Evaluation of General Dentists' and Dental Specialists' Knowledge about Oral Cancer in South Khorasan-Iran 2014

  • Background: Oral cancer is one of the most prevalent cancers and one of the top ten causes of death in the whole world. Most oral cancers are diagnosed at late stages. Since dentists play a critical role in early detection of oral cancer, they should be knowledgeable and skillful in oral cancer diagnosis. The aim of this study was to survey dentist knowledge about oral cancer in Southern Khorasan Province. Materials and Methods: This descriptive, cross-sectional study was conducted with dentists who participated in an in-service educational program at the Faculty of Dentistry of Birjand University of Medical Sciences in spring 2014. A questionnaire including demographic information with 11 questions regarding oral cancer was prepared. The participants were required to be complete the questionnaires within a specific time span. The data were analyzed using SPSS 15 software by t-test and one-way ANOVA at 0.05 confidence level. Results: A total of 73 dentists out of 80 answered the questionnaires - 36 (49.3%) were females and 37 (50.7%) were males. Total mean score of knowledge was $7.91{\pm}1$ of 11. Mean scores of knowledge of male and female participants were $7.70{\pm}1.83$ and $8.13{\pm}1.94$ respectively. Mean knowledge score of general dentists was $7.41{\pm}1.79$ and of dental specialists was $9.44{\pm}1.0$ In spite of higher knowledge score of women compared to men and general dentists compared to dental specialists, these differences were not statistically significant (p=0.09). Tukey testing showed a significant difference between groups with 1-4 years of experience (8.74) and over twenty years of experience (6.50) ( p=0.001). Conclusions: Considering the good knowledge level of young dentists and the specialists and the importance of early diagnosis of oral cancer, it seems necessary to pay more attention to academic education for dentistry students, as well as holding retraining courses for experienced dentists, so that their knowledge not be reduced over time.

Taichi for Arthritis : systematic review - focused on Korean studies (국내에 보고된 관절염 태극권 연구에 대한 체계적 고찰)

  • Objectives : Taichi for arthritis was developed in 1997 by Paul Lam, M.D. of family medicine in Australia. It is an exercise treatment program and a good approaching method of the oriental medicine for arthritis. However, clinical and academical researches were rare. The purpose of this study is to investigate the effectiveness of the Taichi for arthritis. Methods : We used the medical literature search system, MEDLIS and the database of academic journal, DBpia to search medical studies published in korea. Keywords were "Taichi", "Tai Chi Chuan" and "Arthritis". We classified literatures with two criteria, year of publishment and study design. Then we classified as placebo design and summarized whole material. We determined its effectiveness by positive-negative results of Taichi for arthritis. Results : Taichi for arthritis could release pain of the rheumatoid or degenerative arthritis. Also it is thought to increase muscular power and enhance physical flexibility, balance. Conclusions : There were limits to ensure effectiveness because of small number of studies comparing Taichi with other exercise program for arthritis. However the Taichi for arthritis could have positive effect to treat the rheumatoid and degenerative arthritis. We hope further studies to be followed in oriental medical field.

A Survey on the Job Stress of Home Care Nurses (가정전문간호사의 업무 스트레스)

  • Purpose: The purpose of this study was to investigate the job stress in home care nurses. Method: The subjects were 139 home care nurses of hospital in an urban cities. The data were collected from 26 Man to 27 June by self report adherence questionnaire. Job stress was measured using CHN-PWRSQ(Community Health Nurses Perceptions of Work-Related Stressors Questionnaire). Result: The average score of job stress was 60.8. There were significantly high levels of job stress in those who have less than 10 years office period, those who have less than 2000000 won monthly salary than in their counterparts. With regard to the level of job stress by item, 'driving long distance and traffic jam', 'risk of traffic accident', 'bad weather condition', 'uncooperative family members', and 'uncooperative client/patient' were higher than other items. The age and office period were negatively correlated with job stress. Conclusion: The average score of job stress was 60.8. The Effect of Emotional Intelligence on Organizational Performance in Clinical Nurses - A Preliminary Study for an Education Program of Organizational Performance - (임상간호사의 감성지능이 조직성과에 미치는 영향 - 조직성과 증진 교육프로그램을 위한 기초연구 -)

  • Purpose: The purpose of this study was to discover the effect of emotional intelligence on organizational performance in clinical nurses regarding anger expression, job satisfaction, job commitment, and turnover intention. Methods: Data were collected by self-administered questionnaires from 162 clinical nurses in three university hospitals in 2008, and analyzed by the SPSS 14.0 Window program, using descriptive statistics, pearson correlation, and stepwise multiple regression. Results: Emotional intelligence showed a statistically significant association with each of anger expression-in, anger expression-control, job satisfaction, job commitment, turnover intention, and organizational performance. The multiple regression analysis also showed that organizational performance was statistically significant with job satisfaction, job commitment, and emotional intelligence respectively. 38.0% of the variation of organizational performance was explained by these variables. Conclusion: Emotional intelligence significantly affected organizational performance together with job satisfaction and job commitment. From this result, it could be concluded that nursing interventions for emotional intelligence have a positive effect on improving organizational performance. Further studies are needed to develop nursing intervention programs for emotional intelligence and test its effects on organizational performance.

Primary Caregivers' Self-Efficacy and Stress Coping Strategy According to Home Care Nurses' Communication Styles (가정전문간호사의 의사소통 유형에 따른 주돌봄자의 자기효능감과 스트레스 대처방식)

  • Purpose: This study aimed to investigate the difference between primary caregivers' self-efficacy and coping strategy according to the communication styles of home care nurses. Methods: Data were collected from 123 primary caregivers of patients who were registered at a home care nursing center in D city and who had been receiving home care for more than 3 months from January 1 to February 27, 2018. The questionnaire included items on communication style, self-efficacy, and stress coping strategy. The data were analyzed using descriptive statistics, t-test, and ANOVA. Results: Regarding primary caregivers' self-efficacy in terms of communication style, the caregivers showed higher efficacy in providing informative and friendly communication (F=14.07, p=.001). Regarding home care nurses' communication style and the stress coping strategy of the primary caregivers, the informative-friendly communication style was adopted the most for the problem-solving coping strategy (F=7.17, p=.001). Regarding the social support-seeking coping, home care nurses' friendly communication style was the most adopted (F=4.40, p=.014). Conclusion: This study suggests that home care nurses will plan to provide informative and friendly communication-oriented nursing care, and to improve self-efficacy and positively influence the coping method by using the communication styles appropriate to the state of the primary caregiver.

  • The research topics of the appropriate technology include environment, medical, education, and energy etc. Therefore, having a review on appropriate technology requires huge efforts of specialists from many difference research areas. Social network analysis has been applied to understand the relationships among the components in the network and to provide insights by visualizing the huge network having many components. In this study, the co-authors and their research topics at the international conference on appropriate technology (ICAT) held by Academic Society for Appropriate Technology (ASAT) from 2017 to 2019 were analyzed using the statistical and visualization tools. The networks between co-authors were analyzed using the pretreated data which were collected from ASAT. The annual trend or characteristics of the general information on ICAT were also analyzed.

A Study on a PDA-Based Ubiquitous Healthcare System for the Management of Chronic Diseases (만성질환 관리를 위한 PDA폰 기반 이동형 방문건강관리 시스템 개발)

  • Purpose: The rapid change to an aging society generates an increase in the incidence of chronic diseases. Many chronic patients have been facing their illnesses without enough preparation. In order to solve these problems, we designed and tested a public healthcare service based on ubiquitous technology. Method: Telemedicine has emerged as new medical care system of chronic disease. However, public potential of its technology is difficult to know under current traditional health care system. In this work, we developed a Personal Digital Access (PDA) phone based healthcare system by trained visiting nurses for elderly patients. A field test was performed by SeongBuk Public Health Center in Seoul, Republic of Korea (ROK). Result: Surveys were generated to assess the effects of this system compared to conventional public health system. Findings of trials demonstrate that healthcare coordination enhanced by PDA phone technology is satisfactory to the patients and nurses compared to previous one because prompt responses arouse their recognition of health. Conclusion: Ubiquitous healthcare system applied to public health service proved to be efficient and time-saving to monitor and control the chronic illnesses of large population.

Influencing Factors of Behavior of Reducing Exposure to Endocrine Disrupting Chemicals in Pregnant Women (임부의 내분비계 장애물질 노출저감화행동의 영향요인)

  • Purpose: This study attempted to determine the factors related to behaviors of reducing exposure to endocrine disrupting chemicals (BRE to EDCs) in pregnancy, based on the PRECEDE model. Methods: A cross-sectional survey was conducted with participants (N=239) who met the medical records and eligibility criteria from the Women's Hospital and Public Health Center. Data were collected using a specially-designed questionnaire based on the PRECEDE model and included BRE to EDCs predisposing factors, reinforcing factors, and enabling factors. The data were analyzed using the chi-square test, independent t-test, one way ANOVA, Pearson's correlation coefficients and a hierarchical multiple regression analysis. Results: In Model I it was found that participation in prenatal education influenced the BRE to EDCs significantly; and a regression showed that the determinant variables accounted for 3.4%. In Model II, the predisposing factors of perceived barriers of BRE to EDCs and environmental self-efficacy were added. It was shown that they significantly influenced BRE to EDCs in the order named, and a regression revealed that increases in the determinant variables accounted for 22.5%. In Model III, to which enabling factors were added, the information acquisition experience of BRE to EDCs interacted significantly with BRE to EDCs and a regression showed an increase in the determinant variables accounting for 25.3%. Conclusion: The results of this study, the content of endocrine disruptors must be included in the prenatal care education program for pregnant women, and it should be composed of contents that can reduce the perceived obstacle to BRE to EDCs, enhance the environmental self-efficacy, and provide the information in regard to reducing exposure to EDCs.

Development of a Linking Model between Community Resourses and Industrial Injury Insurance Organs for the Activation of Medical Rehabilitation Services for Industrial Injury Patients (산재환자의 의료재활서비스 활성화를 위한 산재보험시설과 민간시설간의 연계모형 개발)

  • Objectives: The purpose of this study is to develop a linking model between industrial injury insurance organs and local organizations providing social welfare health services, for the activation of medical rehabilitation services for industrial injury patients. Method: Research design for this study was a multi-step research through literature review, field research, and group interviews with persons in charge, to compare local society-oriented medical rehabilitation programs. The term of researches Sep.1 ~ Nov. 30, 2004. Results: 1. Home nurses from Workers Accident Medical Corporation have been taken to be adequate to case managers, who link industrial accident insurance institutions to local society services for the activation of medical rehabilitation services for industrial injury patients. They have been chosen for case managers because they have richer understanding and experiences of objects of industrial accidents than any other specialists, and because they have proved to be able to provide direct home services as a specialist. We have established the center for case management affairs within the workers accident general hospital, organized the committee for case management with doctors in charge, doctors in rehabilitation, rehabilitation consultants, social welfare workers, physical therapists, and nurses, determined objects of case managements from those of long-term recuperation, and constructed a course of case management containing from case management plans to evaluation. 2. We have made files of community resources, and organized the council of industrial accident administration to have it in charge of the adjustment and linking of services in case management affairs. 3. Because there are inequality of community resources between areas, differences in experiences in and bases for linking, and disparity of core organizations with active linking in the system of linking between public and civil sectors, we have taken a system of linking between parallel organs to be the core. In our linking model, workers accident general hospital, hospitals designated for industrial injury, and rehabilitation hospitals are linked in parallel, inadequate long-term recuperation managers are trusted to an workers accident medical corporations through examination by the examination committee in Korea Labor Welfare Corporation, and are dealt with by the committee for case management. Of the hospitals designated for industrial accidents, those running a home caring center provide home caring services for the handicapped at home from industrial injury. 4. Workers Accident Medical Corporation take part in medical rehabilitation, and Korea Labor Welfare Corporation in vocational/social rehabilitation. Furthermore, in the model, the latter should construct a system for job opportunities through Internet portals and provide cyber vocational consultation and introduction. Conclusion: Improvement of systems is needed to apply the linking model to practical affairs. Because this model is centered for practical affairs, it should be put under the analysis of effects, and evaluation of its adequacy to practical application, and its effects and efficiency through experimental running in the 8 workers accident general hospital in Korea.

Awareness of Doctors' Shared Decision-Making in Life-Sustaining Care Decisions

  • Purpose: At the end of life, communication is a key factor for good care. However, in clinical practice, it is difficult to adequately discuss end-of-life care. In order to understand and analyze how decision-making related to life-sustaining treatment (LST) is performed, the shared decision-making (SDM) behaviors of physicians were investigated. Methods: A questionnaire was designed after reviewing the literature on attitudes toward SDM or decision-making related to LST. A final item was added after consulting experts. The survey was completed by internal medicine residents and hematologists/medical oncologists who treat terminal cancer patients. Results: In total, 202 respondents completed the questionnaire, and 88.6% said that the decision to continue or end LST is usually a result of SDM since they believed that sufficient explanation is provided to patients and caregivers, patients and caregivers make their own decisions according to their values, and there is sufficient time for patients and caregivers to make a decision. Expected satisfaction with the decision-making process was the highest for caregivers (57.4%), followed by physicians (49.5%) and patients (41.1%). In total, 38.1% of respondents said that SDM was adequately practiced when making decisions related to LST. The most common reason for inadequate SDM was time pressure (89.6%). Conclusion: Although most physicians answered that they practiced SDM when making decisions regarding LST, satisfactory SDM is rarely practiced in the clinical field. A model for the proper implementation of SDM is needed, and additional studies must be conducted to develop an SDM model in collaboration with other academic organizations.
