The purpose of this study was to compare the degree of micro- leakage of new glass ionomer root canal sealer, Ketac-Endo(ESPE Co., Seefeld, Germany) with that of AH-26(De Trey Co., Ltd., U.S.A.). Root canal treatment using K -file, H -file, Gate Glidden drill was conducted on 49 extracted single-rooted teeth. 45 teeth were randomly divided into 3 experimental groups(15 teeth per group) and 4 teeth were used as the control group. Group 1 was used AH -26 sealer with the lateral condensation method for canal filling, group 2 was used Ketac-Endo with the single cone method and group 3 was used Ketac-Endo with the lateral condensation method. The control group was obturated with the single cone method without sealer. The teeth were covered with two coats of nail varnish after 48 hours of obturation. The teeth were immersed in India ink for 1 week and cleaned with methyl salicylate and then the degree of dye penetration were measured with stereomicroscope. The data were analyzed statistically by one-way ANOVA. The results were as follows: 1. 7 teeth in group 1, 5 in group 2, and 3 in group 3, were showed evidence of microleakage implying appropriate canal filling. 2, The mean average of microleakage was $0.17{\pm}0.32mm$ in group 1, $0.30{\pm}0.37mm$ in group 2. $0.10{\pm}0.21mm$ in group 3, showing that canal filling using the lateral condensation canal filling method with Ketac-Endo showed the least microleakage and using the single cone method with Ketac-Endo showed the largest amount of microleakage, 3. There were no statistically significant difference in the variation of microleakage among groups. From the results above, Ketac-Endo which has the advantage of glass ionomer, whether using the single cone method or the lateral condensation method, showed similar results as AH-26, but for clinical application it is thought that were studies on the properties of Ketac-Endo should be followed.
Objectives: This study evaluated the effects of adhesion variables such as the priming concepts of canal wall and the curing modes of adhesives on the sealing ability of a resin-based root canal filling system. Materials and Methods: Apical microleakage of the Resilon-RealSeal systems filled with 3 different combinations of adhesion variables was compared with the conventional gutta-percha filling using a dye penetration method. Experimental groups were SEDC, Resilon (Resilon Research LLC) filling with self-etch RealSeal (SybronEndo) primer and dual-cure RealSeal sealer; NELC, Resilon filling with no etching, Scotchbond Multi-Purpose (3M ESPE) primer application and light-curing adhesive; and TELC, Resilon filling with Scotchbond Multi-Purpose primer and adhesive used under total etch / wet bonding and lightcure protocols. GPCS, gutta-percha filling with conventional AH26 plus sealer, was the control group. Results: The median longitudinal dye penetration length of TELC was significantly shorter than those of GPCS and SEDC (Kruskal-Wallis test, p < 0.05). In the cross-sectional microleakage scores, TELC showed significant differences from other groups at 2 to 5 mm from the apical foramen (Kruskal-Wallis test, p < 0.05). Conclusions: When a resin-based root canal filling material was used, compared to the self-etching primer and the dual-cure sealer, the total etch/wet-bonding with primer and light-curing of adhesive showed improved apical sealing and was highly recommended.
The purpose of this study was to evaluate effects of smear layer and dentin primers on the sealing ability of root canals. 126 extracted human teeth with single, straight canals and mature apices were used. The Samples were first classified into six groups as follows: presence of smear layer; absence of smear layer; Scotchbond Bond Multi-Purpose; All Bond 2; Mac Bond 2; Clearfil Liner Bond 2. A Positive control was also established. All teeth except the control group were then obturated with thermoplasticized gutta-percha and AH26. Electrochemical and dye penetration technique were later used to evaluate the degree of micro leakage through the root canal. Seventy teeth were then immersed in a 1% potassium chloride solution and An external power supply(DC 10 V) was then applied to the circuit for the electrochemical microleakage test. The degree of Microleakage was determined over period of 28 days before being evaluated. In total, 48 teeth were submitted to the dye infiltration technique. All specimen were suspended in 2% methylene blue dye for 1 week before being longitudinally split. The degree of dye infiltration was measured under a stereo microscope at ${\times}10$ magnification and evaluated. The results were as follows: 1. Apical microleakage increased throughout the test period in all group and one group having a smear layer showed a dramatic increase under electrochemical test (p<0.05). In the group having smear layer, the degree of apical microleakage was the highest, and the micro leakage was much higher than in the smear layer removed group in electrochemical test (p<0.05). Scotchbond Multi-Purpose, All Bond 2, Mac Bond 2 and Clearfil Liner Bond 2 showed lower micro leakage than one group having smear layer. The All Bond 2 and Clearfil Liner Bond 2 treated groups showed the lowest microleakage in electrochemical test (p<0.05). 2. There was no significant difference between the experimental groups in dye penetration technique. These results suggested that the removal of the smear layer from root canal and concomitantly the application of dentin primer into root canal could improve the sealing ability of root canal obturation.
The purpose of this study was to evaluate four different composite resins in vitro for microleakage in Class II box type restorations that have gingival margins apical to the cementoenamel junction. Forty caries free extracted human molars were used in this study. The Class II cavities were prepared 1.0mm below cementoenamel junction with a #701 carbide bur. The teeth were randomly divided into four groups, each group comprising 20 treated cavities according to adhesives and filling materials ; Group 1: Scotchbond Multipurpose/Z 100. Group 2: Ariston Liner/Ariston pHc, Group 3: One Step/Pyramid, Group 4: Prime & Bond NT/SureFil. To simulate the clinical situation during restoration placement, a restoration template was fabricated and composite resin was filled using a three sited light-curing incremental technique. The specimens were stored in the 100% humidity for 7 days prior to thermocycling. The specimens were immersed in 2% methylene blue dye solution for 24 hours and then embeded in transparent acrylic resin and sectioned mesiodistally with a diamond wheel saw. The degree of marginal leakage was scored under the stereomicroscope($\times$20) and the data were analyzed by Kruskal Wallis test and Mann Whitney test. (omitted)
Advisor: Prof. The quality of apical seal obtained with 3 different gutta-percha obturation techniques was compared in 49 recently extracted single rooted teeth. The root canals were instrumented using step-back technique and obturated with laterally condensed gutta-percha, Continuous Wave gutta-percha, and hybrid technique. Teeth were suspended in black India ink for 7 days, cleared, and then examined under a stereomicroscope at ${\times}10$ magnification. The results were as follows; 1. All experimental groups produced favorable apical seal. 2. The mean leakage was $0.23{\pm}0.25mm$ for group 1, $0.17{\pm}0.21mm$ for group 2, and $0.19{\pm}0.23mm$ for froup 3, but there was no statistical difference amoung them. Within the limits of the results of this experiment, the Continuous Wave gutta-percha obturation technique demonstrated relatively favorable apical sealing effect and shorter obturation time. Thus, it is thought that this obturation technique is a acceptable method for clinical use but further studies on this metter should be conducted.
The purpose of this study was to evaluate the effect of heated spreader on the sealing ability of lateral condensation, compared with regular cold spreader. Forty two extracted human teeth with single canal were randomly placed into 3 experimental groups, and four additional teeth were used as positive and negative controls. Each group was prepared with Ni-Ti Profile #40 using step-down technique and obturated with standardized colored gutta-percha cone by standard(cold) lateral condensation technique, warm lateral condensation technique with Endotec and hot spreader soaked in glass bead sterilizer, each with Sealapex sealer. Control groups were not obturated, but prepared. After 2 days in 2% methylene blue, the teeth were invested and made into transparent resin blocks. And then, each block was sectioned horizontally with microtome at 1, 2, 3, 4, 5 mm levels from the apex. The linear extent of dye penetration was examined with stereomicroscope at ${\times}$20 magnification. At each of 5 levels, ratio of the area of gutta-percha was obtained by calculating the area of gutta-percha to the total area of the canal. The data collected were then analyzed statistically using an analysis of variance(ANOVA) and Scheffe test. The results were as follows ; 1. All experimental groups produced the apical microleakage. 2. The mean leakage was 1.57${\pm}$0.76mm for cold spreader group, 0.86${\pm}$0.95mm for Endotec spreader group, and 0.64${\pm}$0.93mm for hot spreader group. The difference between hot spreader group and cold spreader group was statistically significant(p<0.05). 1. At the 1 mm level, the mean ratio of area of gutta-percha was 74.58${\pm}$13.15(%) for cold spreader group, 65.42${\pm}$14.62(%) for Endotec spreader group, and 80.72${\pm}$14.63(%) for hot spreader group. There was statistically significant difference between hot spreader group and Endotec spreader group(p<0.05). 2. At the 2mm level, the mean ratio of area of gutta-percha was 87.86${\pm}$11.22(%) for cold spreader group, 66.55${\pm}$14.02(%) for Endotec spreader group, and 92.93${\pm}$7.24(%) for hot spreader group. There was statistically significant difference between Endotec spreader group and other two spreader groups(p<0.05). 3. At the level 3, 4, 5 mm, there was no statistically significant difference between each group. Within the limits of the results of this experiment, warm lateral condensation technique with hot spreader soaked in a glass bead sterilizer demonstrated favorable apical sealing effect and improved density of gutta-percha mass. Thus, it is thought that this obturation technique is effective for clinical use and beneficial to reduce condensation forces, also economical and easy. Purpose: While studies have examined microleakage in endodontically treated teeth restored with posts, microleakage among post and adhesive systems remains a concern. This study compared the sealing properties of 3 adhesively luted post systems. Materials and methods: Thirty-six endodontically treated permanent maxillary central incisors were divided into 3 groups: Zirconia-glass fiber, Quartz-glass fiber, Polyethylene fiber posts. Post space was prepared and each post was adhesively luted with 3 systems. The specimens were separately immersed in freshly prepared 2% methylene blue solution for 1 week. The cleaned specimens were then embedded in autopolymerizing acrylic resin. The root portion of tooth were horizontally sectioned into three pieces (apical, middle, and coronal portions). An occlusal view of each section was digitally photographed with a stereomicroscope. The methylene blue-infiltrated surface for each specimen was measured. Dye penetration was estimated as the ratio of the methylene blue-infiltrated surface to the total dentin surface. Results: No significant differences were found among post types. The variables of middle section and 3-stage adhesive produced significant differences in microleakage between the following post pairs: zirconia-glass fiber versus quartz-glass fiber, zirconia-glass fiber versus polyethylene fiber, and quartz-glass fiber versus polyethylene fiber (P<.05). There were significant differences between the apical and coronal sections of each post type, and between apical versus middle sections of quarze-glass fiber and polyethylene fiber posts (P<.05). Conclusion: No significant differences were found among post types. The 3-stage adhesive produced significant differences in microleakage between the following post pairs.
The purpose of this study was to evaluate the sealing ability of a calcium hydroxide plug treated with a bonding agent. Ninety extracted human anterior teeth and premolars with single canal were used in this study. Crowns were removed. the canal's were instrumented. and the roots were randomly divided into three groups of 30 each. In control group. a single apical seat was prepared with #60 K file 1mm short of the apex and the root canal was obturated with Gutta-percha and Sealapex by the lateral condensation method. In experimental group 1 and group 2. to prepare an apical isthmus of 1mm in length. the first apical seat was prepared with a #45 K file 1mm short of the anatomical apex and with a #60 K file 2mm short for the second apical seat. Dry calcium hydroxide powders were packed in the apical isthmus with a hand plugger and #60 K file and then. the root canal was obturated with Gutta-percha and Seal apex by the lateral condensation method. In experimental group 2. following an application of the bonding agent to the plug. the root canal was obturated in the same way. The teeth of each group were immersed in a 2% methylene blue dye solution. for 1, 2, and 4 weeks. The distance from the tip of the cone to the deepest penetration was measured using the Tool maker's microscope. The results were as follows : 1. The teeth having the calcium hydroxide plug treated with the dentin bonding agent (experimental group 2) showed the lowest leakage with 1.4705mm and the control group without apical plug(no apical isthmus) showed the highest leakage with 3.1735mm. 2. The control group without apical plug showed higher leakage than experimental group 1 having the calcium hydroxide plug treated without the dentin bonding agent(p>0.05). 3. The control group without apical plug and experimental group 1, having the calcium hydroxide plug treated without the dentin bonding agent. showed higher leakage than experimental group 2. having the calcium hydroxide plug treated with the dentin bonding agent(p<0.001). 4. The immersion time had no significant effect on the degree of leakage. In conclusion, the results showed that the calcium hydroxide plug treated with the dentin bonding agent could decrease the microleakage from the root apex effectively.
The purpose of this study was to evaluate the effects of EDTA and pulsed Nd :YAG laser on apical leak-age of canal obturation. Forty-eight single-rooted teeth were used in this study. The teeth were instrumented up to a size 40 K-file and irrigated with 2.5% NaOCl between each fie size. And the teeth were divided into 4 groups. In group A, the root canals were irrigated with a final flush of 5ml 2.5% NaOCl as a control group. The teeth in group B were irrigated with a final flush of 5ml 17% EDTA. The teeth in group C and D were irradiated by pulsed Nd:YAG laser(laser parameters were set at 1W, 100mJ, 10Hz, and 2W, 100mJ, 20Hz respectively). The results were as follows : 1. Apical leakage was observed in 50% of samples in group A, 30% of samples in group B, 20% of samples in group C, and 10% of samples in group D. 2. The teeth in group B had less leakage than group A, but there was no statistically significant differences(p>0.05). 3. The teeth in group C, D had less leakage than group A, and there was statistically significant differences(p<0.05). 4. The teeth in group C, D had less leakage than group B, but there was no statistically significant differences(p>0.05). 5. There was no significant differences in apical leakage between group C and group D(p>0.05).
The purpose of this study was to evaluate the microleakage electrochemically using different retrograde filling materials, different root resection angle, and different cavity preparation instruments. 104 extracted single-rooted teeth were selected for this study. 100 teeth were used as experimental groups and four was used as controls. Anatomical crowns were resected, root canals were prepared, and the apical 2 mm of roots were removed. The experimental roots were randomly divided into five equal groups. Experimental groups : Group 1. no bevel, cavity preparation with ultrasonic instruments, amalgam filling Group 2. no bevel, cavity preparation with ultrasonic instruments, SuperEBA cement filling Group 3. no bevel, cavity preparation with ultrasonic instruments, desiccated ZOE filling Group 4. $45^{\circ}$ bevel, cavity preparation with ultrasonic instruments, amalgam filling Group 5. no bevel, cavity preparation with conventional bur, amalgam filling Microleakage was measured once a day for 30 days using electrochemical method and were analyzed statistically. The results were as follows : 1. The group with Super EBA cement filling showed the least marginal leakage from second to fourth day(p<0.05), there was no significant difference between the group with amalgam filling during eighth to eighteenth day(p>0.05), but after the nineteenth day here was a higher marginal leakage than the group with amalgam filling(p<0.05). 2. The group with desiccated ZOE filling demostrated that the highest marginal leakage, started on the eighth day(p<0.05). 3. The group using ultrasonic instrument showed lower marginal leakage than the group using bur until the nineteenth day(p<0.05), but there was no significant differnce with the group using bur after twentythird day(p>0.05). 4. The group without bevel showed lower marginal leakage than the group with bevel (p<0.05). 5. Whether bevel or nor had much more effect on marginal leakage than with cavity perparation instrument when the cavity was retrogrdefilled with amalgam(stepwise regression).
