Change of arterial pulse wave characteristic by measurement posture and brachial blood pressure (측정 자세 및 상완 혈압에 의한 맥파 특성 변화)

  • In this study, pulse waves were measured at radial artery using non-invasive tonometric pulse pressure measurement system, SphygmoCor(AtCor, Australia), according to subject's posture. Then it was analysed whether the pulse wave parameters, which contain heart activities, change among three different postures (upright stand, sit, and supine). And it was also verified that the pulse wave parameters change among blood pressure level groups(hypotensive, normotensive, and hypertensive). As a results, posture effects were verified in time information of pulse wave rather than amplitude. But some parameters calculated by ratio of two amplitude, such as augmented index(AI) and ratio of central aortic pulse and radial artery pulse, showed significant difference according to postures. In post hoc test, time to the $1^{st}$ and $2^{nd}$ pulse peak(P_$T_1$, and P_$T_2$), ED(ejection duration), and HR(heart rate) showed significant difference among posture groups with each other. In comparison of blood pressure groups, it was verified that the parameters related to amplitude of pulse wave showed significant difference rather than time information.

An Example of Test on Differences of Pulse Waveform Characteristics at Cun, Guan and Chi (촌, 관, 척 위치의 맥파 특징 차이 검정에 대한 1례)

  • Although the pulse diagnosis position, Guan is apart from Cun or Chi by only $10{\sim}20$ mm at most, traditional medical doctors applies different indent pressures and even they states different pulse images are felt at Cun, Guan and Chi, To support their clinical behaviors, in this study, we tested statistically whether there are differences in pulse waveform measured at these three positions with SphygmoCor system used world widely, A 30 years old female subject without any evidence of cardiovascular diseases was involved in this experiment. Radial pulse waves were recorded at three different positions on left lower arm 10 times at three positions-Cun, Guan and Chi. With ANOVA, we tested whether, among three different positions. there are any differences in 12 parameters of radial pulse waveform and in estimated AIx(Augmentation Index) as an arterial stiffness index extracted from radial pulse waveform. As results, differences in optimal indent pressure h0 were observed at different measuring positions(P<0.001) but not significantly different. And pulse pressure his were found to be different(Chi$22.60{\pm}3.06%,\;18.60{\pm}3.37%\;and\;26.4{\pm}5.02%$ respectively. Consequently. AIx at Gwan seems to be lowest and that at Chi seems to be highest. So. we assert the AIx at Chi is likely to be overestimated. In further studies. we want to examine what make differences in these parameters between measuring positions. And it also seems to be worthy to investigate the relationship between the depth of radial artery and AIx. And, ultimately, we need to determine the best measuring process including measuring position, hold-down pressure, signal quality validation and so on. so to achieve the optimal waveform which represents subject's health condition for both western medicine and traditional medicine.

Characteristics of Pulse Waves in Various Age Categories and Applicability of Pulse Wave to Metabolic Syndrome Using Pen-type Piezoresistive Sensor (펜타입 압저항 센서를 활용한 연령별 맥파 특성 및 맥파의 대사증후군에의 적용 가능성 평가)

  • Objectives : The purpose of this study was to confirm that the pulse analyzer is useful for analyzing characteristics of variables of pulse waves in age categories, evaluating pulse waves of the metabolic syndrome group, compared with those of the non-metabolic syndrome group in Korean adults. Methods : The pulse wave variables were measured in Guan of all 1,056 subjects by the pulse analyzer, using a pen-type piezoresistive sensor. The physical measurement, blood test and survey were also performed by each subject. Results : In the age categories, height of pre-incisura (h2), height of tidal wave (h3), area of percussion wave (Aw), and width of percussion wave (w) increased in accordance with increase in age. While ratio of systolic period area (As) went up according to the increase of age, ratio of diastolic period area (Ad) went down. Radial augmentation index (R-AI), h2/h1, h3/h1, w/t and angle of percussion wave went up by aging, generally. Aw rate (Aw/At) also increased. Among the metabolic syndrome group, in the ages of 19 and 44, ratio of systolic period area (As) was higher and ratio of diastolic period area (Ad) was lower than in the non-metabolic group. w/t, Aw/At, and angle of percussion wave were higher than in the non-metabolic syndrome group. Among the metabolic syndrome group over the age of 60, height of pre-incisura (h2), height of tidal wave (h3), total area (At), area of percussion wave (Aw), radial augmentation index (R-AI), h2/h1 and h3/h1 were higher than in the non-metabolic syndrome group. Conclusions : The pulse analyzer is useful to analyze arterial stiffness in the age categories and in the metabolic syndrome group by some measures.

Comparative Study on the Pulse Wave Variables and Sasang Constitution in Diabetes Mellitus Patients and Healthy Subjects (당뇨병 환자와 건강인의 맥상과 사상체질에 관한 연구)

  • The purpose of this study was to determine whether a pulse analyzer was useful 1) to characterize the variables of pulse wave of diabetes mellitus group, compared with those of healthy subjects, and 2) to determine Sasang Constitution in diabetes mellitus group and healthy subjects. 1. The sum of pulse pressure (energy) and the ratio of systolic period area (As%), called pulse pressure-related variables, were higher in diabetes mellitus group than healthy group, while the height of dicrotic wave (h5) and the ratio of height of dicrotic wave to height of percussion wave (h5/h1), correlated with arterial compliance, were lower in diabetes mellitus group than healthy group. 2. Taeumin of diabetes mellitus group showed higher pressure-related variables than that of healthy group. 3. Soumin of diabetes mellitus group had shorter the time to dicrotic wave (t5) than that of healthy group. 4. Soyangin of diabetes mellitus group showed higher heart rates and lower values in pulse wave time-related variables, including time to dicrotic wave(t5), time to incisura (t4), total time minus time to incisura (t-t4), total time (t), width of percussion wave (w), and the ratio of width of percussion wave to total time (w/t), than that of healthy group. 5. Contact pressure (CP), sum of pulse pressure (energy), height of pre-incisura (h2), height of incisura (h4), width of percussion wave (w), time to incisura (t4), time to percussion wave (t1), variance of total time (Vt), variance of height of percussion wave(Vp) and the ratio of height of incisura to height of percussion wave (h4/h1) were used to develop the rules of Sasang Constitution Classification with about seventy five percents accuracy. These suggested that the pulse analyzer was useful to evaluate the risk degree of diabetes mellitus and to determine Sasang Constitution among either diabetes mellitus group or healthy group.

Signl processing method and diagnostic algorithm for arterial oxygen-saturation measument (산소포화도 측정을 위한 신호처리방법 및 계산 알고리즘)

  • A measurement unit and signal processing algorithm have been developed for predicting arterial oxygen saturation noninvasively. The measurement set-up was composed of a probe including light source and photodetector, optical signal processing section, LED driving circuit, PC interface software for data acquisition and data processing software. Light from the LED's was irradiated onto the finger nail bed and transmitted light was measured at different wavelengths. An effective baseline correction method was developed and measured data were analyzed by using various data processing methods and prediction algOlithms. For performance evaluation, a pulse oximeter simulator (Bio- Tek Instrument Inc.) was used as reference. The best performance in terms of the correlation coefficient and the standard deviation was obtained under the following conditions; when the arterial signals were computed in terms of area rather than peak-valley difference, and when the algorithm calculating by $In(I_p/I_v)/I_{avr}$ value for pulsation waveform was used. In in vivo test, prediction was improved when the developed baseline correction method was used. In addition, wavelengths of 660 nm and 940 nm provided better linearity and precision than wavelengths of 660 nm and 805 nm. 05 nm.

Correlation between Arterial Stiffness and Physiological Parameters (동맥경화도와 생리학적 변수들 간의 상관관계)

  • Arterial stiffness(AS) is an important pathologic state of vascular injury. This study was carried out to elucidate the effect of physiological variables on brachial-ankle pulse wave velocity(BAPWV), index of AS. Four hundred adults(volunteers) participated in this study. Body indices, biochemical, cardiac and inflammatory markers, and right(Rt)- and left(Lt)-BAPWV were measured. Body mass index(BMI), Rt- and Lt-BAPWV, glucose, triglyceride, alkaline phosphatase(ALP), gamma-glutamyl transferase(GGT), creatinine, uric acid, troponin-I(TNI), NT-proBNP and high sensitivity C-reactive protein(hs-CRP) levels were higher than the reference value of each variable. Rt- and Lt-BAPWV were directly correlated with age, body weight, BMI, glucose, ketone, aspartate aminotransferase, alanine aminotransferase, ALP, GGT, total cholesterol, low density lipoprotein, lipoprotein(a), apolipoprotein-B, blood urea nitrogen, heart rate, TNI, creatine kinase, CK-MB, lactic dehydrogenase, myoglobin, hs-CRP, lipase, reumatoid factor, fibrinogen and D-dimer (P<0.05, P<0.01, P<0.001 or P<0.000, respectively), but inversely associated with total bilirubin, uric acid, apolipoprotein-A1 and GFR (P<0.05). These observations suggest that a variety of physiological variables may influence BAPWV, resulting in increased risk or prevention of cardiovascular and/or cerebrovascular attacks. Therefore, physiological variables affecting BAPWV should be regularly controlled.

Seven Days Breaking Up Prolonged Sitting Improves Systemic Endothelial Function in Sedentary Men (일주일간의 간헐적 좌식차단의 혈관기능 개선 효과)

  • PURPOSE: To examine the cumulative (7 days) effect of breaking up prolonged sitting on systemic endothelial function in sedentary men. METHODS: Thirty sedentary men ($33.93{\pm}5.72years$) participated in two randomized 7 days sitting trial (Sit group (control) vs. Breaks group). The protocol of Breaks group is as follows: 4-minute of moderate-intensity marching in place (walking) every 1 hour during business hour (total: 8 breaks/day). Assessment of brachial artery endothelial function using flow-mediated dilation (FMD) and arterial stiffness indices (augmentation index, arterial pressure and pulse wave velocity) were measured before and after 7 days treatment. RESULTS: Brachial artery FMD significantly increased after 7 days breaking up prolonged sitting treatment (Breaks groups, $9.65{\pm}2.61$ to $9.62{\pm}2.6%$) compared with 7 days prolonged sitting (Sit group, $8.37{\pm}3.41$ to $10.11{\pm}3.75%$) (interaction effect, p=.004). Arterial pressure (AP) significantly increased after treatment (Breaks group, $2.75{\pm}2.19$ to $2.38{\pm}1.63mmHg$, p=.002) in Sit group but there was no change (Sit group, $1.00{\pm}3.18$ to $2.50{\pm}9.23mmHg$) in Breaks groups (interaction effect, p=.008). CONCLUSIONS: These finding show that 7 days regular breaking up prolonged sitting improve in FMD, compared with prolonged sitting. Therefore, regular breaking up prolonged sitting may improve systemic endothelial function in sedentary men.

Effects of one-to-one Labor Support on Labor Pain, Labor Stress Response, Childbirth Experience and Neonatal Status for Primipara (일대일 분만지지간호가 초산모의 분만동통, 분만스트레스 반응, 분만경험 및 신생아 상태에 미치는 효과)

  • This study was designed to investigate the effect of one-to-one labor support on labor pain, labor stress response, childbirth experience and neonatal status for primipara by a quasi experiment (nonequivalent control group pretest-posttest design), from April 30, 1999 to February 20, 2000. The subjects of this experiment consisted of eighty two primipara with single gestation, full term, uncomplicated pregnancies. Forty one were in the experimental group and forty one in the control group. Their mean age was 25.95 years, their mean gestation period was 39.9 weeks. A caring package of one-to-one labor support had three components. Physical support consisted of massage, back pressure, touch. Emotional support was provided by a continuous nurse's presence, acceptance and encouragement. Informational support involved teaching breathing skills, relaxation skills and knowledge about the labor process. Data assessed labor pain, pulse rate and blood pressure to measure labor stress response. Also, in measuring the value of labor stress response, plasma epinephrine, plasma norepinephrine and serum cortisol were measured. In the 24 hours after birth, the data for the postpartum mother's childbirth experiences was collected. Umbilical cord arterial blood pH, one minute and five minute Apgar score were measured after birth. Data was analyzed by t-test, $x^2$-test, repeated measures ANOVA, ANCOVA with SAS Program. The results were as follows; 1. Labor pain was significantly low in the experimental group(P=.016). 2. No significant group effects were found, but significant time effects were found for plasma epinephrine, norepinephrine, serum cortisol, pulse rate and blood pressure. 3. The childbirth experience of the experimental group was significantly more positive than the control group (P = .005). 4. The umbilical cord arterial blood pH of the experimental group was significantly higher than the control group(P=.014). There was no significant difference between the two groups in neonatal one minute and five minute Apgar scores. In conclusion, these findings indicate that one-to-one labor support could be effective in decreasing labor pain, and increasing positive childbirth experiences, also increasing the neonatal umbilical cord arterial blood pH for primipara. So, one-to-one labor support could be applied as an effective nursing treatment for primipara.

Improvement of Fourier Transform Arteriography by Use of Ramped RF Profile and Dual Projections (경사 윤곽의 고주파 펄스와 이중 투사법에 의한 Fourier 변환 동맥 혈관 촬영법의 성능 향상)

  • The Fourier transform arteriography (FTA) exploits the periodic variation of arterial flow velociety of arterial flow velocity in stnchronized with cardiac cycles. This technique is intrinsically unique compared to other modern techniques. This technique separates the arteries from the veins using the pulsatile arterial flow without using the presaturation RF pulses. Therefore, it has less RF deposition and is free from the dark band artifacts that can arise from retrograde flow and curved arteries. Furthermore, it is free from the artifacts induced by eddy currents. However, there are some drawbacks such as a single projection view and the saturation of arteries at the end of an imaging slab. These drawbacks are circumvented by applying recently developed techniques. The fast gradient switching capability of modern MRI systems enabled us to incorporate dual projection views into the conventional FTA sequence without increasing the repetition time. In addition, signals from the distal arteries were enhanced by use of a ramped RF pulse and therefore the distal arteries were less saturated. By use of the FTA sequence with dual projection views and the ramped RF pulse, we acquired the sagittal and coronal projection views of femoral arteriograms simultaneously with more enhanced signals of distal arteries than the conventional FTA.

The fruit of Acanthopanax senticosus Harms improves arterial stiffness and blood pressure: a randomized, placebo-controlled trial

  • BACKGROUND/OBJECTIVES: Arterial stiffness and endothelial dysfunction are 2 of the independent predictors for cardiovascular disease, while Acanthopanax senticosus Harms (ASH) is a traditional medicinal plant that can improve cardiovascular health. This study aimed to investigate the efficacy of the fruit of ASH on vascular function in apparently healthy subjects. SUBJECTS/METHODS: A 12-week, randomized, double-blind, placebo-controlled design, consisting of healthy adults with at least 2 of the following 3 conditions: borderline high blood pressure (BP; 120 mmHg ≤ systolic BP ≤ 160 mmHg or 80 mmHg ≤ diastolic BP ≤ 100 mmHg), smoking (≥10 cigarettes/day), and borderline blood lipid levels (220 ≤ total cholesterol ≤ 240, 130 ≤ low density lipoprotein cholesterol ≤ 165, or 150 ≤ triglyceride ≤ 220 mg/dL). Randomly assigned 76 subjects who received a placebo or 2 doses of ASH fruit (low, 500 mg/day; high, 1,000 mg/day) completed the intervention. Brachial-ankle pulse wave velocity (baPWV), flow-mediated dilation, carotid intima-media thickness, and BP were measured both at baseline and following the 12-week intervention. Endothelial nitric oxide synthase (eNOS) phosphorylation was assessed by western blotting. RESULTS: Compared with the placebo group, the low-dose group showed more significant changes after the 12-week intervention period in terms of systolic BP (0.1 vs. -7.7 mmHg; P = 0.044), baPWV (31.3 vs. -98.7 cm/s; P = 0.007), and the ratio of phospho-eNOS/eNOS (0.8 vs. 1.22; P = 0.037). CONCLUSIONS: These results suggest that ASH fruit extract at 500 mg/day has the potential to improve BP and arterial stiffness via endothelial eNOS activation in healthy adults with smoking and the tendency of having elevated BP or blood lipid parameters.
