건강한 성인의 소화 상태 및 소화 불편에 따른 복진용 압통장치를 활용한 정량적 연구 (Quantitative Research Using Modified Digital Algometer according to Digestive Condition and Discomfort in Healthy Adults)

  • Objectives : The aim of this study was to assess the quantitative characteristics of pressure pain threshold (PPT) and pressure depth (PD) at the abdominal conception vessel (CV) acupoints according to subjective digestive status and digestive discomfort levels, accomplished by comparing a large group of healthy men and women, using the modified digital algometer. Methods : A total of 1,504 healthy adults aged 19 years or older participated in this study. A questionnaire was administered to evaluate participants' digestive status and discomfort. PPT (kgf/cm2/s) and PD (mm) measurements were obtained at CV14, CV12, and CV4 acupoints using a modified digital algometer. General characteristics were analyzed using the chi-square test, and differences in PPT and PD were assessed using two-sample t-tests and ANCOVA. Results : Significant difference in PPT was found based on digestive status at CV14 and CV12 for both sexes, and in PD at CV14 for women. Women exhibited significant difference in PPT based on digestive discomfort at CV14, CV12, and CV4, while men showed significant difference at CV14 and CV12. Significant difference in PD was observed at CV14 and CV4 among women. Even after adjusting for age and body mass index, significant difference persisted in PPT based on digestive discomfort at CV14 and CV12 for both sexes. PD exhibited significant differences at CV14, CV12, and CV4 among women. Conclusions : Using the modified digital algometer, this study showed the significant difference of pressure pain threshold at the CV14 and CV12 acupoints for subjective digestive discomfort levels in healthy men and women.

복부 뜸요법이 여대생의 월경불편감에 미치는 효과 (Effect of Abdominal Moxibustion on Menstrual Discomfort in University Students)

  • This study was aimed to identify the effect of abdominal moxibustion on premenstrual syndrome and dysmenorrhea in university students. The data were collected by using questionnaires from April 25, 2005 to June 20, 2005. The subjects were university students from in the Pusan metropolitan area who had complained of menstrual discomfort. Thirteen students were assigned to the experimental group and fourteen students to the control group. The research design of this study was a nonequivalent control group pretest-posttest design. In the research method pretest were conducted through measurement based on scores of general character, menstrual character, premenstrual syndrome and dysmenorrhea in the experimental group and the control group. Experimentation was conducted by abdominal moxibustion being applied on the kwan-won hole and Ki-hae hole for 25 minutes at a time, three times a week, form the ending of one menstrual cycle to the beginning of the next menstrual cycle were in the experimental group. After treatment was finished, post tests conducted through measurement based on scores of premenstrual syndrome and dysmenorrhea in the experimental group. The control group conducted a point of same time and same measurement with the experimental group. The instruments used in this study MDQ(Menstual Distress Questionnaire) developed by moos(1968) and the VAS(Visual Analogue Scale) developed by Cline(1992). The data were analysed by SPSS/ win 12.0 program with ${\chi}$2-test and t-test to verify identification among subjects' general characteristics and menstrual characteristics. Hypothesis verification was analysed with t-test. The results of the study are as follows: The hypothesis, 'the experimental group who received abdominal moxibustion will decrease premenstrual symptoms more than the control group', was supported (t= -2.121, p=.022). The hypothesis, 'the experimental group who received adbominal moxibustion will decrease dysmenorrhea compared to the control group', was supported (t=-4.424, p=.001). As a result of this study, abdominal moxibustion will be used as a nursing aide to improve and prevent menstrual discomfort of university students who have premenstrual syndrome and dysmenorrhea.

대장내시경 검사 전 걷기 운동이 장세척액 복용 시 불편감과 대장 정결도에 미치는 효과 (Effects of Walking on Discomfort and Colon Cleansing during Colon Lavage before Colonoscopy)

  • Purpose: The purpose of this study was to identify the effects of walking on discomfort and colon cleansing for patients having a colon lavage solution before colonoscopy. Methods: This study was a nonequivalent control group post test design. The participants were 89 patients (experimental group: 47, control group: 42) who underwent colonoscopy at G hospital in Incheon. The two groups put on step counter for an hour while taking the colon lavage solution. The experimental group was made up of patients who walked over 3,000 steps and the control group of those who walked less than 3,000 steps. Discomfort was measured using VAS and colon cleansing was measured by a specialist. Collected data were analyzed using $x^2-test$, t-test with SPSS/PC+ window version 15.0. Results: Walking while taking the colon lavage solution decreases abdominal pain before colonoscopy. Also one hour after taking the colon lavage solution, decreased nausea, abdominal pain and discomfort were found in the group which walked over 3,000 steps. Concluson: Based on the above findings, adequate walking can be used as a nursing intervention to increase comfort in patients undergoing colonoscopy.

Efficacy of ramosetron in combination with polyethylene glycol of preparing for a colonoscopy

  • Background: Because of its efficacy and safety, polyethylene glycol (PEG) is generally used to prepare for colonoscopy. However, the side effects of PEG, including nausea, vomiting, abdominal discomfort, pain, and general weakness, tend to decrease patient compliance and satisfaction. The aim of this study is to investigate the efficacy and safety of PEG with 0.1 mg ramosetron on colonoscopy patients who had difficulty taking PEG due to side effects or large volume. Methods: From January to August in 2012, 28 patients who visited Yeungnam University hospital for a colonoscopy were prospectively enrolled. All enrolled patients were previous history underwent colonoscopy using PEG only in our hospital. The efficacy and safety of ramosetron were assessed through the use of a questionnaire, and compared previous bowel preparation. Results: Compared to previous examination, the patients using the ramosetron reported less nausea, vomiting, abdominal discomfort, and abdominal pain, as well as a higher degree of compliance and satisfaction of the patient. There were no side effects reported with the use of ramosetron. However, overall bowel preparation quality was not better than the previous examination. Conclusion: In case of the use of ramosetron in combination with PEG for bowel preparation, patients experienced a higher rate of compliance and tolerance. Looking forward, ramosetron may become an option of pretreatment for bowel preparation.

심한 변비와 복부팽만으로 발견된 소아 크론병 1예 (Pediatric Crohn's Disease presenting as Severe Constipation and Abdominal Distension)

  • Crohn's disease is a chronic inflammatory bowel disease that mainly affects children and young adults. Its cause remains unknown. The incidence of pediatric Crohn's disease is increasing, so it is important for clinicians to be aware of the presentation of this disease in the pediatric population. The majority of patients complain of abdominal pain (72%), with only 25% presenting with the 'classical triad' of abdominal pain, weight loss, and diarrhea. Many children with Crohn's disease present in a 'non-classical' manner, with vague complaints such as lethargy or anorexia, which may be associated with only mild abdominal discomfort. Other symptoms include fever, nausea, vomiting, growth retardation, malnutrition, delayed puberty, psychiatric symptoms, arthropathy, and erythema nodosum. Severe constipation and abdominal distension are uncommon symptoms at diagnosis. We report a case of pediatric Crohn's disease, which was diagnosed after the patient presented with severe constipation and abdominal distension.

소음인체질병증 임상진료지침: 소음병 (Clinical Practice Guideline for Soeumin Disease of Sasang Constitutional Medicine : Lesser Yin Symptomatology)

  • Objectives This research was performed to establish the clinical practice guideline(CPG) for Lesser Yin Symptomatology of Soeumin disease. Methods Dongeui suse bowon(sinchuk edition), textbook for Sasang constitutional medicine, Clinical guidebook for Sasang constitutional medicine, and standardization reports on Sasang constitutional medicine and papers concerning symptomatology of Soeumin Disease, especially Lesser Yin Symptomatology was collected and classified. Additionally experts' conference was held to make agreement on the conflicting issues on a regular basis. Results & Conclusions There was no concerning paper on Lesser Yin Symptomatology. Experts' agreement was needed to establish the CPG. Lesser Yin pattern can be classified into 2 groups; Lesser Yin severe pattern and Lesser Yin critical pattern. There are Lesser Yin pattern accompanied abdominal pain and bowel irritability pattern and Lesser Yin pattern accompanied green tinged watery diarrhea pattern in Lesser Yin severe pattern. There are Visceral syncope pattern and Exuberant yin repelling yang pattern in Lesser Yin critical pattern. Lesser Yin symptomatology has several symptoms like abdominal pain and diarrhea, thirst, oral discomfort, chest discomfort, whole body pain, articular pain and coldness of hands and feet. Additionally there are abdominal pain and diarrhea in Lesser Yin symptomatology accompanied abdominal pain and bowel irritability pattern, there is green tinged watery diarrhea in Lesser Yin pattern accompanied green tinged watery diarrhea pattern and if this symptoms exacerbate, delirious speech and constipation can occur. There are restlessness and coldness on hands and feet in Visceral syncope pattern and severe restlessness and coldness on hands and feet and symptom which the patient cannot drink water in Exuberant yin repelling yang.

급성기 중풍 환자에 있어서 보류관장과 일반관장의 사하효과에 대한 비교 연구 (The Comparison Study on the Effect of Bowel Movement between Bo-Ryu Enema(보류관장, Bao-Liu Enema) and General Enema in Patients at the Acute Stage of Cerebrovascular Accident)

  • Objectives : In Oriental Medicine, the improvement of constipation is very important for patients at the acute stage of cerebrovascular accident. For constipation, oral medicine has usually been used but is not available to stroke patients with dysphagia and mental disorder, and may bring about side effects on the gastrointestinal tract. In such cases, general enema has been used for assistant therapy, We needed to develop a more effective enema technique on constipation of stroke patients. Methods : Sixty-three patients were researched. We treated 29 patients with Bo-Ryu enema and 34 patients with general enema. To compare the effect of the Bo-Ryu enema group with that of the general enema group, we analyzed general characteristics, bowel movement, abdominal examination and related symptoms. Results : Sixty-three patients comprised the Bo-Ryu (n=29) and general (n=34) enema groups. Between the Bo-Ryu and general enema groups, the Bo-Ryu enema was more effective than the general enema in the number of bowel movements, total and corrected amount of stool, tonus of rectus abdominis muscle and abdominal discomfort. However, there were not significant differences in the power of abdominal muscle, digestion and physical power. Among the subgroups, Bo-Ryu enema group was most effective in the number of bowel movements, total amount of stool and tonus of rectus abdominis muscle, and Bo-Ryu enema group was best in the corrected amount of stool and abdominal discomfort. Conclusions : Bo-Ryu enema should be considered as an effective and safe treatment for patients with constipation during the acute stage of cerebrovascular accident.

소양인 망음증으로 진단한 역류성식도염 치험 2례 (2 Case Reports of Gastroesophageal Reflux Disease Diagnosed as Soyangin Mangeum)

  • Objectives This study was designed to report treatment of Soyangin Mangeum with gastroesophageal reflux disease. Methods We treated two patients suffering from dyspepsia and chest discomfort. A female patient was treated with Hyeongbangjiwhang-tang. A male patient was treated with Hyungbangsabaek-san. The progress was evaluated with the visual analogue scale for dyspepsia, chest discomfort, and other symptoms. Results A female patient's symptoms such as dyspepsia, chest discomfort were almost cured. but they increased with patient's emotional state repeatedly. A male patient' symptoms such as abdominal pain, dyspepsia, nausea, heart burn was successfully disappeared. Conclusions Hyeongbangjiwhang-tang and Hyungbangsabaek-san can be considered as a treatment for patient with Soyangin Mangyin diagnosed as gastroesophageal reflux disease.

Isolated Common Hepatic Duct Injury after Blunt Abdominal Trauma

  • Extrahepatic bile duct injury is commonly associated with hepatic, duodenal, or pancreatic injuries, and isolated extrahepatic bile duct injury is rare. We report a patient who presented with an isolated extrahepatic bile duct injury after blunt trauma. A 50-year-old man was referred to our hospital after having suffered a fall down injury. His laboratory findings showed hyperbiliribinemia with elevated aspartate aminotransferase and alanine aminotransferase level. Initial abdominal computed tomography (CT) showed a mild degree of hemoperitoneum without evidence of abdominal solid organ injury. On the 3rd day of hospitalization, the patient complained of dyspnea and severe abdominal discomfort. Follow-up abdominal CT showed no significant interval change. Owing to the patient's condition, Emergency laparotomy revealed a large amount of bile-containing fluid collection and about 1 cm in size laceration on the left lateral side of the common hepatic duct. Primary repair of the injured bile duct with T-tube insertion was performed On postoperative day (POD) 30, endoscopic retrograde cholangiopancreatography showed minimal bile leakage and endoscopic sphincteroplasty and endoscopic retrograde biliary drainage were performed. On POD 61, the T-tube was removed and the patient was discharged.

전신마취 복부 수술 후 적극적인 가온요법이 통증, 체온 및 체온불편감에 미치는 효과 (The Effects of Active Warming on Pain, Temperature, and Thermal Discomfort in Postoperative Patients after General Anesthesia for Abdominal Surgery)

  • Purpose : This study investigated the effects of active warming using a Warm Touch warming system or a cotton blanket in postoperative patients after general anesthesia for abdominal surgery. Methods : This quasi-experimental study utilized two experimental groups and one control group: a cotton-blanket group (n = 25) were warmed with a cotton blanket and a sheet; a forced-air warming group (n = 24) were warmed with a Warm Touch warming system, a cotton blanket, and a sheet; and a control group (n = 25) were warmed with a sheet. Measurement variables were postoperative pain, body temperature, and thermal discomfort. Data were analyzed using a one-way ANOVA, ${\chi}^2-tests$, Fisher's exact test, and a repeated measures ANOVA. Results : The effects of active warming using a Warm Touch warming system and a cotton blanket on postoperative patients was significant in reducing pain (F = 13.91, p < .001) and increasing body temperature (F = 12.49, p < .001). Conclusion : Active warming made a significant difference in pain and body temperature changes. Active warming methods may help patients' postoperative recovery and prevent complications. Further research is needed to explore the effects and side effects of active warming on recovering normothermia.
