Absorbed and effective dose in direct and indirect digital panoramic radiography (직.간접디지털 파노라마 방사선촬영시 흡수선량과 유효선량)

  • Purpose : We evaluated the absorbed doses to the organs and calculated the effective doses when using the digital panoramic radiography. Materials and Methods : The absorbed dose averages in major organs of oral and maxillofacial region were measured using the Dental head phantom (CIRS Co., USA), $^nLi_2B_4O_7$ TLD chip and UD-716AGL dosimeter (Matsushita Electric Industrial Co., JPN) when performing indirect and direct digital panoramic radiography. Effective doses were calculated from correspond to ICRP 2007 recommendations for two panoramic radiography. Results : The absorbed dose average on indirect and direct digital panoramic radiography was highest in parotid glands as measured 1259.6 mGy and 680.7 mGy respectively. Absorbed dose average in another organs were high in order of esophagus, submandibular gland, tongue and thyroid gland on both types of digital panoramic radiography. The absorbed dose average was higher on indirect type than direct one (p<0.05). The effective dose was higher on indirect type than direct one as measured 13.28 mSv and 8.70 mSv respectively. Conclusion : The absorbed doses in salivary gland and oral mucosa were high. However, thyroid gland also demands the attention on radiography due to high tissue weighting factor in spite of the low absorbed dose.

  • Mammography using X-rays is currently the most used for early diagnosis of breast cancer. As the frequency of use of X-ray devices increases, interest in radiation hazards caused by mammography is increasing. Therefore, in this study, in order to measure the exposure dose of the mammary gland in X-ray mammography that requires high contrast and high resolution, the international Atomic Energy Agency (IAEA) stipulates the international standards presented by IEC 62220-1-2: 2015. Based on the beam quality criteria of the recommendation, we tried to present a guideline for evaluating the average mammary gland dose. As a result, the average streamline dose value of the 4.5 cm PMMA phantom was 2.3 mGy at the maximum within the 30 kV range, and was evaluated to be 1.19 mGy based on 28 kV.

  • We studied exposure techniques and exposure dose for simple abdomen A-P projection for 41 medical facilities that are located in Seoul area. 1. The range of tube voltage used was 60 to 84 kVp, the average tube voltage used was 74 kVp 2. Only 17% of added filter was used. 3. Tube current mostly used was 200 mA, some of them used 400 mA. 4. The grid ratio mostly was used 10 : 1, 54 % of the rare earth screen was used in most facilities. 5. The average skin entrance dose was 4.15 mSv and the dose range was 1.05 mSv to 11.0 mSv.

  • Recently, the use of panoramic radiography has shown a constant increase, and significant research is underway. However, radiation exposure attracts less attention in dental radiography than in other types of radiography. We used an OSLD for measurement of the entrance skin dose in eyeballs and the thyroid region, both of which are not covered by examinations but are included in radiographical regions and are sensitive to radiation, as well as orally in Incheon and reported the results. The entrance skin dose was 0.0282 mSv on average for the oral region, and 0.0259 mSv on average for the eyeball, and 0.0261mSv on average, for thyroid gland. While there is no proper shielding method for the eyeball, a thyroid protector is not used by most hospitals and most hospitals are equipped with an apron and a thyroid protector separately; thus, it is necessary to use an integration of an apron and a thyroid protector and medical device manufacturers need to develop a method for controlling the length of the slit in the slit-type area of radiation occurrence in order to reduce unnecessary exposure.

  • Comparison of the dose aspect that radiation therapy treatments using IMRT, tomotherapy, mArc (modulated arc therapy). The experimental subject is non-small cell lung cancer patient. The prescription dose is 58.0 Gy to the volume of PTV(planning target volume). and spinal cord, esophagus, and liver organ is the normal organ(OAR, organ at risk). Average PTV value is 57.60 Gy in mArc and 61.04 Gy in tomotherapy and 58.95 Gy in IMRT. The average dose of the Esophagus is 2.84 Gy in m-Arc, 5.14 Gy in tomotherapy, 1.84 Gy in IMRT. The average dose of the Liver is 19.44 Gy in m-Arc, 12.22 Gy in tomotherapy, 21.97 Gy in IMRT. The average dose of the Spinal cord is 5.72 Gy in m-Arc, 7.08 Gy in tomotherapy, 6.15 Gy in IMRT. Results of this study is no significant difference between mArc and tomotherapy and Linac based IMRT in dose study and also, mArc's dose coverage and dose volume histogram is better than IMRT and tomotherapy. but, This study is limited to a disease of cancer. in addition, fewer number of groups. The wide range the more research can be developed patient-specific treatment techniques and be applied to the patients

A Computed Tomography Analysis of the Success of Spinal Fusion Using Ultra-Low Dose (0.7 mg per Facet) of Recombinant Human Bone Morphogenetic Protein 2 in Multilevel Adult Degenerative Spinal Deformity Surgery

  • Study Design: Retrospective cohort study. Purpose: To report on spinal fusion assessment using computed tomography (CT) after adult spinal deformity (ASD) surgery using ultra-low dose recombinant human bone morphogenetic protein 2 (RhBMP-2). Overview of Literature: The reported dose of RhBMP-2 needed for successful spinal posterolateral fusion in ASD ranges from 10 to 20 mg per spinal level. This study reports the use of ultra-low dose of RhBMP-2 (0.07 mg per facet) to achieve spinal fusion in multilevel ASD surgery. Methods: Consecutive patients who underwent ASD surgery using ultra-low dose RhBMP-2 were recruited. Routine postoperative CT analysis for spinal fusion was performed by two spine surgeons. Inter-observer agreement was calculated for facet fusion (FF) and interbody fusion (IBF) at 6 and 12 months after the procedure. Results: Six consecutive ASD patients with a mean age of 62 years (28-72 years) were examined. Each patient received a total dose of 12 mg with an average dose of $0.69{\pm}0.2mg$ (0.42-1 mg) per single FF and $1.38{\pm}0.44mg$ (0.85-2 mg) for IBF. Total 131 FF and 15 IBF were examined in the study, with 88 FFs and nine IBFs being analyzed specifically at 6 months after the surgery. FF and IBF reported by surgeons A and B at 6 months were 97.7% vs. 91.9% FF, respectively (${\kappa}=0.95$) and 100% vs. 100% IBF, respectively (${\kappa}=1$). Two patients underwent longitudinal follow-up CT at 12 months, and the FF rates reported by surgeons A and B were 100% vs. 95.8%, respectively (${\kappa}=0.96$). Five out of nine facet (56%) non-unions were identified at the cross-links. The remaining four facet pseudarthrosis were noted at 1-2 spinal levels caudal to the cross-links. At the final clinical follow-up, there was no rod breakage, deformity progression, neurological deficit, or symptom recurrence. The Oswestry Disability Index improved by an average of $32.8{\pm}6.3$, while the mental component summary of the 36-item Short-Form Health Survey improved by an average of $4.7{\pm}2.1$, and physical component summary improved by an average of $10.5{\pm}2.1$. Conclusions: To our knowledge, this is the first study to report a CT that defined 92%-98% FF and 100% IBF using the lowest reported dose of RhBMP-2 in multilevel ASD surgery. The use of ultra-low dose RhBMP-2 reduces the RhBMP-2 related complications and healthcare costs.

  • This study examined the following: accuracy of the exposure conditions in the inverter device and three-phase device; output waveform over the exposure conditions; and average and standard deviation of the output waveform. After assessing whether the dose corresponding to the theoretical dose was presented, the following conclusions were obtained: 1. The accuracy of the tube voltage(kVp) and tube current(mA) exposure time(sec) was within the tolerable level prescribed in Korea's Safety Management Standards. In the error, Inverter device was large the tube voltage and exposure time, the three-phase device was large the tube current. 2. In terms of the output waveform of the exposure conditions and the average and standard deviation of the output waveform, the higher tube voltage and larger tube current resulted in greater standard deviation in pulsation. Moreover, the standard deviation of pulsation was shown to be greater in the inverter device than the three-phase device; there was also greater standard deviation in the inverter device considering the exposure time. 3. Regarding the exposure conditions over the output dose, all linearity showed the coefficient of variation which had an allowable limit of error within 0.05. Although the output dose ratio for the inverter device was 1.00~1.10 times no difference that of the three-phase device, there was almost no difference in dose ratio between the tube currents.

Comparison of the Equivalent Dose of the Lens Part and the Effective Dose of the Chest in the PET/CT Radiation Workers in the Nuclear Medicine Department (핵의학과 PET/CT실 방사선작업종사자의 수정체 부위의 등가선량과 흉부의 유효선량의 측정 비교)

  • Comparison of the effective dose of the chest and the equivalent dose of the lens site in the radiation workers working at four medical institutions with the PET / CT room located in one metropolitan city and province from April 1 to June 30, 2018 Respectively. Radioactive medicine were measured at the time of dispensing and at the time of injection. In this experiment, the average dispensing time per patient was 5.7 minutes and the average injection time was 3.1 minutes. The equivalent dose at the lens site was $0.78{\mu}Sv/h$ for 1 mCi, and the effective dose for chest was $0.18{\mu}Sv/h$ per 1 mCi. The equivalent dose at the lens site during injection was $0.88{\mu}Sv/h$ per mCi and the effective dose of chest was $0.20{\mu}Sv/h$ per mCi. The daily effective dose of the chest was $0.9{\pm}0.6{\mu}Sv$ and the equivalent dose of the lens site was $3.6{\pm}1.4{\mu}Sv$ during daily dosing for 20 days. The effective dose of the chest during the day was $0.6{\pm}0.5{\mu}Sv$ and the equivalent dose of the lens was $2.2{\pm}1.0{\mu}Sv$. At the time of dispensing, the equivalent dose of the lens was $0.187{\pm}0.035mSv$, the effective dose of the chest was $0.137{\pm}0.055mSv$, the equivalent dose of the lens was $0.247{\pm}0.057mSv$, and the effective dose of the monthly chest was $0.187{\pm}0.021mSv$. As a result of the corresponding sample test, the equivalent dose and the effective dose of the chest, the effective dose of the chest, the effective dose of the chest, the effective dose of the chest, The equivalent dose of the lens and the effective dose of the chest were statistically significant (p<0.05) with a significance of 0.000. However, there was no statistically significant difference (p>0.05) between the equivalent dose and the effective dose of the chest, the equivalent dose of the lens at the time of injection, and the effective dose of the chest at 0.138 and 0.230, respectively.

Assessment of Gamma-radiation dose Rate in the Ogcheon Lower Phyllite Area, Goesan County, Korea, Using Gamma-ray Spectrometry (감마선분광분석기를 이용한 괴산 옥천하부천매암대 일대의 감마선량 평가)

  • Gamma-radiation dose rates were measured at 77 points around the Ogcheon lower phyllite zone (og2) in Goesan County, Korea, using gamma-ray spectrometry. Sample K contents were in the range 1.8-8.8% (average 4.6%), highest in Kgr. The eU contents were 0.2-217.9 ppm (average 16.7 ppm), highest in og2 (median 29.6 ppm). The eTh contents were 11.9-76.5 ppm (average 29.5 ppm) and the average eTh content of Kgr was 45.4 ppm, higher than those of Ogcheon meta-sedimentary rocks (og1, og2, and og3) (26.6-30.6 ppm). Except for some high-uranium sites in og2, 40K is the main radioactive material contributing to the gamma-radiation dose in the study area. Our results indicate that the outdoor effective dose rate of the area is 0.08-1.71 mSv y-1 (average 0.28 mSv y-1), with most areas apart from three points in og2 displaying dose rates <1 mSv y-1, which is the normal natural radiation background level.

Evaluations and Comparisons of Body Surface Doses during Breast Cancer Treatment by Tomotherapy and LINAC Radiotherapy Devices

  • Effects on skin caused by the dose from linear accelerator (LINAC) opposing portal irradiation and TomoDirect 3-D modeling treatment according to the radiation devices and treatment methods were measured, and a comparative analysis was performed. Two groups of 10 patients each were created and measurements were carried out using an optically stimulated luminescence dosimeter. These patients were already receiving radiation treatment in the hospital. Using the SPSS statistical program, the minimum and maximum average standard deviations of the measured skin dose data were obtained. Two types of treatment method were selected as independent variables; the measured points and total average were the dependent variables. An independent sample T-test was used, and it was checked whether there was a significance probability between the two groups. The average of the measured results for the LINAC opposing portal radiation was 117.7 cGy and PDD 65.39% for the inner breast, 144.7 cGy and PDD 80.39% for the outer breast, 143.2 cGy and PDD 79.56% for the upper breast, 151.4 cGy and PDD 84.11% for the lower breast, 149.6 cGy and PDD 83.11% for the axilla, and 141.32 cGy and PDD 78.51% for the total average. In contrast, for TomoDirect 3-D conformal radiotherapy, the corresponding measurement values were 137.6 cGy and PDD 76.44%, 152.3 cGy and PDD 84.61%, 148.6 cGy and PDD 82.56%, 159.7 cGy and PDD 88.72%, and 148.6 cGy PDD 82.56%, respectively, and the total average was 149.36 cGy and PDD 82.98%. To determine if the difference between the total averages was statistically significant, the independent sample T-test of the SPSS statistical program was used, which indicated that the P-value was P=0.024, which was 0.05 lower than the significance level. Thus, it can be understood that the null hypothesis can be dismissed, and that there was a difference in the averages. In conclusion, even though the treatment dose was similar, there could be a difference in the dose entering the body surface from the radiation treatment plan; however, depending on the properties of the treatment devices, there is a difference in the dose affecting the body surface. Thus, the absorbed dose entering the body surface can be high. During breast cancer radiotherapy, radiation dermatitis occurs in almost all patients. Most patients have a difficult time while undergoing treatment, and therefore, when choosing a radiotherapy treatment method, minimizing radiation dermatitis is an important consideration.
