Comparison of Student′s Clinical Competency in Different Instructional Methods for Fundamentals of Nursing Practicum (기본간호학 실습교육방법에 따른 학생들의 간호수행능력의 비교)

  • The main purpose of this study was to compare the clinical competency in different instructio nal methods for funda- mentals of nursing practicum, standardized patients methods story as text method ,and traditional lecture/model method. Method: The study was designed as a quasi-experimental, nonequivalent control group post-test design with three separate classes of sophomore students attending fundamentals of nursing classes at one baccaleureate nursing school located in metropoli tan Seoul area. Control group was taught by traditiona lecture/ model method and two experimental groups were taught by standardized patients method and story as text method. Data were collected from September, 1999 to June 2001. There were 36 students in the standardized patient method group, 38 students in story as text group, and 40 students in the control group. Data analysis was done using SPSS WINDOW 9.0. Result: The results showed that the standardized patients method and story as text method groups were significantly better in clinical judgement and communication skills than the traditional lecture/model method group. The standardized patients method group was significantly better in clinical nursing skills performance than two other groups. However, there was no significant difference among the three groups in student satisfaction. Conclusion: The standardized patients method is an effective in teaching clinical cometency for student nurses. It is necessary to explore more efficient way to develop standardized patients cases for wider areas of nursing education. Also, it is recommended to develop more research projects with many nursing programs.

Renal Problems in Early Adult Patients with Turner Syndrome

  • Purpose: This study aimed to evaluate the status of renal function and the presence of urinary abnormalities in early adult patients with Turner syndrome (TS). Methods: Sixty-three girls with TS, who are attending pediatric endocrine clinics in Busan Paik Hosp., were studied. Urine and blood chemistry tests were performed in every visiting times. Renal ultrasonography was performed in all patients at the initial diagnosis, and intravenous pyelography, DMSA renal scan and renal CT were also performed, if necessary. Results: Of the 63 patients, the karyotype showed 45,X in 32 (50.8%), mosaicism in 22 (34.9%) and structural aberration in 9 (14.3%). The renal function at the latest visit was shown as normal in all patients. Nephrotic syndrome had developed in one patient. Hematuria was observed in seven patients. Renal anomalies were observed in 20 of the 63 TS (31.7%). Of the 32 TS patients with 45,X karyotype, 13 (40.6%) had renal anomalies, while these were found in 7 (22.6%) of 31 TS patients with mosaicism/structural aberration. But there was no significant statistical difference between two karyotype groups. Conclusion: Based on this study, most of the patients with TS do not have any significant problems related to renal function until early adulthood, regardless of renal malformation or hematuria.

Characteristic and Quality of Life in Patient with Tinnitus who visit the Korean Medical Hospital (한방병원에 내원한 이명환자의 특성 및 삶의 질 분석)

  • Backgroun : Tinnitus is common disorder with many possible causes and a symptom of many different diseases but has no effective treatment. Approximately 20% of tinnitus patients experience the disorder to a degree that their quality of lift and productivity are impaired. Objective : This study was conducted to investigate the characteristic of the tinnitus patients and the impact of tinnitus on quality of life. Methods : 103 patients un recruited in this study who was ill with tinnitus. They wrote out the queslicumaire about tinnitus and the Tinnitus Handicap Inventory(THI). Result : THI score have relation to set the period of birth, the loudness of tinnitus and the items of attending symptoms. Conclusion In this study, tinnitus have influence on patients' quality of life. We expect this study to provide the base data for the effective treatment and the high qualify of patients' life.

An Initial Practice Experience of EMT Students in Fire Station (응급구조과학생의 첫 소방 실습 경험)

  • This study was conducted to identify an initial fire station experience of EMT students, so to better understand their' practice experience in fire station. The subjects were 28 EMT students of C department of Emergency Medical technology in C city, who were demonstrating at the fire station in C city. This study was approached by phenomenological method, collected data were analyzed by Colaizzi's method, the results were as a follows. From the protocol, 201 significant statements were organized into 93 formulated meanings. From the formulated meanings, 30 themes were identified, organized into 16 theme clusters, and then 7 categories. EMT students got experienced 'tension' in resulting from new training situation and at the field practice, 'comport and gratitude', in feeling of identity and a bond sympathy with senior EMT, in training environment and heartfelt care, 'stress' from lack of knowledge and skill, difficulties in field practice for 24 hours, in dealing with making interpersonal relationship with patient and staffs and from the insufficiency of instruction, 'confidence feeling' from the participation of field treatment, in improving of learning, in self-esteem of EMT job, 'confusing feeling' of conflict of the path in work, 'disappointment and doubt' by the discrepancy between learning and actuality, and disappointment of actuality, 'feeling of lack' based on the passive attitude. The results of this study are to use as basic data for students attending fire station experience for the first time.

Respiratory support with heated humidified high flow nasal cannula in preterm infants

  • The incidence of bronchopulmonary dysplasia (BPD) has not decreased over the last decade. The most important way to decrease BPD is by weaning the patient from the ventilator as soon as possible in order to reduce ventilator-induced lung injury that underlies BPD, and by using a noninvasive ventilator (NIV). Use of a heated, humidified, high flow nasal cannula (HHHFNC), which is the most recently introduced NIV mode for respiratory support in preterm infants, is rapidly increasing in many neonatal intensive care units due to the technical ease of use without sealing, and the attending physician's preference compared to other NIV modes. A number of studies have shown that nasal breakdown and neonatal complications were lower when using a HHHFNC than when using nasal continuous positive airway pressure (nCPAP), or nasal intermittent positive pressure ventilation. The rates of extubation failure during respiratory support were not different between patients who used HHHFNC and nCPAP. However, data from the use of HHHFNC as the initial respiratory support "after birth", particularly in extremely preterm infants, are lacking. Although the HHHFNC is efficacious and safe, large randomized controlled trials are needed before the HHHFNC can be considered an NIV standard, particularly for extremely preterm infants.

The Clinical Study of Urticaria (두드러기 치료 조사)

  • This study was clinically performed with the 73 outpatients of urticaria who had visited the Dept. of Dermartology, Oriental Medical Hospital, Dong-eui University from January 1st 2000 to June 30th 2003. They have all taken the oriental herb medicine as Sungmagalguntanggamibang(SGT, 升麻葛根湯加味方) more than 1 time. Urticaria is a common disease and a well known reaction pattern of the skin charaterized by erythema, wheals and edema, that is the results of various causes. Urticaria is classified into acute or chronic type. The etilogy of acute uricaria has been determined in many cases, but the cause of chronic urticaria has been determined in 10$\%$ to 20$\%$ of cases. Character of chronic type urticaria is recurrent pruritus and erythematous skin disease by stimulating factor. Through checking up the present patient state, we verified the effect of SGT in various ways. The results were summarized as follows; 1. An increase of taking a medicine(SGT) and attending a hospital in number let down repeated urticaria occurence. 2. SGT is more effective when the symptoms with the severe pruritic wheals are spread on the whole body in daily occurrence. 3. In urticaria treatment method, gradually reducing a western medicine and going side by side SGT, that is good at symptoms improvement. 4. Etiology of urticaria is classified into intestine-stomach accumulated heat by indigestion(食滯腸胃積熱), wind-moisture(風濕), wind-heat(風熱) etc. The SGT is available in the order of frequency of use.

The Effects of the Designated Doctor System on the Health of Medical Aid Beneficiaries (선택병의원제가 의료급여 수급권자의 건강에 미치는 영향)

  • Purpose: Medical Aid Beneficiaries were surveyed to identify differences in health behaviors, adherence to drug regimen, and quality of life between those people in the Designated Doctor System and those who are not. Methods: A total of 1,327 study subjects were separated into three groups: those in the Designated Doctor System for 2 years, those in for 1 year, and those not in the system. Results: After the introduction of the Designated Doctor System, 55.8% and 67.9%, respectively, of the subjects in the Designated Doctor System complained of inconvenience in relation to hospital use and the patient referral process. Also, the rate of emergency room use or hospitalization guided by the Designated Doctor System was only 8.7% and 6.5%, respectively. There were no significant differences in health behaviors and adherence to drug regimens between those in the Designated Doctor System and those who are not. Conclusion: This study was carried out early in the introduction of the system. Therefore, it is necessary to monitor the positive and negative effects of the Designated Doctor System for a full reflection of its impact.

Effects of the Supportive Program on the Chronic Arthritis Patients (지지 프로그램이 만성 관절염환자의 생리적, 사회$\cdot$심리적 상태와 건강지각에 미치는 효과)

  • The purpose of this study is to identify the effects of the supportive program for arthritis patients. who are attending a rheumatoid arthritis clinic. To achieve this purpose. this study adopted a quasi-experimental. pre- and post-test research design. comparing experimental group with control one. Supportive program was composed of in-depth. 3 times of direct interview and 2 times of advice using telephone for 8 weeks. The object of these interviews and phone was focused on the improvement of patients' preception for health. During this period. the level of pain, hemato-immunologic indices(ESR, CRP). self-efficacy, depression. and perception for health were measured in both grooups. Data were analysed by $x^2-test$, t-test. repeated measures ANOVA and Pearson's correlations. The results were as follows : 1. There were no significant differences in physiological data. 2. The feeling of self-efficacy was significantly increased in experimental group(P=.012), 3. There was no significant differences in depression. 4. The perception for health status was significantly increased in experimental group(P=.002). Thus, the supportive program. which is focused on the close interpersonal communication. proved to be effective. This result justifies the following suggestion that the role of the nursing professionals in out-patient clinic should be extended for more qualified care for the patients.

First Korean Case of 16p11.2 Duplication Syndrome Diagnosed by Chromosomal Microarray Analysis

  • A 10-year and 5 month-old girl with developmental delay, intellectual disability, attention deficit hyperactivity disorder, poor weight gain, and microcephaly was transferred to our pediatric clinic for genetic evaluation. Her height was within the 5-10th percentile, and her weight was under the 3rd percentile. On the social maturity scale, her developmental status was scored as 3 years 9 months for social age, and the social quotient was 35.98. A chromosomal microarray analysis was performed and the microduplication at chromosome 16p was observed: arr[GRCh37] 16p11.2 (29580020_30190029)${\times}3$. Currently, the patient is diagnosed with Grade 2 intellectual disability and is attending a computerized cognitive rehabilitation class twice weekly. In addition, nutritional support and growth follow up are also ensured in the Pediatric Gastrointestinal and Endocrinology clinic.

A systematic review of continuing education for emergency medical technicians (응급구조사의 보수교육에 대한 체계적 문헌고찰)

  • Objectives: This study aimed to combine the satisfaction survey results and suggestion for continuing education of the emergency medical technicians through a systematic review of continuing education of the emergency medical technicians. Methods: This review focused on previous studies published until 2020 that were associated with the continuing education of emergency medical technicians. We followed the guidelines set for each phase of the study and selected the 26 studies that met the selection criteria for the analysis. Results: The level of satisfaction with continuing education for emergency medical technicians was low. The most frequently suggested method for continuing education was patient case presentation. The answers to the desired training subjects of the continuing education program varied. Finally, studies on continuing education have not been published continuously. Conclusion: To enhance the satisfaction and competence of continuing education, we need to provide various programs that reflect the needs of emergency medical technicians attending maintenance training. In addition, various methods should be considered to improve the attendance rate for continuing education, and further investigations and research should be actively conducted on continuing education in the future.
