한국 베들링턴 테리어에서 구리중독증을 유발하는 COMMD1 유전자의 exon 2 결손변이 (The Exon 2 Deletion of the COMMD1 Causing Copper Toxicosis in Bedlington Terriers in Korea)

  • 개의 10번 염색체에 존재하는 Copper metabolism domain containing 1 (COMMD1) 유전자는 체내 구리 대사를 조절하는 COMMD1 단백질을 합성한다. COMMD1 유전자의 exon 2 결손변이는 단백질의 결핍을 유발하여 베들링턴 테리어 견종에서 상염색체 열성 유전질환인 구리 중독증을 일으킨다. 본 증에 이환된 개체는 담즙을 통한 구리의 배설이 저해되어 간 내에 구리가 축적된다. 본 연구에서는 국내 베들링턴 테리어 257두(수컷 109두, 암컷 148두) 혈액 시료를 사용하여 genomic DNA를 추출하였다. 유전자 결손변이의 분자생물학적 진단을 위해 다중 중합효소 연쇄반응법(multiplex PCR)을 이용하여 COMMD1 유전자의 exon 2 결손 발생 및 그 빈도를 조사하였다. 베들링턴 테리어 257두에서, 정상유전자 동협접합자가 131두(51%), 이형접합자가 108두(42%), 변이유전자 동형접합자가 18두(7%)로 확인되었다. 본 연구를 통해 한국 베들링턴 테리어 개체군의 유전변이 발생 및 그 빈도를 확인하였고, 이는 국내 베들링턴 테리어 개체군의 유전자 선택적 교배계획 설립 및 변이 유전자 확산을 예방하기 위한 기초 자료로서 의의가 있다.

간흡충증의 임상 양상 (Clinical Features of Clonorchiasis)

  • 간흡충은 담도계를 침범하여 황달, 담도염 및 담도계암 등의 합병증을 유발하는 기생충으로 예방과 치료가 가능함에도 불구하고 감염의 빈도가 줄지 않고 있다. 한편 간흡충에 감염이 되더라도 증상이 없이 발견되기도 한다. 평균 3.3%의 감염률을 보였으며, 대상 환자 1,063명의 남녀 성비는 3.6:1로 남자가 높았으며, 남녀 모두 40~60대에서 가장 높은 빈도를 차지하였다. 내원 당시 환자들은 증상이 없는 경우가 76.5%로 가장 많았으며, 증상 중에는 우상복부 동통이 전체의 7.6%, 상복부 동통이 7.2%를 차지하였다. ALP는 85.2%에서, GGT는 56.0%에서 정상범위를 보였으며 그리고 호산구수의 증가는 전체의 66.1%를 차지하였다. 복부초음파 소견은 정상적인 경우가 전체의 46.5 %, 지방간은 22.9% 그리고 coarse parenchymal echogenicity는 7.0%를 차지하였으며 ERCP 검사에서는 전체의 32.5%에서 말단부의 담도 확장 소견을 보였다. 간흡충과 동반된 간담도계 질환으로는 지방간이 21.0%로 가장 높은 빈도를 차지했고, 다음으로 담낭 결석이 4.7%로 빈도가 높았으며, 또한 담도계의 종양은 전체 환자의 3.5%를 차지하였다. 간흡충증은 여전히 우리나라에서 빈번히 발견되고 있으며 자각증상이 없이 우연히 발견되는 경우가 많다. 증상이 없더라도 민물고기 생식을 한 적이 있거나, 간검사의 이상, 호산구수 상승 또는 영상학적 검사에서 기생충의 감염이 의심되는 경우에는 간흡충증의 가능성을 염두에 두어야 한다.

The Protective Effects of Different Mycotoxin Adsorbents against Blood and Liver Pathological Changes Induced by Mold-contaminated Feed in Broilers

  • An experiment was conducted to determine the effects of different mycotoxin adsorbents including esterified glucomannan (EGM), hydrated sodium calcium aluminosilicate (HSCAS) and compound mycotoxin adsorbent (CMA) on performance, blood parameters, and liver pathological changes in broilers fed mold-contaminated feed. Two hundred and forty 10-day-old broilers were randomly assigned to one of the five dietary treatments including: i) control diet; ii) mold-contaminated diet; iii) moldcontaminated diet+0.05% EGM; iv) mold-contaminated diet+0.2% HSCAS; v) mold-contaminated diet+0.1% CMA. At 35-days-old, blood and liver tissue samples were collected for analysis. 0.1% CMA improved ADG and ADFI during 10-42 d compared to the moldcontaminated group (p<0.05). The mold-contaminated diet increased total white blood cell (WBC) number, haemoglobin (Hgb) concentration, hematocrit (Hct) level, serum aspartate aminotransferase (AST) and ${\gamma}$-glutamyl transferase (GGT) activities, and decreased red blood cell (RBC) number and serum globulin (GLB) and urea nitrogen (BUN) concentrations (p<0.05). The three mycotoxin adsorbents alleviated the alteration of RBC, WBC, Hgb and AST caused by the mold-contaminated diet. Furthermore, 0.1% CMA increased GLB concentration and decreased Hct level and GGT activity (p<0.05). Liver superoxide dismutase (SOD) activity was reduced, and myeloperoxidase (MPO) activity was increased by the mold-contaminated diet (p<0.05). Both EGM and HSCAS prevented the increase of MPO activity (p<0.05). Liver lesion, including severe vacuolar degeneration of hepatocytes, was observed in chicks fed the mold-contaminated diet. 0.05% EGM prevented these effects except for biliary hyperplasia and mild vacuolar degeneration. 0.2% HSCAS showed medium vacuolar degeneration of hepatocytes. Liver of broilers fed 0.1% CMA revealed a mild vacuolar degeneration. These results indicate that a mold-contaminated diet results in adverse effects on blood parameters and liver morphology. 0.05% EGM and 0.2% HSCAS partially alleviated the adverse effects. However, 0.1% CMA almost completely ameliorated the adverse effects.

  • Cholangiocarcinoma (CCA) is a serious problem in Thailand, particularly in the northeastern region. Active surveillance in rural communities with an appropriat low-cost screening tool is required to facilitate early detection. Therefore, this study aimed to investigate the population at risk of CCA in Bua Yai district, Nakhon Ratchasima province, Northeastern Thailand using the Korat-CCA verbal screening test (KCVST) during June to October 2015. Reliability of KCVST demonstrated a Cronbach alpha coefficient=0.75 Stepwise-multiple regression showed that alcohol consumption was important for CCA screened, followed by agriculture and pesticide use, under-cooked cyprinoid fish consumption, praziquantel use, naïve northeastern people, opisthorchiasis, family relatives with CCA, and cholangitis or cholecystitis or gallstones, respectively. Population at risk for CCA was classified to low risk (63.4%), moderate risk (33.7%), and high risk (1.32%) for CCA. When CCA was screened using ultrasonography, 4 of 32 high risk participants had an abnormal biliary tract with dilated bile ducts. This study indicates that KCVST is a potential useful too which decrease the cost of large scale CCA screening.

간이식에서 간동맥 문합술 (Hepatic artery anastomosis in liver transplantation)

  • Purpose: Liver transplantation is considered as the treatment of choice in many acute and chronic liver diseases, and it is becoming more common. Since successful microscopic anastomosis of hepatic artery is a crucial requirement of successful liver transplantation, we studied and analyzed the result of hepatic artery anastomosis of liver transplantation in our liver transplantation center. Methods: 145 liver transplantations were performed between February 2005 and May 2008. Male to female ratio of the liver transplantation recipients was 3.4 : 1. Anastomosis of portal vein, hepatic vein and biliary tract was performed by the general surgeon, and anastomosis of hepatic artery was performed by the plastic surgeon under the loupe or microscopic vision. After the hepatic artery was reconstructed, anastomosed site status and flow were checked with Doppler ultrasonography intraoperatively and with contrast enhanced CT or angiography postoperatively if necessary. Results: Out of 145 liver transplantations, cadaveric liver donor was used 37 cases and living donor liver transplantation was performed 108 cases including the 2 dual donor liver transplantations. As for the baseline diseases that resulted in the liver transplantation, there were 57 cases of liver cirrhosis and hepatocellular carcinoma due to hepatitis B, taking up the greatest proportion. Single donor hepatic artery was used in 114 cases, and mean artery diameter was 2.92 mm and mean artery length was 24.25 mm. Hepatic artery was used as the recipient artery in every case except the 8 cases in which gastroepiploic artery was used as alternative. Out of 145 cases of hepatic artery anastomosis, 3 cases resulted in the thrombosis of the hepatic artery, requiring thrombectomy and re - anastomosis. In all 3 cases, thrombosis was found in left hepatic artery and there was no past history of hepatic artery chemoembolization. Conclusion: Incidence of hepatic artery thrombosis after the anastomosis of hepatic artery during liver transplantation was 2.1%, which is considered sufficiently low.

제주지역 노인의 담낭결석 유병률 및 위험요인 (Gallstones Risk Factor and Prevalence in the Elderly in Jeju Island)

  • 배경/목적 : 담낭결석은 담도계의 가장 흔한 질환이며 급성복통을 유발하는 질환이다. 담낭결석으로 인한 담낭염이 자주 발생하고 담낭암의 발생 가능성이 높아진다. 본 연구의 목적은 지역사회의 담낭결석 유병률과 위험요인을 알아보고자 한다. 대상 및 방법 : 2008년 7월 한 달 동안 제주도 10곳에서 60세 이상 노인의 남자 297명, 여자 529명 총 826명을 대상으로 하였다. 대상자는 신장, 체중을 측정하고 혈액검사와 상복부초음파 검사를 시행하였다. 통계학적 검증은 T-test, chi-square test로 검정하여 p < 0.05 이하일 경우 유의한 결과로 판정하였다. 결 과 : 담낭결석 유병률은 5.9%(49/826명)이었다. 남성은 6.1%, 여성은 5.8%로 남성이 여성보다 1.03배 높게 나타났다. 연령별로는 $70{\sim}74$세에서 8.0%로 가장 높은 유병률을 보였다. 담낭결석 집단에서 과체중 이상이 69.3%로 높게 나타났으며, 집단 간의 유의한 차이를 보였다(p < 0.047). 결 론 : 연령의 증가에 따라 담낭결석의 발생률이 높게 나타났다. 비만은 담낭결석 위험요인으로 나타났으며 적절한 체중 관리와 식이조절을 한다면 어느 정도 담낭결석 발생을 줄일 수 있을 것으로 생각된다.

말기 암환자의 임종 전 48시간 동안의 임상증상 (Symptom Experience in Terminal Cancer Patients during the Last 48 Hours of Life)

  • 목적: 말기 암환자의 여명을 예측하는 것은 환자와 가족들에게 '평안한 임종'을 맞기 위한 준비를 할 수 있도록 하고, 의료진에게는 적극적 증상완화와 전인간적 돌봄을 가능하게 할 수 있다. 이에 본 연구는 말기 암환자에서 임종 전 48시간 동안의 임상증상을 파악하고 임종을 예측하는 임상증상을 확인하여 환자 가족 및 의료진에게 이 시기에 나타나는 변화들을 이해하여 준비된 평안한 임종을 맞는데 도움이 되고자 한다. 방법: 2003년 7월부터 2006년 3월까지 한 대학병원에 입원하여 사망한 말기 암환자 89명을 대상으로 하여 임종 전 48시간 동안의 임상증상을 조사하였고 입원 시, 임종 48시간-24시간전, 임종 24시간전$\sim$임종시까지 시간대별 증상의 빈도 변화를 분석하였다. 결과: 전체 대상 환자의 중앙 연령은 62세(범위, $16{\sim}97$세)였다. 임종 48시간전 임상증상의 빈도는 의식혼탁(57%)이 가장 흔하였으며 이외 통증(30%), 발열(22%), 호흡곤란(19%)의 순이었다. 원발암에 따른 임종 48시간전 임상증상도 유사한 빈도를 보였으나 담도계암환자에서 발열의 빈도가 다른 암종에 비해 유의하게 높았다(P=0.012). 임종이 임박할수록 입원 시에 호소하던 전신 쇠약감과 식욕부진은 감소한 반면 의식혼탁은 유의하게 증가하였다(P<0.001). 결론: 말기 암환자에서 의식혼탁은 임종이 임박했음을 나타내는 가장 중요한 임상 지표로 생각된다.

총담관낭종의 $^{99m}Tc$-DISIDA 간담도 스캔 (Scintigraphic Features of Choledochal Cyst Using Technetium-99m-DISIDA Hepatobiliary Scan)

  • In order to evaluate the scintigraphic features of choledochal cyst and these diagnostic value, authors investigated the findings of fourteen patients with choledochal cyst undergone hepatobiliary scan with $^{99m}Tc$-DISIDA before surgery. Five cases demonstrated the decreased hepatic uptake at 5-minute image of which four cases revealed severe jaundice. Seven cases demonstrated visualization of the cystic dilated common bile duct within 1 hour after injection. Two cases showed the cyst activity between 1 and 12 hours, but the cyst activity was not visible in five cases. Nonvisualization of the gall bladder was noted in ten cases, while four cases demonstrated visualization of the gall bladder within 1 hour. The time of visualization of gut activity was variably delayed. The intestinal activity was found in three cases within 1 hour and appeared in three cases between 1 and 2 hours and eight cases showed no visible gut activity. In four cases, intrahepatic ductal prominence was visible on the scintigram. Seven cases showed early and persistent accumulation of tracer in the common bile duct. Three cases showed persistent photon-deficient area in the gall bladder region. Two cases showed early photon-deficient area around gall bladder region with progressive accumulation of tracer in the same region. Two cases showed no evidence of activity in the biliary tract but noted late excretion into the small intestine. We concluded that hepatobiliary scan using $^{99m}Tc$-DISIDA is a noninvasive test useful in the evaluation and the diagnosis of choledochal cyst.

흰쥐에서 트리플루살의 위장관 및 간 초회통과효과 (Gastrointestinal and Hepatic First-pass Effects of Triflusal in Rats)

  • In order to elucidate the influence of intestinal and hepatic first-pass effect on the pharmacokinetics of triflusal, the biotransformation of triflusal in the gastrointestinal tract and liver was designed. Moreover, we tried to establish an HPLC method applicable for bioassay and available to pharmacokinetics, not only with the simultaneous determination of triflusal and its active metabolite, 2-hydroxy-4-trifluoromethyl benzoic acid (HTB), but also with improving sensitivity. After the administration of triflusal (10 mg/kg) and HTB (10 mg/kg) into femoral vein, portal vein (only triflusal) and oral route (only triflusal), pharmacokinetic parameters were investigated from the plasma concentration-time profiles of triflusal and HTB in rats. An HPLC method was developed for the simultaneous determination of triflusal and HTB in rat plasma, urine and bile. The HPLC analysis was carried out using a C18 column and acetonitrile-methanol-water (25:10:65, v/v/v) as the mobile phase and UV detection at 234 nm. Furosemide was used as the internal standard. The calibration curves were linear over the concentration range $0.05-5.0\;{\mu}g/ml$ for triflusal and $0.2-200.0\;{\mu}g/ml$ for HTB with correlation coefficients greater than 0.999 and with intra-day or inter-day coefficients of variation not exceeding 10.0%. This assay procedure was applied to the study of metabolite pharmacokinetics of triflusal and HTB in rats. It was supposed that triflusal was almost metabolized in vivo because urinary and biliary excreted amounts of triflusal could be ignored as it was lower than 1.2% of the administered dose. According to the gastrointestinal and hepatic biotransformation pathways of triflusal, it was found that triflusal was hydrolyzed by about 5% in intestine and metabolized by about 53% in liver, and that the bioavailability of triflusal after oral administration of triflusal was 0.44, and also that the fraction of total elimination rate of triflusal which formed HTB in liver $(F_{mi},\;%)$ was about 98%. These results showed that triflusal was almost metabolized in liver, and the total elimination of triflusal in the body was dependent to the formation rate of HTB from triflusal in liver.

췌담관질환에서 담관 솔질세포검사의 유용성 - 통상도말과 $MonoPrep2^{TM}$ 액상세포검사의 전향적 비교연구 - (Utility of Bile Duct Brush Cytology in Pancreaticobiliary Diseases - Prospective Comparative Study of Conventional Smear and $MonoPrep2^{TM}$ Liquid Based Cytology -)

  • Bile duct brush cytology has been employed as a diagnostic tool for the evaluation of pancreatic and biliary tract strictures. The specificity of this method is high however, its sensitivity is quite low. A recent study employing liquid based cytology (LBC) reported results comparable to those achieved via conventional cytology. Therefore, we have attempted to prospectively evaluate the diagnostic utility of bile duct brush cytology in pancreaticobiliary diseases. A total of 46 cases with bile duct stricture were enrolled including 11 cases of benign stricture, 29 cases of bile duct carcinoma, 3 cases of gallbladder cancer, and 3 cases of pancreatic cancer. Both conventional smear and LBC using $MonoPrep2^{TM}$ system were conducted in each case. The cytological diagnosis of each case was classed into the following categories; benign, suspicious for malignancy, and malignancy. The diagnostic accuracy of both cytologic methods was investigated. LBC evidenced a high rate of material insufficiency (13/46), which was attributed to low cellularity. The kappa index of both cytological methods was 0.508. Cytological and tissue diagnoses were correlated in 25 cases conducted from biopsy or operation. The sensitivity, specificity, positive predictive value, and negative predictive value were 41.2% (7/17), 100% (8/8), 100% (7/7), and 44.4% (10/18) in conventional smear; 58.8% (10/17), 87.5% (7/8), 90.9% (10/11), and 50.0% (7/14) in LBC; and 94.1% (16/17), 87.5% (7/8), 94.1% (16/17), and 87.5% (7/8) in any one of both cytological methods, respectively. Based on these results, the sensitivity of LBC was found to be superior to that of conventional smear and we were able to obtain higher positive predictive value upto 94.1% by simultaneously conducting both cytologic methods.
