Determination of Nursing Price using Willingness to Pay (지불용의접근법을 이용한 간호서비스의 가격결정)

  • It will become more and more popular to use the long-term care facilities and home health care services with the chronic disease increasing. It depends on how much the consumers would pay and purchase the services. They might get more benefits from that kind of services than from ordinary hospitalization. So far, the study of determining the medical service price has focused most often on the efforts from the providers' view. But it must be reasonable to include the consumers' value for the service. This study was performed to assess WTP(Willingness to Pay) for home health care service in order to apply to the determination of nursing price in a reasonable manner. In this study, respondents were asked if they would pay for the service's intangible benefits under the four different types(open-ended minimum WTP, open-ended maximum WTP, bidding WTP, referendum WTP). The contingent valuation method is a potentially useful tool in understanding how people value the benefits of the service. As a result, average open-ended minimum WTP was W16,015 per day among 65 respondents. Average open-ended maximum WTP was W29,154 per day among 65 respondents. Average bidding WTP was W26,300 per day among 65 respondents. Average referendum WTP was W22,200 per day among 70 respondents. The results of regression analyses were also consistent with theoretical prediction, e.g., increasing WTP with consumers' value for the service, state of patients, and household income. This study demonstrated that it was more reasonable to consider the consumers' value in determining the services' price. In addition, a further study is needed to test the validity of this CV method and to determine a proper nursing price based on the consumers' view.

The Research on the Oxidative Stress by Free Oxygen Radicals in Sasang Constitution (활성산소(活性酸素)에 의한 산화스트레스환자의 사상체질별(四象體質別) 특성(特性)에 관한 연구(硏究))

  • 1. Objectives The purpose of this research is to find out the Constitutional difference of Oxidative stress by Free oxygen radicals. 2. Methods We enrolled 55 patients who visited our hospital for medical examination from February 1, 2006 to May 31, 2006. This research investigated the Constitutional difference of Oxidative stress by Free oxygen radicals. 3. Results and Conclusions l) The Sasang Constitutional distribution of patients have oxidatative stress by Free oxygen radicals was Taeunmin 30%(8persons of total 27persons), Soeumin 56%(9persons of total 16persons), Soyangin 33%(4persons of total 12persons). The rate of patients have oxidatate stress was high in Soeumin. The Sasang Constitution had significance relation with oxidatative stress by Free oxygen radicals. 2) Triglyceride on the average of Taeumin patients have oxidatative stress by Free oxygen radicals is ststistical significantly higher than Triglyceride on the average of controlled normal persons in Taeumin(p-value=0.010) and Soyangin(p-value=0.015). Blood uric acid level on the average of patients have oxidatate stress by Free oxygen radicals is ststistical significantly lower than Blood uric acid level on the average of controlled normal persons in Taeumin(p-value=0.004) and Soyangin(p-value=0.037). This research has shown that there is a statistical significance between the Triglyceride level, Blood uric acid level and oxidatate stress by Free oxygen radicals.

Assessment of Isokinetic Muscle Strength in the Knee Extensor Through the Ankle Joint Angles (발목관절 각도에 따른 무릎 폄근의 등속성 근력 평가)

  • Purpose:This study was designed to investigate difference in isokinetic muscle strength in the knee extensor muscle and characteristic differences in muscle strength between males and females through the ankle joint angles. Methods: Seventy-four subjects participated in this study. There were two groups: 36 males and 38 females. The mean age of the men was 24.58 years and women was 23.74 years. Subjects were seated on a CON-TREX LP (leg press) lean to back of chair, and there bodies were fixed by straps with the hip joint at an angle of $130^{\circ}$. After randomly fixing the ankle joint at $0^{\circ}$, $20^{\circ}$, and $40^{\circ}$ of plantar flexion (PF) in range of full extension of knee joint. We studied force max average, force max average/kg, power average, and total work through the angle of the ankle joint when the knee joint was extended from $90^{\circ}$ to $180^{\circ}$. Results: In the male group, all maximum measured value showed at the ankle joint $0^{\circ}$, all minimum measured value showed at $40^{\circ}$ PF (p<0.01). In the female group, all maximum measured value showed at the ankle joint $20^{\circ}$ PF, especially the power average increased significantly. All minimum measured value showed $40^{\circ}$ PF (p<0.01). Conclusion: There are differences between males and females in isokinetic muscle strength of the knee extensor through ankle joint angles in healthy adults. Males and Females have different characteristics of muscle strength through the ankle joint angles.

Developing a New BNR (Parallel BNR) Process by Computer Simulation (컴퓨터 시뮬레이션을 이용한 신 생물학적 고도처리 (병렬 고도처리) 공법 개발)

  • Since Korean government imposed a stricter regulation on effluent T-N and T-P concentrations from wastewater treatment plant, a new process has to be developed to meet these rules and this process should remove T-N and T-P, economically, from weak wastewater that is typical for Korea's combined sewer system sewage. In this study, a computer simulator, BioWin from EnviroSim, Inc. was used. Three processes - A2/O, Modified Johannesburg, UCT- had been simulated under same operational conditions and a new process - Parallel BNR Process - had been developed based on these simulation results. The Parallel BNR process consists of two rows of reactors: One row has anaerobic and aerobic reactors in series, and the other row has RAS anoxic1 and RAS anoxic2 reactors in series. In order to ensure anaerobic state in anaerobic tank, a part of influent is fed to RAS anoxic1 tank in second row. This process had been simulated under same conditions of other three processes and the simulation results were compared. The results showed that three existing processes could not perform biological phosphorus removal when the average influent was fed at any operation temperatures. However, the Parallel BNR process was found that biological phosphorus removal could be performed when both design and average influent were fed at any operation temperatures. This process showed the T-N concentration in effluent had a maximum value of 15mg/L when design influent was fed at $13^{\circ}C$ and a minimum value of 14mg/L when average influent was fed at $20^{\circ}C$. Also, T-P concentrations had a maximum value of 1.3mg/L when average influent was fed at $20^{\circ}C$ and a minimum value of 1.1mg/L when design influent was fed at $13^{\circ}C$. Based on these results, we found that this process can remove nitrogen and phosphorus biologically under any operational conditions.

The Effects of Marine Training on Physical -Focused to Teaching Models of Aquatic Training Curricula- (해양훈련이 신체에 미치는 영향 - 해양훈련교과목의 수업모형을 중심으로 -)

  • This study aimed to know the effects of three marine training items, swimming, rowing and yachting on pulse, lung capacity and weight. The experiment subjects were composed of ten each item and were tested for six days. The experiment groups were strictly controlled in eating time, food amount, sleeping time and training intensity. The level of training intensity was 70~80% of maximal pulse rate. In the training intensity of each item the speed was decided after examination in advance, and the trainees kept the speed during training. The contents of training were made up through enough examination. The conclusions were as follows. 1. The effect on pulse in average value showed the decrease of 1.80round/min swimming, 1.51round/min rowing, and 0.11round/min yachting, but it was not admitted as significant difference. And in average value, swimming showed the decrease of 0.26round/m than rowing and 1.69round/m than yachting. 2. The effect on lung capacity showed the increase of 66.66cc swimming, 42.97cc rowing, and 4.22cc yachting, but there was no significant difference. And the average value of swimming showed the increase of 23.66cc than rowing, and 62.44cc than yachting. 3. The effect on weight showed decrease of 3.45g in swimming, 3.24g in rowing, and 2.07g in yachting. Swimming and rowing proved to have significant difference (p<.05). And in average value, swimming showed the decrease of 1.175g than rowing, and 1.38g than yachting. On the whole, in all experiment items, pulse, lung capacity and weight, the change was in the order of swimming, rowing and yachting after experiments.

  • Geotechnical parameter estimation is critical to the design, performance, safety, and cost and schedule management in Tunnel Boring Machine projects. Since these parameters vary within a certain range, relying on mean values for evaluation introduces significant risks to the project. Due to the non-homogeneous characteristics of geological formation, data may not exhibit a normal distribution and the presence of outliers might be deceptive. Therefore, the use of reliable analyses and simulation models is inevitable in the course of the data evaluation process. Advanced modeling techniques enable comprehensive analysis of the project data and allowing to model the uncertainty in geotechnical parameters. This study involves using Monte Carlo Simulation method to predict probabilistic distributions of field data, and therefore, establish a basis for designs and in turn to minimize project risks. In the study, 166 sets of geotechnical data Obtained from 35 boreholes including Standard Penetration Test, Limit Pressure, Liquid Limit, and Plastic Limit values, which are mostly utilized parameters in estimating project requirements, were used to estimate the geotechnical data distribution of the study field. In this context, firstly, the data was subjected to multi-parameter linear regression and variance analysis. Then, the obtained equations were implemented into a Monte Carlo Simulation, and probabilistic distributions of the geotechnical data of the field were simulated and corresponding to the 90% probability range, along with the minimum and maximum values at the 5% probability levels presented. Accordingly, while the average SPT N30 value is 42.86, but the highest occurrence rate is 50.81. For Net Limit Pressure, the average field data is 17.07 kg/cm2, with the maximum occurrence between 9.6 kg/cm2 and 13.7 kg/cm2. Similarly, the average Plastic Limit value is 22.32, while the most probable value is 20.6. The average Liquid Limit value is 56.73, with the highest probability at 54.48, as indicated in the statistical data distribution. Understanding the percentage distribution of data likely to be encountered in the project allows for accurate forecasting of both high and low probability scenarios, offering a significant advantage, particularly in ordering TBM requirements.

The Impact of Sales Revenue on Value Relevance in the Distribution Corporate (유통기업 매출액의 기업가치 관련성)

  • Purpose - For distribution corporate, the method of recognizing sales revenue may be different depending on the type of distribution transaction. Until the change in accounting standards for revenue recognition was made in 2002, the distribution corporate recognized the full amount of sales of goods regardless of the type of transaction. However, in accordance with accounting standards for revenue recognition, which began to be applied in 2003, distribution corporate differ in sales revenue recognition by transaction type. The Purpose of this study is to analyze the impact of sales revenue on the corporate value after the change of the revenue recognition accounting standards. Research design, data, and methodology - We selected a comprehensive wholesale and retail corporate listed on Korea Exchange. The research model extends the Ohlson(1995) model and regresses whether sales revenue affecting the corporate value is discriminatory value relevance between the corporate affected by changes in accounting standards for revenue recognition and those not. Results - The results of the analysis are as follows. First, The average value of stock price, net asset per share, and earnings per share are all higher than those before the change of accounting standards for revenue recognition. However, the average value of sales per share is lower than that before the change of accounting standards for revenue recognition. Second, the relationship between corporate value and net asset per share, earnings per share and sales per share, the coefficient of net asset per share, earnings per share and sales per share are all statistically significant positive value. Therefore, in explaining corporate value, besides net asset per share and earnings per share, sales per share provides additional information. And the coefficient of interaction variable between accounting standard change and sales per share is a statistically significant positive value. This result indicating that after the change of the revenue recognition accounting standards the usefulness of sales revenue has increased. Conclusions - The change in accounting standards for revenue recognition led to a decrease in distribution corporate sales revenue but the higher the relevance of the corporate value of the sales revenue information. These results shows that the change of accounting standards that reflects the transaction type of retailers was a revision to increase the value relevance of sales revenue in valuation of corporate value.

On the Statistical Characteristics of Freak Wave Occurrence (Freak Wave 발생의 통계적 특성에 대하여)

  • In this paper time series wave data are simulated by the Monte Calo method using random numbers to generate random phases of the wave signal. The simulated wave signasl are used to study the characteristics of freak waves. Various sea states are represented by combinations of the significant wave height $H_s$ defined in the spectrum method and the significant wave steepness $S_s$. For a fixed value of $S_s$, the probability of the occurrence of the freak wave is decreased as $H_s$ increases. For a fixed value of $H_s$ the probability of the occurrence of the freak wave increases as $S_s$ increases. The average value of the maximum wave height increase as $S_s$ increases, but the average height of freak wave remains the same and the value approaches two times of $H_s$. For the fixed value of $S_s$, average kurtosis of wave elevation increases as $H_s$ increases, but for a fixed $H_s$, the average kurtosis decreases as $S_s$ increases. The average of abnormality index(AI) is around 2.11 irregardless of $H_s$ and $S_s$. The maximum value of AI lies between 2.5 - 3.0. Therefore it is conjectured that AI maximum due to linear focusing is 3.0.

Studies on the Hemodilution Perfusion with Rygg-Kyvsgaard Oxygenator (혈액희석 체외순환법에 관한 임상적 관찰 -상온하 Rygg-Kyvsgaard 산화기 및 Sigmamotor pump 사용예를 중심으로-)

  • Clinical perfusion data on 16 cases of cardiopulmonary bypass using Sigmamotor pump and RyggKyvsgaard Oxygenator which performed at Seoul National University Hospital during the period of Aug. 1968 to Aug. 1970 was analized. AIl cases were hemodiluted and the perfusion was carried out under the normothermic condition. The age of the patients ranged between 6 and 43 years. The b:dy weight varied between 18.3 and 54.0 kg and the body surface area between 0.78 and 1. 59$M^2$. The priming solution was consiste:I with fresh ACD blood. Hartmann solution and Mannitol. The average amount of priming was approximately 2242 ml. The average hemodilution rate was 17%. The flow rate ranged from 1.7L to 3.5L/Min/$M^2$ and averaged 2.4L/Min/$M^2$ or 78mI/Min/kg. The duration of perfusion varied from 22 to 110 min with average of 56.9 minutes. Some hemodynamic responses were observed. The arterial pressure dropped immediately after the initiation of partial perfusion and was more marked after the total perfusion foIlowed by gradual increase to the safety level. The central venous pressure reflected the reduced blood volume especially in the cases of prolonged perfusion which lasted over 60 min. In most of the cases, red blood cell count decreased and white blood ceIl count increased after the perfusion. Hemoglobin level was decreased, averaging of 12.5mg%, Hct 3.3% and platelets count of 18% postoperatively. Plasma hemoglobin increased mildly, from pre-perfusion average value of 4. 06mg% to postperfusion value of 22.5mg%. Serum potassium was 4.4mEq/L pre-operatively and was decreased to 3.7mEq/L postoperatively. Five cases showed definite hypopotassemia immediately after the operation. Sodium and chloride decreased mildly. These electrolyte changes are thought to be related with hemodilution. diuretics and reduced blood volume during and after the perfusion. Arterial blood pH value revealed minimal to moderate elevation from preperfusion average value of 7.376 to 7.461 during perfusion and then 7.365 after perfusion. The pC02 and hicarbonate showed minimal to moderately lowered values. The total CO2 was decreased. Buffer base decreased during perfusion (Av. 42.6mEq/L) and further decreased after the perfusion (Av. 40.8mEq/L). These arterial blood acid base changes suggested that the metabolic acidosis was accompanied by respiratory alkalosis during and immediately after the perfusion. Authors belived that the acidosis could more effectively be corrected with the more additional dose of bicarbonate than we used by this study. The chest tune drainage during the first 24 hours following operation was 1158 ml in average. One case (Case No. 15) showd definite bleeding tendency and it was believed that the cause might be due to the defect of heparin and protamine titration. The average urinary out put during 24 hours post-perfusion was 1291ml. One case (Case No. ]) showed definite post perfusion oliguria. As conclusion hemodilution using fresh ACD blood. Hartmann and Mannitol solution added with Bivon and high flow rate unler normothermia. was thought to amelioratc the severity of mctabolic acidosis during and after perfusion with relatively satisfactory effect on the diuresis and bleeding tendency.

Evolution and Identification of Thermo-Tolerant Hybrids in the Silkworm, Bombyx mori L.

  • Four thermo-tolerant lines of silkworm, Bombyx mori, (L.) viz., A HT, B HT (Chinese type) and F HT, G HT (Japanese type) were evolved by utilizing the breeding resource material (identified from initial screening at a temperature of 31 ${\pm} 1^{\circ}C$ and relative humidity 85 ${\pm}$ 5%) through conventional breeding. These tolerant lines were crossed with productive breeds and forty four hybrids were evaluated on eight economic traits by the Multiple Trait Evaluation Index Method. Ten hybrids were short-listed based on the average evaluation index value larger than 50 for eight economic traits studied. The identified ten hybrids recorded higher index values (> 50) for most of the traits studied. Single hybrid G ${\times}$ CSR12 indicated average index value larger than 50 for six traits viz., pupation number (58), cocoon weight (67), shell weight (65), average filament length (74), raw silk % (69), reelability % (51) except for shell ratio % (41). The standard deviation of the cocoons in the above hybrid was 8.41 in the hybrid cocoon length and width measurement. However, two selected hybrids viz., A ${\times}$ CSR5 and G ${\times}$ CSR13 recorded average index value larger than 50 for all the traits viz., pupation number (57, 60), cocoon weight (50, 54), shell weight (56, 57), shell ratio percentage (59, 53), average filament length (55, 60), raw silk percentage (63, 67) and reelability percentage (53, 53). The standard deviation of the cocoons in the two selected hybrids viz., A ${\times}$ CSR5 and G ${\times}$ CSR13 was 8.41 and 8.06 respectively in the cocoon length and width measurement.
