고관절 전치환술 환자의 Critical pathway적용효과 (Effects of Critical Pathway(CP) on the patients with Primary Total hip replacement(THR))

  • Purpose: The purpose of this quasi-experimental research is to develop the CP for primary THR patients and verify its effectiveness. Method: The CP is designed for both patients and medical employees to meet the 14-day-long hospitalization with an expert and clinical validity test, and the standardized orders are also created. 21 eligible subjects for the control group(with common care plan) and 27 subjects for the experimental group(with CP service) were assigned. Data were collected from questionnaires to evaluate patients' satisfaction level of nursing care and CP, investigation of physical complications, inspection of LOS(length of hospital stay) and hospital charges from April 2000 to February 2001 at K-hospital in Daegu. The collected data were analyzed by using the SAS program. Results: After implementation of CP, there was a statistically significant reduction in mean length of stay. The hospital charges have no considerable difference between two groups. Frequency of physical complications is reduced considerably. The experimental group scored much higher than the control group on the satisfaction level toward nursing care. And in the analysis of CP satisfaction questionnaires, many subjects have high satisfaction in items of 'I see the treatment process and hospital life', 'I feel that I am participating in my treatment process with health care staff'. The analysis of variances which cause a prolonged LOS supports that it's necessary to strengthen an evaluation of pre-operative outpatients' condition and link the home nursing care system in discharge for more shortening of LOS. Conclusion: The above results show that CP can have a positive impact on satisfaction level of inpatients with primary THR and the hospital.

골시멘트 특성 및 스템 형상에 따른 시멘트 타입 인공관절의 생체역학적 평가 (Biomechanical Evaluation of Cement type hip Implants as Conditions of bone Cement and Variations of Stem Design)

  • The total hip replacement (THR) has been used as the most effective way to restore the function of damaged hip joint. However, various factors have caused some side effects after the THR. Unfortunately, the success of the THR have been decided only by the proficiency of surgeons so far. Hence, It is necessary to find the way to minimize the side effect caused by those factors. The purpose of this study was to suggest the definite data, which can be used to design and choose the optimal hip implant. Using finite element analysis (FEA), the biomechanical condition of bone cement was evaluated. Stress patterns were analyzed in three conditions: cement mantle, procimal femur and stem-cement contact surface. Additionally, micro-motion was analyzed in the stem-cement contact surface. The 3-D femur model was reconstructed from 2-D computerized tomography (CT) images. Raw CT images were preprocessed by image processing technique (i.e. edge detection). In this study, automated edge detection system was created by MATLAB coding for effective and rapid image processing. The 3-D femur model was reconstructed based on anatomical parameters. The stem shape was designed using that parameters. The analysis of the finite element models was performed with the variation of parameters. The biomechanical influence of each parameter was analyzed and derived optimal parameters. Moreover, the results of FE A using commercial stem model (Zimmer's V erSys) were similar to the results of stem model that was used in this study. Through the study, the improved designs and optimal factors for clinical application were suggested. We expect that the results can suggest solutions to minimize various side effects.

  • Background: The purpose of the study, using the ICF Tool in the process of rehabilitation of artificial joint replacement surgery of the hip joint, goal setting and understanding of the problem, through the process of creating intervention strategies, useful clinical practical course for rehabilitation I try to present the data. Methods: Fracture was the left total hip replacement (THR) surgery due to women of 76 years old. I proceed in order screening, evaluation, diagnosis, prognosis, treatment planning and intervention, re-screening. Needs of the patient was walking short distances for using the toilet. In order to improve was carried out arbitration, after you have created a list of issues that limit the ability to walk short distances. Results: It was revealed improved results in self-paced walk test (SPWT) and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) was used to examine whether the goals. In addition, MMT VAS, DGI, and TUG is a detailed goal was improved. Conclusions: It can be shown objectively the results of interventions performed for the purpose of solving the problem which is grasped through clinical Practical course for short-range walking ability enhance patient THR. I considered practical clinical course using the ICF Tool would be useful.

팽창흑연을 사용한 복합재료의 난연 특성에 관한 연구 (A Study for the Fire Retardant-Characteristics of Expandable Graphite Composite Materials)

  • In this study, the composite material of expandable graphite was made to the material development for improving such as a composite material of the sandwich panels or material properties of a fire door and was tested by the ISO 1182, ISO 5660-1(Cone calorimeter Method). For the test, the composite material of expandable graphite, what the expandable graphite ratio was increased by respectively 0g~30g, was classified A1,A2, A3, A4, and made to the test specimens. Through cone calorimeter test, peak heat release rate(HRR) and total heat release(THR), expanded thickness and expansion rate of each composite material of expandable graphite, and fire prone crack and mass loss rate after burning was measured. Thus, the effect of the addition of the expandable graphite and whether is suitable for reference as a fire retardant, was analyzed. Consequently the correlation of THR and fire retardant performance rate was confirmed.

Predictive Value of XRCC1 and XRCC3 Gene Polymorphisms for Risk of Ovarian Cancer Death After Chemotherapy

  • Objective: To investigate any association between XRCC1 and XRCC3 polymorphisms and outcome of platinum-based chemotherapy in ovarian cancer patients. Methods: With a prospective study design was cases were consecutively collected from January 2005 to January 2007. All 310 included patients were followed-up until the end of January 2010. Genotyping of XRCC1 and XRCC3 polymorphisms was conducted by TaqMan Gene Expression assays. Results: A total of 191 patients died during follow-up. Our study showed a lower survival rate in XRCC1 399 Arg/Arg genotype than Gln/Gln, with a significant increased risk of death (HR=1.69, 95%CI=1.07-2.78). Similarly, those carrying XRCC3 Thr/Thr genotype had a increased risk as compare to the Met/Met genotype, with a HR (95% CI) of 1.90 (1.12-3.41). There was no significant association between XRCC1 Arg194Trp and XRCC1Arg280His gene polymorphisms and ovarian cancer death. Conclusion: Our study demonstrates that polymorphisms in DNA repair genes have roles in the susceptibility and survival of ovarian cancer patients.

암모늄염으로 처리된 리기다 소나무의 연소특성 (Combustive Characteristics of Pinus Rigida Treated with Ammonium Salts)

  • 이 연구에서는 암모늄염을 처리한 리기다 소나무의 연소성을 시험하였다. 실온에서 3종류의 암모늄염 즉, 황산암모늄, 제1인산암모늄, 그리고 제2인산암모늄의 20wt% 수용액에 각각 리기다 소나무를 함침시켜 건조시킨 후 콘칼로리미터(ISO 5660-1)를 이용하여 그의 연소성을 시험하였다. 암모늄염으로 처리한 시험편은 처리하지 않은 시험편에 비하여 그의 연소성을 감소 시켰다. 이것은 연소 억제성이 순수 리기다 소나무 시험편에 처리한 암모늄염 때문에 향상된 것으로 생각된다. 또한 암모늄염으로 처리한 시험편은 처리하지 않은 시험편에 비해 낮은 최대열방출률과 낮은 총방출열량을 나타내었다.

유로키나제와 프로유로키나제의 정제 및 특성 비교 (Purification of Urokinase and Pro-urokinase and Comparison of their Characteristics)

  • 본 연구에서는 인뇨에서 분리 정제한 urokinase와 유전자 재조합 CHO(Chinese Hamster Ovary) 세포 배양액으로부터 분리 정제한 pro-urokinase의 물리화학적 특성과 혈액 내에서의 효소활성을 비교 분석하였다. Two chain form인 urokinase와 single chain form인 pro-urokinase를 각각 순수 분리한 후, electrophoresis한 결과 모두 54 kd의 단일밴드를 나타냈다. 그러나 urokinase는 환원시켰을 때 33 kd과 21 kd으로 나누어짐을 확인하였으나 gel filtration결과 분자량이 54 kd 정도임을 확인되어 용액 내에서 단일분자로 존재함을 알 수 있었다. Urokinase와 pro-urokinase가 물리화학적으로 거의 동일한 구조를 가졌다는 사실은 isoelectro focusing에 의한 pI 값이 모두 8.6으로 동일하다는 점과, 아미노산 조성을 분석한 결과 동일하다는 사실로도 알 수 있었다. N-terminal 아미노산 서열을 분석한 결과, urokinase는 이중사슬구조이므로 N-terminal이 두개 존재하여 pro-urokinase의 서열(Ser-Asn-Glu-Leu-His-Gln-Val-Pro-Ser-Asn)이외에도, 159번째의 Ile다음부터 Ile-Gly-Gly-Glu-Phe-Thr-Thr-Ile-Glu가 같은 peak로 나타남을 확인하였다. 효소활성 조사결과 pro-urokinase와 urokinase는 모두 농도 의존적으로 혈전용해 활성을 보였으나, 특이하게도 짧은 반응시간동안에는 urokinase가 동일 농도 하에서 강한 활성을 보인 반면, 2시간이 지난 오랜 반응조건에서는 pro-urokinase가 혈전용해활성을 나타내었다. Fibrinogen에 대한 분해활성을 조사한 결과, urokinase는 혈장 내 fibrinogen을 상당히 손상시키지만, pro-urokinase는 거의 영향을 주지 않아 혈전선택성이 매우 좋음을 알 수 있었다.

된장의 쓴맛 펩타이드 특성 (Characteristics of bitter peptides from Doenjang)

  • 본 연구에서는 재래식 된장, Asp. oryzae를 이용한 고지와 소금의 배합을 달리한 개량식 된장 세가지(A; 쌀고지 :콩 :소금:53 : 100 : 33, B: 쌀고지 .콩 :소금=100 : 100 : 40, C: 쌀고지 :콩 :소금=200 : 100 : 40)를 만들어 일반 성분의 변화와 쓴맛 펩타이드의 특성을 살펴보고 쓴맛 펩타이드가 된장의 수응도에 어떠한 영향을 미치는지를 알아보았다. 1. 4가지 된장 모두 총질소는 발효기간 중 약간씩 증가하였으며 아미노산태 질소는 크게 증가하였다. 특히 재래식 된장의 아미노산태 질소는 다른 시료에 비해 큰폭의 증가를 보였다. 환원당은 쌀고지를 첨가한 된장의 경우 함량이 월등힌 많았으며 숙성됨에 따라 모든 된장이 증가 추세를 보였다. pH는 된장이 숙성됨에 따라 점차로 저하하였으며 고지가 첨가된 된장의 경우 더욱 산성을 띠었다. 2. 숙성시킨 된장에서 쓴맛 펩타이드의 특성을 알아보기 위해 소수성 펩타이드를 2 : 1(v/v) Chloroform-me-thanol로 추출하였다. 추출된 추출물을 겔 크로마토그래피를 사용하여 분자량별로 분리하고 다시 TLC로 정제하였다. 정제된 펩타이드는 쓴맛 검사를 하여서 쓴맛을 나타내는 펩타이드를 얻었다. 쓴맛 펩타이드의 아미노산 조성은 다음과 같다. 재래식 된장 peak 1-1. Trp-(Asp, Arg, Thr, Ser, Glu, Pro)-Phe 개량식 된장 B peak 1 Trp-(lie, Pro, Asp, Lys, Val, Glu)-Trp 개량식 된장 B peak 1 Trp-(lie, Pro, Asp, Lys, Val, Glu)-Trp 개량식 된장 C peak 1-2 Trp-(tyr, Thr, Glu, Pro, Gly)-Phe. 3. 관능검사의 결과를 Pearson 상관관계를 알아본 결과 쓴맛과 전체적인 수응도와의 상관관계는 높지 않았다.

대퇴관절 전치환술을 이용하여 개의 재발되는 대퇴관절 탈구증의 치료 (Treatment of Recurrent Coxofemoral Joint Luxation by Total Hip Replacement in a Dog)

  • 38 kg, 7년령의 골든리트리버가 대퇴관절 탈구증으로 의뢰되었다. 신체검사에서는 왼쪽 뒷다리를 신전시킬 때 통증과 염발음이 확인되었다. 방사선 검사를 통해 왼쪽 대퇴관절 탈구증 및 경도의 퇴행성 골변화가 관찰되었다. 치료로서 TightRope$^{(R)}$를 이용한 최소침습 관절경적 정복을 하였으나, 수술 후 한달이 지나 재탈구가 발생하였다. 이에 대해 무시멘트형 대퇴관절 전치환술을 실시하였다. 수술 후 10개월 경과된 결과, 환자는 정상적으로 앉기, 서기, 보행을 보였으며, 특이한 파행없이 편안하게 뛸 수 있었다. 수술한 뒷다리의 허벅지둘레는 반대쪽 다리의 그것에 비해 108.6%였다. 이 증례를 통해 대퇴관절 전치환술은 대퇴관절 정복이 실패하여 재탈구된 개의 대퇴관절 탈구 치료에 효과적인 수술법이 될 수 있다.

Micro Total Hip Replacement in Two Dogs with Legg-Calvé-Perthes Disease

  • A Maltese (case 1) and a Pomeranian (case 2) presented with acute right and left hind limb weight-bearing lameness. On physical examination, they exhibited severe pain and crepitus on the coxofemoral joint of affected hind limb. In addition, decreased thigh girth measurements were noted compared with the opposite leg. Radiological exam revealed necrotic areas in the femoral head of affected hind limb. These dogs were diagnosed with Legg-$Calv{\acute{e}}$-Perthes disease (LCPD). The dogs underwent micro total hip replacement (THR). After surgery, at 3 years (case 1) and 7 months (case 2), both dogs recovered normal activity. The thigh girth and lameness scores were apparently improved in the affected limbs of both dogs. No complications of prosthesis implants, such as loosening, were noted. The clinical outcomes of these cases indicate that dogs with LCPD can be successfully treated with micro THR and have a good prognosis immediately after surgery.
