A Study on Satisfaction with Care Service depending on Activities of Daily Living and Type of Caregivers among Inpatients with Chronic Disease (만성질환 입원 환자의 일상생활 수행능력과 수발자 유형에 따른 수발만족도)

  • Purpose: The purpose of this study was to examine the satisfaction level of care services depending on the activities of daily living and types of care services for inpatients with chronic disease. Methods: The participants consisted of 156 chronically ill patients. A structured questionnaire and personal interviews were used for collecting the data. The data were analyzed using SPSS (ver. 12.0) program for frequency, t-test, ANOVA, multiple regression analysis. Results: The chronically ill patients taken care of by family members were more satisfied with care services than those taken care of by paid caregivers. Also, the more able the patients were for their daily living activities, they were more satisfied with care services. In addition, type of care services was the most important factor in terms of affecting patients' satisfaction with care services most. Conclusion: The result of this study suggests that nurses need to pay more attention to those with low activities of daily living and taken care of by paid caregivers, who have the probability of low satisfaction of care services.

A Study on the Acupuncture Methods of Joseon Dynasty Using Five Viscera Diagnosis (오장변증(五臟辨證)을 활용한 조선(朝鮮) 침법(鍼法) 연구(硏究))

  • Objective : The objective of this study is to verify the characteristics of acupuncture methods of Joseon Dynasty by looking into the relationship between five viscera diagnosis and acupuncture methods. Material & Method : In the process, I've reviewed the relationship between meridian/exterior and viscera/bowels, along with a thorough comparison of the academic tendency in acupuncture of Ming-China, Qing-China and Joseon. Result & Conclusion : The two fields of meridian and exterior, and viscera and bowels had been theoretically merged, and based upon that, foundation methods applying the five viscera diagnosis were designed. Joseon acupuncture exceeded the existing concept of viscera which simply related itself to the exterior meridian and exterior by integrating the concepts from the visceral manifestation theory. With this, large proportions of medicine related to the visceral manifestation theory were invited into acupuncture, expanding therapeutic boundaries for acupuncture treatment. A historical review on medical texts starts from the Hyangyakjipseongbang("鄕藥集成方") which familiarized the public with mainstream acupuncture knowledge up until the Song dynasty, followed by Uibangyuchwi("醫方類聚"), which sparked up interest on the acupuncture methods based on viscera and bowels. Donguibogam("東醫寶鑑") organized the medical theories up until then, building a foundation upon which viscera/bowel-based acupuncture was able to develop further. In Chimgugyeongheombang("鍼灸經驗方"), viscera/bowel-based acupuncture methods started to blossom, integrating the meridian and exterior theory with the viscera manifestation theory, which in turn provided various methods through five viscera diagnosis. In the Saamdoin acupuncture method(舍岩道人鍼法), diagnostic criteria moved on to the five viscera diagnosis, and new methods resulting from the inter-complimentary and inter-prohibiting relationships between the five phases were introduced, opening a new world of acupuncture.

  • Background: The Bronchiectasis Health Questionnaire (BHQ) is a simple and repeatable, self-reporting health status questionnaire for bronchiectasis. We have translated the original version of the BHQ into Korean using a standardized methodology. The purpose of this study was to assess the validity of the Korean version of the BHQ (K-BHQ) with Korean patients. Methods: Stable state patients with bronchiectasis from two academic hospitals were enrolled in this study. The validity was assessed by investigating the relationship between the K-BHQ scores and the Korean version of the Chronic Obstructive Pulmonary Disease Assessment Test (K-CAT) scores. We also investigated the relationship between the K-BHQ scores and other variables of the modified Medical Research Council's (mMRC) dyspnea scale, lung function, and exacerbations. Results: A total of 126 patients with bronchiectasis were enrolled. The mean age was 64.3 (standard deviation [SD], 9.7). Women comprised 53.2% of the patients. The mean forced expiratory volume in one second (FEV1) was 60% of the predicted value (SD, 18.9%); the mean K-CAT score was 17.6 (SD, 9.1). The K-BHQ scores correlated strongly with the K-CAT scores (r=-0.656, p<0.001). There was significant correlation between the K-BHQ scores and the mMRC dyspnea scale (ρ=-0.409, p<0.001), FEV1 (r=0.406, p<0.001), and number of exacerbations requiring hospitalization (ρ=-0.303, p=0.001). Conclusion: The K-BHQ is valid for assessing the health-related quality of life or health status of Korean bronchiectasis patients.

The Role of Perfusionists during the COVID-19 Pandemic and Clinical Laboratory Technologists: Comparison of Training Systems in Japan and the United States (코로나19를 통해서 바라본 체외순환사의 역할과 임상병리사: 일본과 미국의 양성체계를 비교)

  • In 2022, Korea reported a total of 224 perfusionists, employing nurses or clinical laboratory technologists (also known as medical technologists) to provide hospital self-education or reliable education. In 2021, the total number of perfusionists in Japan was 2,100, mainly supported by clinical engineering technologists. During the same period, the number of perfusionists reported in the United States was 4,212, who had received training in the master's program, post-bachelor certificate program, and bachelor's program. Most personnel in the USA were graduates of healthcare sciences or life sciences. Perfusionists must be knowledgeable in heart anatomy, physiology, pharmacology, pathology, hemodynamics, laboratory analysis, and quality assurance, as well as techniques to operate the cardiopulmonary bypass machine (heart-lung bypass machine). These are jobs similarly handled by clinical laboratory technologists. The importance of perfusionists became more prominent during two major crises: the MERS-CoV and COVID-19 pandemic. Currently, perfusionists play a significant role in the rapidly expanding field of extracorporeal membrane oxygenation (ECMO) and extracorporeal circulation during cardiac surgery. Results of the current study indicate that hospitals offering cardiac surgery and infectious disease hospitalization need to be institutionalized to secure a certain number of qualified perfusionists. In the future, we look forward to establishing a perfusion technology association under the Korean Society for Clinical Laboratory Physiology to provide academic exchanges.

A Survey on Home Care Team's Perception in Health Center about Home-based Physical Therapy among Home Care Service for Long Term Care Insurance (노인장기요양보험제도에 있어 방문물리치료서비스에 대한 보건소 방문사업팀의 인식도 조사)

  • Background: The purpose of this study was to investigate the perception of home care team's home based physical therapy in public health center Method: We surveyed 11 questionnaires for hone care team in health center from 1st to 30th, November in 2008. Results: The person who recognized the exclusion fact of home-based physical therapy in long term care insurance was 64.2% in whole 109 people. About necessity of home-based physical therapy, "absolutely necessary" as the person answer was 43.1%. Home-based physical therapy in the insurance must come to be provided with a precedence was 81.3%. About starting time of hereafter home-based physical therapy "after 1 years" the opinion which was 60.7%. Opinion about operation institution of home-based physical therapy "the pubic hospital or health center" was 52.3%. In composition form of the home-based physical therapy team "with the physical therapist and occupation therapist come together" was investigated with 37.4%. Conclusion: As long term care insurance will be developed, discussion about quality- of-service must be continuous and depth. Relates hereupon, the academic, researchers, and the persons concerned must consider the best quality of life improvement of the citizen and prepare the ground which systemic, rational, and actual on starting of home-based physical therapy in long term care insurance.

The Effects of Clinical Education Program for Nurses in Regional Public Hospital (공공보건의료인력 임상교육효과 평가: 지역거점공공병원 간호사 대상)

  • Background: The purpose of this study is to evaluate the outcomes of clinical education program for nurses in regional public hospital, utilizing the Kirkpatrick's model. Methods: Kirkpatrik's 4-level model was applied to this study. Trainees were asked to fill out questionnaires in the middle and at the end of the program. Also administrators of excellent trainees were asked to fill out the questionnaires regarding nursing management performance after 1-2 months from the end of the training course. Results: All trainees had positive reactions to the clinical education program. Not only the results of individual level (satisfaction and achievement scores, academic achievement scores, practical application rate, and educational transition factors) but also the scores of organization level (nursing management performance scores) are improved. Conclusion: By showing a correlation between the effectiveness factors we need to verify the relationship between these factors in a future study. In addition, development of quantitative and qualitative performance indicators are needed. To establish a long-term education system, it is required to applying the excellent trainee's successful experiences.

Effects of Emotional Labor and Social Support on Turnover Intention of Hotel Workers (호텔 종사자의 감정노동과 사회적 지지가 이직 의도에 미치는 영향)

  • Purpose: This study identified the influence of hotel workers' emotional labor and social support on their intentions to change jobs. Methods: Study participants were 437 workers from seven hotels in Korea who consented to the survey. Among them, insufficient responses from 21 participants were excluded, as well as 107 responses from workers with workloads of under one hour of face to face work. Thus, 309 hotel workers were included in the final analysis. A multivariate regression analysis was used to identify factors influencing the hotel workers' intention to change jobs. Results: The intention to change jobs was high when external behavior was shown, and workers who received social support from a superior exhibited low intention of changing jobs. Conclusion: The study results show that the most influential variables of turnover intention are external behavior and the support of a superior. It is therefore important to educate managers on personnel management methods for reducing external behaviors so that the rate of job changes by hotel workers can be reduced. It is also necessary to prepare and manage measures for strengthening the support system by workplace superiors.

  • Korea is successfully controlled intestinal parasitic infections owing to economic development and high health consciousness. The Division of Vectors and Parasitic Diseases (formerly the Division of Malaria and Parasitology) is in the Center for Laboratory Control of Infectious Diseases of the Korea Centers for Disease Control and Prevention. It has been the governmental agency responsible for controlling and leading scientific research on parasitic diseases. The Division of Vectors and Parasitic Diseases has conducted and funded basic research and disseminated the research results to various medical fields, ultimately promoting public health in Korea. Among the noteworthy achievements of this division are the national surveillance of healthcare-associated parasitic infections, prevention and control for parasitic infections, and the elimination of lymphatic filariasis from Korea. On a broader scale, the division's research programs and academic supports were influential in preventing and treating infectious parasitic diseases through public policies and laws. In this review, we summarize the past and present role of the Division of Vectors and Parasitic Diseases in preventing and treating infectious parasitic diseases in Korea.

Obstructive sleep apnea syndrome in children: Epidemiology, pathophysiology, diagnosis and sequelae

  • The prevalence of pediatric obstructive sleep apnea syndrome (OSAS) is approximately 3% in children. Adenotonsillar hypertrophy is the most common cause of OSAS in children, and obesity, hypotonic neuromuscular diseases, and craniofacial anomalies are other major risk factors. Snoring is the most common presenting complaint in children with OSAS, but the clinical presentation varies according to age. Agitated sleep with frequent postural changes, excessive sweating, or abnormal sleep positions such as hyperextension of neck or abnormal prone position may suggest a sleep-disordered breathing. Night terror, sleepwalking, and enuresis are frequently associated, during slow-wave sleep, with sleep-disordered breathing. Excessive daytime sleepiness becomes apparent in older children, whereas hyperactivity or inattention is usually predominant in younger children. Morning headache and poor appetite may also be present. As the cortical arousal threshold is higher in children, arousals are not easily developed and their sleep architectures are usually more conserved than those of adults. Untreated OSAS in children may result in various problems such as cognitive deficits, attention deficit/hyperactivity disorder, poor academic achievement, and emotional instability. Mild pulmonary hypertension is not uncommon. Rarely, cardiovascular complications such as cor pulmonale, heart failure, and systemic hypertension may develop in untreated cases. Failure to thrive and delayed development are serious problems in younger children with OSAS. Diagnosis of pediatric OSAS should be based on snoring, relevant history of sleep disruption, findings of any narrow or collapsible portions of upper airway, and confirmed by polysomnography. Early diagnosis of pediatric OSAS is critical to prevent complications with appropriate interventions.

Development of a Program to Facilitate Evidence-Based Practice Based on the Transtheoretical Model (범이론적 모형에 기반한 근거기반실무 활성화 프로그램의 개발)

  • Purpose: This study developed a program to facilitate evidence-based practice (EBP) in one nursing organization, and identifies the effects of the program on the nurses' EBP facilitators. Methods: The program was based on the Transtheoretical Model of stages of organizational change, a literature review, the cases of hospitals overseas, and a prior study. To identify the effects of the program, a one-group pretest-posttest study was conducted with 45 nurses who participated in the EBP implementation. Results: The program consisted of EBP educational sessions, consultations with academic nursing faculty and clinical EBP mentors, and support from the administration and relevant departments. After the EBP program, there was a statistically significant difference in belief in the value of EBP between the pretest and the posttest (t=2.31, p=.026). However, no significant differences were found between the pretest and the posttest for organizational support to develop EBP (t=0.62, p=.537), skills in locating and evaluating research reports (s=-1.00, p=.987), knowledge of research language and skills (s=-1.00, p=.986), and time to devote to EBP (s=-23.00, p=.711). Conclusion: The findings provide important data that can be used to develop and implement strategies for enhancing EBP in clinical settings in Korea.
