  • Purpose: This study was to investigate factors influencing the Framingham risk score-Coronary heart disease (FRS-CHD) according to gender and body mass index (BMI) of adults who participated in the 5th Korea National Health and Nutrition Examination Survey (KNHANES V-3). Methods: This study used a cross-sectional design with secondary analysis with KNHANES V-3. The FRS-CHD scores were measured with ages, sex, blood pressure, cholesterol, high density lipoprotein, smoking, and diabetes mellitus. With demographic characteristics, family history of ischemic heart disease, types (intensity) and days of physical activities, perceived stress, drinking, menopause (in female), and BMI scores were measured. The data were analyzed with descriptive statistics, Pearson's correlation coefficients, and multiple regressions. Results: FRS-CHD was significantly associated with types (intensity) and days of physical activities, educational level, occupation, and marital status, explaining 19.1~76.8% of the variance in men. FRS-CHD was significantly associated with types (intensity) and days of physical activities, menopause, and education level, explaining 55.0~59.5% of the variance in women. Conclusion: Factors influencing FRS-CHD were significantly different according to gender and BMI. To reduce the risk of coronary artery disease, it is necessary to develop gender-specific physical activity programs according to BMI.

  • Ambient air pollution (AAP) and particulate matters (PM) have been closely associated with adverse health effects such as respiratory disease and cardiovascular diseases. Previous studies have examined the adverse health effects associated with short- and long-term exposure to AAP and outdoor PM on respiratory disease. However, the effect of PM size ($PM_{2.5}$ and $PM_{10}$) on cardiovascular disease has not been well studied. Thus, it remains unclear how the size of the inhalable particles (coarse, fine, or ultrafine) affects mortality and morbidity. Airborne PM concentrations are commonly used for ambient air quality management worldwide, owing to the known effects on cardiorespiratory health. In this article, we assess the relationship between cardiovascular diseases and PM, with a particular focus on PM size. We discuss the association of $PM_{2.5}$ and $PM_{10}$, nitrogen dioxide ($NO_2$), and elemental carbon with mortality and morbidity due to cardiovascular diseases, stroke, and altered blood pressure, based on epidemiological studies. In addition, we provide evidence that the adverse health effects of AAP and PM are more pronounced among the elderly, children, and people with preexisting cardiovascular and respiratory conditions. Finally, we critically summarize the literature pertaining to cardiovascular diseases, including atherosclerosis and stroke, and introduce potential studies to better understand the health significance of AAP and PM on cardiovascular disease.

양성 갑상선질환과 함께 나타나는 갑상선암 (Concurrent Thyroid Carcinoma and Benign Thyroid Disease)

  • In order to review the clinicopathologic characteristics of the thyroid cancer associated with benign thyroid disease, we evaluated 47 patients treated between January, 1993 and September, 1997 at the Chonnam National University Hospital. In those period, we had operated a total of 690 thyroidectomy of which 320 were diagnosed as thyroid cancer. Forty three(91.4%)occurred in women and four(8.5%)occurred in men. The mean age at operation was 46.7years(range, 15 to 76 years). Forty three of the 47 cancers(91.4%) were papillary carcinomas while 4(8.5%)were follicular. Twenty four of the 47 patients (51%) were occult thyroid carcinomas measured less than 1 cm in diameter. The concurrent benign disease were nodular goiter(n=17), Hashimoto's thyroiditis(n=16), follicular adenoma(n=10), Graves' disease(n=2) and diffuse hyperplasia(n=2). Thirty one patients were diagnosed by preoperative FNAC and they underwent total thyroidectomy. Three were diagnosed by frozen section examination at the time of operation. Among them, one underwent total thyroidectomy and two underwent subtotal thyroidectomy. Eight cases revealed lymph node metastases and 2 cases extended to surrounding muscles. In conclusion, concurrent thyroid cancers and benign thyroid disease are not uncommon and a regular ultrasonic follow-up with selective aspiration cytologic examination is recommaned to enhance their diagnostic accuracy.

Acute Limb Ischemia and Coronary Artery Disease in a Case of Kimura's Disease

  • Kimura disease (KD) is an immune-mediated chronic inflammatory disease of unknown etiology. KD has many complications associated with hypereosinophilia, including various forms of allergic reactions and eosinophilic lung disease. Additionally, hypereosinophilia is associated with hypercoagulability, which may lead to thromboembolic events. A 36-year-old man with KD presented with acute limb ischemia and coronary artery occlusion. He underwent thrombectomy, partial endarterectomy of both popliteal arteries, and coronary artery stent insertion. KD is a systemic disease that affects many organs and presents with thromboembolism and vasculitis. In a patient with KD, physicians should evaluate the vascular system, including the coronary arteries.

The Phenotype of the Soybean Disease-Lesion Mimic (dlm) Mutant is Light-Dependent and Associated with Chloroplast Function

  • The dlm (disease lesion mimic) mutant of soybean (Glycine max L. Merr) shows the similar lesion of a soybean disease caused by a fungus, Corynespora cassilcola. The lesion was examined at cellular and molecular level. Trypan blue staining result indicated that cell death was detectable in the entire region of leaves excluding veins when the lesions had already been developed. We found that the mesophyll cells of palisade layer in the dim mutant appeared to be wider apart from each other. The chloroplasts of the dim mutant cells contained bigger starch granules than those in normal plants. We also found that the lesion development of dlm plant was light-dependent and the starch degradation during the dark period of diurnal cycle was impaired in the mutant. Three soybean pathogenesis-related genes, PR-1a, PR-4, and PR-10, were examined for their expression patterns during the development of disease lesion mimic. The expression of all three genes was up-regulated to some extent upon the appearance of the disease lesion mimic. Although the exact function of DLM protein remains elusive, our data would provide some insight into mechanism underling the cell death associated with the dim mutation.

Chronic Obstructive Pulmonary Disease Combined with Interstitial Lung Disease

  • Although chronic obstructive pulmonary disease (COPD) and interstitial lung disease (ILD) have distinct clinical features, both diseases may coexist in a patient because they share similar risk factors such as smoking, male sex, and old age. Patients with both emphysema in upper lung fields and diffuse ILD are diagnosed with combined pulmonary fibrosis and emphysema (CPFE), which causes substantial clinical deterioration. Patients with CPFE have higher mortality compared with patients who have COPD alone, but results have been inconclusive compared with patients who have idiopathic pulmonary fibrosis (IPF). Poor prognostic factors for CPFE include exacerbation, lung cancer, and pulmonary hypertension. The presence of interstitial lung abnormalities, which may be an early or mild form of ILD, is notable among patients with COPD, and is associated with poor prognosis. Various theories have been proposed regarding the pathophysiology of CPFE. Biomarker analyses have implied that this pathophysiology may be more closely associated with IPF development, rather than COPD or emphysema. Patients with CPFE should be advised to quit smoking and undergo routine lung function tests, and pulmonary rehabilitation may be helpful. Various pharmacologic agents and surgical approaches may be beneficial in patients with CPFE, but further studies are needed.

  • The incidence of inflammatory bowel disease is increasing rapidly in Korea and one quarter of the new patients present in childhood. Crohn's disease and ulcerative colitis are incurable conditions associated with significant morbidity, therefore the focus of treatment in children should be to reduce or eliminate symptoms, optimize nutritional status and growth, prevent complications. This review will deal with the evidence supporting the treatments currently used in children with inflammatory bowel disease.

이하선에 발생한 Catsleman's Disease 1례 (A Case of Catsleman's Disease of the Parotid Gland)

  • Catsleman's disease is a type of angiofolliculr lymph node hyperplasia and its etiology is not known yet. It usually presents with the mediastinal mass but rarely with the parotid mass. There are only five cases previously reported in the literatures. It has two pathologic types, which are hyaline vascular type and plasma cell type. Plasma cell type is frequently associated with systemic menifestations. Diagnosis is based on the histopatholgic findings. Treatment is surgical excision. A case of Catsleman's disease involving the parotid lymph node is presented and literatures are reviewed.

The Relative Influence of Diet and Physical Activity on Obesity in China

  • The purpose of this study is to investigate the relative influence of diet and physical activity on obesity. The subjects were 155 adults aged 35-52 years from 24 neighborhood committees in 4 urban districts of Beijing (male : 78, female : 77). They were divided into normal weight, overweight and obese groups according to their BMI. The general information of the subjects was collected by interview-administered questionnaire. Dietary intake was obtained by three-day(two weekdays and one weekend day) food weighted method, physical activity was assessed by a validated combination of data obtained from activity monitors, bicycling information and activity records. There were no significant differences of age, gender, height, educational, family economic level, smoking and drinking between different groups. The proportion of flour intake was higher in obese group compared to normal weight and overweight groups, and that of vegetables is lower in obese group. The physical activity (PAL) was not significantly different between two groups of the normal, overweight and obese groups. After the adjustment for confounding factors using logistic regression model, we found that the proportion of flour intake was positively associated with obesity, while the proportion of vegetable intake was inversely associated with obesity. It is concluded that dietary patterns were associated with obesity and diets composed of more vegetables and less staple combined with physical activities could contribute to obesity prevention.

Overall and disease-specific survival outcomes following primary surgery for oral squamous cell carcinoma: analysis of consecutive 67 patients

  • Objectives: This study evaluated the predictive factors for survival of patients with oral squamous cell carcinoma (OSCC) and investigated the overall and disease-specific survival (DSS) outcomes. Materials and Methods: A total of 67 consecutive patients who underwent surgery for OSCC from January 2006 to November 2014 were included in this study. Patients were classified according to age, sex, pTNM stages, primary sites, smoking and alcohol drinking habits, depth of invasion, perineural and lymphovascular invasion, cell differentiation and postoperative radiotherapy. Kaplan-Meier methods were used to estimate the survival categorized by patient groups. Cox regression methods were used to investigate the main independent predictors of survival. Results: Nineteen patients died of OSCC during follow-up periods. Another five patients died of other diseases including lung adenocarcinoma (n=1), cerebral infarction (n=1), general weakness (n=2), and pneumonia (n=1). The tongue (n=16) was the most common site for primary origin, followed by buccal mucosa (n=15), mandibular gingiva (n=15), maxillary gingiva (n=9), floor of mouth (n=9), retromolar trigone (n=2), and palate (n=1). Eleven patients had pTNM stage I disease, followed by stage II (n=22) and stage IV (n=34). No patients had pTNM stage III disease in this study. The overall survival of all patients was 64.2% and the DSS was 71.6%. DSS of patients with stage I and II disease was 100%. Stepwise Cox regression showed the two predictors for DSS were pTNM stage (P<0.0001, odds ratio=19.633) and presence of metastatic lymph nodes (P=0.0004, odds ratio=0.1039). Conclusion: OSCC has been associated with poor prognosis; however, there were improved survival outcomes compared with past studies. Advanced-stage disease and presence of metastatic lymph nodes were associated with poorer survival compared with early-stage OSCC and absence of neck node metastasis. Stage I and II OSCC were associated with excellent survival results in this study.
