One-year experience of oral substrate reduction therapy in three patients with Gaucher disease type I

  • Purpose: Eliglustat is an oral substrate reduction therapy (SRT) approved for adults with Gaucher disease type I (GD1) who are extensive, intermediate, or poor CYP2D6 metabolizers. Here we report one-year experience of eliglustat switch therapy from long-term enzyme replacement therapy (ERT) in three adult patients with GD1. Materials and Methods: Medical history, clinical (hemoglobin concentration, platelet count, and bone mineral density) and biochemical parameters (angiotensin converting enzyme, total acid phosphatase, and lyso-gb1) of the patients were collected and evaluated by retrospective review of medical records at every 3, 6, or 12 month after switch to SRT. Results: Patient 1 was a 43-year old female diagnosed GD1 and her clinical and biochemical parameters were stabilized for more than 20 years by ERT. Due to the burden of regular hospital visit, she switched to SRT. During one-year of SRT, clinical parameters and biomarkers were maintained stable. However, after suffering acute febrile illness during SRT, she decided to re-switch to ERT due to concerns about drug interaction. Patient 2 was 41-year old male, younger brother of patient 1 and Patient 3 was 31-year old male. They switched to SRT in clinically stable condition with long-term ERT. The one-year SRT was tolerable without specific safety issue and the clinical parameters were maintained stable. Conclusion: One-year eliglustat therapy in three adult patients with GDI was generally tolerable and effective for maintaining the clinical parameters and biomarkers. However, the drug compliance, concurrent drug interactions, and long-term safety of eliglustat should be carefully monitored.

Assessment of coagulation function by thromboelastography in dogs with mitral valve insufficiency

  • In veterinary medicine, a variety of disease are known to cause coagulation abnormalities. Identification of these coagulation abnormalities have been relied on traditional coagulation assays(platelet concentration, aPTT, PT, D-dimer, fibrinogen) which take only a small part of the coagulation pathways rather than global hemostatic capacity. Among of the hypercoagulable diseases, cardiovascular disease, such as mitral valvular disease, was not regarded as the cause of the hypercoagulability. The value of a thromboelastography (TEG) as an early predictor of coagulopathy, especially hypercoagulability, has been founded. It was associated with decreased R and K values, and increased MA and α angle. The objective of this study was to compare thromboelastography results and those of traditional coagulation tests between twenty adult dogs with mitral insufficiency (MVI group) and eleven adult healthy dogs (Healthy group). As a results, MA values in the patients with mitral insufficiency (68.8±7.8 mm) were significantly higher than the normal patients (60.4±4.8 mm) (P value<0.05). Although a little report has been reported in veterinary medicine, platelet activation seems to be related with hypercoagulability in MVI patients in human medicine. The result of this report can support this pathophysiology in veterinary medicine. In addition to traditional coagulation assay, global assessment of coagulopathy using TEG, especially ability to detect hypercoagulability, may be useful for customized treatment in MVI patients. To achieve this, further study is needed to define pathophysiology and effect of medication.

한국 성인당뇨병 환자의 식생활에 관한 문화기술적 연구 (An Ethnographic Study on Eating Styles of Adult Diabetics in Korea)

  • Purpose: The purpose of this study was to explore adult diabetics' eating styles and factors which influence them. Method: The study adopted an ethnographic method based on a perspective which views the eating style as a cultural phenomenon. Data was collected through a personal interview, participant observation, and documented materials from Oct.2001 to Sept. 2002. In this study, fifteen adult diabetics, with an average age of 57, participated. Data analysis was done by the Spradley's taxonomic analysis technique. Result: The patients' eating styles were rooted in their viewpoint on illness as well as the meaning of food. Eating styles were classified into 4 types: Pathology-centered, symptom-centered, need-centered, and role-centered. Conclusion: A conventional approach to the treatment and management of diabetes did not consider the patient's inner world which may play an important role in the successful management of the disease. We found that it was critical for health care personnel to understand patients' values, beliefs and their way of life in order to facilitate the most successful self-care diet.

수술적 치료를 시행한 함입된 메켈게실에 의한 성인 장중첩증 (Adult intussusception caused by inverted Meckel's diverticulum treated with operation)

  • Intussusception is a serious disease where part of the intestine slides into an adjacent part of the intestine. Adult intussusception is mainly due to benign or malignant neoplasm. Therefore, in most cases of adult intussusception, treatment by surgery would be preferable to conservative therapy. However, we report on a 28-year-old female patient who underwent intussusception operation delayed 3 months. Abdominal computed tomography 3 months ago showed a small bowel intussusception measuring 20 cm long. Three months later, the previously identified small bowel intussusception appeared without change. The patient underwent surgery, and ectopic gastric mucosa was observed in the biopsy. Therefore, Meckel's diverticulum was diagnosed.

성인 선천성 심장기형의 외과적 치료 (Surgical correction of congenital heart defects in adult)

  • The records of 248 patients over 16 years of age who had undergone a surgical correction of a congenital cardiovascular malformation during the period of 10 years from August, 1978 to July, 1988 were reviewed. During this period, the incidence of congenital cardiovascular malformation in adult was 18.2% of 1376 total heart disease operated on and 25.5% of 986 congenital heart defects. Among them, there were 200 patients in acyanotic group and 48 patients in cyanotic group. Male versus female ratio was 1:1.28. The oldest patient was 59 years old female who had atrial septal defect. The mean age was 24.4 years old. The distribution of the lesions showed a large preponderance of atrial septal defects [37.19o] followed in frequency by ventricular septal defects [918.1%], patent ductus arteriosus [17.3%], tetralogy of Fallot [16.1%], and a variety of other complex malformations[3.2%]. In the pediatric age group, relative frequency was different from that of this adult group, showing ventricular septal defects, tetralogy of Fallot, patent ductus arteriosus and atrial septal defects in order of incidence. The hospital mortality and late mortality were 6.0% and 1.7% respectively. The causes of hospital death were low cardiac output in 10 patients, arrhythmia in 2, air embolism in 1, sepsis in 1 and respiratory failure in l. Clinical improvement upto NYHA functional class I or II postoperatively has been achieved and sustained in all patients following repair except the patients of late death and receiving reoperation. This result confirms that congenital heart defects in the adults can be corrected with a good outcome and an aggressive operative approach seems justified.

중풍 조기검진 환자의 혈중지질에 대한 연구 - NCEP Adult Treatment Panel III를 중심으로 - (Study for Serum Cholesterol Level with Patients on Premature Examination of Stroke - Based on National Cholesterol Education Program Adult Treatment Panel III -)

  • The aim of this study is to investigate the serum cholesterol level of patients who visited Oriental Hospital of Daejeon University to predict stroke, in Korea. In the time of period June 15th 2009 to June 20th 2009, 45 patients visited Oriental Hospital of Daejeon University to check the risk of stroke. We checked their blood to find out the serum cholesterol level and investigated their past history according to Adult Treatment Panel III guidelines. Male has higher risk of coronary heart disease than the female. The average of the total serum cholesterol of the participants was lower than the average of Korean. The percentage of the participants who have less HDL-cholesterol than 40 mg/dL was 33.3% and this result is less than the average percentage of Korean. Only 8.9% of the participants were placed in group who need to use lipid-lowering drug to reduce the lipoprotein density. Checking the characters of the person is necessary to use lipid-lowering drugs. This study can be the baseline data of actual condition of serum cholesterol level.

  • 단심실성 심장기형의 교정방법으로 Fontan 수술이 행해지지만 이 수술의 위험요소를 가진 대상군에서는 고식적 수술을 시행한 후 단계적으로 Fontan 수술을 시행하는 것이 보편적이다. Fontan 수술의 시기는 고식적 수술 후 감소한 수술 위험요소와 고식적 수슬상태의 지속에 의하여 증가하는 위험 요소를 재평가한 후 결정되며, 고식술 후 비교적 조기에 시행되는 것이 보통이다. 국립의료원에서는 36세 성인 단심실증 환자에서 과거에 단방향성 Glenn 수술을 시행하고 17년 후에 완전 Fontan 수술로 전환한 증례가 있어 보고하고자 한다.

코어 운동이 성인 남성의 동적 균형과 몸통의 수평 회전에 미치는 효과 (The Effect of Core Stabilization Exercise on Dynamic Balance and Trunk Horizontal Rotation in Adult Men)

  • Background: This study aims to evaluate the effect of core stabilization exercise on the dynamic balance and horizontal rotation of the trunk in young adult men. Through this study, it is expected that various core stabilization exercises will prevent and treat musculoskeletal disease. Design: Randomized Controlled Trial. Methods: The study subjects were recruited from young adult men in their 20s and 30s living in Seoul, and after the randomized controlled trial, it was divided into an experimental group training core stabilization exercise(n=15) and a control group(n=15). The evaluation methods of this study were modified Star Excursion Balance Test(mSEBT), Functional Reach Test(FRT), and Trunk Rotation Test(TRT). The experimental group performed three sets of crunches and deadbug exercises twice a week for eight weeks, and the control group did not perform any exercises similar to core stabilization exercise during the experimental period. Results: The result of the experiment, the experimental group showed significant improvement in mSEBT(p<.05), FRT(p<.05) and TRT(p<.05). Conclusion: In conclusion, core stabilization exercises improved dynamic balance and horizontal rotation of the trunk. As a result of this study, core stabilization exercise can prevent and treat musculoskeletal diseases even in healthy people.

Influence of History of Brain Disease or Brain Trauma on Psychopathological Abnormality in Young Male in Korea : Analysis of Multiphasic Personal Inventory Test

  • Objective : The purpose of this study is to confirm whether brain disease or brain trauma actually affect psychopathology in young male group in Korea. Methods : The authors manually reviewed the result of Korean military multiphasic personal inventory (KMPI) in the examination of conscription in Korea from January 2008 to May 2010. There were total 237 young males in this review. Normal volunteers group (n=150) was composed of those who do not have history of brain disease or brain trauma. Brain disease group (n=33) was consisted of those with history of brain disease. Brain trauma group (n=54) was consisted of those with history of brain trauma. The results of KMPI in each group were compared. Results : Abnormal results of KMPI were found in both brain disease and trauma groups. In the brain disease group, higher tendencies of faking bad response, anxiety, depression, somatization, personality disorder, schizophrenic and paranoid psychopathy was observed and compared to the normal volunteers group. In the brain trauma group, higher tendencies of faking-good, depression, somatization and personality disorder was observed and compared to the normal volunteers group. Conclusion : Young male with history of brain disease or brain trauma may have higher tendencies to have abnormal results of multiphasic personal inventory test compared to young male without history of brain disease or brain trauma, suggesting that damaged brain may cause psychopathology in young male group in Korea.

조선시대(朝鮮時代) 순종(純宗)의 질병(疾病)에 관한 고찰 - 『조선왕조실록(朝鮮王朝實錄)』을 중심으로 - (A Research on the Disease of King Sunjong in the Joseon Dynasty)

  • King Sunjong was the 27th King and the last King in the Joseon Dynasty. He lived an unhappy life as the prince and the King of perishing country. At the age of 22, He witness the death of his mother Queen Myeongseong by Japanese assassin. He has a weak constitution and have many diseases. He get varicella(水痘) in infancy, and suffered from smallpox(紅疫) at 6, and catched the measles(天然痘) at 12. At the age of 25, Having drunk the coffee that contained opium(阿片), he had bloody excrement(血便) and lose the 18 teeth. The Symptoms that appeared frequently at adult age are indigestion(滯症), diarrhea(泄瀉), trophedema(足部浮腫), odontopathy(齒科疾患) etc. This indigestion(滯症) and diarrhea(泄瀉) are occurred by gastroenteropathy(胃腸病). This odontopathy(齒科疾患) is the aftereffect from having 18 false teeth. We assume that this trophedema(足部浮腫) is occurred by cardiac disease(心臟病) and renal disease(腎臟病). The chronic diseases of King Sunjong are gastroenteropathy(胃腸病), renal disease(腎臟病), cardiac disease(心臟病), rheumatism. We assume that the immediate cause of his death is the aggravation of the renal disease(腎臟病) and cardiac disease(心臟病). The medical treatments for him are hardly included in "Formal Records of the Joseon Dynasty(朝鮮王朝實錄)" which is the prime governmental document. Many royal doctors in court used the traditional Korean medicine, but western doctors from Japan often treated him by medical techniques such as injection(注射), consperg(散藥), liquid medicine(水藥).
