STABILITY AFTER SURGICAL CORRECTION OF MANDIBULAR PROGNATHISM USING BILATERAL SAGGITAL SPLIT RAMUS OSTEOTOMY AND FIXATION WITH POLY-L/DL-LACTIDE COPOLYMER SCREWS ($BIOSORB^{TM}FX$) (하악지 시상 골절단술 이후 흡수성 나사를 사용하여 고정한 환자에서 술 후 안정성에 대한 연구)

  • This study was designed to assess skeletal stabilily after surgical correction of mandibular prognathism by bilateral saggital split ramus osteotomy(BSSRO) and fixation with absorbable screws. From July 2001 to September 2003, 30 patients with Class III malocclusion were treated with BSSRO and mandibular setback. They underwent fixation with Biosorb$^{TM}$ FX screws. Cephalograms were obtained preoperatibely, 2 or 3 days postoperatively, and about 12 months after the operation. Changes in the position of lower incisor tip, B point, and pogonion were examined on lateral cephalograms. The mean mandibular setback just after surgery was 10.6mm. 12 months after surgery, mean relapse at pogonion represented 17.9% and 15.1% at B point. Our results suggest that fixation of the bony segments with absorbable screws after BSSO may be used effectively in properly selected cases.

Histological Changes in the Normal Tissues of Rat after Local Application of the Holmium-166-Chitosan Complex attached to Biodegradable Solid Material (생분해성 고형물에 흡착시켜 실험동물에 국소 투여한 홀미움- 166-키토산 복합체의 투여량, 기간 및 부위에 따른 조직의 괴사 정도와 양상)

  • Purpose: The aim of this study was to find out a clinically appliable method to insert a biodegradable solid material containing holmium-166-chitosan complex into the surgical field, and to evaluate the histological changes in the normal tissues after ${\beta}$ -ray irradiation from holmium-166 according to the dose, period and type of tissues. Materials and Methods: 3.0 mCi, 50 ${\mu}l$ of the liquid state $^{166}$Ho-chitosan complex was attached to the absorbable gelatin sponge. The radiation activity measured by dose caliberator was 1.5 mCi. These $^{166}$Ho-chitosan complex containing absorbable gelatin sponges were inserted into the thigh muscles and over the femur bones of the Wistar rats. The cases were evaluated at 2 weeks after insertion, and 4, 6 weeks with respect to the histological changes of the soft tissues and bone, the depth of the tissue necrosis, and the changes of the $^{166}$Ho-chitosan complex containing absorbable gelatin sponges. Results: At 2 weeks, the muscles showed coagulation necrosis, degenerating myocytes, regenerating myocytes, intermuscular edema, and inflammatory cells. The necrosis depth was 3.3 mm. In the bones, there was no osteocyte in the lacuna of cortex (empty lacuna), marrow fibrosis, inflammation. The necrosis depth was 2.9 mm. At 4 weeks, in the muscle, calcification and increased fibrosis with necrosis depth by 3.3 mm were the additional findings. In the bone, marrow fibrosis with necrosis depth by 3.3 mm were detected. At 6 weeks, soft tissue shrinkage, increased fibrosis and granulation tissue formation, and nearly resolving inflammatory reaction were the findings. Conclusion: The local application of the $^{166}$Ho-chitosan complex attached to biodegradable gelatin material with surgery in the laboratory animals resulted in no mortality and morbidity, and satisfactory tissue necrosis. Holmium-166 can be applied to the treatment of the malignant tumor patients.

Congenital Peritoneopericardial Diaphragmatic Hernia in a Dog (개에서의 선천성 복막-심낭 횡경막허니아 1예)

  • A 5 kg, seven-month-old, female Pekingese dog was presented to Seoul National University Veterinary Medical Teaching Hospital with the history of anorexia and exercise intolerance. Muffled cardiac sound and mild abdominal pain were detected in the physical examination. In positive contrast peritoneography, contrast medium was observed in enlarged pericardial sac through the diaphragm. According to the history taking, physical examination and contrast radiographic study, the dog was diagnaosed congenital peritoneopericardial diaphragmatic hernia. Following midline celiotomy, herniated falciform ligament and greater omentum were repositioned to abdrminal cavity. The diaphragmatic defect was closed with absorbable suture. Clinical signs related to peritoneopericardial hernia disappeared immediately after surgical treatment. There had been no evidence of recurrence of the peritoneopericardial hernia for 1 year.

  • Equinus contracture of the gastrocnemius-soleus complex is associated with many foot and ankle diseases. We introduce a new simple technique that can be performed using a minimal incision with simple devices. Briefly, 0.5 cm sized medial and lateral longitudinal skin incisions are made at the level of the distal one third of the calf. The musculotendinous junction of the gastrocnemius is gently transected using an Arachnoid blade. The skin is, then, closed with non-absorbable sutures.

Traumatic Tracheal Rupture by Blunt Chest Injury -Report of a Case- (비관통성 흉부 손상에 의한 기도 파열 -1례 보고-)

  • Tracheal rupture by a blunt trauma is an uncommon injury, and its clinical presentations are variable. It is a kind of the modern hazard. Herewith, we report a successful management of the tracheal rupture. A 22 year-old female was transferred from other hospital 4 hours after a car crash. Physical examination, simple chest X-ray, Chest CT and fiberoptic bronchoscopy revealed rupture of the membranous portion of the trachea about 5cm in length extending to the right main bronchus. Ruptured membraous portion of the trachea was sutured directly with absorbable suture. Her postoperative course was uneventful, and follow-up fiberoptic bronchoscopy revealed intact membranous portion of the trachea.

  • Tracheobronchial rupture following tracheal intubation is a rare complication. We experienced a case of tracheal rupture following double-lumen endotracheal tube intubation. A 76 year old female was admitted due to coughing and chest discomfort. The operation was performed with the diagnosis of congenital broncho esophageal fistula. During the operation, accidently the main trachea was ruptured longitudinally. There was no history of surgical trauma. The ruptured trachea was repaired with prolene and monofilament absorbable sutures. The cause of tracheal rupture was suspected overinflation of the cuff. The patient was discharged from the hospital without any significant complications.

Two Case Report of Treatment by Closed Prescrotal Urethrotomy in Two Dogs with Urethral Calculi (폐쇄식 고환앞 요도절개술을 통한 요도결석견 치유 2례)

  • Two dogs were presented urinary disorder in the associated with urolithiasis. Clinical signs were increased frequency of urination with mild straining pollakiuria, general depression and anorexia. On physical examinations, the pain was revealed at the palpation of the urethral area. Urinalysis showed high specific gravity, high pH, and triple phosphates. Radiography showed an increased radiopacity, and ultrasonography showed hyperecho in the just behind of os penis. Urolithiasis was diagnosed on the basis of clinical signs, radiography, and ultrasonography. In the prescrotal urethrotomy, urethra on the midline was incised and uroliths were eliminated. After elimination of uroliths, incision area was closed with 4-0 synthetic absorbable suture. In postoperative, there was good prognosis without hemorrhage, inflammation, and urethral stricture.

Iatrogenic Tracheal Posterior Wall Perforation Repaired with Bronchoscope-Guided Knotless Sutures Through Tracheostomy

  • A 68-year-old man presented with a posterior tracheal wall injury caused by percutaneous dilatational tracheostomy. The wound was immediately covered with an absorbable polyglycolic acid sheet. Ten days after the injury, the perforation was closed with knotless sutures using a Castroviejo needle-holder through the tracheostomy. The successful repair in this case indicates the feasibility of the knotless suture technique for perforations. The technique is described in detail in this report. The patient was weaned from the mechanical ventilator on postoperative day 25. In cases of posterior tracheal posterior wall perforation, every effort should be made to repair the perforation through an existing opening.
