Two cases of Atopic Dermatitis Treated with Chijabyukpi-tang (치자벽피탕(梔子檗皮湯)으로 호전을 보인 아토피피부염의 치험 2례)

  • Objective : This case report presents the effect of Chijabyukpi-tang(梔子檗皮湯) on Atopic Dermatitis. The patient was diagnosed with Yangmyung-Yeoljeung(陽明熱證) based on Shanghanlun provisions. Methods : We have classified the patients according to the shanghanlun six meridian patterns diagnostic system and the patients are treated with Chijabyukpi-tang. The severity of Atopic dermatitis was evaluated by SCORAD index, visual analouge scale(VAS) and pictures. Results : After the treatment, the SCORAD index of 2 patients decreased from 78 to 20.2 and from 67.4 to 38.2 respectively. Conclusions : Although Chijabyukpi-tang is unusual treatment for Atopic dermatitis, this case report showed the effectiveness of using Chijabyukpi-tang on Atopic dermatitis according to shanghanlun six meridian patterns diagnostic system.

Clinical Efficacy of External Preparation Containing Herbal Extracts m Atopic Dermatitis Patients (아토피 피부염 환자에서 한방 추출물이 포함된 외용제품에 대한 임상 효과)

  • Background : Atopic dermatitis is a chronic and recurrent allergy eczema, called as 'Tae-yul'. It is a characteristic dermatitis, diagnosed from baby to adult, that carries severe itching. Objectivd : The aim of this trial is to examine the effect and safety of Medibebe in patients with atopic dermatitis. Methods : We made a comparison between before and after the use this product. Volunteers who satisfied the requirements were enrolled in the study. Degrees of severity of atopic dermatitis were measured by SCORAD index. Results : After 4 weeks' use of Medibebe, SCORAD index showed a decrease of $20.0{\pm}10.2$(95% C.I: 17.1-22.9) compared with when it was not applied, which is a statistically meaningful change(p-value<0.0001). Subjective pruritus and Sleeping loss reported after both 2 and 4 weeks' use of Medibebe decreased in a statistically meaningful way(p-value<0.0001) compared with before the application. but, 2 cases out of all 50 subjects for the clinical trial were excluded on the way. Conclusion : As a result of applying the 4 items of Medibebe(cream, lotion, soap, and body cleanser) to patients with atopic dermatitis and watching the progress, it is concluded and considered that these cleansing and moisturizing products can be used by atopic patients safely and effectively with almost no side-effect.

Parenting Stress and Needs of Mothers of Children with Atopic Dermatitis (아토피 피부염 환아 어머니의 양육 스트레스와 간호 요구)

  • Purpose: The main purposes of this descriptive survey study were to describe parenting stress and needs of mothers of children with atopic dermatitis. Method: Data were collected from 111 mothers of children under 6 year-old with atopic dermatitis registered at one university affiliated medical center in Seoul. The instruments used for this study were a self-report questionnaire, Parenting Stress Index (PSI) Short Form by Abidin, and an open question to identify nursing needs. Descriptive analysis, Pearson correlation and contents analyses were done. Result: The mean score for parenting stress was 77.25 (range: 12-60), for parent domain 31.50 (range: 12-60), parent-child domain 21.02 (range: 11-55) and child domain was 24.72 (range: 11-51). There was a significant positive relationship between parenting stress and severity of symptom (r=0.192, p<.005). The maternal stressors included extra housework related to symptom management, treatment related to atopic dermatitis, personality and behavior of child, and financial problems. Conclusion: the mothers of children with atopic dermatitis have high parenting stress from various stressors and needs. It is recommended that nursing interventions should include stress management and take into consideration the educational needs of mothers.

  • 1. Objectives: The purpose of this study is to evaluate the effects of Yangkyuksanhwa-tang which is based on sasang constitutional medicine for dermatitis as psoriasis, atopic dermatitis. 2. Methods: This two patients were treated by Soyangin's constitutional medications according to the result of sasang constitutional diagnosis. We evaluated the severity in dermatitis that two methods ; doctor's view and patient's subjective itching. 3. Results & Conclusoins: This cases study shows an efficent result of using Yangkyuksanhwa-tang in treatment of dermatitis as psoriasis, atopic dermatitis.

  • Atopic dermatitis, caused by immune hyper-responsiveness, is wide spread in humans as well as in the dogs, especially in industrialized condition. Pet dogs are generally exposed to the same environment as their owners, and a significant portion of these animals are also known to suffer from this allergic disease. However, diagnosis and treatment methods of atopic dermatitis in animals have not been well established. We explored the possibility of using recently developed PG102 for the treatment of atopic dermatitis in the canine population. PG102 is a water soluble extract prepared from Actinidia arguta, and has been shown to produce significant therapeutic effect in variable allergy animal models. After oral administration of PG102 at a dose of 12.5 mg/kg daily for 4 weeks, severity of disease was greatly improved. IgE is one of representative members used to diagnose allergic diseases in humans. However, it is not well established whether there is any correlation between the serum level of IgE and atopic dermatitis. Our data indicated that dogs diagnosed to have atopic dermatitis contained higher level of serum IgE than the normal dogs and that treatment of dogs with PG102 significantly lowered the serum level of IgE. Taken together, this study demonstrated that PG102 treatment yielded significant amelioration of canine atopic dermatitis and down-regulation of serum IgE and that the serum level of IgE can be used as a convenient member for diagnosis of atopic dermatitis in dogs.

Effect of Gupoongjeseuptang (GPJST) on DNCB (dinitrochlorobenzene)-induced Atopic Dermatitis-like Model NC/Nga Mice (구풍제습탕(驅風除濕湯)이 DNCB로 유도된 NC/Nga mice의 아토피 피부염에 미치는 영향)

  • Objectives : The purpose of this study is to investigate the effect of Gupoongjeseuptang (GPJST) on atopic dermatitis by in vivo experiment using NC/Nga atopic dermatitis mouse, which has histological and clinical similarities to the atopic dermatitis of human. Methods : To investigate the effect of GPJST on atopic dermatifis, we evaluated atopic dermatitis-like skin lesions by clinical skin index and analyzed immunological parameters in peripheral blood mononuclear cells (PBMCs), splenocytes, draining lymph node (DLN) and performed skin histology in ears and dorsal skin of atopic dermatitis-like skin NC/Nga mouse in vivo. Results : In vivo, clinical skin severity score were significantly lower in GPJST group than control group. IgE, IL-6, $TNF-{\alpha}$, IgG1, IgM, IgG2a and IgG2b levels in serum decreased remarkably in GPJST group than control group. Also, total absolute number of $CD3^+CD69^+$, and $CCR3^+$ cells recovered as normal in PBMCs and $CD3^+$, $CD3^+CD69^+$ decreased significantly compared with control group in isolated DLN from NC/Nga mouse and total absolute number of $CD11b^+Gr-1^+$, $CCR3^+CD3^+$ in dorsal skin of NC/Nga mouse decreased by GPJST. We analyzed ear and neck-back skin after biopsy and dyeing by hematoxyline/eosin (H&E) and toluidine staining (mast cells marker) and obtained results that GPJST are very effective to histological symptoms (dermal and epidermal thickening, hyperkeratosis and inflammatory cell (CD4, $CCR3^+$) infiltration). Conclusions : This study demonstrates immunological activity of GPJST on atopic dermatitis-like model mice.

  • This report describes the effectiveness of mahaenggamseog-tang(麻杏甘石湯) on the patients who suffered from atopic dermatitis and psoriasis. We administered mahaenggamseog-tang with heat-clearing and lung-dispersing(淸熱宣肺) effect to two dermatitis patients who showed sweating with panting(汗出而喘), vexation and thirst(煩渴), aversion to heat(惡熱), agitation(煩燥), swelling of the face and eyes(面目浮腫). The severity of dermatitis was evaluated by visual analogue scale(VAS), pictures, SCORAD index(case 1), PASI(case 2). After the treatment, VAS/SCORAD index of case 1 changed from 9/71.5 to 0/6.1; VAS/PASI of case 2 from 8/36.8 to 1/6.8. Mahaenggamseog-tang could improved the symptoms of atopic dermatitis and psoriasis in this study. This study offers an approach to managing chronic dermatitis more effectively using Mahaenggamseog-tang which used mainly for bronchial diseases and obesity.

Comparing the Therapeutic Effects of Aloe vera and Olive Oil Combination Cream versus Topical Betamethasone for Atopic Dermatitis: A Randomized Double-blind Clinical Trial

  • Objectives: Atopic dermatitis (AD) is a prevalent and chronic, pruritic inflammatory skin condition that can influence all age groups. AD is associated with a poor health-related quality of life. This randomized clinical trial was performed to compare the effectiveness of Olivederma (combination of aloe vera and virgin olive oil) or betamethasone regarding disease severity, quality of life, serum IgE and eosinophil count. Methods: Thirty-six AD patients were randomly allocated to topical Olivederma or betamethasone, and were followed for 6 weeks. Results: Total SCORAD severity scores showed significant decrease in both groups, while it was more prominent in Olivederma group (64.5% improvement in Olivederma vs. 13.5% improvement in Betamethasone, p-value < 0.001). Quality of life (DLQI questionnaire) of AD patients was significantly improved after 6 weeks treatment with Betamethasone (22.3%, p < 0.001) and Olivederma (60.7%, p-value < 0.001). Olivederma group showed a significantly lower DLQI score in comparison with Betamethasone treated patients after 6 weeks of therapy (p < 0.001). Improvements in eosinophil count and serum IgE was observed. Conclusion: In summary, this study shows that Olivederma is superior to topical Betamethasone after 6 weeks of therapy with regard to disease severity, quality of life and eosinophil count.

Evaluation of the Atopic Dermatitis-mitigating and Anti-inflammatory Effects of Kyung Hee Allergic Disease Herbal Formula (KAHF)

  • The purpose of this study was to investigate the effects of Kyung Hee Allergic Disease Herbal Formula (KAHF) on atopic dermatitis (AD) and its mode of action. Our clinical study showed KAHF reduced Severity Scoring of Atopic Dermatitis (SCORAD) indexes and subjective symptom scores. In parallel, the decreased levels of interferon (IFN)-$\gamma$ and interleukin (IL)-5 in serum, which contributed to its AD-mitigating effect was observed. To reveal the underlying mechanisms of KAHF in AD, its anti-inflammatory effect on lipopolysaccharide (LPS)-induced responses in RAW 264.7 cells was examined. KAHF was found to significantly inhibit the productions of nitric oxide (NO), prostaglandin $E_2$ ($PGE_2$), and IL-$1{\beta}$ in LPS-stimulated RAW 264.7 macrophages. Consistently, KAHF potently inhibited protein and mRNA expressions of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2). Furthermore, KAHF inhibited LPS-induced activation of nuclear factor (NF)-$\kappa}B$. Taken together, our data suggest that KAHF has a beneficial effect on several eicosanoid-related skin inflammations, such as atopic dermatitis.

Isoegomaketone Ameliorates Atopic Dermatitis via MAPK and STAT Pathway-based Pro-Inflammatory Cytokine Regulation

  • Isoegomaketone(IK), isolated from the radiation-induced mutant cultivar of Perilla frutescens var. crispa, is a major phytochemical compound that has attracted attention in pharmacological research. In this study, we demonstrated that IK exerts anti-inflammatory and protective effects on human mast cells and in an atopic dermatitis mouse model. IK inhibited tumor necrosis factor-α(TNF-α), interleukin-6 (IL-6), and IL-8 expression in human mast cells (HMC-1) stimulated with phorbol myristate acetate(PMA) and calcium ionophore A23187 (PMACI). IK significantly reduced the PMACI-induced phosphorylation of ERK and JNK, but not p38. IK also inhibited the PMACI-induced phosphorylation of STAT1 and STAT3. Oral administration of IK in atopic dermatitis mouse model ameliorated skin inflammation severity, as measured by skin thickness and pro-inflammatory cytokine levels such as TNF-α, IL-8, IL-4, and IL-13. These results might suggest that IK is a potent therapeutic agent against skin inflammation and atopic dermatitis.
