Change of the Maximal Isometric Contraction to the Spastic Muscle by NMES (신경근전기자극에 의한 경직근의 최대 등척성 수축력 변화)

  • The purpose of this study was compared to MTICs(maximal tolerated isometric contraction), decrement and recovery. For this, using isokinetic exercise analysis device(Biodex Medical Systems Inc., Biodex System 3PRO, USA), low rate(20 Hz) and high rate(100 Hz) NMES(neuromuscular electrical stimulation) were applied to the quadriceps muscles of fifteen patients with spastic hemiplegia caused by lesions in the central nervous system. The results were as follows: 1. It was shown to fast decrement in the middle of phase at low rate NMES and to slow decrement of MTIC response at high rate NMES(p<.01). 2. It was shown to fast recovery at high rate NMES and to slow recovery at low rate NMES in recovery tendency of MTIC(p<.01). These conclusions suggest that NMES of high rate caused to slow fatigue and fast recovery different from low rate NMES.

The Relationship between Strength Balance and Joint Position Sense Related to Ankle Joint in Healthy Women (정상 성인 여성의 발목관절에서 근력 균형과 관절위치감각의 상관관계)

  • Purpose: The purpose of this study was to determine whether there is a relationship between strength balance and joint position sense related to the ankle joint in healthy women. Methods: Twenty-six healthy women were recruited. Isokinetic strength and joint position sense (JPS) were measured using a Biodex System 4 pro Dynamometer and a Biodex Advantage Software Package. Prior to measuring the JPS and isokinetic strength, the dominant foot was determined according to the Waterloo Footedness Questionnaire. After the JPS test, isokinetic strength was evaluated in velocity $60^{\circ}s$, including practice trial ($90^{\circ}s$). Using the measured isokinetic strength, a Strength Balance Index (SBI) was calculated. Results: Relative to the SBI, the degree of imbalance was varied; but there were imbalances. For each starting position, JPS error showed no significant differences (p>0.05). The relationship between SBI and JPS was found during the inversion to eversion process, eversion to inversion, and dorsi flexion to plantar flexion. Conclusion: There are moderate to mild relationships between JPS and SBI during ankle movement. It is suggested that to prevent ankle injuries, strength balance should be considered along with the other potential factors including anatomical alignment, proprioception, and soft tissues problems.

The Effect of Muscle performance on knee flexion / extension by chin in exercise (턱 당김 운동이 무릎 굽힘폄의 근 수행력에 미치는 영향)

  • Background: The purpose of the present study is to report the effect of muscle performance on knee flexion and extension by chin in exercise. Methods: This study was Participated in 10 healthy subjects. For conducting the chin-in exercise, subjects are lay down with supine position. Using the rounded towel, subjects was performed contraction of longus colli and longus capatis muscle by push the towel. Chin-in exercise was conducted 3 times a day, 4 times a week for 4 weeks. By using the Biodex system4, We measured absolute muscle strength, relative muscle strength, total exercise quantity and average rate of production in knee flexion and extension. The data was analyzed by the repeated-measure ANOVA for comparing before, after exercise 2 weeks, after exercise 4 weeks changes of factors. Results: After chin-in exercise, there was significant difference of before, after 2 weeks and 4weeks results in absolute muscle strength, relative muscle strength, total exercise quantity, average rate of production, agonist/antagonist ratio(p<.05). Conclusion: As a results of this study, chin-in exercise may help to improve muscle ability of knee joint activation and knee joint action performance.

Effects of Repetitive Sit to Stand Training on the Knee Extensor Strength and Walking Ability in Subject with Total Knee Replacement Patients

  • Purpose: The purpose of this study was to verify the effectiveness of repetitive sit to stand training to improve knee extensor strength and walking ability of total knee replacement patients. Methods: In this study, 12 patients with total knee replacement patients were recruited from a rehabilitation hospital. They were divided into two groups: a repetitive sit to stand group (n=6) and a control group (n=6). They all received 30 minutes of continuous passive motion (CPM). After that, the repetitive sit to stand group performed repetitive sit to stand training, and the control group performed resistance exercise for 15 minutes five times a week for 2 weeks. After 2 weeks of training, knee extensor strength and spatiotemporal gait parameters were measured. Knee extensor strength was measured by Biodex system 3, walking ability was measured by Biodex gait trainer 2. Paired t test was performed to verify the difference between before and after intervention within the group, and analysis of covariance was used to verify the differences between the two groups. Results: After the training periods, the repetitive sit to stand group showed a significant improvement in knee extensor muscle strength, walking speed, step length of the operated side, and step length of the non-operated side (p<0.05). Conclusion: The results of this study showed that repetitive sit to stand training was more effective in improving knee extensor muscle strength and walking ability. Therefore, to strengthen knee extensor muscles and improve the walking ability of total knee replacement patients, it is necessary to consider repetitive sit to stand training.

The Effectiveness of the IKDC Subjective Score in Clinical Outcome Study after Anterior Cruciate Ligament Reconstruction (전방 십자 인대 재건술 후 결과 판정에 있어 IKDC 주관적 점수의 유용성)

  • Purpose: The goal is to identify the effectiveness of the IKDC subjective score in the clinical outcome study after reconstruction of the anterior cruciate ligament. Materials and Methods: Twenty-four patients after ACL reconstructions using twelve hamstring autografts and twelve Achilles allografts were evaluated. Lysholm knee score, IKDC subjective score, Knee Outcome Survey score and Tegner activity score were evaluated for the subjective outcome value. Static instability tests, Biodex dynamometer and one-legged hop for distance tests were done for the objective outcome value. Three functional performance tests (FPTs) including Carioca test, Shuttle run test and Co-contraction test were performed for deciding the final results. Each scale was compared with FPTs results and Pearson's correlation test was used to test the correlation between the parameters. Results: IKDC subjective score, single hop test, and quadriceps power in low velocity of Biodex dynamometer tests had a positive correlation with the total FPTs results. Conclusion: IKDC subjective score can be an effective test to evaluate the functional status as well as the subjective outcome after ACL reconstruction.

Study on the Smart 1RM System Development and Effect Verification for Health Improvement and Management of National Healthcare (국민 건강관리 및 체력증진을 위한 스마트 1RM 시스템 개발 및 효과 검증에 관한 연구)

  • In this study, we developed a smart 1RM system for national health management and physical fitness, which enables quantitative 1RM measurement in various types of exercise using digital pulley technology, and to test the effect on training by using it. We developed the smart 1RM system, which is composed of portable muscle strength measuring device, Bluetooth communication based mobile phone data transmission and circuit diagram, and height adjustable system body. We recruited the 30 participants with 20th aged and divided into training and non-performing groups with 15 participants randomly. The participants performed 5 sets of elbow, lumbar, knee extension / flexion 10 times using smart 1RM system and the experimental period was 3 days a week for a total of 8 weeks. The experimental results showed that the maximum strength of the elbow, lumbar, and knee joints was significantly improved before and after maximal muscle strength training in the training group. Oxygen intakes during 1RM exercise mode showed 10.91% than endurance. To verify the validity of the smart 1RM maximal strength data, the reliability was 0.895 (* p <0.00). This study can be applied to the early rehabilitation treatment of the elderly and rehabilitation patients more quantitatively using the national health care.

The Change of the Maximal Isometric Contracture of the Normal and Spastic Muscles in Voluntary Exercise (수의적 운동에 의한 정상근과 경직근의 최대 등척성 수축력 변화)

  • This study aimed at examining the maximal isometric contraction caused by voluntary exercise and at comparing its aspects of decrease and restoration in their different repeated application, as to the quadriceps muscles of thigh in the subjects composed of patients with spastic hemiplegia and normal adults. Using isokinetic exercise analyser(Biodex Medical Systems Inc., Biodex System 3PRO, U.S.A.), experiment was conducted as to the normal group composed of fifteen adults and the patient group composed of fifteen patients with spastic hemiplegia. As to each group, MVIC(maximal voluntary isometric contraction) of the quadriceps muscle of thigh caused by voluntary exercise and the aspects of decrease and restoration of the isometric contraction were examined with the method to induce isometric exercise, and their SDI(strength decrement index) and SRI(strength recovery index) were also calculated. The results can be summarized as follows: 1. As for decrease of maximal isometric contraction, both groups showed slow decrease in voluntary exercise, but the normal group showed rapid decrease later phase. 2. As for SDI, no significant differences could be observed in comparison between groups. 3. As for restoration of maximal isometric contraction, both groups showed slow restoration in voluntary exercise, but the normal group showed rapid restoration early phase. 4. As for SRI, comparison between groups showed significant differences in voluntary exercise. These results lead us to the conclusions that spastic muscle is characterized by slow decrease and restoration of MVIC in comparison with normal muscle in voluntary exercise.

Isokinetic Muscle Strength and Muscle Endurance by the Types and Size of Rotator Cuff Tear in Men

  • Background: Our study was to determine the effect on shoulder isokinetic muscle strength and muscle endurance in isolated full-thickness supraspinatus tendon tear and combined other rotator cuff tear. Methods: Total of 81 male patients (mean age $57.8{\pm}7.4$ years) who were diagnosed as a full-thickness supraspinatus tendon tear were included. They were classified into isolated or combined tear. The isokinetic muscle strength and muscle endurance were measured using the Biodex multi-joint system $PRO^{(R)}$ (Biodex Medical Systems, Shirley, NY, USA) in following movements: shoulder abduction, adduction, flexion, extension, external rotation, and internal rotation. Then, the difference in muscle function according to the type of tears were assessed. Fifty-seven patients had isolated supraspinatus tendon (mean age $56.9{\pm}7.3$ years). They were classified into either anteroposterior tear or modified mediolateral tear. The size were measured using T2-weighted magnetic resonance imaging scans in sagittal plane. Results: Between subjects categorized into the type of tear, we found significant inter-categorical differences in isokinetic muscle strength during abduction, adduction, flexion, extension, and internal rotation, and in muscle endurance during flexion, extension, and internal rotation. Anteroposterior diameter tear, we did not show significant differences in either isokinetic muscle strength or muscle endurance during any movements. However, with modified mediolateral diameter, we found significant differences with isokinetic muscle strength during adduction, and in muscle endurance the external rotation and internal rotation. Conclusions: We found that a supraspinatus tendon tear associated with more numbers of rotator cuff tears has lower isokinetic muscle strength and muscle endurance than a tear found alone.

The Effects of Vestibular Sensory Stimulation Training on Balance and Gait in the Patients with Stroke (전정감각 자극훈련이 뇌졸중 환자의 균형과 보행에 미치는 영향)

  • Purpose: The aim of this study is to examine the effects of training in vestibular sensory stimulation on balance and gait of stroke patients. Methods: Twenty patients were randomly assigned to either the experimental group (n=10) or the control group (n=10). Patients in the experimental group received rotational stimulation training, vertical-horizontal stimulation training, gait training on a flat surface with vestibular sensory stimulation, and gait training on soft ground with vestibular sensory stimulation. Patients in the control group received general treadmill gait training. The intervention was applied four times per week, 25 minutes each time, for a period of four weeks. We measured Berg Balance Scale (BBS), Biodex Balance System, Timed up to Go (TUG) test and Dynamic Gait Index (DGI) to evaluate balance and gait ability. Results: BBS differed significantly in both groups between before and after the intervention (p<0.05) and changes in BBS after the intervention differed between the two groups (p<0.05). According to the Biodex Balance System test result, only the experimental group showed significant changes in balance in the conditions of static eyes open (SEC), dynamic eyes open (DEO), and dynamic eyes closed (DEC) (p<0.05). TUG test results differed significantly between prior to and after the training in both the experimental group and the control group (p<0.05) and changes in TUG after the intervention differed significantly between the two groups (p<0.05). DGI results showed significant change after the intervention in the experimental group only (P<0.05). Conclusion: Training in vestibular sensory stimulation was effective in improving static-dynamic balance and gait ability of stroke patients.

Effects of Game-based Visual Feedback Training on Postural Balance Control (게임기반의 시각 피드백 훈련이 자세균형 조절에 미치는 영향)

  • In this study, we analyzed the effects of game-based visual feedback training on postural balance control in young adults. We provided postural balance training for four weeks in fifth minute a day and three days a week using training system of postural balance based forceplate. We evaluated the ability of postural balance using balance SD(Biodex, medicalscience Inc., USA) for the validation of game contents based visual feedback training program. The results showed that postural stability and limits of stability were improved significantly before and after the training(p<0.05). Our study indicates that postural balance training of visual feedback based game could be adapted for improving postural balance. Also, for application of this game-based visual feedback training in older adults, we could develope of various game contents for disease types and conduct quantitative analysis and data collection of postural balance in the aged.
