A Case Study on Obese Patient with Oligomenorrhea and Polycystic Ovary (다낭성 난소 소견을 동반한 비만여성 경지(經遲) 치험 1례)

  • Objective The purpose of this study is to evaluate the effectiveness of obesity management program with herbal medication(Changbudodamtanggamibang) on an obese female patient with oligomenorrhea resulted from polycystic ovary syndrome. Methods I applied herbal medication(Changbudodamtanggamibang), acupuncture, auricular acupuncture, electrolipolysis, low calorie diet, aerobic exercise, behavioral modification therapy and fumigation therapy to her. Results Her weight decreased from 64.3kg to 54.0kg, BMI from $26.4kg/m^2$ to $22.2kg/m^2$, PBF from 38.9% to 29.6%, and WHR from 0.89 to 0.82. Menstrual period was normalized from 60days to 34days. ConclusionThese results provides an evidence that obesity management program with herbal medication(Changbudodamtanggamibang) is effective on oligomenorrhea of an obese female patient with polycystic ovary syndrome.

The Effects of a Cardiac Rehabilitation Program on Health Behavior Compliance, Cardiovascular Function, and Quality of Life for the Patients with Ischemic Heart Disease (심장재활 프로그램이 허혈성 심장환자의 건강행위 이행, 심혈관 기능 및 삶의 질에 미치는 효과)

  • This study is aimed at developing a cardiac rehabilitation program and enlightening the effects of the program on patient's health behavior compliance, cardiovascular functional capacity, and quality of life. Using a quasi-experimental approach the nonequivalent control group pretest - posttest design was accepted for this study. The subjects of this study consisted of 55 patients with ischemic heart disease at the Cardiac Center of 'G' Hospital located in Inchon from May 1, 1998 to April 30, 1999. The patients were divided into two groups: the experimental group, which participated in the cardiac program with 30 patients and 25 patients of a control group were not involved in the program. There were two phases in the cardiac rehabilitation program: the first phase was a team approach education. It focused on reducing the risk of ischemic heart problems. The second phase was individual training by using a home based exercise program, which was comprised of 8 weeks, three sessions per week, 40-60 minutes per session, and followed by consultation. Every session involved 20-40 minutes of aerobic exercise at 40-60% of heart rate reserve, 11∼13 RPE and 10 minutes of warm-up and 10 minutes of cool-down exercises. The experimental tools for the study were the health behavior compliance scale developed by Lee, Yoon-hee (1992), and quality of life scale developed by McGirr et al.(1990). RPPsubmax were measured by the treadmill. The collected data was processed by SPSS and analyzed by χ²test and t-test. The results of this study were as follows: 1. The health behavior compliance in experimental group was significantly increased (t=5.091, p=.000) when compared to the control group. 2. RPPsubmax also decreased significantly in the experimental group when compared to the control group(t=-2.109, p=.040). 3. The quality of life significantly improved in the experimental group (t=3.853, p=.000) as compared to the control group. As the above results of this study revealed, the effectiveness of the cardiac rehabilitation program of the study was confirmed. It increased the health behavior compliance for reducing the risk of further coronary events, enhanced the cardiovascular functional capacity, and eventually improved the patient's quality of life.

A Case Study on the Effect of Hypobaric-Hypoxic Intermittent Training on the Blood Constituents and Average Heart rate of Professional Handicapped Cyclists

  • The purpose of this study was to investigate the effects of hypobarichypoxic training program on competitive performance. This was done by observing their conditioning and measuring their blood constituents before and after a multi-staged intermittent training program, over 2 weeks. Three national handicapped cyclists were placed in a multi-leveled hypobaric-hypoxic (flat-4000 meter (m) high elevation) environment with consistent temperature and humidity ($23{\pm}2^{\circ}C$, $50{\pm}5%$) for 2 weeks. After the training, the blood constituents and average heart rate (HR) were measured and the following results were obtained. In all three athletes, there were no unique changes in red blood cell count, hemoglobin, and hematocrit, while there was a rise in the reticulocyte count. Observations of the difference in average HR during exercise at varying altitudes showed that athlete A had an average increase in the HR for the first 5 days at 2000 m. For athlete B, the comparison of the first and last training sessions at an altitude of 2000 m showed an HR increase of approximately 17%. For athlete C, there was a steady increase in the HR until day 7 of the training. As such, hypobaric-hypoxic training suggested that improvement of aerobic exercise performance in these athletes and it is recommended that there be a development for future training programs at high altitude, geared towards handicapped athletes of various disciplines.

The effects of a 24 Weeks of combined exercise programs have on physical configuration, blood components and physical strength for normal and geriatric diseased senior citizens residing in the country side (중소도시 노인들의 24주간 복합운동 프로그램이 성인병 질환자 및 정상인의 신체구성, 혈액성분, 체력에 미치는 영향)

  • This thesis is to research the before and after changes of physical configurations, blood components and physical strength for normal and geriatric diseased senior citizens at the end of 24 week of combined exercises constructed of aerobic and muscular strength training to create most suitable and effective complex exercise program for geriatric diseased patients. For this experiment 20 normal and 20 geriatric diseased patients in the age of 65 residing in "K" city were selected to carry out the 24 weeks of combined exercises in regularly. The result of the research showed that geriatric patients increased significantly in everything, but normal group showed significant change in only WHR. There was a slight improvement in the blood components for the average participants but it only differed slightly from the diseased participants so there were no major changes reflecting the outcomes from both before and after. After concluding the program both groups displayed positive improvements in stamina but no significant alterations in physical strength., agility, muscle endurance and balance. The positive factors for each groups could be that the norms were able to maintain their health and enhancement in stamina and diseased were able to prevent their condition from worsening. Additionally, over 50 percent of all senior citizens have one or more geriatric diseases but the participation of any physical activity is in the decrease. Henceforth, this is a field that still needs a lot of work and combined exercise programs should be created and followed through so it may enhance in the improvement of health and quality of life as well.

The Effect of 4-Week Health Promotion Summer Camp on the Metabolic Syndrome and Insulin Resistance among Obese Elementary Students (비만 초등학생의 4주 여름 건강증진캠프 참여가 대사증후군 및 인슐린저항성에 미치는 효과)

  • The purpose of the current study was to investigate the effects of 4-week health promotion intervention program consisting of high-intensity aerobic exercise to improve energy consumption and cardiorespiratory capacity, circuit training to strengthen muscular strength and endurance and education for lifestyle changes on the obesity level, physical fitness, insulin resistance and metabolic syndrome among overweight or obese elementary school students. Twenty three overweight or obese children as obese group and 15 normal body weight children as normal group were recruited. The health promotion program consisted of two exercise sessions and one education session, 3days/week in 4-week. Obesity level(body mass index, waist circumference, %body fat), physical fitness(muscular strength, muscular endurance, flexibility, cardiopulmonary fitness), insulin resistance(homeostasis model assessment of insulin resistance) and metabolic syndrome risk factors(blood pressure, fasting glucose, triglycerides, high density lipoprotein cholesterol) were measured. There was not significant decrease in obesity level; however, there were significant improvement in physical fitness, insulin resistance and metabolic syndrome risk factors after program participation among obese children. As a result, through participation in the summer camp consisting mainly of exercise, the improvement of the physical fitness level and the decrease of insulin resistance had an effect on the reduction of the metabolic syndrome frequency.

Effect of Case Management Program on Self-Efficacy, Self-Care Behaviors and Glycometabolic Control in Patient with Diabetes (당뇨병 환자의 자기효능감, 자가간호행위 및 혈당조절에 미치는 사례관리 프로그램의 효과)

  • - Abstract - Objectives: We examined the effects of diabetes mellitus (DM) case management program developed by National Health Insurance Corporation in Korea. Methods: One trained nurse performed the case management for 30 DM patients recruited through the reimbursement information of National Health Insurance in a rural community. The case manager had four home visits and two telephone calls for 12 weeks. We evaluated the effectiveness of the program in terms of self-efficacy, self-care behaviors and glyco-metabolic control. Results: The self-efficacy score was significantly increased from 70.0 of pre-intervention to 80.5 of post-intervention (p<0.001). The self-care score was also significantly increased from 2.86 to 3.60 (p<0.001). The scores of its components such as nutritional care, self-test of sugar level, foot care, oral care, and aerobic exercise were significantly improved (p<0.001). However, the improvement of score in medication management was statistically marginal (p=0.071). The fasting blood sugar level and HbA1c level was significantly decreased from 128.6 mg/dl to 114.5 mg/dl (p<0.001) and from 6.78 % to 6.54 % (p<0.001) respectively. Conclusions: The case management program was effective to improve the self-efficacy and self-care behaviors, and to control blood sugar level in DM patients.

Development of the Home-Based Pulmonary Rehabilitation Program for Patients with Chronic Lung Disease (만성 폐질환 환자에서 재택 호흡재활치료방법 개발 연구)

  • Background : Even though it is well known that pulmonary rehabilitation (PR) improves exercise capacity, and the quality of life, in patients with chronic lung diseases, not many patients can attend hospital based intensive PR in Korea. The purpose of this study was to develop a method for a home-based PR program, and study its effectiveness. Materials and Methods : Twenty patients with chronic lung diseases were randomly divided into two groups : a home PR group comprising of 10 male patients, with a mean age of 70 years, and a control group comprising of 10 male patients, with a mean age of 65 years. We developed exercise programs, depending on the exercise capacity of each patient, which were easy to do at home. The PR program consisted of a 12 week period of enforced aerobic (mostly walking) and muscle strengthening exercises, as prescribed by the exercise specialist, in accordance with the functional capacity of the patient. In addition to the education, nutritional and psychiatric consultation was undertaken, and respiratory muscle training arranged. Patients visited hospital every 2 weeks for evaluation and exercise prescription. Results : All patients finished the 12 week course of therapy. Following the home PR, the endurance times and work capacity of the upper and lower extremities were significantly increased in the treatment group in comparison to the controls. The six minute working (Eds note:should) 'working' read "walking"?) distance was increased from $465{\pm}60m$ to $508{\pm}37m$ and the maximal inspiratory pressure from $72.8{\pm}27.2cmH_2O$ to $91.4{\pm}30.9cmH_2O$. The quality of life, as assessed by St Georges Respiratory Questionnaire (SGRQ), was also improved following PR. Conclusion : The home PR program we developed seemed to be applicable, and effective, to most of the patients with chronic lung diseases in this study.

Effects of Combined Exercise on Irisin, Body Composition and Glucose Metabolism in Obese Elderly Women with Type 2 Diabetes Mellitus (복합운동이 제2형 당뇨병 비만 여성노인의 Irisin, 신체조성 및 당 대사에 미치는 영향)

  • The purpose of this study was to investigate the effects of combined exercise on irisin, body composition and glucose metabolism in obese elderly women with type 2 diabetes mellitus. The subjects were thirty-six obese elderly women with type 2 diabetes volunteers, aged 65 to 85 years, composed of the combined exercise type 2 diabetes mellitus group (n=20) and non-exercise type 2 diabetes mellitus group (n=16). The 60 minute combined exercise program (outdoor walking exercise & elastic-band exercise) was performed 3 times per week for 12 weeks. Exercise intensity of outdoor walking exercise was performed as medium intensity (RPE 5~6) and elastic-band exercise was progressively increased every four weeks (1-4 weeks: OMNI-RES 3~4, 5-8 weeks: OMNI-RES 5~6, 9-12 weeks: OMNI-RES 7~8). The results of the study in the combined exercise type 2 diabetes mellitus group were as follows; Irisin and skeletal muscle mass had significantly increased (p<.001), percentage of body fat had significantly decreased (p<.001). Further, HbA1c (p=.020) and fasting glucose (p<.001) was significantly decreased, and HOMA-β was significantly increased (p<.001). Correlation results showed that change of irisin had a significant negative correlation between percentage of body fat mass (r=-.423, p=.010), HbA1c (r=-.351, p=.036) and fasting glucose (r=-.424, p=.010). Also, irisin changes showed a positive correlation with aerobic endurance (r=.355, p=.034) and HOMA-β (r=.411, p=.013). In conclusion, the practice of regular combined exercise was found to increase the level of irisin in elderly women with type 2 diabetes and have a positive effect on body composition changes. In addition, HbA1c, fasting glucose and insulin secretion was improved, which helped to regulate glucose metabolism. Walking exercise and elastic band exercise are recommended as effective exercise for the prevention and management of diabetes in obese elderly women with type 2 diabetes mellitus.

Relationship between the Practice of Physical Activity Guideline for Adolescents and Smartphone Problem Experience (청소년 신체활동지침 실천과 스마트폰으로 인한 문제경험과의 관계)

  • This study aims to understand the relationship between the practice of physical activity guideline for adolescents presented by government agencies and smartphone problem experience. To that end, the association between the practice of physical activity guideline and smartphone variables was analyzed by utilizing 62,225 data from "2017 Youth Health Behavior Survey" and multiple regression analysis was conducted using the SPSS 23.0 program. The analysis showed that all physical activity variables, except for "physical activity for more than 60 minutes," have negative effects on the smartphone problem experience. In addition, students who practice physical activity guideline presented by government agencies, such as aerobic exercise, muscular exercise, participation in sports clubs, and sedentary behavior, experienced less smartphone problem compared to those who did not practice, which showed significant differences in gender. According to the results of this study, it is thought that academic in-depth research using this guideline and differentiated physical activity promotion programs by gender will be possible in order to prevent smartphone addiction.

Effect of Circuit Training on Estrogen Hormone, Serum Lipids in Obese Middle-aged Women (Circuit Training이 비만중년여성의 여성호르몬, 혈중지질에 미치는 영향)

  • This study is supposed to offer data related to whether circulation training(aerobic and anaerobic exercise) focused on overweight women has positive effects on reducing fat and increasing muscles for middle-aged women. This study aims to investigate how the circulation training effect overweight middle-aged women's female hormone, blood lipid, which offers basic data of exercise program to keep the overweight middle-aged women healthy. Participants for the study are 30 to 45-year-women who were willing to take part in a M Sports Diet Program in G gu, B metropolitan city, did not have any disease. They were 29 overweight women and showed more than 30% of body fat percentage. Female hormone, blood lipid were measured twice before and in 12 weeks after exercise. The results obtained from this study are given as in the following. Although estrogen has significantly increased after the circulation training, there was no meaningful difference from the control group. After 12-weeks-circulation training, even though there were no meaningful differences. before the training, the circulation training group was significantly higher than the control group. Although TC has significantly decreased after the circulation training, there was no meaningful difference from the control group. the circulation training group was significantly lower than the control group. Even though TG was no significantly decreased after the circulation training, there was no significantly difference from the control group. the circulation training group was significantly lower than the control group. Even though HDL-C has significantly increased after the circulation training, there was no meaningful difference from the control group. the circulation training group was significantly higher than the control group. Even though LDL-C has significantly decreased after the circulation training, there was no significantly difference from the control group. the circulation training group was significantly lower than the control group.
