The Oriental Medical Study of Myofascial Pain Syndrome about Shoulder (견부(肩部)의 근막통증증후군(筋膜痛症症候群)에 대(對)한 고찰(考察))

  • The shoulder is the most flexible joint in human body, so many people suffer from the shoulder pain. In order to improve medical care about shoulder muscle disease, Myofascial Pain Syndrome(M.P.S) is compared with the oriental medical theory. The findings of this study are as follows; 1. Myofascial Pain Syndrome(M.P.S) is the sensory, motor, and autonomic symptoms caused by myofascial trigger points. For the objectivity of the oriental medical theory, practical application is necessary. 2. The meridian and meridian-muscle of the shoulder is su-sam-yang(手三陽), su-sam-um(手三陰). Meridian-muscle theory is similar to anatomical muscle and myofascia. 3. There is similarity in the trigger point and Ashi(阿是)-point, taut band and palpable tender-point(硬結), referred pain and Hangki(行氣). In this study, myofacial pain syndrome is similar to the oriental medical theory. If myofacial pain syndrome is applicated in treatment, the cure of shoulder pain and objectivity of the oriental medical theory is improved.

An Analysis of the Study Tendency on Meridian Muscle (국내의 경근(經筋) 연구동향에 대한 고찰)

  • Objectives : The aim of this research is to analyze the current trend of the studies about eridian muscle and to provide background for further studies. Methods : Reviewing 33 domestic oriental medical studies about meridian muscle, and comparative analysis was made. These studies were classified by method, theme and subtitle. Results : 1. According to the classification by study method, number of literary studies are 22(67%), which is more than half, number of experimental studies are 5(15%) and clinical studies are 6(18%). 2. According to the classification by study theme in literary study, percentage of 'Structure amp; Movement of Meridian Muscle' took 64%, Theory study of Meridian Muscle' took 14%, 'Application of Concept of Meridian Muscle' took 14%, 'Treatment of Meridian Muscle disorder' took 9% arranged in order. 3. In 'Theory study of Meridian Muscle', there were not only literary approaches but also Deficiency-Excessiveness(虛實) and historical approaches. Study about 'Structure & Movement of Meridian Muscle' includes analysis of muscle and Myofascial pain syndrome. On this background, it is necessary to recognize the linkage and motion analysis of Meridian Muscle. Therefore, studies were changed into interpretation about Anatomy trains, analysis of motion. The study about 'Treatment of Meridian Muscle disorder' provided the various treatment method-Acupuncture, Manual therapy, Ashi(阿是)-point therapy, CHUNA therapy etc.- in literary study. The study about the 'Application of Concept of Meridian Muscle' has been performed in relation to Embedding Therapy, Kyungkuen chuna, Ki-gong therapy. 4. Experimental Studies were all Anatomical Studies. Studies were done in trial of discovering the actual existence, but revealed problem in interpretating the meaning of Meridian Muscle. 5. Clinical Studies based on Ashi(阿是)-point therapy CHUNA Muscles Along Meridians Release Therapy etc, were performed. Experimental studies about Meridian Muscle were assessed as low grade according to Jadad Scale. There were no studies which were based on well-organized Meridian Muscle theory. Conclusions : There needs to be more discussion about concept of Meridian Muscle and proceed more reliable experimental studies with organized Meridian Muscle theory. Further objective studies about treatment of Meridian Muscle should be done.

  • Objectives : Scalenus anticus syndrome and Piriformis syndrome is representative of myogenic Nerve entrapment syndrome, and their clinical symptoms are similar to HIVD(herniated Intervertebral disc) of cervical or lumbar spine. But, distinguished by muscle test, these syndrome apply to Meridian-muscle therapy. Methods : Meridian-muscle therapy consists of Ashi(阿是)-point therapy, taping therapy, myofascial release technique, manipulation, their based on the traditional meridian-muscle theory. This theory is similar to myofascial pain syndrome in western medicine. The study population consisted of 9 patients who were already diagnosed as Nerve entrapment syndrome with radiological examination & physical examination and muscle test. The evaluation of clinical outcome was done by Visual Analogue Scale (VAS) and Pain Assesment Questionnaire(PAQ). Results and Conclusions : After treatment, All patient's VAS is decreased as $2.11{\pm}1.59$ and the evaluation of clinical effect was excellent(6 cases) or good(3 cases) according to PAQ. Conclusively, Meridian muscle therapy is efficacious against Nerve entrapment syndrome.

  • I have come to next conclusions in consequence of documentary study about medical books of many generations regarding acupuncture and moxibustion therapy of arthalgia syndrome. 1. Jing point of regular channels is most used. Next there are extraordinary point, ashi point in the acupuncture and moxibustion therapy of arthalgia syndrome. 2. The gall baldder channel of foot-shaoyang is most used. Next there are the urinary bladder channel of foot-taiyang, the large intestine channel of hand-yangming, the stomach channel of foot-yangming, the small intestine channel of hand-taiyang, the spleen channel of foot-taiyin, the liver channel of foot-jueyin, the triple-warmer channel of hand-shaoyang, the du channel, the lung channel of hand-taiyin, the kidney channel of foot-shaoyin, the pericardium channel of hand-jueyin, the heart channel of hand-shaoyin, the ren channel in the order of frequency in used channel among the twelve channels. 3. Three yang channels of foot is most used. Next there are three yang channels of hand, three yin channels of hand, three yin channels of foot in the order of frequency in use among the twelve channels. 4. The gall baldder channel of foot-shaoyang is most used. Next there are the urinary bladder channel of foot-taiyang, the large intestine channel of hand-yangming, the triple-warmer channel of hand-shaoyang, the stomach channel of foot-yangming, the small intestine channel of hand-taiyang, the liver channel of foot-jueyin, the lung channel of hand-taiyin, the spleen channel of foot-taiyin, the kidney channel of foot-shaoyin, the du channel, the pericardium channel of hand-jueyin, the ren channel the heart channel of hand-shaoyin in the order of frequency in used point among the twelve channels. 5. Jing point in the order of frequency of use, expel wind-evil and remove wetness-evil on the Acupuncture and Moxibustion Therapy of Arthalgia Syndrome.

A Review of Recent Studies about Fire and Warm Needling for De Quervain Syndrome (드퀘르벵 증후군의 온침 및 화침 치료에 대한 최신 연구 동향)

  • Objectives : The purpose of this study is to examine the efficacy of fire needling and warm needling for De Quervain Syndrome by reviewing clinical studies for recent 10 years. Methods : Randomized controlled trials, non-randomized controlled trials, and case series containing more than 20 cases about fire needling and warm needling for De Quervain Syndrome published since 2011 were searched through four foreign online databases (CNKI, Pubmed, EMBASE, Cochrane Library) and five Korean online databases (OASIS, Science ON, DBPIA, KISS, RISS). The number and characteristics of participants, treatment points and main treatment methods involving other combination treatments, treatment cycle or total periods of treatments, evaluation indices, efficacy, and adverse events were analyzed. Risk of bias of included randomized clinical trials was assessed using a revised tool for assessing risk of bias in randomized trials (RoB 2). Results : A total of 6 randomized clinical trials and 2 case series involving 471 participants were included. Tender point or 'Ashi point' was the most commonly used treatment point, followed by LU4. Treatment frequency ranged from once a day to once a week. One to three outcome measures were used to evaluate the results of the studies, with the efficacy rate the most frequently used, followed by visual analogue scale. Overall risk of bias of all included randomized clinical trials was judged to have some concerns. Conclusions : All selected studies showed that fire needling and warm needling treatments for De Quervain syndrome were more effective than other clinical methods or acupuncture treatments. However, as the number of clinical studies is still too small and the risk of bias of the studies is not low, it is believed that more systematic and objective studies should be conducted.

Effect of Eight Vacuity Theory Adopted Complex Acupuncture Therapy on Patients with External Epicondylitis (주관절 외측상과염 환자에 대한 팔허(八虛) 이론에 근거한 복합침구치료 치험 2례)

  • Objectives : The objective of this study was to report two cases that patients with external epicondylitis were improved by based on 'eight vacuity' theory bloodletting therapy, acupuncture therapy and bee venom pharmacopuncture. Methods : In this study, patients were treated with bloodletting therapy, then acupuncture therapy and bee venom pharmacopuncture to Ashi point on eight vacuity of elbow in the patients without retention of needles. Subsequently, electroacupuncture and Dong-qi acupuncture treatment were implemented in order to strengthen the effect of acupuncture. In terms of evaluation methods, visual analog scale(VAS) score, range of motion(ROM), grip strength and physical examination were used to assess external epicondylitis-related symptoms. Results : VAS score decreased, ROM and grip strength were increased and physical examination was improved in all cases. Conclusions : It was suggested that the symptoms of intractable external epicondylitis could be improved by eight vacuity theory adopted bloodletting therapy, acupuncture therapy and bee venom pharmacopuncture.

The Study of Acupuncture Treatment of Lateral Epicondylitis. -Based on PubMed (외측상과염의 침 치료에 관한 최신 연구 동향 -PubMed를 중심으로)

  • Objectives : The purpose of this study is to investigate the trends of acupuncture treatment on lateral epicondylitis. Methods : We investigated the recent clinical studies of acupuncture treatment for lateral epicondylitis via searching PubMed from 2007.1.1. to 2016.1.1. This study examined published years, the authors, journals, countries, types of study and classified articles by techniques of acupuncture. Results : 20 theses out of 38 were included, excluding papers duplicated or whose texts are not identified or not satisfying both acupuncture and lateral epicondylitis simultaneously. The theses were constantly published every year. Analyzed by journal, the most published journal was Zhongguo Zhen Jiu. The studies of China had the greatest number of the searched theses. The types of study were reviews, case series, and study protocol. The acupuncture technique used were Hwachim(burning acupuncture therapy), sweet bee venom pharmacopuncture, percutaneous needle electrolysis, electroacupuncture, small needle-knife, combination with local points and Tianzong(SI11), and anti-ashi-point. Conclusions : In order to develop acupuncture treatment of lateral epicondylitis, clinical studies on high level with objective evaluation scale are needed.

A Comparative Study of Clinical Papers about Pharmacopuncture in Korea and China - Focusing on Papers about Musculoskeletal and Nervous Diseases - (중국과 국내에서의 약침 관련 임상논문 비교 연구 - 근골격계 및 신경계 질환을 위주로 -)

  • Objectives : To research the trend of the study related to pharmacopuncture about musculoskeletal and nervous diseases in China and to make a comparison with clinical application on pharmacopuncture about musculoskeletal and nervous diseases in China and Korea. Methods : I reviewed and analyzed four Chinese Journals relating pharmacopucture from 2004 to 2008 and two Korean Journals relating pharmacopuncture from 1999 to 2008. Results : The following results were obtained in this study. 1. The pharmacopuncture was often used for diseases of spines and joints. 2. In the medicine of pharmacopuncture, Salviae Miltiorrhizae Radix, Angelicae Gigantis Radix were mainly used in China, and Bee-venom in Korea. 3. As a method of needling, local acupoint and Ashi-point were mainly ueed in both China and Korea. 4. In combined treatments, acupuncture, electronic acupuncture, Chuna, heating acupuncture were mainly used in China, and acupuncture, herb medicine, physiotherapy in Korea. 5. In the case of control group study, compared with other treatments pharmacopuncture was not verified the superiority of effectiveness in China, but in Korea, the effectiveness of a pharmacopuncture could be compared with that of others and the effectivenss of different pharmacopuncture could be compared. Conclusions : As seen above, in Korea, clinician should carry out clinical application about musculoskeletal and nervous diseases with various herbal injections besides Bee-venom. And I think that it will be starting point to use Salviae Miltiorrhizae Radix and Angelicae Gigantis Radix that were often used in China.

Venesection therapy is much used for five sensory organ disease. Besides that internal disease, pain paralysis disease of muscle and joints, sugical disease, disease of woman and children, fever sunstroke CVA emergency case follow that in the order of frequency of use. 2. It is used for swollen tongue, eye pain, pharyngitis, swelling and pain in the throat, bleeding from the eye ear nose mouth or subcutaneous tissue, tonsillitis, aphthae and so on in the five sensory organ disease. Focus, sosang, jinjin yuye, taiyang, baihui are used for five sensory organ disease in the order of frequency of use. 3. It is used for malaria, headache, precordial pain, head-wind, abdominal colic, diseases characterized by acute diarrhea and vomiting, and so on in the Internal disease. Superficial venules and lymph vessesls, taiyang, quze are used for Internal disease in the order of frequency of use. 4. It is used for low back pain, hypochondriac pain, numbness, knee pain, tinea pedis, red swelling pain of hand and arm, flaccidity-syndrome, and so on in the pain paralysis disease of muscle and joints. Weizhong, superficial venules and lymph vessesls, Ashi point, zhigou are used for pain paralysis disease of muscle and joints in the order of frequency of use. 5. It is used for furuncle, tinea capitis, and so on in the sugical disease. Focus, weizhong are used for sugical disease in the order of frequency of use. 6. It is used for inflammatory disease with redness of skin, and so on in the disease of woman and children. Focus, weizhong, yanglingquan, yaoshu, sanyinjiao are used for disease of woman and children in the order of frequency of use. 7. It is used for fever, CVA, sunstroke, cadaverous coma, common cold, and so on in the fever sunstroke CVA emergency case. Sosang, weizhong, chize are used for fever sunstroke CVA emergency case in the order of frequency of use. 8. The urinary bladder channel of foot-taiyang is most used. Next there are the du channel, the stomach channel of foot-yangming, the lung channel of hand-taiyin, the gall baldder channel of foot-shaoyang, the triple-warmer channel of hand-shaoyang, the large intestine channel of hand-yangming, the spleen channel of foot-taiyin, the kidney channel of foot-shaoyin the pericardium channel of hand-jueyin the liver channel of foot-jueyin, the ren channel, the heart channel of hand-shaoyin, the small intestine channel of hand-taiyang in the order of frequency in use. 9. Superficial venules and lymph vessesls, focus, five shu points, extra-point, back point are used in the venesection therapy, those are characteristic of locating an acupuncture point.

Acupotomy for Osteoarthritis of the Knee; A Systematic Review and Meta-Analysis

  • The purpose of this study was to evaluate the effectiveness and safety of acupotomy for the treatment of patients with knee osteoarthritis. There were 9 databases searched to retrieve randomized controlled trials until August 3, 2019 regarding acupotomy versus conventional Western medicine, conventional Western medicine treatment with and without acupotomy, and Korean medicine treatment with and without acupotomy, and meta-analysis was performed. Of 303 potentially relevant studies retrieved, 43 were systematically reviewed. All studies were conducted in China. Effective rate, visual analogue scale, and Western Ontario and McMaster Universities Osteoarthritis index were used as the evaluation scales. The Ashi point was selected most frequently. In all studies, the intervention group was more effective than the control group. Meta-analysis revealed that acupotomy showed statistically significant beneficial results. Although acupotomy had a beneficial effect on knee osteoarthritis, the risk of bias of the included studies was not low. The majority of the results from the evaluation scales used were highly heterogeneous (> 50%) which reduced confidence in the estimation of effect, or had a small sample size. Further clinical research and development is required in the future.
