Development of Data Acquisition System for Quantification of Autonomic Nervous System Activity and It's Clinical Use (자율신경계의 활성도 측정을 위한 Data Acquisition System의 개발 및 임상응용)

  • Background: Power spectrum analysis method is a powerful noninvasive tool for quantifying autonomic nervous system activity. In this paper, we developed a data acquistion system for estimating the activity of the autonomic nervous system by the analysis of heart rate and respiratory rate variability using power spectrum analysis. Materials and methods: For the detection of QRS peak and measurement of respiratory rate from patient's ECG, we used low-pass filter and impedence method respectively. This system adopt an isolated power for patient's safety. In this system, two output signals can be obtained: R-R interval heart rate) and respiration rate time series. Experimental ranges are 30-240 BPM for ECG and 15-80 BPM for respiration. Results: The system can acquire two signals accurately both in the experimental test using simulator and in real clinical setting. Conclusion: The system developed in this paper is efficient for the acquisition of heart rate and respiration signals. This system will play a role in research area for improving our understanding of the pathophysiologic involvement of the autonomic nervous system in various disease states.

Comparative Study of Acute Dyspepsia, Functional Dyspepsia, Organic Dyspepsia by HRV(Heart Rate Variability) (심박변이도를 통한 급성, 기능성, 기질성 소화불량증의 비교연구)

  • Objectives : This study was designed to evaluate the correlation between patterns of dyspepsia(acute dyspepsia, functional dyspepsia, dyspepsia with the organic disease) and autonomic nerve system dysfunction using HRV analysis. Methods : The patient group consisted of 88 patients diagnosed as a dyspepsia (Acute dyspepsia group-35 patients, Functional dyspepsia group-28 patients, Dyspepsia with the organic disease group-28 patients) who visited in the Oriental Medical Hospital of Dong-eui University from 2005.3.OO to 2008.8.OO. And the control group consisted of 33 patients diagnosed as a normal state of stomach during the same period. We checked HRV of the 4 groups over 5 minutes and compared the HRV index between groups. Results : 1. HF, LF, VLF and TP were significantly lower in the acute dyspepsia, functional dyspepsia and organic dyspepsia patient group than in the control group. 2. HF, LF, VLF and TP were higher in the acute dyspepsia patient group than in the organic dyspepsia patient group, but the differences were not statistically significant. 3. HF, LF, VLF and TP were higher in the acute dyspepsia patient group than in the functional dyspepsia patient group, but the differences were not statistically significant. 4. HF, LF, VLF and TP were lower in the functional dyspepsia patient group than in the organic dyspepsia patient group, but the differences were not statistically significant. Conclusions : Compared to the control group, all of the dyspepsia patient groups showed the tendency that the overall activity of the autonomic nervous system and the activity of sympathetic nerves decreased. Although there was no significant difference in the suppression of the autonomic nervous system, chronic dyspepsia patient group was lower than acute dyspepsia patient group, functional dyspepsia patient group was lower than the organic dyspepsia patient group in HRV.

  • Congenital central hypoventilation syndrome (CCHS) is a disorder of the autonomic nervous system characterized by a decreased response to hypercarbia. CCHS is frequently associated with congenital megacolon; the combination is called Haddad syndrome. CCHS is associated with dysfunction in respiratory features of the autonomic nervous system and with other disorders, including facial deformities, cardiovascular symptoms, and tumors. Patients with CCHS frequently have a mutation in the homeobox protein 2b (PHOX2B) gene. Most mutations involve heterozygous expansion of alanine repeats (GCN). Interestingly, a higher polyalanine repeat number is associated with a more severe clinical phenotype. To clarify the role of PHOX2B in disease pathogenesis, we introduce and review the clinical and molecular features of CCHS and Haddad syndrome.

Indicator of Diagnosing Fatigue by Analyzing Questionnaire Data (설문지를 이용한 피로 진단 지표 연구)

  • Objectives This study was designed to development the diagnosis indicators for the fatigue group without disease using analysis of questionnaire data. Methods The subjects were divided into three groups, the fatigue group with disease, the fatigue group without disease, non-fatigue group without disease. Thirteen kinds of questionnaire (Fatigue visual analogue scale, Fatigue severity scale, Pain visual analogue scale, Short-form McGill pain questionnaire score, Headache impact Test-6, Neck disability index, Oswestry low back pain disability questionnaire score, Blood stasis assessment scale, Physical activities questionnaire score, Beck depression inventory, Anger expression inventory score, Mood disability scale, Job stress scale) were carried out for the subjects. Results The results of this study are ; 1) The weight, the systolic and diastolic blood pressure were different among three groups. 2) Among nine questionnaires related to the physical or life conditions, the averages of eight questionnaires were not same one another. 3) Among four questionnaires related to the mental condition or autonomic nervous system, the averages of Beck depression inventory were not same one another. Conclusion The fatigue group without disease can be diagnosed through using the both the questionnaires related to the physical or life condition and the those related to the mental condition or autonomic nervous system.

  • We present a patient with a primitive neuroectodermal tumor arising from the right atrium who experienced multiple syncope episodes daily, which had first appeared 1 month after surgery. The symptoms continued to worsen over the course of chemotherapy, and the autonomic function test (AFT) was performed after the 14th chemotherapy cycle. The AFT revealed orthostatic hypotension and reduced baroreflex-dependent sympathetic reactivity. Physical counterpressure techniques were applied with a visual biofeedback intervention, and were found to be effective in reducing the syncope episodes.

Effect of Electro Acupuncture on Heart Rate Variability of Patients with Functional Voiding Disease (배뇨장애 환자에 대한 전침치료가 심박 변이도에 미치는 영향)

  • Objectives : This study was designed to evaluated the effect of electro acupuncture on Heart Rate Variability of Patients with functional voiding disease. Methods: Subjects were voluntarily recruited by newspapers and internet. Electro acupuncture was performed three times a week for 3 weeks. Acupuncture point for EA group was B32, Electrical stimulation frequency was 2Hz, duration 20min, and intensity was up to pain threshold according to patients. The patients' Heart Rate Variability was measured before, after 3 weeks of treatment for 5 minutes. Results : In time domain analysis, There were significant increase of SDNN and RMSSD(p<0.05) after 3 weeks treatment. In frequency domain analysis, There were significant increase of LF and LF /HF ratio(p<0.05) after 3 weeks treatment. There were no significant changes in other components of heart rate variability. Conclusion : This study suggests that electroacupuncture treatments can be applicable to improve autonomic nervous system in patients with functional voiding disease.

A Study of the Relationship between Granulocyte/Lymphocyte Ratio and Dysmenorrhea, History of Gynecological Disease, Heart Rate Variability in Some Young Women (과립구/림프구 비율과 일부 젊은 여성들의 월경양상, 산부인과적 질환 과거력 유무, HRV와의 관계)

  • Objectives: The purpose of this study is to investigate the relationship between granulocyte/lymphocyte ratio and dysmenorrhea, history of gynecological disease, heart rate varibility(HRV). Methods: From May 1st 2011 to July 10th 2011, a total of 40 female subjects (age 20-39) were recruited. We evaluated the menstruation condition by questionnaires including VAS(Visual Analog Scale), VRS(Verbal Rating Scale) and MVRS (Multidimensional Verbal Rating Scale), also measured subject's WBC differential count and HRV. Results: 1. There was no statistically significant difference in granulocyte/lymphocyte ratio according to menstruation cycle, duration, amount, color, and blood clot. 2. There was no statistically significant difference in VAS, VRS, MVRS mean scores between normal group and abnormal group according to granulocyte/ lymphocyte ratio. 3. The past prevalence of gynecological disease of abnormal group showed significantly higher compared with that of normal group($x^2$=6.578, p=0.010). 4. LF/HF ratio and granulocyte ratio significantly showed positive correlation ($r_s$=0.311 p=0.048) and LF/HF ratio and lymphocyte ratio significantly showed negative correlation($r_s$=-0.319 p=0.045). Conclusion: These findings suggest that the gynecologic diseases are related to disorder of autonomic nervous system, but not dysmenorrhea. And the balance state of sympathetic/parasympathetic nerve infered through granulocyte/lymphocyte ratio and HRV seems to be consistent. However, reconfirmation through further studies is needed.

Factors Affecting Heart Rate Variability in the Industrial Workers (사업장 근로자의 심박동 변이도에 영향을 미치는 요인)

  • The purpose of this research is to seek for efficient health maintenance device and to suggest desirable daily habit, based on the inquiry on interrelationship between workers' daily lives and their heartbeat change level. The paper survey about general features, case history and daily habits was conducted on workers during medical examination in Jeollabukdo, and examined their change of heartbeat as well. The results of research deducted from data analysis are as follows; 1. There found very positive interrelations between time-domain analysis and frequency-domain analysis, and MHR and LF/HF ratio had negative connection with other analyses. 2. The recipient showed high time-domain analysis when they are younger, have worked shorter or have spouse, and it contributes to stable sympathetic nerve and parasympathetic nerve as it stimulates autonomic nervous system. 3. According to the result of frequency-domain analysis, recipients showed higher TP and LF when they are younger, and the highest HF when they are under 34.The level of VLF was higher for university graduates than the ones who finished high school. The recipients showed higher TP and HF when they don't have spouse, and lower TP, LF and HF when they have worked longer. 4. The level of RMSSD and TSRD was high for the people who don't have case history, and HF was high when they don't have any disease in progress. 5. According to the result concerning correlation of daily habits with time-domain analysis and frequency-domain analysis, cigarette, alcohol and sleeping hours don't affect heartbeat change, but the ones who regularly workout showed higher result in every analysis. It shows that the autonomic nervous system of recipients who regularly exercise response more actively. The result mentioned above suggests that the change of heartbeat is a direct index which shows the change of autonomic nervous system, and it depends on the exercise the most. Thus, workout is proved to be the best method in order for workers to take care of their health.

A case of Acute Autonomic and Sensory Neuropathy (급성 자율 감각 신경병증 1례)

  • Acute autonomic neuropathy is a rare disease. Since the first case was reported by Young et.al., in 1969, a number of similar cases have been described, with some variation of the accompanied neurologic deficits. Acute autonomic and sensory neuropathy(AASN) is characterized by the acute onset of autonomic dysfunction and sensory disturbances. A 16-year-old girl experienced high fever($40^{\circ}C$) and erythematous rash on whole trunk and face followed by pain and sensory loss over the whole body, dysphagia, ataxia, urinary retention, and postural hypotension. There was no evidence of limb weakness. The electrophysiologic studies of this patient revealed sensory polyneuropathy and the various autonomic function test showed autonomic dysfunction. The recovery of her autonomic and sensory symptoms is incomplete, three months after the onset of the symptoms. The etiology of the acute autonomic and sensory neuropathy is not known. Most previous authors have suggested the dysautonomia may be an acute immunological damage to peripheral fibers of the autonomic nervous system. We report a case of acute autonomic and sensory neuropathy.
