Radiographic and Ultrasonographic Images in Canine Lymphoma (개에서 림프종의 방사선학적 및 초음파학적 진단)

  • In 2001, eight small-sized dogs and three medium-sized dogs with or without clinical signs such as vomiting, dyspnea and enlargement of superficial lymph nodes were diagnosed as lymphoma by radiography, ultrasonography and cytology. Among them, six were female mainly over 7 years old and others were male around 4 years old. There were six cases of multicentric form lymphoma, three cases of mediastinal form, an alimentary and an extranodal form, respectively. Radiographic findings were enlarged cranial mediastinal lymph node, sternal lymph node, and tracheobronchial lymph nodes on thorax and hepatomegaly, splenomegaly, and enlarged sublumbar lymph nodes on abdomen. Ultrasonographic findings revealed enlarged abdominal lymph nodes and multifocal hypoechoic lesions on the liver. Especially, severe wall thickness of small bowel was observed in a dog with alimentary lymphoma. Three dogs were dead during chemotherapy, and three dogs were dead without any treatment. Three dogs did not return and two treated dog showed normal status. Radiography and ultrasonography, although not giving an final diagnosis for lymphoma, are useful for assessment and diagnosis of lymphoma.

Comparison of Direct Digital Radiography and Conventional Film Screen Radiography for Detection of Peritoneal Fluid in Dogs (개에서 복수의 평가에 있어서 필름-증감지 방사선 사진과 디지털 방사선 사진의 비교)

  • This study was performed to evaluate the sensitivity of conventional film-screen radiography (CFSR) and direct digital radiography (DDR) for detection of various amounts of free peritoneal fluid. Ten adult male healthy beagles were used in this study. Radiographic examinations were performed in the right lateral and ventrodorsal positions. Fluid was injected in increments of 2.0 ml/kg of body weight up to 20.0 ml/kg of body weight. The images of CFSR and DDR were evaluated by two veterinary radiologists for evidence of abdominal fluid without knowledge of injected fluid volume. Data were evaluated by using the receiver operation curve (ROC) analysis and the area under the curve (AUC). There was no significant difference in detection of peritoneal fluid between DDR and CFSR in the ROC analysis. The accuracy of CFSR (0.805) was relatively higher than that of DDR (0.733), based on the ROC analysis and AUC. AUC of CFSR was higher in most injection doses. These results suggest that CFSR is more accurate than DDR for the detecting peritoneal fluid. Therefore, for situation in which digital radiographs are equivocal or small amount of fluid is suspected, other imaging modalities, such as ultrasonography would be helpful for determining the presence of fluids.

Colonoscopic Diagnosis and Treatment of Colonic Obstruction by the Leaves in a Dog (개에서 발생한 나뭇잎에 의한 대장폐색의 대장 내시경적 진단 및 치료)

  • A 6-years-old 30 kg male Germen Shepherd dog was presented with the history of abdominal pain, intermittent vomiting and anorexia. Abdominal survey radiography and ultrasonography revealed marked colonic distention. Positive contrast radiography revealed a colonic obstruction between the transverse colon and descending colon. Foreign bodies were identfied during colonoscopy. The descending colon was inflated by the air of endocope then foreign bodies were moved to the descending colon. Foreign bodies were removed by the enema. The dog's clinical signs resolved following foreign body removal.

  • Peritoneopericardial diaphragmatic hernia(PPDH) is uncommon and congenital disease in dogs and cats. In PPDH, the peritoneal organs such as liver, small intestine, stomach and omentum are displaced into the congenital defect between pericardial sac and diaphragm and cause the abnormal round and enlarged cardiac silhouette. Abnormal cardiac silhouette contacts with the cranial diaphragmatic border consistently and soft tissue- and/or gas- density structures are summated over the cardiac density in radiography. The contrast medium flows from peritoneal cavity into the pericardial sac and demonstrates the herniated abdominal organs and the abnormal defect in positive peritoneography. In this study, 4 dogs was diagnosed as PPDH using radiography, peritoneography and thoracic ultrasonography and showed various clinical signs according to the kind, amount and clinical state of herniated abdominal organs.

The Effect of Body Mass Index on Entrance Surface Air Kerma in Abdominal X-ray Radiography Using Automatic Exposure Control (자동노출제어를 이용한 복부 일반 X선 검사에서 체질량지수가 입사표면공기커마에 미치는 영향)

  • The purpose of this study was to determine the effect of body mass index (BMI) on entrance surface air kerma (ESAK) in abdominal X-ray radiography using automatic exposure control (AEC). This study included 321 patients who underwent abdominal X-ray using AEC, and we correlated ESAK with height, weight, BMI and compared mean ESAK according to BMI grades (Underweight, Normal, Overweight, Obese 1, Obese 2). As a result, Weight ($R^2=0.777$, p<.001) and BMI ($R^2=0.835$, p<.001) were positively associated with ESAK, but no significant association was found between height ($R^2=0.075$, p<.001) and ESAK. The mean ESAK with respect to BMI grades showed statistically significant difference and in the post-hoc analysis, the existence of 5 subgroups at the significance level of 0.05 indicated that there were differences in the ESAK in all BMI grades. Also, as the increment of ESAK between two neighboring BMI grades increases from Underweight to Obese 2, the exposure dose dramatically increased as the BMI increased. Thus, an excessive exposure dose due to increasing BMI when using AEC should be acknowledged and Efforts to reduce dose should be taken, such as: by fixing the exposure conditions.

Assessment of Entrance Surface Dose and Image Distortion in Accordance with Abdominal Obesity in the Chest Radiography (흉부 X-선 검사에서 복부비만에 따른 입사표면선량과 영상 왜곡도 평가)

  • Abdominal obesity is one of the most influential index to predict of insulin resistance syndrome/metabolic syndrome in social demographic characteristics. It is matter of fact that radiation dose are increasing with development of medical treatment and device. In this study, we estimated distortion between reference image and entrance surface dose when take a chest radiography forward chest phantom assumed abdominal obesity. When angle of chest phantom incline $5^{\circ}$ forward, thoracic transverse and longitudinal diameter increase 1.22% and 0.44% each. Also cardiac transverse diameter increase 1.01% and cardio-throracic ratio (CTR) decrease 0.27% in the same situation of incline to $5^{\circ}$ forward. Thoracic transverse diameter shows the largest increase, and CTR was decreased. But entrance surface dose to phantom increase significantly 6.12% when angle of chest phantom incline $5^{\circ}$ forward. In conclusion, we have to pay attention to accurate positioning, to prevent a distortion of image through incline, and make patients not to expose to additional radiation.

  • A 5-year-old Pekingese bitch was presented with a history of vaginal bleeding and copulation before 4-week. Ovarian remnant syndrome was diagnosed on the clinical changes, vaginal cytology and exploratory laparotomy. An abdominal mass was diagnosed on radiography, ultrasonography and Computed Tomography. The remnant of ovarian tissue and an abdominal mass were removed surgically under general anesthesia. Ultrasonographic examination of the removed ovarian tissue (water bath scanning) revealed three corpora lutea. The condition resolved following surgical excision of the remaining ovarian tissue and an abdominal purulent mass.

The Influence of the Change of Patient Radiation Exposure Dose Distribution on the Grid Condition and Detector Acquisition Dose on the Exposure Distance in the Use of Amorphous Silicon Thin Film Transistor Detector with AEC (자동노출제어장치를 이용한 비정질 실리콘 평판형 검출기에서 격자의 조건에 따른 환자선량 변화와 촬영 거리의 변화가 검출기 획득선량에 미치는 영향)

  • This study attempts to propose an appropriate method of using digital medical imaging equipments, by studying the effects of automatic exposure control(AEC), grid ratio and the change of radiography distance on the patient dose and detertor acquisition dose during the procedure of acquiring image through a digital medical imaging detector. The change of dose following the change of grid ratio's exposure and radiography distance was measured, by using an abdominal phantom organized with tissue equivalent materials in an amorphous silicon thin film transistor detecter installed with AWC. The case to use grid ratio 12 : 1, focal distance 180cm to radiography distance 110cm in AEC, the patient dose increased rather when we used grid ration 10 : 1, focal distance 110cm. When AEC was not used,the dose necessary for image acquisition decreased as the grid ratio became higher and the distance became further. but detector acquisition dose was not reduced when in applied AEC. When purchasing digiral medical imaging equipments, optional items such as AEC and grid shall be accurately selected to satisfy the use of the equipments. Radiography error made by radiation technologist and unnenessary patient dose can be reduced by selecting equipments with a radiography distance marker equipment when it did not apply AEC. These equipments can also be helpful in maintaining high imaging quality, one of the merits of digital detectors.

Diagnosis of Persistent Right Aortic Arch with an Aberrant Left Subclavian Artery using CT in a Poodle Dog

  • A 6-month-old, female poodle presented with a three-month history of persistent regurgitation immediately after eating. On physical examination, the patient was emaciated and dehydrated. Thoracic radiography showed ventral displacement of the trachea and increased radiopacity in the mediastinum, cranial to the heart base. A severely dilated esophagus was identified cranial to the heart on esophagram. Computed tomography (CT) revealed the esophagus was filled with gas, fluid and a little of contrast and dilated from caudo-cervical to cranio-thoracic part. The esophageal diameter was markedly decreased at the heart base. In addition, the trachea was displaced to the left-ventral side of the right aortic trunk and an aberrant left subclavian artery originating from the aorta was identified. There was no evidence of abdominal vascular anomaly. Based on diagnostic imaging, persistent right aortic arch (PRAA) with an aberrant left subclavian artery was diagnosed. The patient did not undergo surgery and died at 15 days after diagnosis. This report describes imaging diagnosis, including CT and radiography in a weaned dog with regurgitation due to esophageal obstruction by PRAA. When PRAA is suspected and conventional radiography or contrast study is insufficient for diagnosis, CT may be helpful for diagnosing PRAA.
