Recovery of Symptoms and Signs in Four Cases of Acute Tonsillitis Treated with Hyunhwang-haedok-tang (현황해독탕으로 호전된 급성편도선염 치험 4례)

  • This study was designed to report the clinical effects of Hyunhwang-haedok-tang on patients who have suffered from acute tonsillitis. Patients complained of fever, sore throat, swallowing difficulty, headache, cough. We give a diagnosis of acute tonsillitis. Therefore, we treated patients with oriental medicine, especially Hyunhwang-haedok-tang, and found that those symptoms and signs were improved by VAS, which is visual analogue scale. Through this study, Hyunhwang-haedok-tang on patients who have suffered from acute tonsillitis, proved to be effective.

Moxifloxacin Ameliorates Oleic Acid-induced Acute Lung Injury by Modulation of Neutrophilic Oxidative Stress in Rats (Moxifloxacin의 Secretory $PLA_2$억제가 올레인 산으로 유도된 호중구성 급성 폐손상에 미치는 영향)

  • Background: Based on the known immunoregulatory functions of moxifloxacin on phagocytes, the therapeutic effect of moxifloxacin on oleic acid (OA)-induced acute lung injury (ALI) was investigated. Methods: Moxifloxacin (10 mg/kg) was given to male Sprague-Dawley rats that had been given oleic acid (OA, $30{\mu}L$) intravenously. Five hours after OA injection, parameters demonstrating ALI were assessed to measure the effects of moxifloxacin on acute lung injury. Results: The pathological findings of OA-induced ALI's was diminished by moxifloxacin. Through ultrastructural and $CeCl_3$ EM histochemistry, moxifloxacin was confirmed to be effective in decreasing oxidative stress in the lung as well. Indices of ALI, such as lung weight/body weight ratio, protein content in bronchoalveolar lavage fluid, and lung myeloperoxidase were decreased by moxifloxacin. In diaminobenzidine immunohistochemistry, fluorescent immunohistochemistry, and Western blotting of the lung, moxifloxacin had decreased the enhanced expression of secretory phospholipase $A_2$ ($sPLA_2$) by OA. Conclusion: We concluded that moxifloxacin was effective in lessening acute inflammatory pulmonary edema caused by OA, by inhibiting the neutrophilic respiratory burst, which was initiated by the activation of $sPLA_2$.

Case of Combined Eastern-Western Medicine Treatment on a Patient Diagnosed with Benign Paroxysmal Positional Vertigo after Acute Vestibular Neuritis (양성 돌발성 체위 현훈으로 이행된 급성 전정신경염 환자의 한양방 협진 치료 1례)

  • Objective: This clinical study was performed with a female patient to evaluate the effects of combined Eastern-Western medicine treatment on benign paroxysmal positional vertigo after acute vestibular neuritis.Method: We used acupuncture, herbal medication, Western medication, fluid therapy, and the canalith repositioning maneuver to treat a female patient suffering from dizzy spells. The vertigo scale was checked to assess any improvement in symptoms.Results: The patient had originally recovered from acute vestibular neuritis, but after the first discharge, she came down with benign paroxysmal positional vertigo and had to be rehospitalized. She eventually got better, however, and her score on the vertigo scale improved.Conclusion: This study demonstrates that combined Eastern-Western medicine treatment may be an effective option for treating benign paroxysmal positional vertigo after acute vestibular neuritis, despite the difficulty of the treatment in this case.

Acute Stress Disorder Patients Treated with Yanshimtang-Gamibang: Three Case Reports (급성 스트레스 장애에 대한 양심탕가미방(養心湯加味方) 치험 3례)

  • Objectives: The purpose of this case report was to propose Yangshimtang-Gamibang on three patients diagnosed with acute stress disorder. Methods: Yangshimtang-Gamibang was treated on three patients diagnosed with acute stress disorder after experiencing a traffic accident. The effects of Yangshimtang-Gamibang were evaluated through BAI (Beck Anxiety Inventory), NRS (Numerical rating scale), NDI (Neck Disability Index), ODI (Oswestry Disability Index), respectively, and quality of sleep was assessed with total sleep time and number of times waking up during sleep. Results: After the treatment, there was significant decrease in the value of BAI score, a measurement of anxiety, as well as the value of NRS, NDI, ODI, a measure of pain. Also, the sleep quality of three patients improved. Conclusions: The results indicate that Yangshimtang-Gamibang may have an effect on people with acute stress disorder who recently experienced a traffic accident.

Acute Tubular Necrosis associated with the Ketogenic Diet in a Child with Intractable Epilepsy

  • The ketogenic diet (KD) has been used as an effective antiepileptic therapy for intractable childhood epilepsy. However, various adverse effects have been reported with use of the KD. We report a case of a child who developed acute tubular necrosis subsequent to therapy with KD. A 5-year-old girl had myoclonic epilepsy with developmental delay. She was under the treatment with antiepileptic drugs since the age of 3 months and on the KD during the past 18 months. Proteinuria persisted intermittently with the initiation of the KD and subsequently increased in the past 2 months. She was admitted with intermittent mild fever, vomiting, and lethargy for the past 3-4 weeks. At the time of admission, she presented with hypertriglyceridemia, heavy proteinuria, renal Fanconi syndrome, and acute kidney injury. Renal sonography showed a marked increase in the size and parenchymal echogenicity of both kidneys. A renal biopsy revealed acute tubular necrosis accompanied by early interstitial fibrosis. After the withdrawal of the KD and supportive therapy, without changing other anticonvulsants and their dosages, improvement of renal function was observed. Proteinuria had disappeared after 1 month and kidney size returned to normal after 8 months. It is hypothesized that the KD can induce and/or aggravate the renal tubulointerstitial injury in some patients who are under the treatment with anticonvulsants.

The effect of backward walking training on balance, balance confidence and falls efficacy in patients with acute stroke: A pilot randomized controlled trial (후방 보행훈련이 급성기 뇌졸중 환자의 균형, 균형 자신감, 낙상 효능에 미치는 영향: 무작위 대조군 예비연구)

  • Background: The requirements for postural and motor control in backward walking training (BWT) may improve balance and walking speed in patients with acute stroke. The aim of this study was to analyze the effect of BWT on balance, balance confidence, and fall efficacy in this population. Design: Randomized controlled pilot trial. Methods: This study included 14 subjects with acute stroke (onset of illness less than one month). They were randomly allocated to a BWT (n=7) or forward walking training (n=7) group and observed five times in a week for a period of two weeks. Measurements were taken before and after the experiment using the Berg balance scale (BBS), Activities-specific balance confidence scale (ABC), and Fall efficacy scale (FES). Results: The BBS, ABC and FES scores obtained in both groups after the experiment were significantly higher than those before the experiment (p<0.05). In addition, the BBS, ABC, and FES scores in the experimental group were significantly higher than those in the control group (p<0.05). Conclusion: These findings indicate that BWT improved balance and balance confidence and decreased the risks of fall in patients with acute stroke. Further study is needed to better understand the effects of backward walking in acute stroke patients.

A Case Report of Cold Hypersensitivity Caused by Acute and Chronic Blood Deficiency (급만성(急慢性) 혈허(血虛)로 유발된 냉증(冷症) 치험례)

  • Purpose: This study intends to report the effects of Koong Gui Tang on Cold Hypersensitivity caused by acute and chronic Blood Deficiency. Methods: The patients having Cold Hypersensitivity were treated with oriental medicine including herbal medication therapy. Thereafter their health status was analyzed with their own empirical assessments and VAS. Results: After the series of medical treatment, Cold Hypersensitivity caused by acute and chronic Blood Deficiency has shown considerable improvement. Conclusion: This case study shows that Koong Gui Tang therapies are medically effective on Cold Hypersensitivity caused by acute and chronic Blood Deficiency.

  • Objective: The purpose of this study was to investigate acute and subacute toxicity and sarcoma-180 anti-cancer effects of herbal acupuncture with Carthami- Tinctorii fructus (CF) in mice and rats. Method: Balb/c mice were injected intraperitoneally with Carthami - Tinctorii fructus (CF) for $LD_{50}$ and acute toxicity test. Sprague Dawley rats were injected intraperitoneally with Carthami- Tinctorii fructus (CF) for subacute toxicity test. The Carthami- Tinctorii fructus herbal-acupuncture was injected on Chung-wan (CV12) of mice with Sarcoma-180 cancer cell line. Results: 1. $LD_{50}$ was uncountable as none of the subjects expired during the test. 2. In acute toxicity test, toxic symptoms were not detected, but the body weight of mice was increased in treatment Ⅰ, treatment Ⅱ groups, compared to the normal group.(p<0.05) 3. In acute toxicity test of serum biochemical values of mice, glucose was increased in treatment Ⅰ and treatment Ⅱ groups, total cholesterol was increased in treatment I group, GOT was decreased in treatment Ⅱ group, and GPT was decreased in treatment Ⅰ group, compared to the normal group.(p<0.05) 4. The clinical signs and the body weight of mice treated with 0.1 cc, 0.2cc Carthami- Tinctorii fructus (CF) were not affected during the subacute toxicity test. 5. In subacute toxicity test, treatment groups didn't show significant changes in complete blood count test (CBC) of rats, compared to the nonnal group.(p<0.05) 6. In subacute toxicity test of serum biochemical values of rats, uric acid was decreased in treatment Ⅰ and treatment Ⅱ groups, compared to the nonnal group, triglyceride was decreased in treatment I group, compared to the normal group, GOT and GPT were decreased in treatment I and treatment Ⅱ groups, and alkaline phosphatase was decreased in treatment Ⅰ and treatment Ⅱ groups, compared to the normal group.(p<0.05) 7. Median survival time was increased in all the treatment groups for Sarcoma-180 cancer cell treated with Carthmni- Tinctorii fructus (CF).(p<0.05) 8. Natural killer cell activity was significantly increased in all the treatment groups compared to the normal group.(p<0.05) 9. Interleukin-2 productivity was decreased in treatment Ⅰ and treatment Ⅱ groups.(p<0.05) Conclusion: According to the results, we can conclude herbal-acupuncture of Carthami-Tinctoriifructus (CF) caused negligible toxkity, and had anti-tumor effects in mice.

The clinical signs and the body weight of mice treated with 0.1cc and 0.2cc Haeng-In extract were not affected during the acute toxicity test. 3. In acute toxicity test of serum biochemical values of mice, total protein and albumin were decreased in treatment group Ⅰ. Glucose was increased, and total cholesterol was decreased in treatment groups. GPT was increased in treatment group Ⅰ. 4. In subacute toxicity test, toxic symptoms were not detected in the treatment groups. 5. In subacute toxicity test, the body weight was increased in treatment groups on 14th and 21st day. 6. In subacute toxicity test. liver weight was increased in treatment group Ⅱ, and spleen weight was increased in treatment group Ⅱ. Lung weight was increased in an the treatment groups.(p<0.05) 7. In subacute toxicity test, severe tissue injury was found in lung and liver, especially treatment group Ⅰshowed more significant lung damage compared to treatment group l. 8. In subacute toxicity test, WBC. MCH and MCHC were increased in an the treatment groups, RBC, HGB and HCT were decreased in treatment group H(p<0.05). 9. In subacute toxicity test of serum biochemical values of rats, triglyceride was decreased in all the treatment groups. ALP was decreased in treatment group Ⅰ. and creatinine was decreased in treatment group Ⅱ. BUN/CR was increased in treatment group Ⅱ(p<0.05). 10. Median survival time of Sarcoma-180 cancer cell treated with Haeng-In was increased in all the treatment groups by twenty percent, compared to the control group(p<0.05). 11. Natural killer cell activity about the Sarcoma-180 cell was decreased at the ratio of 100:1 but was increased at the ratio of 10:1. In treatment group Ⅱ, increase was found at the ratio of 100:1 and 50:1 (p<0.05). 12. Interleukin-2 productivity of the Sarcoma-180 cell was decreased in treatment group I, but was increased in treatment group Ⅱ(p<0.05). Conclusion: According to the results, we can conclude Herbal-acupuncture with Anneniacae amarum semen caused toxicity, and had effects in Sarcoma-180 cancer cell.

Comparison of Acute Cardiovascular Effects of Using Heated Tobacco Productsand Cigarette Smoking (가열담배 사용과 연소담배 흡연의 급성 심혈관 효과 검증)

  • The aims of this study were to compare the differences in hemodynamics between cigarette smoking and using heated tobacco products (HTPs) and to determine the acute effects of using HTPs on cardiac autonomic regulation. Another goal was to examine the acute cardiac autonomic responses when using different tobacco sticks in HTPs. Sixteen healthy male smokers completed an open-label, randomized, crossover trial consisting of non-smoking (NS), cigarette smoking, and the use of two different HTPs (IQOS with HEETS; lil SOLID with Fiit). Sub-trials, which included NS, lil SOLID with Fiit, and lil SOLID with HEET, were performed on eight smokers among the total subjects. Hemodynamic variables, such as systolic blood pressure (SBP) and diastolic blood pressure (DBP), and heart rate variability were measured before, during, and 30 minutes after using each tobacco product. Using HTPs resulted in a significant increase in both SBP and DBP, comparable to smoking cigarettes. Cardiac sympathetic activity significantly increased, and cardiac vagal tone (CVT) significantly decreased after acute exposure to HTP aerosol, similar to the effects of cigarette smoke exposure. Furthermore, differences in the withdrawal of CVT were observed when using different tobacco sticks in the same HTPs. The findings of this study indicate that acute exposure to HTP aerosol increases the hemodynamic burden and disrupts cardiac autonomic balance, similar to exposure to cigarette smoke. Moreover, depending on the type of tobacco stick inserted into the HTP device, acute withdrawal of CVT may have been enhanced.
