The Usefulness of the Ivy Sign on Fluid-Attenuated Intensity Recovery Images in Improved Brain Hemodynamic Changes after Superficial Temporal Artery-Middle Cerebral Artery Anastomosis in Adult Patients with Moyamoya Disease

  • Objective : MR perfusion and single photon emission computerized tomography (SPECT) are well known imaging studies to evaluate hemodynamic change between prior to and following superficial temporal artery (STA)-middle cerebral artery (MCA) anastomosis in moyamoya disease. But their side effects and invasiveness make discomfort to patients. We evaluated the ivy sign on MR fluid attenuated inversion recovery (FLAIR) images in adult patients with moyamoya disease and compared it with result of SPECT and MR perfusion images. Methods : We enrolled twelve patients (thirteen cases) who were diagnosed with moyamoya disease and underwent STA-MCA anastomosis at our medical institution during a period ranging from September of 2010 to December of 2012. The presence of the ivy sign on MR FLAIR images was classified as Negative (0), Minimal (1), and Positive (2). Regions were classified into four territories: the anterior cerebral artery (ACA), the anterior MCA, the posterior MCA and the posterior cerebral artery. Results : Ivy signs on preoperative and postoperative MR FLAIR were improved (8 and 4 in the ACA regions, 13 and 4 in the anterior MCA regions and 19 and 9 in the posterior MCA regions). Like this result, the cerebrovascular reserve (CVR) on SPECT was significantly increased in the sum of CVR in same regions after STA-MCA anastomosis. Conclusion : After STA-MCA anastomosis, ivy signs were decreased in the cerebral hemisphere. As compared with conventional diagnostic modalities such as SPECT and MR perfusion images, the ivy sign on MR FLAIR is considered as a useful indicator in detecting brain hemodynamic changes between preoperatively and postoperatively in adult moyamoya patients.

간호학생의 약물용량계산능력과 정확도에 관한 융합적 연구 (Convergence Study on Ability and Accuracy in Drug Dose Calculations of Nursing Students)

  • 본 연구는 간호학생의 약물용량계산에서의 취약점을 찾기 위해 학생들의 자기기입식 일반적 사항과 약물계산수업 전 자신감(이하, 자신감), 수업수행도(이하, 수행도) 및 약물용량계산 시험지를 SPSS 19.0 으로 분석하였다. 본 연구결과 약물계산능력을 나타내는 서술식 두 문제의 평균 정답률은 72%였고, 단순계산능력은 성별 간 차이가 없었으나 응용계산능력과 계산정확도는 남학생이 유의하게 높았고, 고등학교계열별 계산능력에는 차이가 없었다. 자신감과 수행도 및 수행도와 계산정확도 간 양의 상관관계가 있었고, 자신감의 수행도에 대한, 수행도의 계산정확도에 대한 설명력은 각각 4.2%와 3.2%였다. 1개 대학 간호학생 대상의 본 연구결과를 일반화할 수는 없으나, 반복적이고 지속적인 수학학습으로 수행도를 높이고 약물계산에 대한 자신감을 향상시킴으로써 계산정확도를 높여 투약오류예방을 위한 중재프로그램 개발 및 적용의 필요성을 제언한다.

전대뇌동맥 경색으로 인한 뇌량 분리 증후군 환자의 치험 1례 (Case Study of a Patient with Callosal Disconnection Syndrome Secondary to an ACA Infarction)

  • A callosal disconnection syndrome results in the functional independence of each hemisphere caused by ACA infarction, head injury or callosal operation for epilepsy. Clinical features are characterized by left unilateral tactile anomia, left unilateral agraphia, left unilateral apraxia, difficulty in copying drawings, dyscalculia and alien hand syndrome. Taeumchowe-tang(太陰調胃湯) is used to treat Taeumin(太陰人) Wewansuhanpyohanbyung(胃脘受寒表寒病). A 47 year-old man, following callosal infarction, was detected on MRI with left agraphia, dyscalculia and alien hand syndrome, but no left ideomotor apraxia or left apraxia. He was treated with Taeumchowe-tang. Significant improvement of clinical symptoms was seen, so it is here reported.

Precise-Optimal Frame Length Based Collision Reduction Schemes for Frame Slotted Aloha RFID Systems

  • An RFID systems employ efficient Anti-Collision Algorithms (ACAs) to enhance the performance in various applications. The EPC-Global G2 RFID system utilizes Frame Slotted Aloha (FSA) as its ACA. One of the common approaches used to maximize the system performance (tag identification efficiency) of FSA-based RFID systems involves finding the optimal value of the frame length relative to the contending population size of the RFID tags. Several analytical models for finding the optimal frame length have been developed; however, they are not perfectly optimized because they lack precise characterization for the timing details of the underlying ACA. In this paper, we investigate this promising direction by precisely characterizing the timing details of the EPC-Global G2 protocol and use it to derive a precise-optimal frame length model. The main objective of the model is to determine the optimal frame length value for the estimated number of tags that maximizes the performance of an RFID system. However, because precise estimation of the contending tags is difficult, we utilize a parametric-heuristic approach to maximize the system performance and propose two simple schemes based on the obtained optimal frame length-namely, Improved Dynamic-Frame Slotted Aloha (ID-FSA) and Exponential Random Partitioning-Frame Slotted Aloha (ERP-FSA). The ID-FSA scheme is based on the tag set estimation and frame size update mechanisms, whereas the ERP-FSA scheme adjusts the contending tag population in such a way that the applied frame size becomes optimal. The results of simulations conducted indicate that the ID-FSA scheme performs better than several well-known schemes in various conditions, while the ERP-FSA scheme performs well when the frame size is small.

  • We have synthesized several bicyclic heteroaromatic compounds with bridgehead nitrogen from N-amine salts of heteroaromatic amines. 2-Amino and 2-unsubstituted thiazolo[3,2-b][l,2,4]triazole derivatives 2a-b were prepared by the cyclization reaction from N-amine salts of aminothiazole-5-yl(N-methoxyimino)acetate with cyanogen bromide and formamidine acetic acid salt, respectively. 2-Methylthiazolo[3,2-b][1,2,4]triazole 2c was obtained from N-acetylated N-amine salt of aminothiazole-5-yl(N-methoxyimino)acetate by the cyclization reaction in the presence of polyphosphoric acid (PPA). 2-Substituted and 2-unsubstituted thiazole[3,2-b][1,2,4]triazole derivatives 2a-c were coupled with 7-aminocephalosporanic acid (7-ACA). Coupled cephalosporin derivatives 1a-c did not have good antibacterial activities in vitro.

  • The fast separation and determination of cefatrizine${\cdot}$propylene glycol and inmpurities - TACA: 7-amino-3-(1,2,3-triazol-4-yl)thiomethyl-3-cephem-4-carboxylic acid and 7-ACA; 7-amino cephalosporanic acid - was performed by the high poerformance liquid chromatography using octadecyl siland (ODS) column. Methanol and ammonium phosphate buffer [$0.03M(NH_4)_2\;HPO_4$, (pH 7.5)] was used analyze, as eluent. The experimental value of the contents of cefatrizine${\cdot}$propylene glycol and impurities agree with the theoretical value of those.

지주막하 출혈에 따른 Vasospasm에 대한 Transcranial Doppler의 임상적 적용 (Transcranial Doppler Detection of Vasospasm Following Subarachnoid Hemorrhage)

  • Vasoconstriction of intracerebral arteries is the leading cause of delayed cerebral infarction and mortality following aneurysmal subarachnoid hemorrhage. Transcranial Doppler studies show and increase in the flow velocities of basal cerebral arteries, which usually start around day 4 following a subarachnoid hemorrhage, and peaking by days 7 to 14. Angiographic studies confirm the presence of at least some degree of MCA vasospasm when the flow velocities are higher than 100 cm/sec. Mean velocities in the 120 to 200 cm/sec range correspond to 25 to 50% luminal narrowing. MCA and ACA vsospasm is detected with around 90% specificity. Sensitivity is 80% and 50% respectively. A 200cm/sec threshold and rapid flow velocity increase exceeding 50 cm/sec on consecutive days, has been associated with subsequent infarction. Transcranial Doppler is also used to monitor the effects of endovascular treatment of vasospasm. Flow velocities decrease following successful angioplasty or papaverine infusion. Overall, transcranial Doppler studies are considered to have acceptable accuracy for the evaluation of vasospasm in aneurysmal subarachnoid hemorrhage, with limitations that have to be taken into consideration in the clinical setting.

  • $7{\beta}$-[4-Phenyl-5-(3.4.5-trimethoxyphenyl)-4H-1.2.4-triazol-3-yl]thioacetoamidocepharosporanic acid was synthesized by condensation of 7-Aminocephalosporanic acid (7-ACA) with [4-Phenyl-5-(3.4.5-trimethoxyphenyl)-4H-1.2.4-triazol-3-yl] thioacetylchloride. This compound was tested for antimicrobial activity in vitro against nine species of microorganisms. It showed remakable antibacterial activity against Bacillus subtilis, Micrococcus luteus, and Staphylococcus aureus.

  • Nucleolar and coiled-body phosphoprotein 1 (NOLC1) is a phosphoprotein that transiently associates with the mature nucleolar H/ACA and C/D box small nucleolar ribonucleoproteins (snoRNPs). Several lines of evidence indicate that NOLC1 plays an important role in the synthesis of rRNA and the biosynthesis of ribosomes. In the present study, we examined the transcriptional regulation mechanisms that govern the expression of NOLC1. We first performed functional dissection of the NOLC1 promoter. We demonstrated that transcription factors NF-${\kappa}B$ and CREB could bind to the minimal NOLC1 promoter. This was demonstrated by electrophoretic mobility shift assays and chromatin immunoprecipitation. Mutagenesis and overexpression assays revealed that NF-${\kappa}B$ and CREB positively regulated the NOLC1 promoter. These findings may provide new insight into the mechanisms that regulate NOLC1 expression.

한국어 마찰음과 파찰음의 음향학적 및 공기역학적 특성에 관한 연구 (An Acoustic and Aerodynamic Study of Korean Fricatives and Affricates)

  • 21 normal Korean native speakers participated as subjects to investigate the acoustic and aerodynamic study of Korean fricatives and affricates and to make good use of the results for the patients with articulation problems. Their productions of [sa], [s'a], [ca], [$c^{h}a$], [c'a], [asa], [as'a], [aca], [$ac^{h}a$], and [ac'a] were analyzed with CSL and AP II instruments. The results are as followings: (1) Fricatives showed higher frequency in minimum and maximum frequency and longer duration than affricates. (2) Fricatives showed higher peak flow rate and longer rise time than affricates. (3) When we compared the different phonemes with each other, their differences were usually statistically significant, but when we compared CV and VCV syllables, they did not show significant difference, even VCV's showed higher and longer values than CV syllables. (4) Normaly, lax fricatives and affricates showed lower frequency and higher peak flow rate, shorter frication duration, and longer rise time.
