Asymmetry in mesial root number and morphology in mandibular second molars: a case report

  • Ambiguity in the root morphology of the mandibular second molars is quite common. The most common root canal configuration is 2 roots and 3 canals, nonetheless other possibilities may still exist. The presence of accessory roots is an interesting example of anatomic root variation. While the presence of radix entomolaris or radix paramolaris is regarded as a typical clinical finding of a three-rooted mandibular second permanent molar, the occurrence of an additional mesial root is rather uncommon and represents a possibility of deviation from the regular norms. This case report describes successful endodontic management of a three-rooted mandibular second molar presenting with an unusual accessory mesial root, which was identified with the aid of multiangled radiographs and cone-beam computed tomography imaging. This article also discusses the prevalence, etiology, morphological variations, clinical approach to diagnosis, and significance of supernumerary roots in contemporary clinical dentistry.

Common Hepatic Artery Originating from Left Gastric Artery: A Rare Variant Encountered in Gastric Cancer Surgery

  • The hepatic artery can have numerous variations. However, a common hepatic artery originating from the left gastric artery and the entire hepatic blood supply furnished by the left gastric artery is an extremely rare anomaly. We encountered this anomaly in a patient with advanced gastric cancer. A surgeon should recognize this image appearance and identify the anomaly. Without knowledge of this anomaly and given the strategy for extensive lesser sac dissection generally employed during gastric cancer surgery, a surgeon could easily inadvertently divide this vessel, resulting in critical liver damage. We report a case of common hepatic artery originating from left gastric artery and review of the literatures.

유방암 환자의 모의치료, CT 스캔 및 치료 과정에서 발생되는 준비 오차 분석 (Analysis of Set-up Errors during CT-scan, Simulation, and Treatment Process in Breast Cancer Patients)

  • 목적: 방사선 치료 시 3차원 영상 획득에 방사선치료 전용으로 개발된 모의 CT를 사용하고 있으나 아직까지도 많은 병원에서는 일반 진단용 CT를 이용하고 있다. 따라서 본 연구에서는 21명의 유방암 환자를 대상으로 모의치료, 진단용 CT기를 이용한 CT 스캔, 및 치료 과정 사이의 준비 오차를 분석하였다. 대상 및 방법: 준비 오차는 isocenter, SSD, CLD, 및 수술 시 삽입된 클립의 위치들의 변화를 계산하여 분석하였다. 모의조사에서 얻어진 x-ray 영상에 나타난 해부학적 구조물과 CT 스캔 시 isocenter를 표시하기 위해 환자의 몸에 부착된 marker를 기준으로 정해진 isocenter에서 얻은 DRR 영상상의 구조물을 비교하여 잘 일치하지 않을 경우 새로운 isocenter가 정해졌고 이러한 isocenter의 위치 변화를 계산하였다. 결과: 21명의 환자 중 7명의 경우 DRR상과 모의치료 필름상의 해부학적 구조물이 21명의 환자 중 7명이 일치하지 않았으므로 치료계획을 실행하기에 앞서 새로운 isocenter를 정하였다. Isocenter 이동을 근거로 계산된 진단용 CT와 모의 치료간에 발생되는 평균 준비오차의 표준편차는 횡측 방향으로 2.3 mm, longitudinal 방향으로 1.6 mm, 그리고 AP 방향으로 1.6 mm이다. 모의치료와 CT data의 AP 방향 및 tangential 방향에서 측정된 SSD 값의 평균오차 및 표준편차는 각각 $1.9{\pm}2.3\;mm$$2.8{\pm}3.7\;mm$이다. 모의치료와 DRR간의 CLD 오차의 변화범위는 0에서 6 mm 이고 모의치료와 portal 영상간의 오차범위는 0에서 5 mm이다. 클립을 기준으로 계산된 그룹의 systematic error는 횡측 방향으로 1.7 mm, AP 방향으로 2.1 mm, 그리고 SI 방향으로 1.7 mm이다. 결론: 연구 결과 SSD, CLD, 클립의 움직임 및 isocenter의 위치변화 측면에서 분석될 경우 그다지 큰 오차는 발생하지 않았음을 보여준다. 그러므로 본 연구결과 유방암 환자의 경우 진단용 CT를 사용한다 하더라도 준비오차는 모의 CT를 사용하는 경우와 비교하여 차이가 없음을 알 수 있다. 그러나 모의치료와 CT스캔 사이의 준비오차를 감소하기 위해서는 CT 영상 획득 시 환자 위치고정에 특별한 주의를 기울여야 한다.

치과임플란트 치료와 관련된 상악구치부의 임상적 연구 (CLINICAL STUDY ON KOREAN POSTERIOR MAXILLAE RELATED TO DENTAL IMPLANT TREATMENT)

  • Purpose of study: The purpose of this study was to provide adequate diagnostic guideline for the maxillary sinuses prior to dental implant treatment for edentulous posterior maxillary areas. For this purpose, our procedure involves the estimation of the remaining alveolar bone height, the examination of the anatomical variation in the maxillary sinuses (e.g. sinus septum), and the evaluation of the incidence of preoperative pathological conditions in the maxillary sinuses. Materials and Methods: We selected 189 patients to undergo computerized tomography (CT) in order to account for the posterior maxillary anatomy found in patients of Korean ethnicity. We evaluated the following using Dentascan software: Remaining alveolar bone height, incidence of sinus septum, and rate of preoperative pathologic conditions in the maxillary sinus. The average amount of remaining alveolar bone height was analyzed using the student's t-test for differences according to anatomical site, and the ANOVA was used for the differences according to age group with the level of significance set at 0.05. Results: Alveolar bone heights of upper first premolar, second premolar, first molar, and second molar was 12.24 mm, 10.37 mm, 7.16 mm, and 7.15 mm, respectively with statistical significance (P < 0.05). Incidence of sinus septum as an anatomic variation was 17 out of 189 cases (9.0%). Incidence of mucosal thickening as a pathologic variation was 82 out of 189 cases (43.4%). Conclusion: In treatment planning of posterior maxillary edentulous area of Koreans, the consideration of augmentation surgery for maxillary sinus is required in maxillary molar area before dental implant installation, and preoperative screening of the asymptomatic maxillary sinuses can be regarded as a reasonable preoperative procedure in the planning of dental implant treatment on the posterior maxillary edentulous area.

하악 제2대구치 치근의 형태학적 구조 및 표면적에 관한 연구 (A STUDY OF THE ROOT SURFACE AREA, ANATOMIC STRUCTURE AND LINEAR VARIATION OF THE ROOT SURFACE AREA OF THE MANDIBULAR SECOND MOLAR)

  • The thirty six mandibular second molars, which were extracted because of hopeless tooth due to advanced periodontal disease, were measured the length of mesial and distal root and the distance from cementoenamel junction to root separation. The molars were cross-sectioned every 1.5 milimeter from cementoenamel junction to root apex perpendicular to long axis and each section was photographed, projected and measured with a calibrated Digital Curvi-Meter(Com Curvi-8. Japan). The root surface area (RSA), percentage of the RSA and the linear variation of the RSA were calibrated for each 1. 5 mm section. The results were as follows. 1. The mean length of the roots was 12. 98mm for mesial root, 11.84 mm for distal root. The mesial root was longer than distal root.(p<0.01) 2. The mean distance from the cementoenamel junction to the point at which the root separate from the root trunk was 3.82mm for the buccal furcation and 4.75mm for lingual furcation. The buccal root separation was coronal than the lingual root separation.(p<0.01) 3. The total root surface area was $317.78mm^2$. 4. The mean surface area of the root trunk was $150.06mm^2$ and averaged 42.54% of the total root surface area. 5. The mean root surface area was $88.79\;mm^2$ for the mesial root, $78.93mm^2$ for distal root, The mesial root surface area was wider than the distal root surface area.(p<0.05) 6. In comparision, the mean root trunk surface area of the mandibular 2nd molar was wider than that of mandibular 1st molar(p<0.01), but each root of 2nd molar was smaller than that of 1st molar(p<0.01).

Auto-segmentation of head and neck organs at risk in radiotherapy and its dependence on anatomic similarity

  • Purpose: The aim is to study the dependence of deformable based auto-segmentation of head and neck organs-at-risks (OAR) on anatomy matching for a single atlas based system and generate an acceptable set of contours. Methods: A sample of ten patients in neutral neck position and three atlas sets consisting of ten patients each in different head and neck positions were utilized to generate three scenarios representing poor, average and perfect anatomy matching respectively and auto-segmentation was carried out for each scenario. Brainstem, larynx, mandible, cervical oesophagus, oral cavity, pharyngeal muscles, parotids, spinal cord, and trachea were the structures selected for the study. Automatic and oncologist reference contours were compared using the dice similarity index (DSI), Hausdroff distance and variation in the centre of mass (COM). Results: The mean DSI scores for brainstem was good irrespective of the anatomy matching scenarios. The scores for mandible, oral cavity, larynx, parotids, spinal cord, and trachea were unacceptable with poor matching but improved with enhanced bony matching whereas cervical oesophagus and pharyngeal muscles had less than acceptable scores for even perfect matching scenario. HD value and variation in COM decreased with better matching for all the structures. Conclusion: Improved anatomy matching resulted in better segmentation. At least a similar setup can help generate an acceptable set of automatic contours in systems employing single atlas method. Automatic contours from average matching scenario were acceptable for most structures. Importance should be given to head and neck position during atlas generation for a single atlas based system.

Assessment of the anterior loop of the mandibular canal: A study using cone-beam computed tomography

  • Purpose: Sufficient area in the interforaminal region is required for dental implant placement, and the anterior loop of the mandibular canal is located within the limits of this area. The aim of this study was to evaluate the prevalence and extent of the anterior loop in a Brazilian sample population using cone-beam computed tomography (CBCT). Materials and Methods: CBCT images from 250 patients (500 hemimandibles) obtained for various clinical indications were randomly selected and evaluated to determine the presence and length of the anterior loop. The length of the anterior loop was then compared based on gender, age, and the side of the mandible. The data were analyzed using the Pearson chi-square test and linear regression analysis. Results: An anterior loop was identified in 41.6% of the cases, and its length ranged from 0.25 mm to 4.00 mm (mean, $1.1{\pm}0.8mm$). The loop had a greater mean length and was significantly more prevalent in males (p=0.014). No significant differences were found between the right and left sides regarding length (p=0.696) or prevalence (p=0.650). Conclusion: In this study, a high prevalence of the anterior loop of the mandibular canal was found, and although its length varied greatly, in most cases it was less than 1 mm long. Although this is a prevalent anatomical variation, safety limits for the placement of implants in this region cannot be established before an accurate evaluation using imaging techniques in order to identify and preserve the neurovascular bundles.

  • 긴엄지발가락폄근의 보조힘줄은 긴엄지발가락폄근 변이의 일종이며 드물지 않게 관찰된다. 저자들은 긴엄지발가락폄근 힘줄이 파열된 38세 여성 환자에서 신체진찰상 긴엄지발가락폄근의 부분파열이 의심이 되어 보존적 치료를 고려하였으나 자기공명영상에서 주힘줄은 완전 파열되고 주힘줄과 나란히 주행하는 보조힘줄이 발견되어 수술적 치료를 시행한 증례를 보고하고자 한다.

  • The interforaminal region is usually considered as a safe region in the chin bone graft without important vital anatomical structures to be damaged. But the accurate anatomy of the interforaminal region, with its potential clinical relationships, is controversial. Moreover some complications suggesting damage of incisive terminal branches after chin bone harvesting are reported such as sensory discomfort and pain etc. In order to verify incisive innervation of symphyseal area, we examined the cross-sectional CT scan images taken for preoperative planning of implant placement with chin bone graft and some parameters were measured; (1) visuality rating of incisive canal (2) vertical and horizontal diameter of canal (3) distance from lower border of the incisive canal to the lower border of the mandible (4) shortest distance from anterior border of the incisive canal to the anterior border of the mandible. We report the positive outcome that decrease the complications related with the damages of incisive branch during bone harvesting from the chin.

한국인 치아치은 집합체구조의 삼차원적인 분석 (Three dimensional analysis of Korean dentogingival complex)

  • Purpose: Variation in the morphology of gingival papilla may be determined by the shape and position of anatomic crown as well as contact area and embrasure form of individual teeth. However, periodontal biotype classification is regarded to be subjective because of the lack of definite criteria. In this study, we defined the objective parameters which constitute the periodontal biotype and measured their relationship. Materials and Methods: 109 of dental casts were prepared using three dimensional scanner and specialized reconstruction software, then acquiredvirtual models were sent to the 20 professional dentists to define the specific periodontal biotypes. Several parameters around periodontal structures were measured from the virtual models; facial surface area of the anterior tooth (AT), anterior papillary area (AP), proportion of the dento-papillary complex, clinical papillary length (PL), and clinical papillary angle (PA). Statistical analysis was performed to confirm the relationship among parameters. Results: Coincidence rate of periodontal biotype within observers was $63.77{\pm}16.05%$. Coincidence rate between observers was $76.15{\pm}16.43%$. Among the parameters measured, PL showed the most positive correlations and PA presented the most negative correlations. The parameter of the AP and PL of six maxillary anterior teeth showed significant correlation coefficient. Conclusion: Anterior papillary area and clinical papillary length would be objective parameters for determining the consistent periodontal biotypes.
