The purpose of this study was to assess the effect of applied pressure to abdomen on lumbar and abdominal muscle activation during upper limb exercise. The experimental group consisted of twenty-seven healthy male subjects (mean age=$22.40{\pm}2.19years$, mean height=$175.30{\pm}2.19cm$, mean weight= $67.67{\pm}7.44kg$, RM=$8.43{\pm}2.76kg$). In each different pressure condition (OmmHg, 30mmHg, 70mmHg, 100mmHg), upper limb exercise was performed in total of 10 trials with 10 RM dumb-bell exercise. Lumbar and abdominal muscle activity was measured using surface bipolar electrode electromyography(EMG). EMG activity was measured from upper rectus abdominis, external oblique abdominis, internal oblique abdominis, and elector spinae. The raw EMG signal was processed into the root mean square(RMS). All RMS EMG data were normalized and express as a percentage of the EMG(%EMG). Collected data were statistically analyzed by SPSS/PC Ver 10.0 using two-way analysis of variance for repeated measures($4{\pm}3$) and Bonferroni post hoc, test. Lumbar and abdominal muscle activation was significantly increased when 100 mmHg was applied(p<.05). Upper rectus abdominis activation was significantly increased compared as other muscles activation(p<.05). However, there were no interaction between pressure and muscles(p>.05). The findings of this study can be used as a fundamental data when lumbar orthosis is applied and external pressure can be used as a therapeutic tool.

Objective: To investigate the appropriate position for abdominal drawing-in maneuver (ADIM) exercise by rehabilitative ultrasound image. Design: Cross-sectional study. Methods: Twenty-eight young adults with no history of low back pain participated in the study. Three positions compared were crook lying position with hip $60^{\circ}$ flexion, standing position with the feet hip width apart and knees straight, and saddle standing positionunsupported with the knees $20^{\circ}$ flexed. Once in the appropriate position, the subjects were verbally cued to draw in their abdominal wall, with the intention of pulling their navel inward toward their lower back. The thickness of each transversus abdominis (TrA), internal oblique (IO), and external oblique (EO) muscles were measured via ultrasound and recorded at the end of inspiration. Results: When compared to the TrA thickness of rest, the TrA thickness was significantly increased in all three positions (crook lying, standing, and saddle standing) during the ADIM (p<0.05). IO thickness was significantly greater in standing and saddle standing than in crook lying (p<0.05). EO thickness was constant in all the three positions. Conclusions: The present study suggests that standing and saddle standing positions could be recommended for the ADIM to maximize recruitment of the TrA and IO activation. Specifically, the saddle standing position with knees flexed to $20^{\circ}$ was observed to increase the TrA activation more than the standing position. These findings should be considered when core stability exercises such as the ADIM are conducted.
Purpose : Kegel exercises reported that it is effective in managing stress-related or complex urinary incontinence through contraction and relaxation of the pelvic floor muscles. In many previous studies, it was confirmed that Kegel exercise is involved in respiration as well as urinary system diseases. However, there is a lack of research on the effect of pelvic setting when performing Kegel exercises. Therefore, this study was conducted to investigate the effect on maximum voluntary ventilation (MVV) and abdominal muscle thickness through Kegel exercise after lumbar-pelvic motor control using pressure biofeedback unit (PBU). Methods : The subjects of this study were 10 healthy female students in their 20s. Subjects measured MVV with a spirometer. In hooklying, external oblique, internal oblique, and transverse abdominis of the dominant hand were measured using ultrasound. The measured value was an average of three times. After one week of intervention, measurements were made in the same manner. Before Kegel exercise, pelvic setting training was performed using PBU. In hooklying, PBU was placed in the waist and set to 40 mmHg, and it was adjusted to 60 mmHg through pelvic muscle contraction. For Kegel exercise, the pelvis was first set using PBU, and then the pelvic floor muscles were contracted for 8 seconds and relaxed for 8 seconds, 10 times, 1 set, and 3 sets. Results : In MVV, a significant difference was confirmed after exercise than before exercise (p<.05). There was also a significant difference in abdominal muscle thickness before and after exercise (p<.05). Conclusion : Based on the results of this study, Kegel exercise using PBU had an effect on MVV and abdominal muscle thickness. However, since this study was conducted without a control group as a preliminary study, additional research should be conducted to supplement this.
Background: This study comparatively evaluated the effects of equipment-based pilates exercises (EPE) and lumbar stability exercises (LSE) in patients with chronic low back pain in terms of their Visual Analogue Scale(VAS), Oswestry Disability Index(ODI), and abdominal muscle thickness. Methods: A total of 30 participants were recruited and randomly assigned to either the EPE or the LSE. The VAS, ODI, and abdominal muscle thicknesses of the participants were measured before and after the intervention. Results: The EPE were more effective in terms of the duration of a sustained reduction in VAS scores. post hoc test revealed that EPE were more efficacious in terms of a sustained improvement in ODI scores. With respect to changes in abdominal muscle thickness, there was a significant difference in the thickness of internal oblique muscles(IO) and the external oblique muscles(EO) between the two groups. Conclusion: In this study, both types of exercise interventions resulted in improvements in the VAS, ODI scores, and abdominal muscle thickness in patients with chronic low back pain. However, EPE were found to be more effective than LPE in terms of longer sustained improvements in VAS and ODI scores. Also, with respect to abdominal muscle thickness, the thickness of EO and IO improved only in the EPE group.
Purpose : The aim of this study is to determine changes to the thickness of core muscles, e.g., transversus abdominis (TrA), external oblique (EO), and internal oblique (IO), after plank and Kegel exercises and to compare the effects of the two exercise methods. Method : The study divided men and women in their 20s into two groups, Kegel and plank, by randomly allocating 30 males and 30 females to the targeted groups. To achieve the purpose of this study, we examined the thickness of core muscles after the participants performed plank or Kegel exercises. Results : 1. In the Kegel group, abdominal muscle thickness increased with time, and there was a statistically significant difference in the thickness of the TrA. 2. In the plank group, abdominal muscle thickness increased with time, and there was a statistically significant difference in the thickness of the TrA. 3. After the experiment, muscle thickness of the abdominal muscles in the plank exercise group were higher, but there was no significant difference between the Kegel exercise group and the plank exercise group. Conclusion : Both plank and Kegel exercises are recommended for core muscle stabilization, and based on the results of this study, Kegel exercise is either used as a core stabilizing exercise or as a plank exercise for the weaker patients or women.
Background: The continuous co-contraction of the trunk muscles through trunk stabilization exercises is important to patients with lumbar spinal stenosis (LSS). However, intentional abdominal muscle contraction (IAMC) for trunk stabilization has been used only for specific training in the treatment room. Objects: The purpose of this study was to provide feedback to adults with LSS to enable IAMC during activities of daily living (ADLs). Methods: The participants with spinal stenosis were divided into an experimental group of 15 adults and a control group of 16 adults. Electromyographic signals were measured while the subjects kept their both hands held up at $90^{\circ}$. The measured muscles were the rectus abdominis (RA), external oblique (EO), internal oblique (IO), and erector spinae (ES). Pelvic tilt was measured using a digital pelvic inclinometer. The degree of pain was measured using the visual analogue scale (VAS) and functional capacity was measured using the Korean version of the Oswestry disability index (KODI). Results: While the experimental group showed statistically significantly higher activities in the RA, EO, and IO after the intervention compared with the control group. Pelvic tilt was significantly decreased only in the experimental group. Both the experimental and control groups exhibited statistically significant declines in the VAS and KODI (p<.01). In terms of the levels of changes, the experimental group exhibited a statistically significant larger decline only in the VAS and the pelvic tilt when compared with the control group (p<.05). Conclusions: The subjects could stabilize their trunks, and relieve their pain and dysfunctions and reduce pelvic tilt by learning abdominal muscle contraction during ADLs. The combination of therapeutic exercises and IAMC may have greater effects on patients with LSS.
Active straight-leg raise (ASLR) is a physical evaluation procedure to test lumbar spine stability. Several previous studies have reported various methods to control the activation of abdominal muscles during ASLR. We investigated the effects of three different hip positions in frontal plane on abdominal muscles to increase or decrease the difficulty level of lumbar spine stability exercise during ASLR in pain free subjects. Eleven young and healthy subjects voluntarily participated in this study (6 men, 5 women; mean age=$24.0{\pm}1.2$ years, height=$160.0{\pm}7.3cm$, weight=$55.0{\pm}10.6kg$, body mass index=$21.5{\pm}2.3kg/m^2$). The subjects had three trials on each ASLR with hip $10^{\circ}$ adduction, neutral hip, and hip $30^{\circ}$ abduction. Separate repeated-measures analysis of variance (ANOVA) and the post hoc Bonferroni tests (with ${\alpha}$=.05/3=.017) were performed for each muscle among the three different hip positions in frontal plane (ASLR with hip $10^{\circ}$ adduction, neutral hip, and hip $30^{\circ}$ abduction). The ipsilateral external oblique (EO), contralateral EO, ipsilateral internal oblique/transverse abdominis (IO/TrA), and contralateral IO/TrA were significantly greater in ASLR with hip $30^{\circ}$ abduction compared with ASLR with hip $10^{\circ}$ adduction. Also, the ipsilateral EO, contralateral EO, and ipsilateral IO/TrA were significantly greater in ASLR with hip $30^{\circ}$ abduction compared with ASLR with neutral hip. These results suggest that ASLR with hip $30^{\circ}$ abduction and neutral would be useful method to strengthen the EO and IO/TrA. And, ASLR with hip $10^{\circ}$ adduction would be effective in early stages of lumbar stabilization program due to low activation of EO and IO/TrA during maintaining of ASLR position with low load.
Purpose: This study aimed to compare changes in abdominal muscle thickness in different standing postures with a handheld load between subjects with and without chronic low back pain (CLBP). Methods: Twenty subjects with CLBP and 20 controls participated in this study. Ultrasound imaging was used to assess the changes in the thickness of the transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) muscles. Muscle thickness in three different standing postures (standing at rest, standing with loads, standing with lifting loads) was compared with the muscle thickness at rest in the supine position and was expressed as a percentage of change in the thickness of the muscle. Results: While standing with loads, the change in IO muscle thickness in the CLBP patients increased more significantly than in the pain-free controls (p < 0.05). The standing with lifting loads posture showed a significant increase in the change in thickness of the TrA compared with the standing with loads posture (p < 0.05). In addition, the standing with lifting loads posture showed a significant decrease in the change in the thickness of the EO when compared with the standing with loads posture (p < 0.05). Conclusion: The automatic activity of the IO muscle in subjects with CLBP increased more than that of the pain-free controls in the standing with loads posture. These findings suggest that IO muscle function may be altered in those with CLBP while standing with loads. Additionally, TrA the activation level was found to be associated with increased postural demand caused by an elevated center of mass.
Purpose: The purpose of this study was to examine the influence of multi-directional dynamic stabilization exercise on the thickness of the external oblique abdominis, internal oblique abdominis, and transversus abdominis. Methods: For the study subjects, 40 adults were taken and divided into two groups and stabilization exercise was performed for 6 weeks. Changes in muscle thickness during the pre-experimental period, and at 2 weeks, 4 weeks, and 6 weeks were analyzed using repeated ANOVA. Results: The thickness of the external oblique abdominis, internal oblique abdominis, and transversus abdominis in the multi-directional dynamic stabilization exercise group showed significant differences per period and an interaction was observed between period and group (p<0.01), while there were no significant differences in changes per group (p>0.05). Conclusion: The effects of multi-directional stabilization exercises on abdominal muscle thickness differed. The results presented herein can be utilized as basic data for future studies and in development of rehabilitation treatments.

The purpose of this study is to examine the effects of feedback breathing exercise on respiratory muscle activity. Thirty stroke patients were randomly and equally assigned to an experimental group and a control group. The experimental group received rehabilitation exercise treatment for 30 minutes and feedback respiratory exercise for 30 minutes and the control group received rehabilitation exercise treatment for 30 minutes and conducted motomed exercise for 30 minutes. All of them conducted exercises five times per week for four weeks. Respiratory muscles including the upper trapezius(UT), longissimus dorsi(LD), rectus abdominis(RA), external abdominal oblique(EAO) and, internal abdominal oblique(IAO) were measured using MP 150WSW prior to and after the experiment. Regarding pulmonary functions prior to and after the experiment, the experimental group showed significant differences in all sections but the control group did not show significant differences in any sections. As for in-between group differences after the experiment, there were significant differences in the UT, LD, RA, and IAO but no significant differences in the EAO. In conclusion, respiratory muscle activity was more effective for the experimental group than the control group. It is considered that feedback respiratory exercise may induce improvement in respiratory muscles in stroke patients through feedback breathing exercise.
