Tri-enzyme modified electrochemical biosensor for paracetamol detection (파라세타몰 검출을 위한 전기화학적 다중효소 바이오센서)

  • A new disposable amperometric tri-enzyme biosensor for the detection of paracetamol has been developed. The paracetamol sensors developed uses horseradish peroxidase modified screen-printed carbon electrodes (HRP-SPCEs) coupled with immobilized enzymes, tyrosinase and aryl acylamidase, prepared using a poly (vinyl alcohol) bearing styrylpyridinium groups (PVA-SbQ) matrix. Optimization of the experimental parameters has been performed and the paracetamol biosensor showed detection limit for paracetamol is as low as $100{\mu}M$ and the sensitivity of the sensor is $1.46nA{\mu}M^{-1}cm^{-2}$.

Distribution of Abused Drugs in 275 Alcohol-positive Blood Samples of Korean Driver (음주운전자 275명 혈액 중 마약류 및 남용약물의 분석)

  • Even though driving under the influence of drug (DUID) is a worldwide problem, we, Korea has no regulation system yet except for alcohol, and there are little cases reported related to DUID. In order to investigate the type of abused drugs for drivers in Korea, we tried to analyze controlled and non-controlled drugs in alcohol-positive blood samples. 275 whole bloods, which were positive for alcohol on the roadside test, were collected from the police for two months ($Nov.{\sim}Dec.$ 2006). The analytical strategy was constituted of three steps: First, alcohol in blood samples were confirmed and quantified by gas chromatography. Second, controlled drugs were screened by $Evidence_{investigator}\;^{TM}$ (Randox, U.K.) as preliminary test. It was based on immunoassay by biochip array analyzer. Nine groups of drug abuse were screened: amphetamines, methamphetamines, cannabis, cocaine, opiates, barbiturates, methadone, benzodiazepines I (oxazepam) & II (lorazepam). Finally, confirmation of these drugs was performed by GC-MS. Blood samples were extracted by solid-phase extraction by $RapidTrace^{TM}$ (Zymark, U.S.A.). After trimethylsilyl (TMS) derivatization, eluates were analyzed to GC-MS. Total 49 drugs were investigated in this study including controlled drugs, antidepressants, 1st generation antihistamines, dextromethorphan, nalbuphine, ketamine, etc. For rapid detection, we developed the automated identification system. It was made up a new software, "DrugMan", modified Chemstation data analysis menu and newly developed macro modules. A series of peak selection, identification and reporting of the results were performed automatically by this system. Concentrations of alcohol in 275 blood samples were ranged from 0.011 to 0.249% (average, 0.119%). Among 149 blood samples, just six samples (4.0%) were showed positive results to the immunoassay: one methamphetamine and five benzodiazepines group I. By GC-MS confirmation, only benzodiazepines were detected and methamphetamine was not detected from immunoassay positive blood sample. Besides these drugs, 5 chlorpheniramines, dextromethorphan, diazepam, doxylamine, ibuprofen, lidocaine and topiramate were also detected in whole bloods by GC-MS. Conclusively, the frequency of drug abuse for Korean drivers was relatively low. There was none case which illegal drug was detected. However these results were limited to alcohol positive blood samples, so it is necessary to analyze more samples including alcohol negative blood.

Effects of Sugars Addition in Alcohol Fermentation of Oriental Melon (첨가 당의 종류가 참외의 알코올발효에 미치는 영향)

  • This study investigated effects of types of added sugar on alcohol fermentation of oriental melon. According to the results, pH was not significantly different according to types of added sugar and fermentation process. Total acidity increased with fermentation process in all groups by recording around 1.4% at the ninth day of fermentation. For sugar content, its initial level was $22^{\circ}Brix$, and alcohol fermented oriental melon fluids added by sucrose, fructose and glucose recorded similar levels or $6.6{\sim}6.8^{\circ}Brix$ while the fluids added by honey and fructo-oligosaccharide showed a slightly higher level or $8.1^{\circ}Brix$ at the ninth day of fermentation. Although free sugar content was different in the early phase of fermentation according to types of added sugar such as sucrose, fructose and glucose, it reduced with fermentation process to nearly non-detection at the ninth day of fermentation. As organic acids, lactic acid and acetic acid were observed in all phases of fermentation and their contents became higher gradually with fermentation process. Alcohol content showed the highest level in alcohol fermented oriental melon fluid added by sucrose by recording 12.80% and was relatively low in the fluids added by fructose and oligosaccharide. For alcohol, acetaldehyde, n-propanol and iso- amylalcohol contents were not significantly different according to types of added sugar and methanol content was the lowest in the fluid added by fructose by recording 84.99 ppm.

Application of Crossover Analysis-logistic Regression in the Assessment of Gene- environmental Interactions for Colorectal Cancer

  • Background: Analysis of gene-gene and gene-environment interactions for complex multifactorial human disease faces challenges regarding statistical methodology. One major difficulty is partly due to the limitations of parametric-statistical methods for detection of gene effects that are dependent solely or partially on interactions with other genes or environmental exposures. Based on our previous case-control study in Chongqing of China, we have found increased risk of colorectal cancer exists in individuals carrying a novel homozygous TT at locus rs1329149 and known homozygous AA at locus rs671. Methods: In this study, we proposed statistical method-crossover analysis in combination with logistic regression model, to further analyze our data and focus on assessing gene-environmental interactions for colorectal cancer. Results: The results of the crossover analysis showed that there are possible multiplicative interactions between loci rs671 and rs1329149 with alcohol consumption. Multifactorial logistic regression analysis also validated that loci rs671 and rs1329149 both exhibited a multiplicative interaction with alcohol consumption. Moreover, we also found additive interactions between any pair of two factors (among the four risk factors: gene loci rs671, rs1329149, age and alcohol consumption) through the crossover analysis, which was not evident on logistic regression. Conclusions: In conclusion, the method based on crossover analysis-logistic regression is successful in assessing additive and multiplicative gene-environment interactions, and in revealing synergistic effects of gene loci rs671 and rs1329149 with alcohol consumption in the pathogenesis and development of colorectal cancer.

Amount of bacteria over time according to the use of antibacterial and wet wipes behavior (항균티슈와 물티슈 사용에 따른 시간별 세균 수 변화의 차이)

  • Background: This study was conducted to analyze the time for re-detection of bacteria after surface disinfection using wet wipes, isopropyl alcohol, and benzalkonium chloride antibacterial tissue and provide standards for re-execution of surface disinfection with wet and antibacterial tissues. Methods: Seven laptops were wiped with wet tissue and isopropyl alcohol and benzalkonium chloride antibacterial tissues. Test areas were rubbed with a sterile cotton swab at baseline and after 30, 60, and 120 min. After plating on a tryptic soy agar medium, the number of colonies was counted by culturing at 36.5℃ for 24 h. Results: The average number of bacterial colonies was 5.85 ± 4.33 before isopropyl alcohol wiping and nil after wiping. The average number of bacterial colonies was 12.28 ± 14.67 benzalkonium chloride wiping and nil after wiping. Before wiping with wet wipes, the average number of bacterial colonies on laptop surfaces was 3.42 ± 5.22. Bacteria decreased after wiping with wet wipes but increased again over time. Conclusions: Wet wipes can temporarily reduce bacteria but are unsuitable for removing bacteria.

15-Year Long-Term Follow-Up on the Progression to Liver Cancer in a Patient with Alcoholic Hepatitis Who Continued Drinking: A Case Report and Clinical Approach (음주를 지속한 알코올성 간염 환자의 간암 진행에 대한 15년 장기 추적 관찰 : 증례 보고 및 임상적 접근)

  • Objectives: This case report aims to present the long-term clinical progression of a patient diagnosed with alcoholic hepatitis, which subsequently advanced to liver cirrhosis and hepatocellular carcinoma over a 15-year period. The report highlights the impact of continued alcohol consumption on the progression of alcoholic liver disease and provides a clinical analysis of the course of the disease. Methods: A single patient diagnosed with alcoholic hepatitis was analyzed retrospectively. The patient was treated with both traditional Korean medicine and conventional medicine over multiple hospitalizations between 2012 and 2021. Clinical outcomes were monitored through blood chemistry, imaging studies, and Child-Pugh scores. Results: Despite consistent treatment, the patient's continued alcohol consumption accelerated the progression from alcoholic hepatitis to cirrhosis and, eventually, hepatocellular carcinoma. The patient's condition remained classified as Child-Pugh B throughout the follow-up, and he ultimately expired in 2023. Conclusion: This case emphasizes that alcohol cessation is crucial in preventing the rapid progression from alcoholic hepatitis to liver cancer. Early detection through noninvasive fibrosis tests such as FibroScan, proactive management of complications, and the use of prognostic tools such as Maddrey Discriminant function (MDF), in addition to patient education, are essential for better outcomes.

Development of PZT Piezoelectric Biosensor for the Detection of Formaldehyde (Formaldehyde 측정을 위한 PZT 압전 바이오센서 개발)

  • A biosensor with PZT piezoelectric ceramic crystal was developed for the detection of formaldehyde gas. Poled PZT piezoelectric ceramic disk was made from ZrO2, TiO2 and Nb2O5, together with the addition of PbO and polyvinyl alcohol, through various processes of mixing, calcination drying, crushing, forming, sintering, polishing, ion coating and poling. Oscillator circuit of sensor was made of operational amplifier(AD811AN). Formaldehyde dehydrogenase was immobilized onto a piezoelectic ceramic crystal, together with the cofactors, reduced glutathione and nicotinamide adenine dinucleotide. The effect of flow rate on the sensitivity was determined by varing the flow rate of carrier gas from 24.7mL/min to 111.7mL/min through detector cell. The results indicated that as the flow rate was increased, the recovery rate was increased. And a significant increase in the sensitivity was observed in enhanced flow rate of carrier gas. Frequency difference(ΔF) of immobilized PZT piezoelectic disk increased proportionally to the concentration gas and reproduced to repeated exposures of formaldehyde gas(28ppm, Δ68Hz).

Determination of ethyl carbamate in maesil wine by alcohol content and ratio of maesil (Prunus mume) during ripening period (알코올 농도 및 담금비에 따른 숙성 기간별 매실주의 에틸카바메이트 함량조사)

  • This study was designed to investigate the formation of ethyl carbamate (EC) during the ripening of Maesil with sugar and Soju (19.5~35% alcohol contents) using a homemade method. Maesil, sugar and Soju were purchased at ordinary market in June of 2012. The preparation of sample for analysis was conducted by method of Henry et al. The analysis of GC/MS was used SIM mode (m/z 89, 74, 62). Quantification was performed in terms of the 62 ion and was based on an internal standard procedure. Good linearity was obtained with a regression coefficient ($r^2$ = 0.993). Low detection limits (LOD) was achieved 4.31 ug/kg and recovery for alcohol was 74.8%. During 90 days, fermentation with sugar was not detected EC (under LOQ). 15 days ripened Maesil wine contained EC between non detected~32.7 ug/kg and 90 days ripened Maesil wine was 19.7~87.4 ug/kg. Higher proportion of Maesil and Soju increased EC contents. EC levels were increased 32.7 ug/kg to 87.4 ug/kg in the ratio of Maesil to 35% alcohol-Soju (1:1). In the ratio of Maesil to 35% alcohol-Soju (1:3) was increased non detected to 69.7 ug/kg. After 90 days, Maesil wine was filtered Maesil through a seive and ripened by 180 days to investigate the formation of EC compared with non filtered. Treatment of filtered, EC contents was much higher level compared with non filtered. Therefore, this result showed that alcohol contents contribute to increase EC formation more than Maesil.

Effect of Daekumeumja Herb-acupuncture on Alcohol-induced Suppressed Cell Proliferation and Expression of Nitric Oxide Synthase in Hippocampus of Rats (대금음자(對金飮子) 약침이 알코올 독성 흰쥐의 해마에서 신경세포생성과 NOS 발현에 미치는 영향)

  • Objectives : The purpose of this study is to assess the effect of Daekumeumja herb-acupuncture on neural cell proliferation and nitric oxide synthase(NOS) expression in hippocampus of ethanol-intoxicated Sprague-Dawley(SD) rats. Methods : SD rats were randomly assigned into 5 groups ; the normal group, the alcohol-treated(control) group, the alcohol- and 1 mg/kg Daekumeumja- treated(sample A) group, the alcohol- and 5mg/kg Daekumeumja-treated (sample B) group, the alcohol- and 10 mg/kg Daekumeumja-treated (sample C) group(n = 6 in each group). Normal group were received with Saline, while control group were injected intraperitoneally with alcohol(2 g/kg) once per day for 5 days. Sample groups were treated Daekumeumja herb-acupuncture on Chungwan(CV12) for 5 consecutive days. Bromo-deoxyuridine(BrdU) was injected into all animals once per day for 5 days. For the detection of BrdU-positive cells and NADPH-d- positive cells in hippocampus, immunohistochemistry was performed. Results : 1. In the dentate gyrus area, the number of BrdU-positive cells in the sample $B(278.08{\pm}6.46)$, $C(331.33{\pm}16.68)$ groups was significantly(p<0.05) increased compared with the control group. 2. In the dentate gyrus area, the number of NADPH-d-positive cells in the sample $C(86.50{\pm}10.02)$ groups was significantly(p<0.05) increased compared with the control group. 3. In the CAI area, the number of NADPH-d-positive cells in the sample $A(63.90{\pm}2.69)$, $B(75.70{\pm}3.01)$, $C(97.70{\pm}4.06)$ groups was significantly(p<0.05) increased compared with the control group. 4. In the CA 2-3 area, the number of NADPH-d-positive cells in the sample $B(30.20{\pm}1.89)$, $C(62.70{\pm}2.08)$ groups was significantly(p<0.05) increased compared with the control group. Conclusion : These results indicate that, neural cell proliferation and NOS expression in hippocampus was reduced in ethanol-intoxicated group. Treatment of Daekumeumja herb-acupuncture increased this diminution. Daekumeumja could be able to effect on the prevention of the amnesia and learning disability in alcoholism.

Cancer Detection Rates in a Population-Based, Opportunistic Screening Model, New Delhi, India

  • Background: In India, cancer accounts for 7.3% of DALY's, 14.3% of mortality with an age-standardized incident rate of 92.4/100,000 in men and 97.4/100,000 in women and yet there are no nationwide screening programs. Materials and Methods: We calculated age-standardized and age-truncated (30-69 years) detection rates for men and women who attended the Indian Cancer Society detection centre, New Delhi from 2011-12. All participants were registered with socio-demographic, medical, family and risk factors history questionnaires, administered clinical examinations to screen for breast, oral, gynecological and other cancers through a comprehensive physical examination and complete blood count. Patients with an abnormal clinical exam or blood result were referred to collaborating institutes for further investigations and follow-up. Results: A total of n=3503 were screened during 2011-12 (47.8% men, 51.6% women and 0.6% children <15 years) with a mean age of 47.8 yrs (${\pm}15.1yrs$); 80.5% were aged 30-69 years and 77.1% had at least a secondary education. Tobacco use was reported by 15.8%, alcohol consumption by 11.9% and family history of cancer by 9.9% of participants. Follow-up of suspicious cases yielded 45 incident cancers (51.1% in men, 48.9% in women), consisting of 55.5% head and neck (72.0% oral), 28.9% breast, 6.7% gynecological and 8.9% other cancer sites. The age-standardized detection rate for all cancer sites was 340.8/100,000 men and 329.8/100,000 women. Conclusions: Cancer screening centres are an effective means of attracting high-risk persons in low-resource settings. Opportunistic screening is one feasible pathway to address the rising cancer burden in urban India through early detection.
