한국 간호교육기관의 성 건강 이론 교육과정 분석 (Analysis of the Sexual Health Education Curriculum of Nursing Schools in Korea using Posner's Theory)

  • Purpose: This study was aimed to analyze the sexual health curriculum for the nursing baccalaureate and associate's degrees in Korea. The curriculum proper based on Posner's theory presented the analysis of purpose, content, organization, and underlying assumption. Methods: This study was conducted with sexual health education guidelines, nursing practice standards, 181 curriculums, and teaching materials. Data were collected through literature, online homepage from 181 nursing school, and textbooks from July to September, 2013. Data were analyzed using percentage and mean with SPSS 12.0. Results: The purposes were mostly included in the low grade cognitive learning domain. The contents included 20 key elements among 22, so the scope was not inclusive. There was an unbalance between content's depth and scope, because total mean credit of sexual health nursing education was only 19.81 hours. The spiral structure of organization showed continuity, sequence, and integration with international standards. The interdisciplinary integration and transcultural value were advantages of the curriculum. Conclusion: This study provided a view on understanding sexual health nursing curriculum and implication for advanced education. The proclaiming of the standard and concept mapping of sexual health curriculum may contribute to the curriculum development for the advanced nursing.

  • Recent changes in health problems as well as the health delivery system demand expanding nursing roles to improve quality care and at the same time provide cost-effective health services. Nursing leaders of the future will be expected to be excellent care givers who are well prepared as researchers and managers and administrators. The purpose of developing an ND program is to train nursing leaders for the future. The program objective are to demonstrate the role as 1) an advanced practitioner who will provide theory based practice 2) a researcher and manager The target population of the ND program is as follows : 1) Nurses with a BSN degree who are seeking to advance on the career ladder 2) Non-nursing BS, BA graduates who are seeking nursing as a second career 3) Transfer students who are mature and motived to be nursing leaders It is believed that the ND program would meet the diverse needs for potential students, for their role in nursing and the health delivery system and create a meaning and challenging position in the profession of nursing. A task force team was organized to develop the ND program and its activities are as follows; 1) Information on ND programs were collected from 3 universities and reviewed 2)An advanced education needs assessment was completed and it was found that 87.6% of the subjects responded positively about advanced degree work. 3) A tentative curriculum for an ND program was developed. It is a great challenge to develop a new program for nursing however, it is also our task and responsibility to further the development of nursing.

전문간호사가 주도하는 급성대동맥증후군 환자의 병원간 다이렉트 전원(direct interhospital transfer)의 임상 결과 (Clinical Outcomes of Direct Interhospital Transfers of Patients with Acute Aortic Syndrome Led by Advanced Practice Nurses)

  • Purpose : This study aimed to evaluate the clinical outcomes of direct interhospital transfers (IHTs) of patients with acute aortic syndrome (AAS) led by advanced practice nurses (APNs). Methods : From September 2014 to June 2017, the study retrospectively investigated 183 patients with AAS who were transferred to a high-volume tertiary hospital. Results : One hundred forty-eight (81%) patients were admitted through direct IHTs, and 35 (19%) patients were admitted through non-direct IHTs. The direct IHT group had a significantly shorter time from symptom onset to hospital arrival than the non-direct IHT group (11.4 vs. 32.1 h, p=.043). There were no significant differences in other clinical outcomes, such as peri-transfer status, mortality, hospital length of stay, and readmission, between the two groups. In the direct IHT group, 55% of transfers were led by APNs. There was no significant difference in outcomes between APN- and physician-led transfers. Conclusions : Implementation of direct IHTs markedly shortened the time from symptom onset to hospital arrival in patients with AAS. Finally, direct IHTs can potentially improve the outcomes of patients with AAS, a condition with time-dependent mortality and morbidity. In addition, APNs can effectively lead the direct IHT of patients with AAS.

  • Purpose: The aim of this study was to develop and to analyze the task of hospital based home care nurse practitioners in Korea. Method: The definition of home care nurse practitioners and job description was developed based on developing a curriculum(DACUM) by 7 panels who have experienced in home care nursing. One hundred fifty four nurses who were working at hospital based on home care were participated. Result: Fourteen kinds of duties were identified : the selection of home care patients; basic home care nursing; advanced home care nursing; patient/family education and counseling; medical decision making and coordination of patient service; management of home care supplies and drugs for patients; management of medical records; management of home care the agency; management of home care personnel; management of the home care supplies for agency; home care public relations; improvement of home care quality; management of long-term care service; and self-improvement. Ninety-six tasks were classified. Conclusion: The abilities for quality improvement and the advanced nursing practice of home care nurses should be empowered.

상급종합병원 전문간호사의 리더십 경험: 포커스 그룹 면담 (The Leadership Experience of Advanced Practice Nurses in a Tertiary Hospital Setting: Focus Group Interview )

  • Purpose : This study aimed to identify and describe the leadership experience of advanced practice nurses (APN). Methods : Data were collected through five focus group interviews in 2022 with a total of 24 APNs in groups of 4-6 participants. All interviews were recorded, transcribed, and data were analyzed using qualitative content analysis. Results : Nine categories emerged from three main themes. First, "Roles of APN leadership" comprised a trusted clinical expert, a moderator for the entire team, a resource person for nurturing the next generation, and a change agent for improving clinical practice. Second, "Facilitators and barriers to APN leadership" included ambiguity of APN role, support system, and institutional backing. Third, "Strategies for strengthening APN leadership competencies" comprised systematic leadership education and speaking up for APNs. Conclusion : APNs are passionate about their expertise and practice, but lack the legal and organizational authority and support to provide successive leadership. Systematic education including leadership and organizational advocacy will enable APN to provide leadership that benefits patients, institutions, and the wider healthcare system.

호스피스 간호사 및 호스피스 전문간호사 수요추계에 관한 연구 (A Study on the Projected Manpower of Hospice and Palliative Care Nurses & APNs in Korea)

  • Purpose: The purpose of this study was to suggest the projected manpower of hospice and palliative care nurses & APNs(advanced practice nurses)needed in the future. Method: Need model, ratio model and expert opinion were used for projecting the number of hospice and palliative care nurses & APNs. Result: 1. The number of Korean hospice facilities was 64 in 2002. The number of hospice nurses in 2001 was 194 and that of beds was 407. 2. The number of hospice target patients was estimated at a minimum of 16,415 to a maximum of 25,254 in 2002, 12,366 to 26,389 in 2005, and 14,057 to 30,000 in 2020. 3. The number of hospice and palliative nurses needed to meet the demands in 2002, 2005 and 2020 was estimated at a minimum of 1,136 to maximum of 1,748, 1,187 to 1,826, and 1,349 to 2,076, respectively. 4. The number of hospice & palliative care APNs needed to meet the demands in 2002, 2005 and 2020 was estimated at 232, 242, and 274, respectively. Conclusion: The legalization of hospice is expected to increase demands for hospice nurses and advanced practice hospice and palliative care nurses in the future.

대장암 생존자에게 적용한 전문간호사 주도 심리교육 프로그램의 효과 (Effects of the Advanced Practice Nurse-Led Psychoeducational Program for Colorectal Cancer Survivors)

  • Purpose: This study aimed to investigate the effects of an advanced practice nurse-led psychoeducational program on distress, anxiety, depression, coping with cancer (CWC), health promotion behavior (HPB), and quality of life (QOL) among colorectal cancer survivors. Methods: This study was designed as a quasi-experimental study with a non-equivalent control group pretest-posttest. The participants were survivors of colorectal cancer who underwent follow-up care. There were 39 survivors: 19 in the experimental group and 20 in the control group. The experimental group performed a psychoeducational program for 120 minutes per session, once a week for a total of six weeks, while the control group received routine education and counseling. Distress, anxiety, depression, CWC, HPB, and QOL were investigated before, immediately after, and 4 weeks after the intervention. The data were analyzed with SPSS/WIN ver. 24.0, using repeated measures ANOVA. Results: There were significant interactions between time and group for distress and anxiety. In addition, CWC interacted with the total of CWC and interpersonal coping, and QOL interacted with the total of QOL and functional status. However, there were no significant differences in the depression or HPB scores. Conclusion: Based on the results of this study, we expect that this program can be used as an effective intervention for colorectal cancer survivors.

The influence of the Clinical Learning Environment and Learning Transition on Satisfaction with a Gerontological Nursing Clinical Practicum in Nursing Students

  • This study used a descriptive survey design to examine the impact of the clinical learning environments and learning transition nursing students experienced the gerontological nursing clinical on the satisfaction with clinical practice. A convenient sample of 211 4th year nursing students who had the gerontological nursing clinical practicum from one College of Nursing at Private University in South Korea was recruited and completed the surveys from October to December 2019. This study showed that the satisfaction with a gerontological nursing clinical practicum was significantly correlated with clinical learning environments and learning transition. The results of this study highlights the need to create a safe and positive clinical learning environment for quality gerontological nursing clinical practicum, so hospitals and nursing schools need to make efforts to promote clinical sites as an educational learning environment in collaborative relationships.

동태적 직무분석을 이용한 암 환자 케어 코디네이터의 직무 분석 (Dynamic Job Analysis of the Cancer Care Coordinator in a General Hospital)

  • Purpose: The purpose of this study was to explore roles of cancer care coordinator's by developing job description, job specification and job process map using dynamic job analysis. Method: The development process consisted of three stages of vertical job analysis and three stages of horizontal job analysis by modifying Song(1997)'s dynamic job analysis. Focus group interview was used to validate the content of the job analysis. Results: Cancer care coordinator's job description was categorized into six major categories, fourteen intermediate categories and one hundred forty specific jobs. Major categories are professional nursing practice, consultation and counsel, coordination and collaboration, education, research and leadership. Cancer care coordinator's job specification included master's degree with over five years of clinical experience preferably relevant clinical area, professional knowledge on pathophysiology of cancer, case management and cost control, competency for communication and counselling skills and clinical decision making. Cancer care coordinator's job process map was framed with time(horizontal) and activities(vertical). Conclusion: The Outcomes of this study will guide to develop possible areas of oncology advanced practice nurses in hospital setting and facilitate the use of oncology nurse practitioners by developing care coordinator roles in cancer care.

간호대학생의 좋은 죽음 인식 (Perception of Good Death among Nursing Students)

  • 본 연구는 간호대학생들의 좋은 죽음에 대한 인식 정도를 알아보고 하위요인 간의 점수 비교를 통해 좋은 죽음의 속성 중 더 중요하게 생각하는 요인을 비교 분석하기 위한 목적으로 수행된 서술적 조사연구이다. 연구의 목적을 설명하고 참여에 동의한 102명을 편의 모집하였다. 연구도구는 Schwartz 등의 'Measure of concept of a Good Death'을 번역한 도구로 온라인을 통해 진행하였다. 자료분석은 IBM SPSS Statistics 26 프로그램의 빈도분석, 기술통계, t-검정, 일원배치 분산분석 등의 통계기법을 활용하였다. 좋은 죽음 인식의 세 가지 하위요인별 점수의 평균차이를 보인 요인은 임상실습의 경험으로 도출되었다. 임상실습을 경험한 학생들의 좋은 죽임 인식의 평균(3.13±0.36점)이 임상실습 경험이 없는 학생들(2.90±0.31점)보다 통계적으로 유의하게 높았다(t=3.156, p=.002). 특히 임상실습 유경험자(3.46±0.39점)가 무경험자(3.16±0.37점)보다 통계적으로 유의하게 친밀감에 대한 좋은 죽임 인식의 평균이 높았다(t=3.476, p=.001). 본 연구를 통해 간호대학생들의 좋은 죽음에 대한 인식은 임상실습 여부에 따라 차이가 있음을 확인하였다. 본 연구의 결과는 간호대학생의 좋은 죽음 인식에 대한 효과적인 교육프로그램 개발에 도움이 되는 기초자료를 제공할 수 있을 것으로 기대된다.
