally, one of the main objects of "Treatise on Cold Damage and Miscellaneous Disease" is preservation and supply of fluid and humor.

  • Carbamoyl phosphate synthetase I (CPS1) deficiency is a rare autosomal recessive urea cycle disorder that causes hyperammonemic crisis. CPS1 is the first enzyme encoded by the CPS1 gene, which catalyzes the first step of the urea cycle. In CPS1 deficiency, ammonia, the toxic metabolite produced by the interruption of the urea cycle, is accumulated in the blood and brain, leading to hyperammonemic encephalopathy and irreversible brain damage. Here, we report a fatal case of neonatal-onset CPS1 deficiency in a 4-day-old girl presenting with recurrent seizures, who was revealed to be homozygous for c.1529delG ($p.Gly510Alafs^*5$).

A survey for tick-borne disease agents from farm deer in the eastern area of Jeonbuk (전라북도 동부지역 사슴에서 진드기매개성 병원체에 대한 감염 실태 조사)

  • Ticks cause economic losses to the deer industry by decreasing the growth and production of the farmed animals. The mediation of ticks affects humans and animals by causing contagious disease both directly and indirectly. Blood from farmed deer from the areas near Jangsu branch was collected for screening of infectious protozoa and rickettsial disease. Seventy deer blood samples were collected from 30 different deer farms located in Jinan, Jangsu and Muju. This blood samples were used for blood slide smear examination and hematological analysis. DNA from these samples was extracted and was used for PCR analysis for detection of gene fragments of Theileria spp, Babesia spp, Anaplasma spp and Ehrlichia spp. In the blood slide smear examination and PCR analysis all samples did not show presence of protozoal and rickettsial diseases. Eight blood samples showed anemia, 1 sample showed iron deficiency and 7 samples showed regenerative anemia. Results for PCR analysis showed 2 samples were positive for T orientalis. All DNA samples were negative for Babesia spp, Anaplasma spp, and Ehrlichia spp.

The Effects of LI4, TE3, TE5 and LI11 Moxibustion on Radial Artery Blood Flow and Heart Rate Variability in Stoke Patients with Hemiplegia (합곡.중저.외관.곡지 뜸치료가 뇌졸중 편마비 환자의 환측 요골동맥 혈류 및 심박변이도에 미치는 영향)

  • Objectives: Moxibustion has been used for various post-stroke symptoms and has also been known to have effect on peripheral blood flow. This study investigated the effect of moxibustion on radial artery blood flow by Doppler ultrasound and on heart rate variability in post-stroke hemiplegia patients. Methods: Moxibustion was applied on the points of LI4, TE3, TE5 and LI11 on the affected side, and blood flow of the radial artery was measured using the Minimax-Doppler-K device. Blood flow velocity and pulsation index were analyzed before, during, and after moxibustion. Simultaneously LF, HF, and LF/HF as variables for HRV were measured by FM-150. Results: The mean value of blood flow velocity in all patients (n=23) showed significant increase between before and after moxibustion, but there was no significant difference in pulsation index and LF, HF, LF/HF ratio between before and after moxibustion. In Yin (n=9) and Yang (n=14) groups, both showed significant increase of blood flow velocity between before and after moxibustion, but there was no significant difference in pulsation index and LF, HF, LF/HF ratio between before and after moxibustion. In Deficiency (n=14) and Fullness (n=9) groups, only the Deficiency group showed significant increase of blood flow velocity between before and after moxibustion, while the Fullness group showed no significant difference in blood flow velocity between before and after moxibustion, and also there was no significant difference in pulsation index and LF, HF, LF/HF ratio between before and after moxibustion. Conclusions: This study suggests that moxibustion on LI4, TE3, TE5 and LI11 on the affected side of stroke patients increase the peripheral blood flow in the affected arm, which was most remarkable in those with Yin and Deficiency pattern.
