From an accident at 2:00 a.m. in November 2nd, 2012 that one fire-fighter died while suppressing the E land World Inc. Fire in Bupyeong-Gu, Incheon, we concluded the problem and the improvement method of using the wireless communication (the two-way radio) that the fire safety officer and the safety crew use when identifying the safety of the fire scene. And after the investigation, we could find that the communication between the fire safety officer and the safety crew was difficult in the scene of the fire by voice because of much noise, so they could rely solely on the wireless communication. Therefore, we investigated the wireless communication and its frequency now used in Incheon Fire Safety Devision. Also, we compared with those of the military and the police. As a result, by making use of the wireless communication and its frequency currently possessed in Incheon Fire Safety Division, we found that the fire safety officer and the safety crew rescuing people at the risk of their own life in the various fire scene can communicate effectively.

This paper proposes Power System Health Index(PSHI) newly. The paper describes several kind of power system health indices based on two main categories, which are adequacy and security. In adequacy, four kinds of health indices of Frequency, Voltage, Reserve(Operating Reserve Power and Frequency Regulation Reserve Power) and Overload of lines and transformers are proposed. In security, four kinds of health indices of Voltage(154kV, 345kV and 765kV), Overload of lines and transformers, Power flow constraint among areas and SPS are proposed. All indices are mapped with three domains, which are indicated as Health, Margin and Risk, defined with expert interview. While domains of health, margin and risk is defined similar with the conventional well being analysis of power system. The criterion of the domains is proposed using an interview with expert operators and practical reliability codes in Korea. The several kinds of health index functions, which are linear ratio, piecewise linear ration and reverse ratio function etc. are developed in this paper. It will be expected that the developed health indices can help operators to control power system more successfully and also prevent power system from accident as like as black out in future because operator can make a decision immediately based on more easily visual information of system conditions from too much indices acquisition of complex power system.
Numerous losses of decay heat removal capability have occurred at U during stutodwn while its significance to safety is needless to say. A study is carried out as an attempt to assess what could be done to lower the frequency of these events and to mitigate their consequences in the unlikely event that one occurs. The shutdown risk model is developed and analyzed using Event/Fault Tree for the typical pressurized water reactor. The human cognitive reliability (HCR) model, two-stage bayesian approach and staircase function model are used to estimate human reliability, initiating event frequency and offsite power non-recovery probability given loss of offsite power, respectively. The results of this study indicate that the risk of a Pm at shutdown is not much lower than the risk when the plant is operating. By examining the dominant accident sequences obtained, several design deficiencies are identified and it is found that some proposed changes lead to significant reduction in core damage frequency due to loss of cooling events.
Korean society is rapidly changing to aging society comparing the other industrialized countries, however, the studies of elderly driver's driving behavior and accidents are not enough in Korea for elderly driver's accident prevention. This study focused on the elderly driver's psychological effects on elderly driver's driving behavior and traffic accidents; carefulness and aberrant driving behavior. - Elderly driver's traffic accidents The high percentage of elderly driver's accidents occurs in intersections and when turning left. There was a significant difference of the opponent vehicle's speed when left turn, between elderly driver and young driver; the elderly driver choose the higher speed of opponent vehicle than young driver when left turning. This result means that elderly driver has some problems with deciding the vehicle's speed and gap acceptance(Sunyeol Lee, Soonchul Lee, and Inseok Kim, 2006)(Table 1). - Carefulness and driving confidence In order to understand elderly driver's carefulness, this study compared the elderly driver's driving confidence. Driving confidence was consisted of 4 factors; environment of traffic condition, safe driving, driving ability and attention. Elderly driver's confidence was lower than young driver's. Elderly driver in high driving confidence group, showed longer driving history and they were tend to commit violations more frequently than elerly driver in low driving confidence group. Young driver, whose driving confidence level was high answered more driving history, annual mileage, the frequency of committing traffic violation and the experience of accident within lats 5 years(Soonchul Lee, Juseok Oh, Sunjin Park, Soonyeol Lee and Inseok Kim, 2006)(Table 2). This study examined the total time required until deciding to turn left in the no traffic signal intersection between elderly driver and young driver. The result showed that the time of elderly driver was significant longer than young driver(Sunyeol Lee et al, 2006)(Table 3). - Elderly driver's aberrant behavior Driver behavior Questionnaire(DBQ) was measured to understand the aberrant behavior; violation, error and lapse. The tend of aberrant behavior was observed by aging(Sunjin Park, Soonchul Lee, Jonghoi, Kim and Inseok Kim, 2006). Elderly driver's DBQ score was lower than young driver's(Table 4). Elderly and young driver showing longer driving history were in low DBQ score group. Elderly driver had high error score and young driver had high violation score. Young driver's aberrant driving behaviour was associated with annual mileage and the frequency of committing traffic violation. Elderly driver's aberrant driving behaviour was associated with annual mileage and experience of accident. Especially elderly driver whose violation, error and lapse score was high answered more committing experience of accident within last 5 years.

Power cables installed in domestic factories or underground can cause accidents depending on the manufacturing process, installation, and environmental conditions during use. When an accident occurs in a power cable, it can cause enormous economic loss and social confusion. Hence, the importance of preventive management of the cable through diagnosis is increasing to prevent it. Therefore, in this paper, a diagnostic sample cable was produced by simulating a part that could be a problem due to the installation, manufacturing defects, or deterioration of cables that can occur in the field. Dielectric loss Tangent (tan 𝛿; TD), and Partial Discharge(PD) tests were performed. Partial discharge and AC (60Hz) withstand voltage equipment using High-Frequency Current Transformer (HFCT) were applied After applying a VLF (Very Low Frequency) power supply with a frequency of 0.1Hz was applied. As a result, B and C phase defect samples at a 2.0U0 voltage through the VLF could measure the internal partial discharge in the A-phase normal sample cable from the noise at a 0.5U0 to 2.0U0 voltage. In addition, the 1.5U0 voltage was measured through the AC (60Hz) withstand voltage equipment of the commercial frequency to verify its effectiveness. Partial discharge in the run-off state was measured at a voltage of 1.0U0, and there was a risk when installing the equipment. AC power equipment showed a difficulty of movement by volume or weight. The diagnostic method, through the VLF of the quadrant state, revealed its safety and effectiveness.
This is clinicostatistical study of mandibular fractures. This study was based on a series of 677 patients with mandibular fracture during the period of 1982 to 1987. This results obtained were as follows : 1. In respect of incidence, there were high frequency in May and September, and in large city(63%). 2. The age frequency was the highest in the 20s-year old group (38.7%), and the ratio of male to female was 4.64 : 1. 3. The most common etiologic factor was blow(31.5%), but in children that was traffic accident followed by fall down. 4. The most common site of bone fractures was symphysis, followed by angle, condyle. In comparison of right and left sides, left side was more frequently involved(364 cases) than right side(257 cases). 5. The patients arrived in hospital immediately within 24 hours after accident were 62.9% of all, and 42.4% was arrived via private medical and dental clinic. 6. In respect of treatment, open reduction was 55.5% of all, closed reduction was 37.2%. In children, closed reduction was done in 50.6%.
The major purpose of this paper to identify safety climate practices, and to find the affecting variables that influence to the difference in the level of safety climate between plants and employees. And this paper attempted to find the interventions for improving safety climate in the chemical plants. The questionnaires were developed from literature review, especially made by HSE(Health and Safety Executive) in the UK and distributed to managers and workers. The frequency analysis was applied for identifying the level of safety climate. The affecting variables(plant size, accident occurrence, accident experience, injury experience and severity, and length of employment) are tested through analysis of variance(ANOVA). The results of frequency analysis showed that both managers and workers recorded generally high level of safety climate, and the major underlying problems are inadequate H&S procedures/rules, pressure for production, and rule breaking. According to the outcomes of ANOVA, the variable 'length of employment' is the only variable which makes the level of safety climate different. From the survey of safety climate practice, this study finds the level of safety climate and three major underlying problems in safety climate factors of the responded plants, and presents two interventions for improving safety performance. Despite of these outcomes, the applied factors are remained questionable for reflecting as the best ones for identifying safety climate in the chemical industry. In addition, the bias caused by self-report exist in the reliability of the response, and the equivalent size of respondents.
While the natural gas supply industry has continuously been growing, its potential hazard has also risen since the natural gas facilities essentially require installations that carry highly flammable and pressurized gas close to the populated areas, posing a serious consequence of significant property damage as well as human casualties in the event of accident. Therefore Quantitative Risk Assessment (QAR) has been recognized as a appropriate method to reduce the risk as far as possible, considering the reality of unachievable zero-risk. However, it is hard to perform effective QRA on hundreds of gas facilities because of insufficient number of expert and long-term analysis. In this paper, we suggest a conceptual QRA system framework to support more efficient risk analysis in gas supply facilities. In this system, the experts make questionnaires and internal calculation formula needed in accident frequency/consequence analysis of the facility through pre-analysis on the point of analysis, called incident point, and general users locate the point on the map and input the value required by the questionnaire to obtain the risk. Ultimately, this is suggested based on the idea that the specialization is available in QRA analysis process and the validity of the system is verified through actual system construction and application.
Rumble strip was introduced in highway pavement for driver's safety in Korea on 2000. With proving its effectiveness, total length of highway with rumble strip has increased. This research team suggested a modified form of the existing rumble strip, which was placed on new concrete pavement during construction. The modified construction equipment was applied on test section at PyungTeak-Eumsung highway with some performance experiments. Through the equation of stopping sight distance proposed AASHTO, the modified form is safer than the existing one due to 0.65m of extra width. The indoor noise test showed that the modified form $3.5{\sim}9dB(A)$ noisier than the main pavement, which wasn't applied by rumble method. Therefore, it was one of alternatives to prevent sleeping. The modified form made the frequency effected on man's eye. There were no differences of the noise and vibration between modified form and existing one. However, that driver who participated on operating test on that section felt that the former was safer than the latter. It can be concluded that modified form can be applied to the new concrete pavement, that will decrease traffic accident.

In 2017, the ratio of the number of victims and deaths in the construction industry was the highest with 25.2% and 29.6%, respectively. Especially, as safety accidents at construction sites continue to increase, the economic loss is greatly increased too. Therefore, in order to prevent safety accidents in the construction work, the safety risk assessment index by type of construction was developed, and the main results of this study are as follows. First, 17 factors related to safety accidents at construction sites were derived through survey and interview survey, and this study suggested 9 items(process, type of construction, progress rate, contract amount, number of floors, safety education, working days and weather) throughout the expert advisory meeting. Second, the risk assessment index for safety accidents was developed based on the ratio and intensity of safety accidents. Third, to verify the risk assessment model, the construction safety risk assessment index by type of construction was derived by surveying and analyzing the statistics of the construction accident. In addition, the risk strength was calculated by dividing human damage caused by construction safety accidents into those killed and injured. The risk assessment index based on the frequency and intensity of safety accidents by type of construction is expected to be utilized as basic data when assessing the risk of similar projects in the future.
