Effect of Analytical Method on the Pharmacokinetic Evaluation of Bromosulfophthalein: Comparison of HPLC and UV Spectroscopy Method (Bromosulfophthalein의 체내동태 평가에 미치는 분석법의 영향: HPLC 법과 UV 흡광광도법의 비교)

  • The aim of this study was to evaluate the difference of analytical methods for the pharmacokinetic study of bromosulfophthalein (BSP), an indicator of hepatobiliary function. The plasma and bile concentrations of BSP after intravenous administration were measured according to custom UV spectroscopy and HPLC, respectively. Plasma concentration of BSP measured by UV spectroscopy was similar to that measured by HPLC. There was no significant difference in the distribution volume, total body clearance, area under the curve and mean residence time of BSP between different analytical method groups. However, bile concentration of BSP measured by UV spectroscopy was overestimated compared with concentration measured by HPLC method. Biliary clearance of BSP obtained from UV spectroscopy method was almost 3 times higher than that obtained from HPLC method. Thus, a feasibility of UV spectroscopy method for high throughput pharmacokinetic evaluation of BSP was limited to the study based on the plasma concentration of BSP, not bile concentration.

Antifibrotic Effects of Oriental Herbs Extraction on Liver Cirrhosis (한약재(자하거, 백출, 차전자) 추출물의 간경화 억제효능에 관한 연구)

  • Hepatic cirrhosis is an important feature of chronic liver disease. Liver cirrhosis is characterized by hyperaccumulation of fibrous tissue components and is commonly observed in latter or terminal states of chronic hepatic disease. The antifibrotic effects on liver cirrhosis by oriental herbs extraction material were examined in bile duct ligated rats. Oriental herbs extraction (0.99 mg/kg rat weight/day) was administrated to cirrohotic rats for 4 weeks. Liver collagen content of bile duct ligated rats was significantly increased. And liver histology showed collagen fiber deposition was increased as well as the normal architecture was lost with large zone of necrosis being observed. Herbs extraction administrated rats showed significantly decreased liver collagen content, accumulation of collagen fiber in histological analysis, and biochemical markers of hepatic diseases. Those results demonstrate the usefulness of herbs extraction materials as an antifibrotic agent for liver cirrhosis.

Factors Affecting Adhesion of Lactic Acid Bacteria to Caco-2 Cells and Inhibitory Effect on Infection of Salmonella Typhimurium

  • In this study, seven strains isolated from mustard leaf kimchi were screened for their tolerance to simulated gastric and bile juices, the adhesive properties to Caco-2 cells, and the inhibition ability of Salmonella Typhimurium ATCC 29631 adhesion. Lactobacillus acidophilus GK20, Lactobacillus paracasei GK74, and Lactobacillus plantarum GK81, which were resistant to bile as well as gastric juices, possessed high bile-salt hydrolase (BSH) activity towards both sodium glycocholate and sodium taurocholate. The strongest in vitro adherence of $53.96{\pm}4.49%$ was exhibited by L. plantarum GK81 followed by L. acidophilus GK20 with adhesion levels of $40.72{\pm}9.46%$. The adhesion of these strains was significantly (p < 0.05) reduced after exposure to pepsin and heating for 30 min at $80^{\circ}C$. Addition of $Ca^{2+}$ led to a significant (p < 0.05) increase of the adhesion of L. acidophilus GK20, but the adhesion ability of L. plantarum GK81 was not different from the control by the addition of calcium. In the competition and exclusion experiment, the adhesion inhibition of S. Typhimurium by L. plantarum GK81 strain was much higher than the other strains. Moreover, the exclusion inhibition of S. Typhimurium by L. acidophilus GK20 was considerably high, although the inhibition activity of this strain was lower than L. plantarum GK81.

Identification of parasite DNA in common bile duct stones by PCR and DNA sequencing

  • We attempted to identify parasite DNA in the biliary stones of humans via PCR and DNA sequencing. Genomic DNA was isolated from each of 15 common bile duct (CBD) stones and 5 gallbladder (GB) stones. The patients who had the CBD stones suffered from cholangitis, and the patients with GB stones showed acute cholecystitis, respectively. The 28S and 18S rDNA genes were amplified successfully from 3 and/or 1 common bile duct stone samples, and then cloned and sequenced. The 28S and 18S rDNA sequences were highly conserved among isolates. Identity of the obtained 28S D1 rDNA with that of Clonorchis sinensis was higher than 97.6%, and identity of the 18S rDNA with that of other Ascarididae was 97.9%. Almost no intra-specific variations were detected in the 28S and 18S rDNA with the exception of a few nucleotide variations, i.e., substitution and deletion. These findings suggest that C. sinensis and Ascaris lumbricoides may be related with the biliary stoneformation and development.

  • Alagille syndrome is characterized by paucity of interlobular bile ducts, chronic cholestasis, characteristic facial abnormalities, cardiovascular abnormalities, posterior embryotoxon, vertebral arch defects, skeletal abnormalities, and glomerular renal involvement. We experienced a case of Alagille syndrome in a 10 month-old male presenting with jaundice. He had chronic cholestasis, characteristic face, cardiovascular abnormalities (aortic stenosis, dextrocardia, double chamber of left ventricle), and situs inversus. Histological examination of liver biopsy specimen revealed paucity of interlobular bile ducts with septal fibrosis, cirrhotic transformation and severe cholestasis. He underwent liver transplantation, but died of cardiopulmonary arrest associated with cardiac anomaly.

Effects of Spice Added Natto Supplementation on the Lipid Metabolism in Rats (향미성 Natto의 섭취가 흰쥐의 지질 대사에 미치는 영향)

  • The purpose of this study was to investigate the effects of Natto supplementation on lipid metabolism in rats. Male rats were fed on casein(CON group), steamed soybean(SS group), conventional Natto(N-1 group), garlic(2%) added Natto(N-2 group), red pepper oleoresin(0.2%) added Natto(N-3 group) and galic(2%) and red pepper oleoresin(0.2%) added Natto(N-4 group) as a protein source for 4 weeks. Serum and liver cholesterol levels were significantly lower in the experimental groups than in the control group. Especially, dietary N-2, N-3 and N-4, compared with CON, reduced markedly the concentration of serum cholesterol. The concentration of serum HDL-cholesterol was higher markedly in the N-4 group than in the control group. The concentration of serum triglyceride was significantly lower in the N-1, N-2, N-3 and N-4 groups than in the control group. Garlic and red pepper added Natto fed groups(N-2, N-3, N-4) showed more excreion of fecal bile acid when compared with those values of casein fed group. The results indicate that spice added Natto supplementation may have more beneficial roles in lipid metabolism through reduced cholesterol levels and increased fecal bile acid excretion.

Effect of DIS Process on in vitro Physiological Functionality of Aloe vera Gel (알로에 겔의 in vitro 생리기능 특성에 미치는 삼투탈수공정의 영향)

  • In vitro physiological functions such as jack bean (Canavalia ensiformis) urease inhibitory activity and retarding effect of glucose/bile acid of Aloe vera gel concentrated by the optimized DIS (Dewatering Impregnation & Soaking) process conditions were examined. Urease inhibitory activity of DIS aloes ranged from 84.6 to 94.4%, which was similar to or higher than 86.3% of fresh aloe. Also, urease inhibitory activity of DIS aloes was maintained at initial levels after heat treatment (90$^{\circ}C$, 10 min.) and drying treatment (freeze or hot air drying). Urease inhibition pattern from Lineweaver-Burk plot indicated general non-competitive inhibition, and inhibition constants ($K_{IE}$ and $K_{IES}$) of DIS aloes were 41-149 and 87-163 $\mu$L/mL, respectively. DIS(glucose) and DIS(polyethylene glycol) exhibited the highest retarding effect of glucose and bile acid. Their retarding effects were about 1.6 and 1.8 folds higher than that of fresh aloe after 0.5 and 1 hr of the dialysis, respectively. Conclusively, the above in vitro physiological functions of Aloe vera gel concentrated by DIS process suggested that aloe products treated with DIS would have the potential benefits for protection against Helicobacter pylori and reduction of blood glucose and cholesterol levels.

Isolation of the Cholesterol-Assimilating Strain Pediococcus acidilactici LRCC5307 and Production of Low-Cholesterol Butter

  • The objective of the present study was to evaluate the cholesterol-assimilation ability of lactic acid bacteria (LAB), which were isolated from kimchi, a Korean traditional fermented cabbage. The isolated strain, using modified MRS medium, showed 30.5% cholesterol assimilation activity and was named Pediococcus acidilactici LRCC5307. Types and concentrations of bile were investigated for their effects on increasing the cholesterol assimilation ability of the LRCC5307 strain, a 74.5% decrease in cholesterol was observed when 0.2% bile salts were added. In addition, the manufacture of low-cholesterol butter using LRCC5307 was examined. After fermentation, LRCC5307 with butter showed 8.74 Log CFU/g viable cells, pH 5.43, and a 11% decrease in cholesterol. These results suggest that LRCC5307 could help in the production of healthier butter by decreasing cholesterol and including living LAB.

A Review of the Techniques, Current Status and Learning Curves of Laparoscopic Bile Duct Exploration

  • Laparoscopic cholecystectomy is of one the most common general surgical operations performed today. Concomitant choledocholithiasis occurs in roughly 10-20% of patients with symptomatic gallstones. Laparoscopic bile duct exploration (LBDE) offers a single-stage minimally-invasive solution to the management of choledocholithiasis. LBDE may be performed either via the transcystic route or via laparoscopic choledochotomy. A number of strategies to improve success are available to the surgeon to help in the problem of complicated choledocholithiasis, these range from simple maneuvers to the use of laser or mechanical lithotriptors. With the advances in laparoscopic surgery, it is also possible to handle complex surgical conditions such as Mirizzi syndrome or recurrent pyogenic cholangitis laparoscopically, even though these have yet to be accepted as standard of care. Following laparoscopic choledochotomy, options for closure include: primary closure, closure over a T-tube, and closure over an endobiliary stent. T-tube placement has been associated with increased operating time and hospital length of stay compared to primary closure, with no significant differences in morbidity. Based on the available literature, LBDE appears comparable to ERCP with regards to procedural efficacy and morbidity. LBDE remains relevant to the general surgeon and is best viewed as being complementary to endoscopic therapy in the management of choledocholithiasis.

A Case Report of Korean Medicine Treatment for a Patient with Acute Cholangitis Accompanied by Common Bile Duct Stone (총담관결석을 동반한 급성 담관염 환자의 한방치험 1례)

  • Objective: This case report outlines an investigation into the efficacy of Korean medicine treatment in a patient who presented with acute cholangitis accompanying common bile duct stones. The patient underwent percutaneous transhepatic gallbladder drainage and subsequently experienced resolution of the common bile duct stones. Methods: The patient was treated with Shihosogan-tang, general acupuncture, ear acupuncture, moxibustion, and cupping therapy. Changes in symptoms were evaluated using a self-reported numerical rating scale (NRS) score and a visual analog scale (VAS) score each morning. Laboratory tests were conducted to examine serum amylase, serum lipase, and liver function. Results: After 29 days, the NRS scores for indigestion and fatigue and the VAS score for abdominal pain all decreased. Additionally, the laboratory test results showed improvement. Discussion: The results suggest that Korean medicine could be effective in treating symptoms of acute cholangitis. However, further research is necessary.
