PURPOSE. On maxillofacial tumor patients, oral implant placement prior to postoperative radiotherapy can shorten the period of prosthetic reconstruction. There is still lack of research on effects of post-implant radiotherapy such as healing process or loading time, which is important for prosthodontic treatment planning. Therefore, this study evaluated the effects of post-implant local irradiation on the osseointegration of implants during different healing stages. MATERIALS AND METHODS. Custom-made implants were placed bilaterally on maxillary posterior edentulous area 4 weeks after extraction of the maxillary first molars in Forty-eight Sprague-Dawley rats. Experimental group (exp.) received radiation after implant surgery and the other group (control) didn't. Each group was divided into three sub-groups according to the healing time (2, 4, and 8 week) from implant placement. The exp. group 1, 2 received 15-Gy radiation 1 day after implant placement (immediate irradiation). The exp. group 3 received 15-Gy radiation 4 weeks after implant placement (delayed irradiation). RESULTS. The bone mineral density (BMD) was significantly lower in the immediate irradiation groups. BMD was similar in the delayed irradiation group and the control group. The irradiated groups exhibited a lower bone-to-implant contact ratio, although the difference was not statistically significant. The irradiated groups also exhibited a significantly lower bone volume and higher empty lacuna count than the control groups. No implant failure due to local irradiation was found in this study. CONCLUSION. Within the limits of this study, the timing of local irradiation critically influences the bone healing mechanism, which is related to loading time of prostheses.
연구목적: 최근CAD/CAM으로 제작한 지르코니아 코어의 변연적합도에 관한 연구가 많이 진행되었지만, 여러 종의CAD/CAM 시스템의 변연 뿐만 아니라 내면적합도까지 상호 비교한 연구는 아직 미흡한 실정이다. 따라서 본 연구에서는 3종의CAD/CAM 시스템으로 제작한 전치부 단일 치아 지르코니아 코어의 변연 및 내면적합도를 Replica technique을 이용해 평가하고자 하였다. 연구 재료 및 방법:3종의CAD/CAM 시스템 (Cerasys $system^{(R)}$, KaVo $Everest^{(R)}$, $LAVA^{TM}$)을 이용하여, 시스템 별로 5개씩의 단일치아 지르코니아 코어를 제작하였다. 제작된 지르코니아 코어를 레플리카 테크닉 (Replica Technique)을 이용해 복제하고 복제된 시편을 협설, 근원심으로 정중앙을 절단하여 변연 및 내면의 간격을 측정하였다. Measuring microscope ($AXIO^{(R)}$)를 이용하여 50배 확대하여 촬영하고 $I-Solution^{(R)}$을 이용하여 측정하였고, ANOVA를 이용하여 통계적으로 분석하였다. 결과:지르코니아코어의 평균 변연간격은 $Cerasys^{(R)}$가 $84.74{\pm}27.57{\mu}m$, KaVo $Everest^{(R)}$가 $80.23{\pm}21.07{\mu}m$, $Lava^{TM}$가 $96.37{\pm}11.45{\mu}m$이었고 평균내면간격은각각 $Cerasys^{(R)}$는 $94.99{\pm}18.74{\mu}m$, KaVo $Everest^{(R)}$는 $92.31{\pm}25.18{\mu}m$, $LAVA^{TM}$는 $94.99{\pm}18.74{\mu}m$이었으며, 세 시스템간의 변연 및 내면간격의 평균적인 적합도에서 통계적 유의차는 없었다. 변연 및 내면간격의 비교에서는 KaVo $Everest^{(R)}$가 $LAVA^{TM}$보다 전반적으로 내면간격이 작은 것으로 보였다 (P< .05). 내면간격 중 절단부위의 간격은 세 시스템에서 공통적으로 다른 부위보다 크게 나타났다. 결론:본 연구에서 $Cerasys^{(R)}$, KaVo $Everest^{(R)}$, $LAVA^{TM}$시스템의 변연적합도는 차이가 없었고, 내면적합도는 KaVo $Everest^{(R)}$가 $LAVA^{TM}$ 보다 적었으나 세 시스템에서 공통적으로 절단부위 간격이 크게 나타났다. 세 시스템 모두 변연적합도 및 내면적합도가 임상적으로 허용범위 내에 있었다.
The treatment of craniofacial anomalies has been challenging as a result of technological shortcomings that could not provide a consistent protocol to perfectly restore patient-specific anatomy. In the past, wax-up and impression-based maneuvers were implemented to achieve this clinical end. However, with the advent of computer-aided design and computer-aided manufacturing (CAD/CAM) technology, a rapid and cost-effective workflow in prosthetic rehabilitation has taken the place of the outdated procedures. Because the use of implants is so profound in different facets of restorative dentistry, their placement for craniofacial prosthesis retention has also been widely popular and advantageous in a variety of clinical settings. This review aims to effectively describe the well-rounded and interdisciplinary practice of craniofacial prosthesis fabrication and retention by outlining fabrication, osseointegrated implant placement for prosthesis retention, a myriad of clinical examples in the craniofacial complex, and a glimpse of the future of bioengineering principles to restore bioactivity and physiology to the previously defected tissue.
Extension of advanced odontogenic infection from deep neck fascial spaces into the mediastinum is heralded by chest pain, dyspnea, fever, and radiographic demonstration of mediastinal widening. The critical care should be done in a team approach by multiple medical and dental departments, such as, oral & maxillofacial surgery, otolaryngology, anesthesiology, chest surgery, and infection medicine. Especially, fluid & drug therapy, adequate incision & drainage and systemic supportive psychosedation care are important. But, acute hyperventilation can be produced by several distinct causes: severe anxiety, respiratory alkalosis, increased blood catecholamine levels, and a decrease in the level of the ionized calcium in the blood. The orofacial fears about acute pain, trismus, dysphagia, swelling and oral surgical treatment lead to the severe anxiety and increased blood catecholamine level by stress. Therefore, the most dental patient should be cared gently as the stress reduction protocol. In spite of the care, hyperventilation was occurred during psychosedation and local anesthesia for incision and drainage of the masticatory fascial space abscess with deep neck infection & mediastinitis. We suggest that the dental patient with advanced odontogenic infection must be attention for the manifestation of hyperventilation, especially in the medically compromised conditions.
PURPOSE. The purpose of this study was to determine the effect of changes in the horizontal plane angle on the mesiodistal width ratios of the maxillary anterior teeth during the acquisition of frontal view photographs, derive these ratios for Korean adults on the basis of the data obtained, and analyze them using the Golden Proportion as a reference. MATERIALS AND METHODS. In experiment I, 30 plaster casts were mounted on an articulator and positioned on the angle-measuring device with a center setting of $0^{\circ}$. The device was rotated to $10^{\circ}$ in $1^{\circ}$ increments in a counterclockwise direction. At each angle, photographs were obtained and analyzed. Experiment II was based on 60 patients who visited the Department of Prosthodontics at Kyungpook National University Dental Hospital from February 2012 to February 2015. The patients were divided into three groups [Male (M), Female (F), Total (M + F)]. Frontal views were obtained for all groups and analyzed. RESULTS. From $1^{\circ}$ to $10^{\circ}$, the relative mesiodistal width ratios for the maxillary anterior teeth showed no significant differences from those at $0^{\circ}$. In all three groups, the relative width ratio of the maxillary central incisor was smaller than that specified in the Golden Proportion; the opposite was true for the canine. CONCLUSION. Our results suggest that the mesiodistal width ratios of the maxillary anterior teeth do not follow the Golden Proportion in Korean adults, and that a change in the horizontal plane angle from $1^{\circ}$ to $10^{\circ}$ during frontal photography does not affect these ratios.
Dental extraction is potentially stress-inducing in many disabled patient. The body's response to dental stress involves the cardiovascular system(an increase in cardiovascular workload), the respiratory organ and the endocrine system(change in metabolism). To minimize the stress, the stress reduction method was established. The obtained contents were as follows: (1) Recognize the patient's degree of medical risk, (2) Complete medical consultation before dental therapy, (3) Schedule the patient's appointment in the morning, (4) Monitor and record preoperative and postoperative vital signs, (5) Use psychosedation during therapy, (6) Use adequate pain control during therapy, (7) Short length of appointment : do not exceed the patient's limits of tolerance, (8) Follow up with postoperative pain/anxiety control, (9) Telephone the risk patient later on the same day that treatment was given. Though the stress reduction method above was applied to the dental extraction in disabled patients with the advanced infected teeth, the complications(syncope, shock, bleeding & infection, etc.) may be occurred. For prevention of complications associated with the extraction, the authors treated the advanced infected teeth with endodontic drainage and incision & drainage before extraction. The final extraction and wound closure were then done after about 3 weeks.
Objectives: This study aimed to review the definition, qualification conditions, accreditation field, and system of Japanese recognized dental hygienists. It is expected that it will be used as basic data for the system introduction of the Korean advanced dental hygienists. Methods: From May to November 2020, a literature review was conducted on Japanese dental hygienists and Japanese certified dental hygienists. Results: The lifelong education system training courses to become Japanese certified dental hygienists consisted of basic training, special training, and specific training. Each training session lasted for 15 h, and the number of training hours required to complete the course was 30 h. The training items for the field of recognition A were prevention of lifestyle disease, home care, oral function management, rehabilitation for dysphagia, prevention of diabetes, and oral management by medical and dental partnership. The training items for the field of recognition B were dentistry for the disabled, dentistry for the aged, community oral health, and oral health care. Conclusions: The Japanese recognized that dental hygienist system is valuable as a demonstration model in introducing the Korean advanced dental hygienist system.

There are five principal causes for excessive bleeding in the immediate postextraction phase ; (1) Vascular wall alteration (wound infection, scurvy, chemicals, allergy) (2) Disorders of platelet function (genetic defect, drug-aspirin, autoimmune disease) (3) Thrombocytopenic purpuras (radiation, leukemia), (4) Inherited disorders of coagulation (hemophilia, Christmas disease, vitamin deficiency, anticoagulation drug-heparin, coumarin). If the hemorrhage from postextraction wound is unusually aggressive, and then dehydration and airway problem are occurred, the socket must be packed with gelatine sponge(Gelfoam) that was moistened with thrombin and wound closure & pressure dressing are applied. The thrombin clots fibrinogen to produce rapid hemostasis. Gelatine sponges moistened with thrombin provide effective coagulation of hemorrhage from small veins and capillaries. But, in dental alveoli, gelatine sponges may absorb oral microorganisms and cause alveolar osteitis (infection). This is a case report of bleeding control by continuous rubber strip & iodoform gauze drainage (without gelfoam packing) of active bleeding infection sites of three teeth extraction wounds in a 46-years-old female patient with advanced liver cirrhosis.
Purpose: The aim of this retrospective study was to compare the survival rate of oxidized titanium implants and sandblasted large-grit acid etched implants in soft bone. Methods: 201 oxidized titanium implants were inserted in 84 patients between May 1999 and May 2004. 120 sandblasted large-grit acid etched implants were inserted in 74 patients between December 2000 and May 2004. The patients were followed-up 0${\sim}$5 years in ITI group or 0${\sim}$6 years in BRA group, respectively. The following information was collected from the patient records: age, gender, systemic disease, implant type, number, length and diameter of the implants, their location in the jaws, bone quantity, the number of failed implants, the causes of failure, and advanced surgery for bone augmentation. Results: In the oxidized titanium implants, 8 implants showed early failure, and 1 implant showed late failure, respectively. The cumulative survival rate was 95.48%. In the sandblasted large-grit acid etched implants, 1 implant showed late failure and cumulative survival rate was 99.10%. The cumulative survival rate and the survival rates in the case of the advanced procedure during the implant placement were not significantly different in both groups. Conclusions: Oxidized titanium implants and sandblasted large-grit acid etched implants can be used successfully in soft bone regardless of the surgical methods used during the implant placement. Background: This study aimed to evaluate the patient's pain and quality of life after suture removal at either 3 or 7 days following the bilateral surgical extraction of impacted lower third molars. Methods: This study was a prospective, randomized controlled clinical trial carried out in 30 patients, who acted as their own control. Each patient required the bilaterally impacted mandibular third molars to be extracted. The impacted teeth were removed and the wound margins were approximated and sutured with black braided silk. The suture material was removed on day 3 on one side and on day 7 on the other. Each participant was asked to complete a questionnaire after the removal of the suture material on each designated day. Results: Regarding overall clinical symptoms, the mean VAS scores of male and female participants on day 3 were not significantly different from those on day 7. A significant difference was found in female participants, in that overall daily activity was better on day 7. There were significant differences in the ability to smile and laugh in both sexes and the ability to chew in the male participants was better on day 7. Conclusions: There were no significant differences in the patient's pain and quality of life between suture removal on day 3 or on day 7 following surgery to remove impacted lower third molars.
