Incidence and Predictors of Falls in Institutionalized Elderly (요양시설 노인의 낙상 실태와 영향 요인)

  • Purpose: The purpose of this study was to determine the incidence and influencing factors of falls in the institutionalized elderly. Method: A descriptive survey design was used with a convenience sampling of 430 institutionalized elderly. Data were collected using a structured questionnaire. Results: The falls rate after admission in institution was 8.4%. Types of injury after falls were mostly contusion and fracture. Interventions for fall prevention were environmental management and regular evaluation of risk factors for falls. Fall experience was significantly different according to admission periods. Type of falls was significantly different according to state of ADL. Predictors for falls were longer admission periods and dependent state in ADL. Conclusion: Falls is important health related problem in the institutionalized elderly, so effective fall prevention program is needed for their health promotion.

Predictors of Delirium in Patients after Orthopedic Surgery (정형외과 수술 후 섬망 발생요인 분석)

  • Purpose: The purpose of this study was to investigate the predictors of delirium in patients after orthopedic surgery. Methods: Participants were 121 orthopedic surgery patients from one university affiliated hospital located in Seoul. The instrument of Delirium Observation Screening Scale (DOS) developed by Schuurmans et al. (2003) was utilized. Data were collected from September 1st, 2010 to March 31st, 2011 and analyzed using SPSS 12.0 with descriptive statistics, t-test, chi-square test and logistic regression. Results: The delirium in patients after orthopedic surgery was occurred in 9 (7.4%) out of 121 patients. Several factors were associated with the delirium occurrence age, admission route, preadmission Activity of Daily Living (ADL), preadmission hearing aid use, preadmission walking degree, diagnosis, type of surgery, Intensive Care Unit (ICU) stay after surgery, restraint, drainage tube, time of admission to surgery, preoperative albumin and preoperative sodium. Preadmission ADL, preoperative sodium and time of admission to surgery were the significant predictors of the delirium occurrence. Conclusion: Study results may help nurses predicting and detecting delirium early and providing preventive measures to the patients with high risk of delirium after orthopedic surgery.

Effects of flow variation in the first stage nozzle on the performance of a partial arc admission in a steam turbine (증기터빈 1단 노즐의 조속현상이 터빈성능에 미치는 영향)

  • Power plant industry has been developed at high-capacity, high-technology, and innovation. Steam turbine became the most useful equipment that dominate more than 50% of all the world electricity production. And developed new materials of the turbine blade and extended length of the turbine last blade brought reform in steam turbine performance upgrade. In this paper, when do partial load driving in high-capacity steam turbine, optimum driving method found whether there is something. In operating steam turbine, there is a lot of loss from secondary wake and throttle of the 1st stage nozzle by the biggest leading factor that load fluctuation affects in high-pressure steam turbine performance. Effect of internal efficiency by 1 stage nozzle is the biggest here, but here fluid flow and flow analysis were not yet examined closely definitely. So, Analyzed design data and acceptance performance test result to applying subcritical pressure drum type 560 MW, supercritical-pressure once through type 500 MW, and 800 MW steam turbines actually. In conclusion, at partial load driving, partial arc admission(PAA) is more efficient than full arc admission(FAA) efficiency. This is judged by because increase being proportional with gross energy of stream that is pressure - available energy if pressure of stream that is flowed in to the turbine increases, available energy becomes maximum and turbine efficiency improves. Therefore, turbine performance is that preview that first stage performance fell if decline is serious in partial load because first stage performance changes according to load.

Effect of Leading Edge Shape on the Blade Surface Temperature of a Partial Admission Supersonic Turbine (부분입사형 초음속 터빈의 블레이드 표면 온도에 블레이드 앞전 형상이 미치는 영향)

  • In this paper, numerical analysis of the surface gas temperature on turbine blades has been performed to investigate the temperature profiles characteristics of a partial admission supersonic turbine driven by high temperature and pressure gas of pyro-starter with two different types of turbine blade edge shape. In order to examine the surface gas temperature on turbine blades at initial starting, computations tlave been carried out at several turbine rotational speeds in the range of $0{\sim}10,000$ rpm for each type of turbine edge shape. "Sharp" edge and "Round" edge types were taken as the turbine edge shape factor. As turbine rotational speed increased, the average temperature of turbine blades was further decreased. It was also found that the surface temperature of turbine blades with a sharp edge was lower than round-type edge turbine blades.

Factors and Satisfaction in Selecting University and Departments of One University freshmen

  • In this paper, we propose to identify the factors and satisfaction of the selection of University and departments of One university freshmen. Research subjects were 499 freshmen in 2021 at K University located in G city. The data were analyzed by descriptive statistics, t-test, ANOVA, Scheffe' test and pearson's correlation coefficient using SPSS WIN 18.0. The results of this study showed that the internet/SNS was the highest in university information media, the school teacher was the highest in information provider, and the employment rate had the most impact on university registration decision. The University satisfaction score was 3.43, and there was a significant difference in gender(t=5.527, p=.019) and admission type(F=5.527, p<.001). The department satisfaction was 3.86 and there was a significant difference in the admissions type(F=3.004, p=.018). Univdrsity satisfaction and Department satisfaction showed a significant positive correlation(r=5.527, p<.001). Universities should improve their competitiveness through systematic admission information system.

  • Objectives Low back pain (LBP) is one of the most common reason for people in Korea to visit Korean medical institutions. To assess actual amounts of use in the treatment of LBP in Korean medicine and to provide objective base line data for policy decision making, research regarding the current state of LBP patients' treatment in Korean medical institutions are in need. Methods The current study was designed as a retrospective chart review to investigate descriptive characteristics of LBP patients. The clinical records of 304 patients who were hospitalized for the treatment of LBP in Korean rehabilitation medicine inpatient clinics of five different Korean medicine hospitals were analyzed. The percentage of patient characteristics such as sex, age, average duration of admission, insurance type, diagnosed LBP related disease code, and rates of interventions applied were assessed. Results 1. The female sex was significantly predominant among patients with LBP : 105 patients (34.5%) were male and 199 patients (65.5%) were female. Percentage of the patients' age appeared as followed : 76 people (25.0%) were in their 50s, 64 people (21.1%) were in their 40s, 51 people (16.8%) were in their 30s, 37 people (12.2%) were in their 60s, and 33 people (10.9%) were in their 70s. Approximately half of the total LBP patients investigated were older than 50. 2. The average duration of admission was 16.2 days. Approximately one third (30.3%) of the patients were hospitalized for 8 to 14 days. 3. Female patients tended to stay admitted in hospitals slightly longer than male patients. Elderly (age 60~79) patients stayed in hospitals longer (17.8 days) compared to younger (age 20~39) patients (13.5 days). 4. More than half of the patients (171 cases, 56.3%) had their hospital bills covered with automobile insurance. 40.1% (122 cases) of the patients had medical insurance to cover their hospital bills. The average duration of admission of patients who had automobile insurance was 14.2 days, while that of the patients who had medical insurance was 18.4 days. 5. "Sprain and strain of the lumbar spine and pelvis" was the most commonly used (195 cases, 64.1%) disease code in patients with LBP. Patients diagnosed as "lumbar and other intervertebral disc disorders with radiculopathy" required the longest admission duration (22.1 days). 6. Herbal medication was applied to all of the patients during admission. Acupuncture was applied to all of the patients except one case diagnosed as spinal stenosis. Physical therapy, cupping therapy, moxibustion therapy, chuna therapy, and pharmacopuncture therapy were applied to 94.7, 92.8, 85.2, 83.9, and 49.7% of the patients, respectively. 7. There were certain differences among Korean medicine hospitals in terms of the LBP patients' duration of admission, type of insurance, frequency of the disease code use, type of intervention applied. Conclusions It is thought that the current study can be used as reference data in assessing the current state of LBP treatment in Korean rehabilitation medicine and a basis for future research. Provided improvements of certain limitations of the current study in future researches, such data would act as better base line data in policy decision making.

  • This paper investigates the mathematical model of multi-server retrial queueing system with the Batch Markovian Arrival Process (BMAP), the Phase type (PH) service distribution and the finite buffer. The sufficient condition for the steady state distribution existence and the algorithm for calculating this distribution are presented. Finally, a formula to solve loss probability in the case of complete admission discipline is derived.

Clinical Analysis of 58 Cases of Aortic Dissecting Aneurysm (해리성대동맥류 58례에 대한 임상적 고찰)

  • Dissecting aortic aneurysm is a life threatening condition which necessitates prompt diagnosis and management. Between January 1987 and September 1993,58 patients was admitted to our department. Mean age at admission was 53 years.[range 25-82]. Clinical findings included chest pain in 48 cases[83%],renal failure in 12[20%],aortic insufficiency in 11[19%] and stroke in 9[15%]. Predisposing factors were hypertension in 50 cases[86%],Marfan`s syndrome in 6[10%] and diabetes melitus in 1 [2%]. 23 patients[ type A 13,type B 10 ] underwent surgical treatment. Surgical technique for type A included graft replacement of ascending aorta in 7 cases,graft replacement and aortic valve resuspension in 3,and Bentall`s operation in 3 cases. Type B patients were operated when specific indications applied. There were three [Two in type A and 1 in type B] deaths in the operation group and nine [ 5 in type A and 4 in type B] deaths in the medical group. These results support our current policy in the treatment of dissecting aortic aneurysm.

Time Trend of Out-of-pocket Expenditure among Cancer Inpatients: Evidence from Korean Tertiary Hospitals

  • Background: This study aimed to examine out-of-pocket expenditure for cancer treatments of hospitalized patients and to analyze changing patterns over time. Materials and Methods: This study examined data of all cancer patients receiving inpatient care from two tertiary hospitals from January 2003 to December 2010. Medical expenditures per admission were calculated and classified into those covered and uncovered by the Korean National Health Insurance (NHI) and co-payment. Results: The medical expenditure per admission increased slowly from 3,455 thousand Korean won (KRW) to 4,068 thousand KRW. While expenditures covered by the NHI have increased annually, co-payments have generally decreased. The out-of-pocket expenditure ratio, which means the proportion of uncovered expenditure and co-payment among total medical expenditure dropped sharply from 2005 to 2007 and was maintained at a similar level after 2007. Medical expenditures, NHI coverage, and the out-of-pocket expenditure ratio differed across cancer types. Conclusions: It is necessary to continually monitor the expenditure of uncovered services by the NHI, and to provide policies to reduce this economic burden. In addition, an individual approach considering cancer type-specific characteristics and medical utilization should be provided.

Difference in Length of Stay and Treatment Outcome of Pulmonary Tuberculosis Inpatients between Health Insurance Types (의료보장유형에 따른 폐결핵 입원환자의 재원기간과 치료결과 차이분석)

  • The purpose of this study is to identify patient and hospital characteristics with pulmonary tuberculosis and to analyze factors which were influencing length of stay and treatment. The Korean National Hospital Discharge In-depth Injury Survey database from 2006 to 2012 was used for analysis. Study subjects were 4,704 patients and analyzed by using frequency, chi-square and logistic regression through using STATA 12.0. To avoid selection bias, we used propensity score matching. Analysis results show that the length of stay and treatment of pulmonary tuberculosis was different between insurance types. Patients characteristic(female, comorbidity, admission by outpatient department, medical insurance type) and hospital characteristic(500-999 beds, over 1000 beds) significantly influence length of stay. Admission by outpatient department and over 1000 beds are significantly influence treatment. Based on these findings, it is necessary to clarify between length of stay and treatment outcome by medical aids beneficiaries and audit hospitals follow discharge guidelines in pulmonary tuberculosis patients.
