Power Doppler ultrasound-guided sialography using the phenomenon of increased blood flow: A technical report

  • Purpose: This report presents a procedure for performing power Doppler ultrasound-guided sialography using the phenomenon of increased blood flow and illustrates its application to practical patient cases. Materials and Methods: The salivary gland was scanned using ultrasound equipment (GE LOGIQ5 Expert® device; GE Medical Systems, Milwaukee, WI, USA) to identify pathological findings related to the patient's chief complaint. To identify the orifice of the main duct, it should be cannulated using a lacrimal dilator. After inserting the catheter into the cannulated main duct, the position of the catheter within the duct was confirmed by ultrasound. A contrast agent was injected until the patient felt fullness, and ultrasound (B-mode) was used to confirm whether the contrast agent filled the main canal and secondary and tertiary ducts. Then, power Doppler ultrasound was performed to determine whether the salivary gland had increased blood flow. Results: In 2 cases in this report, a power Doppler ultrasound scan showed a significant increase in blood flow after contrast medium injection, which was not observed on a preoperative scan. Conclusion: Power Doppler ultrasound was found to be a simple, safe, and effective tool for real-time sialography monitoring.

  • Using a spatial compound imaging technique in a medical ultrasound imaging system, the average speed of sound in a medium of interest is measured, and imaging of its distribution is implemented. When the brightness reaches the highest level in an ultrasonic image obtained as the speed of sound used in focusing is varied, it turns out that the focusing has been accomplished satisfactorily and that the speed of sound which has been adopted becomes the sought-after average speed of sound. Because spatial compound imaging provides many different views of the same object, the adverse effect of erroneous speed-of-sound estimation tends to be more severe in compound imaging than in plain B-mode imaging. Thus, in compound imaging, the average speed of sound even in the case of speckled images can be accurately estimated by observing the brightness change due to different speeds of sound employed. Using this new method that offers spatial diversity, we can construct an image of the speed of sound distribution in a phantom embedded with a 10-mm diameter plastic cylinder whose speed of sound is different from that of the background. The speed of sound in the cylinder is found to be different from that of the surrounding medium.

  • 최근 간질환 검사의 비침습적인 방법으로 탄성초음파가 각광받고 있는 추세이다. 그러나 진단결과와 탄성초음파 결과 값에 따른 표준 진단 매뉴얼이 없는 실정이다. 따라서 본 연구에서는 탄성초음파 수치에 따른 간질환의 종류와 B-mode 영상 간의 경향을 알아보고자 하였다. 그리고 환자의 우측 늑간 초음파 B-mode 영상평가 파라미터인 평균, 신호 대 잡음비, 최대신호 대 잡음비, 엔트로피로 분석하고, 동일 부위에 시행한 탄성초음파 수치와의 비교를 통해 경향을 연구하였고, 그 기준을 제시하고자 하였다.

초음파 탄성 영상 알고리듬 (Algorithms for Ultrasound Elasticity Imaging)

  • 초음파를 사용해 음속도, 감쇠 계수, 밀도, 비선형 B/A 파라미터 등을 측정하여 인체 조직의 특성을 정량적으로 영상화하고자 하는 연구가 1980년대부터 많이 진행되어 왔으나 아직 상용화 단계에는 도달하지 못했다. 하지만 1990년대 초에 시작된 탄성 영상법은 최근 들어 초음파 진단기에 상용화되어 임상에서 B-모드 영상법과 함께 전립선, 유방, 갑상선, 간, 혈관 등을 진단하기 위한 보완적이며 더 정량적인 모드로 사용되고 있는 단계에 진입하였다. 본고에서는 주로 준정적 또는 정적탄성 영상법에 사용되는 여러 가지 알고리듬을 소개하고 특성을 비교하고자 한다. 대부분의 알고리듬은 상호상관함수 또는 자기상관함수 방법에 그 기반을 두고 있으며 전자는 래그를 변화시켜가면서 시간 이동량을 찾지만 후자는 보간 과정 없이 고정된 래그에서의 위상차로부터 시간 이동량을 바로 구해 변위를 추정하는 점이다.

유방 탄성초음파의 Color overlay pattern에 대한 유용성 (Usefulness of Color-overlay Pattern of Breast Elastic Ultrasonography)

  • 유방암 발생률이 해마다 증가하여 그에 따라 유방 종양을 진단하는 검사인 조직검사의 횟수도 같이 증가하고 있다. 조직검사는 침습적 검사로써 환자들의 불안감과 감염, 출혈 등의 부작용이 발생하고 있다. 본 연구에서는 유방 병변 환자를 대상으로 회색조 초음파와 탄성초음파검사 모두 실시한 69명을 대상으로 회색조 초음파는 BI-RADS Category에 따라 분류하고, 탄성초음파검사는 횡파의 전파 속도를 환산하여 나타낸 kPa값을 Color overlay pattern으로 분류하였다. 민감도와 특이도의 합이 가장 높은 최적의 Cut-off value는 54.70 kPa로 나타났다. Color overlay pattern에서 Dark Blue 42명, Light blue ~ Red 27명으로 분류된 결과와 BI-RADS Category 분류 결과인 양성 40명, 악성 29명으로 분류된 결과가 유사하다. 따라서 Color overlay pattern에서 Dark Blue에 분류될 경우에는 양성, Light blue ~ Red에 분류될 경우에는 악성으로 의심할 수 있을 것으로 판단된다. 결론적으로 유방 탄성초음파는 무분별한 유방 조직검사의 횟수를 줄이고 양성종양과 악성종양을 구분하는데 획기적인 역할을 할 것으로 사료된다.

Genetic Analysis of Ultrasound and Carcass Measurement Traits in a Regional Hanwoo Steer Population

  • Ultrasound measurements of backfat thickness (UBF), longissimus muscle area (ULMA) and marbling score (UMS) and carcass measurements of carcass weight (CW), backfat thickness (BF), longissimus muscle area (LMA), and marbling score (MS) on 7,044 Hanwoo steers were analyzed to estimate genetic parameters. Data from Hanwoo steers that were raised, finished in Hoengseong-gun, Gangwon-do (province) and shipped to slaughter houses during the period from October 2010 to April 2013 were evaluated. Ultrasound measurements were taken at approximately three months before slaughter by an experienced operator using a B-mode real-time ultrasound device (HS-2000, FHK Co. Ltd., Tokyo, Japan) with a 3.5 MHz linear probe. Ultrasound scanning was on the left side between 13th rib and the first lumbar vertebrae. All slaughtering processes and carcass evaluations were performed in accordance with the guidelines of beef grading system of Korea. To estimate genetic parameters, multiple trait animal models were applied. Fixed effects included in the models were: the effects of farm, contemporary group effects (year-season at the time of ultrasound scanning in the models for UBF, ULMA, and UMS, and year-season at slaughter in the models for CW, BF, LMA, and MS), the effects of ultrasound technicians as class variables and the effects of the age in days at ultrasound scanning or at slaughtering as linear covariates, respectively for ultrasound and carcass measures. Heritability estimates obtained from our analyses were 0.37 for UBF, 0.13 for ULMA, 0.27 for UMS, 0.44 for CW, 0.33 for BF, 0.36 for LMA and 0.54 MS, respectively. Genetic correlations were strongly positive between corresponding traits of ultrasound and carcass measures. Genetic correlation coefficient between UBF and BF estimate was 0.938, between ULMA and LMA was 0.767 and between UMS and MS was 0.925. These results suggest that ultrasound measurement traits are genetically similar to carcass measurement traits.

희박 어레이 기반의 효율적인 양방향 화소단위 집속 기법의 구현 (Efficient Implementation of Synthetic Aperture Imaging with Virtual Source Element in B-mode Ultrasound System Based on Sparse Array)

  • 본 논문에서는 양방향 화소단위 집속 기법을 희박 어레이 기법을 사용하여 효율적으로 구현하는 방법에 대해 제안한다. 제안한 방법은 각 주사선을 구성할 때 합성에 이용되는 부구경 사이의 간격을 멀리함으로써 적은 수의 부구경으로 큰 크기의 합성구경을 형성하여 시스템의 복잡도를 감소시키면서 초음파 영상의 해상도와 프레임율을 향상시키게 된다. 하지만 이때 합성에 사용되는 부구경 사이의 간격 증가로 인해 그레이팅 로브 값은 상승하게 된다. 이러한 그레이팅 로브는 일반적인 합성구경 기법에서는 제거할 수 있다. 하지만 본 논문에서는 일반적인 합성구경 기법과는 달리 양방향 화소단위 집속 기법에서는 부구경 사이의 간격이 클 경우 발생하는 그레이팅 로브를 제거할 수 없음을 이론적으로 해석하였다. 또한 이러한 그레이팅 로브 값을 송신 집속점을 근거리에 위치시키고 합성에 사용되는 부구경간의 간격을 수신부구경의 크기에 비례하여 적절히 선택함으로써 의료용 초음파 영상에 적합한 -40dB 이하로 억제하는 방법과 조건을 제시하였다. 모의 생체와 실제 인체 실험 결과 제안한 방법을 통해 보다 적은 수의 부구경을 이용하여 더욱 빠른 프레임율의 양방향 화소단위 집속 기법의 구현이 가능함을 확인하였다

Speckle Noise Reduction and Edge Enhancement in Ultrasound Images Based on Wavelet Transform

  • For B-mode ultrasound images, we propose an image enhancement algorithm based on a multi-resolution approach, which consists of edge enhancing and noise reducing procedures. Edge enhancement processing is applied sequentially to coarse-to-fine resolution images obtained from wavelet-transformed data. In each resolution, the structural features of each pixel are examined through eigen analysis. Then, if a pixel belongs to an edge region, we perform two-step filtering: that is, directional smoothing is conducted along the tangential direction of the edge to improve continuity and directional sharpening is conducted along the normal direction to enhance the contrast. In addition, speckle noise is alleviated by proper attenuation of the wavelet coefficients of the homogeneous regions at each band. This region-based speckle-reduction scheme is differentiated from other methods that are based on the magnitude statistics of the wavelet coefficients. The proposed algorithm enhances edges regardless of changes in the resolution of an image, and the algorithm efficiently reduces speckle noise without affecting the sharpness of the edge. Hence, compared with existing algorithms, the proposed algorithm considerably improves the subjective image quality without providing any noticeable artifacts.

Measurement and Clinical Analysis of Carotid Intima, Media and Intima-Media Thickness

  • Individual clinical significance of each layer of CCA (common carotid artery) has not been well studied. We intended to measure the intima thickness (IT), media thickness (MT), and intima-media thickness (IMT) of CCA separately and tried to analyze the clinical significance. One hundred fifty one consecutive patients (mean age: $57{\pm}15$ years; 77 males, 74 females) underwent CCA scanning using high-resolution ultrasound. The images were off-line analyzed using B-mode ultrasound image processing, devised for individual measurement of IT and MT as well as IMT. The mean coefficients of variation of new method measuring IT, MT, and IMT separately were 0.16% for IMT and 0.21% for both IT and MT. The IT (p < 0.01), MT (p < 0.01) as well as IMT (p < 0.001) of patients with atherosclerotic disease were significantly thicker than that of the patients without atherosclerotic disease. Patients with hypertension showed significantly thicker IT (p < 0.01), MT (p < 0.001), and IMT (p < 0.001) than that of the patients without hypertension. However, only IT was thicker in patients with smoking (p<0.01) than that of the patients without smoking.

Implementation of an Ultrasound Elasticity Imaging System

  • Recently, active research has been going on to measure the elastic modulus of human soft tissue with medical ultrasound imaging systems for the purpose of diagnosing cancers or tumors which have been difficult to detect with conventional B-mode imaging techniques. In this paper, a real-time ultrasonic elasticity imaging system is implemented in software on a Pentium processor-based ultrasonic diagnostic imaging system. Soft tissue is subjected to external vibration, and the resulting tissue displacements change the phase of received echoes, which is in turn used to estimate tissue elasticity. It was confirmed from experiment with a phantom that the implemented elasticity imaging system could differentiate between soft and hard regions, where the latter is twice harder than the former, while operating at an adequate frame rate of 20 frames/s.
