Intervention Mapping 설계를 통한 중학생 대상 흡연음주예방 교육프로그램 개발 (Development of a Smoking and Drinking Prevention Program for Adolescents using Intervention Mapping)

  • Objectives: We describe the development of a smoking and drinking prevention program for adolescents, using intervention mapping. Methods: The study sample consisted of 1,000 high school second-grade students from 6 high schools in Seoul. The PRECEDE model was applied for the needs assessment. We carried out a social diagnosis by assessing the factors such as the quality of life, happiness level, and satisfaction with school life; an epidemiological diagnosis on the perceived health status, stress levels, and priority of health issues; a behavioral diagnosis on the smoking and drinking rate and the intention to smoke and drink; and an educational diagnosis on knowledge, beliefs, attitudes, self-efficacy, outcome expectations, social norms and life skills. Results: The development process included a needs assessment, identifying factors that influence smoking and drinking among adolescents. Intention, knowledge, perceived norms, perceived benefit, perceived cost, perceived susceptibility, self-efficacy, and life skills were identified as determinants. Three performance objectives were formulated to describe what an individual needs to do in order to avoid smoking and drinking. Subsequently, we constructed an intervention matrix by crossing the performance objectives with the selected determinants. Each cell describes the learning objectives of the smoking and drinking prevention program. The program used methods from the transtheoretical model, such as consciousness raising, outcome expectations, self-reevaluation, self-liberation, counterconditioning, environmental reevaluation, and stimulus control. The program deals with the effects of smoking and drinking, self-improvement, decision making, understanding advertisements, communication skills, social relationships, and assertiveness. Conclusions: By using the process of intervention mapping, the program developer was able to ensure a systematical incorporation of empirical and new data and theories to guide the intervention design. Programs targeting other health-related behavior and other methods or strategies can also be developed using this intervention mapping process.

  • 모바일 기기의 확산과 더불어 최근 모바일 애플리케이션 시장은 괄목할 만한 성장세를 보이고 있지만 다른 디지털 컨텐츠와 마찬가지로 불법복제의 폐해가 심각하다. 본 연구는 과거 PC환경에서의 불법행위에 대한 태도와 행위의도에 영향을 미치는 것으로 나타난 요인들이 새로운 컴퓨팅환경, 즉 모바일 기기에서도 그 영향력을 유지하는지 실증적으로 확인하고자 한다. 윤리적 의사결정 이론들을 바탕으로 설정된 연구모형은 대학생들을 대상으로 실시한 설문조사를 통해 수집한 자료를 구조방정식모형을 이용하여 실증적으로 검증하였다. PC환경에서의 불법행위 습관은 스마트폰 앱 불법복제에 대한 태도와 불법행위 의도에 영향을 미치는 것으로 나타났으며 PC소프트웨어 불법복제에 대한 태도는 스마트폰 앱의 불법적 사용에 대한 태도에 유의한 영향을 미쳤다. 불법행위에 따른 법적, 성능적 위험성에 대한 의식이 태도에 미치는 영향과 PC환경에서 불법행위 능력에 대한 자각이 불법행위 의도에 미치는 영향은 유의하지 않았다.

  • `거짓말' 은 인간사회에서 옳지 못한 행동으로 비난을 받아왔으나 동시에 일상생활에서는 대인관계를 지속하고 사회에서 살아남기 위한 필수적인 기술로 자리매김해 왔다. 본 연구에서는 거짓말에 대한 이런 이중적인 모습이 '외형적 태도' 와 '암묵적 태도' 라는 두 가지 다른 근원에서 비롯된 것으로 보았다. 또한 일상생활에서 빈번하게 일어나는 순간적인 거짓말은 외형적 태도보다는 암묵적 태도와 더 큰 관련이 있을 것으로 가정하였다. 이를 확인하기 위해 거짓말에 대한 외형적, 암묵적 태도와 실제 거짓 행동을 측정하였으며 외형적 태도는 자기보고식 설문지로, 암묵적 태도는 암묵연합검사(IAT)로 측정하였다. 또 실제 거짓행동은 거짓말을 하면 이득을 얻을 수 있는 상황을 조성, 실험참가자들이 이를 얻기 위해 순간적으로 거짓말을 하는지를 관찰하여 평가하였다. 그 결과, 실제 거짓말을 한 사람들은 외형적 태도상에서는 거짓말을 하지 않은 사람들과 차이가 없었으나 암묵적으로는 거짓말에 대해 더욱 긍정적인 태도를 가기고 있는 것으로 나타났다.

Typology of Lao Vernacular Dwellings and Settlements in Context of Cultural and Physical Environment

  • A typology study is a comparative study of the physical characteristics of the built environment divided into distinct types such as architecture, culture, and environment. Lao vernacular dwellings were initially created based on the Lao people's behavior in terms of beliefs, traditional culture, lifestyle, and local wisdom with regard to the environment, climate, geography, and materials. The main research method used in this study is a comparative case study of three ethnic dwellings. The multiple data collection tools employed included second source data and primary data to analyze the findings of the unique characteristic typology of Lao vernacular dwellings. The objective of this paper is to compare the cultural and physical contexts, the patterns and origins of settlement, the architecture, the cultures, and the local indigenous knowledge related to the dwellings and settlement of the three main ethnic groups. These three groups are rich in tradition and their documentation is therefore essential for the intellectual history of the society of Lao; this research could therefore aid in enhancing our understanding of the three typologies of Lao vernacular architecture and settlement within the historical and sociocultural contexts of each ethnicity to convey an understanding of Lao vernacular dwellings by analyzing their different types to help identify the differences and similarities among architectural artifacts by recognizing the invisible connections between them, whereby the figurative ornamentation is based on the relationship between man and nature. This study also provides a glimpse of the living culture and characteristic features of Lao vernacular architecture.

Factors Affecting Preferences of Iranian Women for Breast Cancer Screening Based on Marketing Mix Components

  • Background: According to recent statistics, the breast cancer rate is growing fast in developing countries. In North West of Iran, the incidence of breast cancer after esophageal and gastric cancers has the highest rate. Previous studies have also indicated that women in this region show reluctance to do breast cancer screening. There is a great need for change to promote breast cancer screening among women. Social marketing is a discipline that uses the systematic application of commercial marketing techniques to promote the adoption of behavior by the target audience. Materials and Methods: In the present qualitative study, thirty-two women with breast cancer were interviewed about their experiences of breast cancer screening. A semi-structured interview guide was designed to elicit information specific to the 4 P's in social marketing. Results: Three main categories emerged from the analysis: price, service and promotion. Subcategories related to these main categories included factors effective in increasing and decreasing cost of screening, current and desirable features of screening services, and weakness of promotion. Conclusions: Screening programs should be designed to be of low cost, to meet patients' needs and should be provided in suitable places. Furthermore, it is essential that the cultural beliefs of society be improved through education. It seems necessary to design an executive protocol for breast cancer screening at different levels of primary health care to increase the women's willingness to undergo screening.

중학교 교사의 흡연예방교육 실천의도와 영향요인: 계획된 행위이론의 활용 (Korean Middle School Teachers' Intentions to Participate in Adolescents' Smoking Prevention Programs)

  • Objectives: To prevent smoking among adolescents, we should facilitate school-based smoking prevention programs and provide supports for teachers to participate actively in these programs. This study investigated Korean middle teachers' intentions, perceptions, and attitudes toward participating in smoking prevention programs. Methods: The study design was cross-sectional. Our conceptual framework utilized the Theory of Planned Behavior by Fishbein and Ajzen. We conducted open-ended elicitation interviews with teachers. We developed the survey questionnaire contents with data from these interviews, and distributed the questionnaires in 2002 to 194 school teachers from four schools in Seoul and Kyounggi-Do. Teachers' beliefs(behavioral, normative & control) and intentions about participating in smoking prevention programs were measured by 7-point scales. Results: The majority of teachers surveyed reported participating in adolescents' smoking prevention programs, while only less than 10% teachers reported having educational training for such programs. Teachers' attitudes toward participating in smoking prevention programs were positive, but they did not feel strong subjective norms about participating in the programs. They reported several barriers as well as facilitating conditions in participating in those programs. In correlation analysis, teachers' subjective norms and attitudes toward participating in smoking prevention were significantly correlated with their intentions to participating in those programs. Teachers with positive perceptions about smoking prevention programs were more likely to have strong intentions to participation in them, while teachers who received educational training and instructions on how to teach in smoking prevention programs were more likely to have positive perceptions than those who did not. Conclusion: We concluded that smoking prevention programs for adolescents can be made more effective by increasing societal expectations that teachers participate in these programs, and by providing additional resources dedicated to facilitating teachers' active participation in them.

이민 임산부의 스트레스와 분만경험에 대한 현상학적 연구 (A phenomenologic study on the stresses and the experiences of pregnant women and postpartum mothers who had immigrated to the United States)

  • Koreans are one of the fastest growing immigrant groups in America. Adjsting to life in foreign country produces a great deal of stress. Differences in culture, language, expectation and social behavior can lead to misunderstandings. The pregnancy and delivery event is one of maturational crisis in life cycle. The purpose of this research was to understand the structure of the lived experience of pregnant women and postpartum mothers who had immigrated to the United States. The research question was "What is the structure of the experience of pregnant women and postpartal mothers?" The sample consisted of 16 women registrated at the Obstetrics and Gynecology Department of one local clinic in Hawaii. The unstructured interviews were carried out from Jnuary 5, through January, 30, 1994. They were audio-recorded and analyzed using Van Kaam's method. There are different views on the causative factors of stress. Maladjustement to the immigrant life, spousal conflicts, anxiety related to bringing up the bay and conflicts between mother-in-law and daughter-in-law are considered to be related factors. The experiences of pregnant women over the period of the pregnancy can be varied and can include change of body image, emotional and physical change. The experiences of postpartum mothers over the postpartum period can be varied and can include postpartum depression emotional irritability, fear related to bringing up the baby and disappointment with husband. Positive experiences over the period of pregnancy and postpartum were the strengthen-ing beliefs. Sources of support were, first, spouse then mother and faith. Support was also received from the physician in charge and through self-control. The nurse, by providing empathetic support, should be a person with whom they can express their feelings and share their experiences.

IMS지향성과 기업문화 적합도가 IMS활동의 이행수준과 성과에 미치는 영향

  • With a sample of 147 Korean small and medium size companies, this study examined the relationships among degree of information orientation, corporate culture, degree of information management implementation and selected business performances in the process of implementing IMS improvement programs, such as IMS(Information Management System). Information orientation is defined as company-wide understanding and implementation of the underlying philosophy, principles, approached, and tools of information improvement programs. It is assumed that successful implementation of information improvement programs requires a information-oriented mind-set of the employees. The key elements of information orientation include continious improvement structured processes, organixation-wide participation and customer-focused spirit. Culture id defined as the value and beliefs of em organization that shape its behavior. It is also assumed that successful implementation of information improvement programs require strong support from s corporate culture that emphasizes cintinious improvement. Adopting the competing values model of Quinn and McGrath(1985), corporate culture is classified into 'flexible' versus 'controlled culture' and 'outer-directed' versus 'inner-directed culture'. Fitness was defined through the relationship between levels of information oriented and types of corporate culture. The results were as follows. First, it was found that when a company with high information orientation promoted information innovation programs, such as IMS, it reported higher degree of information management implementation and improvement in business performances. Second, the results showed the importance of 'flexible culture' and 'outer-directed culture' in performing information, innovation. Regarding the types of corporate culture, the analysis found that developmental culture, rational culture and group culture were effective. Third, companies with high information oriented and flexible culture or companies with high information orientation and outer-directed culture reported the highest implementation in Information management activities. Fourth, the results showed that the level of information management implementation had a mediating effect on the relationship between information orientation and business performance. It was also found that enhanced non-financial performance led to the improvement of financial performance. This study attempted to exaime the factor that lead information management program to success. In order to reach success, first, it is suggested that companies have positive mind set toward continious information improvement. Secondly, it is recommended that a flexible and outer-directed culture appropriate for continious information improvement is cultivated.

고혈압 환자들의 관점에서 본 혈압관리에 대한 지식, 태도, 실천: 질적연구 (The Knowledge, Attitude and Practice of Blood Pressure Management from the Patient's Viewpoint: A Qualitative Study)

  • Objectives: This study adopted a qualitative method to explore the layman's beliefs and experience concerning high blood pressure and its management in order to develop a strategy to increase adherence to proper medical treatment. Methods: Semi-structured interviews that focused on personal experiences with hypertension and its management were conducted with 26 hypertensive patients. The participants were selected according to a BP above 140/90 mmHg (hypertension stage 1), based on the seventh report of the Joint National Committee on prevention, detection, evaluation and treatment of high blood pressure (JNC-VII). The interviews lasted for approximately 30 minutes (range: 20-60 minutes). The resulting questions were formulated into open-ended questions. The interview questionnaire was composed 17 items to examine non-adherence to treatment and 19 items to examine adherence to treatment. Results: Most participants recognized that the direct cause of high blood pressure was unhealthy behavior rather than inheritance. Thus, the hypertensive patient believed they could recover their blood pressure to a normal level through removing the direct cause of hypertension (weight reduction, diet, exercise) instead of taking drugs. The reasons for these statements were that the drugs for controlling hypertension are not natural or they are artificial, and they may have side effects, and drugs are not treatment for the root cause of hypertension. Most of the hypertensive patients chose to manage their behaviors as soon as they knew their blood pressure was high. Therefore, we should not divide the subjects into two groups according to their taking drugs or not, but they should be divided into two groups according to their willingness or not to manage their condition. Conclusions: For developing a strategy for an individual approach to hypertension management, we need to develop a client-centered attitude and strategy. That is, we need to tailor our approach to individual cases to avoid generalizations and stereotyping when developing an adherence increasing strategy.

Evaluation of Breast Self-Examination (BSE) Application in First and Second Degree Relatives of Patients with Breast Cancer

  • Background: The aim of this study was to determine beliefs concerning breast self-examination in first- and second-degree relatives of patients with breast cancer and evaluate their breast self-examination (BSE) application. Materials and Methods: A survey study was conducted in an oncology polyclinic and general surgery clinic of a hospital in Ankara, the capital of Turkey with a sample of 140 women. Results: It was determined that 60.7% of the participants had conducted BSE and 48.1% had undergone a clinical breast examination. Perceived selfefficacy of the women who performed BSE were significantly higher compared with women who did not practice BSE (p<0.001) Furthermore, perceived barriers were lower among those who had performed BSE (p<0.001). Logistic regression analysis indicated that women who perceived higher self-efficacy (OR: 1.119, 95% CI: 1.056-1.185, p<0.001) and had regular CBE (OR: 8.250, 95% CI: 3.140-21.884, p<0.001) and educational status (OR: 5.287, 95% CI: 1.480-18.880, p<0.01) were more likely to perform BSE. Conclusions: Findings from this study indicated that perceived barriers, perceived self-efficacy, and educational status could be predictors of BSE behavior among the first- and second-degree relatives of patients with breast cancer. Therefore, BSE training programs that emphasize self-efficacy and address perceived barriers are recommended.
