Objective : to investigate the modalities and treatments with atopic dermatitis. Methods : We made up a questionnaire consisting of symptoms, birth, family history, previous treatments of atopic dermatitis. We interviewed mild to severe 60 atopic dermatitis patients with questionnaire, from August to October, 2008 who were participating in clinical trials of Kyunghee University East-west neo medical center. Results : The results obtained are summarized as follows: 1. 49(81.7%) out of 60 patients reported previous or current use of medical treatment, and most common type of medical treatment was topical corticosteroid (95.9%). Oral antihistamine (61.2%), oral corticosteroid (61.2%) were also commonly used. 2. 31(51.7%) out of 60 patients reported previous or current use of oriental medical treatment, and most common type of oriental medical treatment was herbal medicine(100%). Acupuncture (41.9%), Herbal external supplements (22.6%) were also used. 3. 36(60.11% ) out of 60 patients reported previous or current use of folk remedies, and the most common type of folk remedy was extract of tree(Mokcho-ak)(75.0%). Aloe(61.1%), Charcoal(22.2%) were also commonly used. Conclusion : It seems the frequency of folk remedies used were related to severity of AD. However, there were no significant difference between medical treatment and oriental medical treatment.
A combined prescription of GGB and CHS (G&C prescription group) was used to investigate its effects on immune related factors and histological changes in atopic dermatitis(AD) induced mice. Significant decrease of atopic dermatitis clinical index in G&C prescription group. In DLN, G&C prescription group significantly modulated the immune cells. G&C prescription group also showed significant effect on the immune cells of the dorsal skin as well as DLN. The group indicated significant decrease of the biosynthesis of IL-4, IL-5, IL-13, GM-CSF, cytokines in serum. On the other hand, the biosynthesis of TNF-$\alpha$ was decreased. G&C prescription group significantly decreased the immunoglobulin IgE levels in serum. The results suggest that G&C prescription significantly improves atopic dermatitis through regulation of immune cells and cytokines. Comparative studies with Protopic ointment also showed that G&C prescription showed significant effect in AD patients, and active application of the prescription in clinicals is anticipated. However, the reason for the results that oppose to those of previous studies should be investigated. Also, therapeutic effects of both internal and external applications should be studied individually as well as for any synergistic effects. Safety, toxicity, as well as stability studies should follow to develop G&C prescription into long-term external clinical product.
Objective : The objective of this study is to report a case of atopic dermatitis (AD) treated with Hoeyeok-tang. Methods : We retrospectively reviewed the course of herbal medicine treatment in a 20-year-old male patient with a topic dermatitis. Changes in main symptoms were confirmed based on the gross findings and assessed using SCORing Atopic Dermatitis(SCORAD) Index. Accompanying symptoms and side effects that can be referred to for diagnosis were analyzed By referring to the clinical records. Results : According to the Shanghanlun provision, the patient's AD was diagnosed with Terminal-yin Gwakran 389th provision. The patient was treated with Hoeyeok-tang for 90 days. The severity of pruritus decreased, and SCORAD Index was decreased from 44.6 to 8.4. VAS for eye fatigue and nausea was decreased from 10 to 5, also. No particular side effects occurred to the patient while taking the Hoeyeok-tang. Conclusions : The result suggests the clinical applicability of 389th provision of Shanghanlun.

Atopic dermatitis (AD) is a chronic inflammatory skin disease and the pathogenesis of AD is associated with the release of various cytokines/chemokines due to activated $Th_2$ immune responses. Synthetic oligodeoxynucleotides (ODNs) containing unmethylated CpG dinucleotide in the context of particular base sequence (CpG motifs) are known to have the immunostimulatory activities in mice and to convert from Th2 to Th1 immune responses in AD. We aimed to investigate that CpG ODN, especially phosphodiester form, can stimulate the protective immunity in NC/Nga mice with AD. We isolated BMDCs from NC/Nga mice and then, cultured with GM-CSF and IL-4 for 6 days, and treated for 2 days by either phosphorothioate ODN or phosphodiester ODN. CpG ODN-treated DCs resulted in more production of IL-12. When CpG ODN-treated DCs were intravenously injected into the NC/Nga mice, the NC/Nga mice with CpG ODN-treated DCs showed significant improvement of AD symptoms and decrease of IgE level. Histopathologically, the NC/Nga mice skin with CpG ODN-treated DCs showed the decreased IL-4 and TARC expression comparing with non-injected mice. These results may suggest that phosphodiester CpG ODN-treated DCs might function as a potent adjuvant for AD in a mouse model.
Objectives : Atopic Dermatitis (AD) is chronic skin disease characterized by allergic hypersensitivity reactions. Samhwangsasim-tang Gamibang(SHSST) can treat skin disease by cooling down blood heat, clearing away congenital fever and detoxicating. This study was conducted to investigate the effects of SHSST on AD induced by oxazolone in mice. Methods : We investigated the effects of SHSST on skin tickness, skin lesions, erythema and melanin index, water content and histopathological changes in vivo. The effects on body weights and spleen body weight ratio were also investigated in mice with AD. Results : In our results, oral administration of SHSST ameliorated skin lesion such as flare, erythema, exudate and petechia and lowered skin thickness induced by oxazolone. In addition, SHSST lowered erythema index and elevated water content compared to AD control respectively. In the histopathological observation, oral administration of SHSST prevented epidermal hyperplasia and spongiotic changes which is a hallmark of skin inflammation respectively. Finally, SHSST did not affect spleen/body weight ratio. Conclusions : These data imply that SHSST can ameliorate skin lesion of AD such as flare, erythema, exudate and petechia, and be used to treat AD patients with relative safety.
Food allergies play an important role in Atopic Dermatitis (AD). Dietary manipulation is essential in the management of AD. However, there has been a paucity of data reporting the prevalence of food allergies in AD patients in Korea. In this study, the Food Open Challenge Tests (FOCT) were conducted to investigate food allergies in AD patients. The skin-prick test and the detection of specific IgE, as well as allergy history of patient were used to detect food allergies in all AD patients. Elimination diet was conducted for two weeks prior to FOCTs. The prevalence of food allergies by FOCT is as follows: milk (67.3%); chicken (64.2%); pork (62.8%); eggs (61.0%); beef(55.4%) ; wheat (52.0%) and soybean (45.2%). Allergenic food items in Korean AD patients were different from people in other foreign countries. The seven major foods those tested positively by FOCTs were completely eliminated from the replaced diets for two weeks, and were subsequently reintroduced one at a time. Results from FOCTs were not comparable with allergy history or skin-prick tests or specific IgE detection. The sensitivity and specificity of skin-prick tests and specific IgE detection were lower than FOCTs. Allergy history, skin-prick tests, and specific IgE detection are useful for the identification of food allergen but its clinical significance differed according to food items. Therefore, we conclude that even though a 10-day delay was necessary after food challenge, FOCT is a useful and valid method to confirm food allergies and may be essential for the effective control of food allergies for treatment of AD.
Atopic dermatitis(AD) is a chronic disease that affects the skin. "Atopic" refers to a group of diseases where there is often an inherited tendency to develop other allergic conditions, such as asthma and hay fever. In AD, symptoms vary from person to person. The most common symptoms are dry, itchy skin and rashes on the face, inside the elbows and behind the knees, and on the hands and feet. Although AD may occur at any age, it most often begins in infancy and childhood. The cause of AD is not known, but the disease seems to result from a combination of hereditary and environmental factors along with malfunction of the body's immune system. In contrast to that, the results from literatual study in oriental medicine are as follows; 1. The causes of AD are considered to be Mosusigisadok(母受時氣毒), Waegampoongsubyeol(外感風濕熱), Guasiksinlayeolmul(過食辛辣熱物), Ohbokonje(誤服溫劑). 2. The symptoms are fever, flushed face, red eyes, oliguria, constipation and itching. 3. Chungyeolhaedok(淸熱解毒) would be the basic method of treatment, and it divides into two categories; Subhyung(濕型) and Gunhyung(乾型). In Subhyung(濕型), AD is treated by Josub(燥濕), Chungyeol(淸熱) and Jiyang(止痒) whereas Chungyeol(淸熱), Jiyang(止痒), Jesub(除濕) in Gunhyung(乾型). 4. To prevent further damage and enhance quality of life, it is necessary to keep the temperature and humidity favorable, and healing the skin and keeping it healthy are important. Developing and sticking with a daily skin care routine is critical to preventing flares. Changing the diet and psychological stability may also be helpful to relieve symptoms of AD.
아토피 피부염(AD)에 대한 고삼백선피복합방(GBBB)의 치료적 가능성을 증명하기 위해 동물모델에서 다양한 면역 관련 인자들과 조직학적 변화에 대한 GBBB의 영향을 평가하였다. 실험 결과는 다음과 같다. GBBB도포군은 관능평가 결과에서 AD의 개선징후를 나타냈다. GBBB 도포군은 대조군에 비해 혈액 내 백혈구, 호중구, 림프구, 단핵구 면역세포 비율이 54%, 63%, 57%, 86%로 감소되었다. 또한 GBBB도포군은 대조군에 비해 혈청 내 IL-4, IL-5, IL-13, Histamine 및 IgE 생성량이 각각 40%, 80%, 62%, 61%, 57%로 유의성 있게 감소되었다. H&E 염색에서 GBBB 도포군은 대조군에 비하여 비만세포가 피부로 침윤되는 것이 억제되었으며 피부두께 역시 상대적으로 감소되었다. 이 결과를 바탕으로 GBBB는 피부에 직접 도포하는 AD 치료제로서 가능성이 있음이 확인하였다. 후속연구에서는 분자생물학적 연구를 통해 GBBB의 면역조절 기전을 구체적으로 밝히고자 한다.
Soy protein based formula (SPF) has been developed for infants who are at a high risk for atopic dermatitis (AD) and cow's milk protein allergy (CMA). We performed this study to evaluate the therapeutic efficacy and safety of SPF compared to conventional hydrolyzed cow's milk formula (hCMF) in the feeding of infants with AD and CMA. 38 infants (12 to 24 months of age) diagnosed with CMA and AD were randomized to receive either SPF or hCMF for 12 weeks. Follow-up was conducted at 4, 8 and 12 weeks. Growth parameters of the infants were evaluated during each visit. Clinical evaluations, including AD severity scores, pruritus, specific immunoglobulin E (IgE) (cow's milk protein and soy protein) levels of peripheral blood, were made at enrollment and week 12. Analysis was performed on the 32 infants (SPF: n=16, hCMF: n=16) who completed the 12-week intervention. Eczema area and severity index (EASI) scores, a measure of the severity of AD, and pruritus were significantly reduced after 12 weeks compared to enrollment in the both groups; however, the median changes for EASI scores and pruritus were not statistically different between the two groups. The growth parameters did not differ significantly between both groups at any assessed time point. This study suggests that SPF could be useful in decreasing the severity of AD without affecting infant growth status. Therefore SPF could provide an adequate and safe alternative to hCMF in treating infants with AD and CMA during the first 12 to 24 months of their life.
Atopic dermatitis (AD) is a chronic inflammatory skin disease that induces changes in various inflammatory skin cells. The prevalence of AD is as high as 18% in some regions of the world, and is steadily rising. However, the pathophysiology of AD is poorly understood. To identify the proteins involved in AD pathogenesis, a comparative proteomic analysis of protein expression in peripheral blood mononuclear cells isolated from AD patients and healthy donors was conducted. Significant changes were observed in the expressions of fourteen proteins, including the vinculin, PITPNB, and Filamin A proteins. Among the proteins, $\alpha$-SNAP and FLNA decreased significantly, and PITPNB increased significantly in AD patients compared with control subjects; these findings were further confirmed by real-time PCR and Western blot analysis. The comparative proteome data may provide a valuable clue to further understand AD pathogenesis, and several differentially regulated proteins may be used as biomarkers for diagnosis and as target proteins for the development of novel drugs.
