Removal efficacy and cytotoxicity of a calcium hydroxide paste using N-2-methyl-pyrrolidone as a vehicle

  • Objectives: This study investigated the removal efficacy and cytotoxicity of a newly developed calcium hydroxide paste (cleaniCal, Maruchi) using N-2-methyl-pyrrolidone (NMP) as a vehicle in comparison with ApexCal (Ivoclar Vivadent) and Calcipex II (Nishika), which use different vehicles such as polyethylene glycol and propylene glycol, respectively. Materials and Methods: Thirty maxillary premolars with oval-shaped canals were divided into 3 groups and the teeth were filled with one of the pastes. After removal of the paste, micro-computed tomographic (${\mu}$-CT) imaging was obtained to assess the volume of residual paste in the root canal of each tooth. The teeth were then split longitudinally and the area of the paste-coated surface was evaluated by stereomicroscopy. The cytotoxicity of each product was assessed using an agar overlay assay. The effect of each vehicle on cell viability was evaluated using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. The data were analyzed using one-way analysis of variance and Tukey's tests to detect any significance (p < 0.05). Results: In the ${\mu}$-CT and stereomicroscopic analysis, cleaniCal exhibited less remnants of medicament than ApexCal and Calcipex. cleaniCal showed a higher cytotoxicity than the other pastes in the agar overlay assay. Furthermore, NMP exhibited lower cell viability compared to the other vehicles. Conclusions: cleaniCal showed better removal efficacy compared to the other products. However, clinicians should be aware of the higher cytotoxicity of the NMP-based material and consider its possible adverse effects on periradicular tissue when it is overfilled.

A PHOTOELASTIC STUDY OF THE STRESS DISTRIBUTION IN THE ALVEOLAR BONE BY VARIOUS MOLAR UPRIGHTING SPRINGS (Molar Uprighting Spring에 의해 발생되는 치조골내의 응력분포에 관한 광탄성학적 연구)

  • This study was performed to analyze the effects of forces to the alveolar bone by various molar uprighting spring such as helical uprighting spring. T-loop spring, Modified T-loop spring and open coil spring. The simplified two-dimensional photoelastic model was constructed with a lower left posterior quadrant containing the second molar, the first and second premolars and the canine, with the first molar missing. Several molar uprighting springs were fabricated from 0.017 by 0.022 inch blue Elgiloy and applied to the photoelastic model. Two-dimensional photoelastic stress analysis was performed, and the stress distribution was recorded by photography The results obtained were as follows; 1. In all the kinds of the springs, the center of rotation of the mandibular second molar was oserved at the apical 1/5-1/6 between the alveolar crest and the root apex. 2. In all the kinds of the spring, the stress induced in the mesial root surface of the mandibular second molar was relatively homogeneous but there was some difference in the magnitude of the stress. 3. In the kinds of the springs, the distal crown tipping moment of the second molar was increased in turn as open coil spring, helical uprighting spring, T-loop spring, and modified T-loop spring. 4. The largest extrusive force was occured in the T-loop spring, intrusive force was occured in Modified T-loop spring only, and the largest distal tipping force was occured in open coil spring. 5. In the T-loop spring with activation, the stress induced in the mesial root surface of the second molar was increased gradually from the root apex to the alveolar crest and highly concentrated in the alveolar crest.

Morphological diversities of leaf in Korean Native Orostachys Species (자생 바위솔속(Orostachys) 엽의 형태적 다양성)

  • Korean native Orostachys species collected from 22 regions investigated morphological characters.O. japonicus species(No.3∼No.9) were similar leaf morphology and characterized by lanceolate leaves, cuspidate leaf apex.O. malacophyllus species(No.12∼No. 17) were morphologically characterized by obovate leaves and acute leaf apex. This species could be differentiate from other species by no thorn and leaf outlines formed by densely populated red dots.O. iwarenge species(No.18∼No.22) were diversified, such as obovate and elliptical leaves with acute, obtuse and round leaf apices. However, this species could be differentiate from other species bl'no thorn and grey powdery green colored leaves. Species collected from Maemuldo(No. 10) and Pocheon(No. 11) was assumed that these 2 species could be the new species which were not named classified.

Clinical problems of computer-guided implant surgery

  • Background: The utilization of a cone-beam computed tomography (CT)-assisted surgical template allows for predictable results because implant placement plans can be performed in the actual surgery. In order to assess the accuracy of the CT-guided surgery, angular errors and shoulder/apex distance errors were evaluated by data fusion from before and after the placement. Methods: Computer-guided implant surgery was performed in five patients with 19 implants. In order to analyze differences of the implant fixture body between preoperative planned implant and postoperative placed implant, angular error and distance errors were evaluated. Results: The mean angular errors between the preoperative planned and postoperative placed implant was $3.84^{\circ}{\pm}1.49^{\circ}$; the mean distance errors between the planned and placed implants were $0.45{\pm}0.48mm$ horizontally and $0.63{\pm}0.51mm$ vertically at the implant neck and $0.70{\pm}0.63mm$ horizontally and $0.64{\pm}0.57mm$ vertically at the implant apex for all 19 implants. Conclusions: It is important to be able to utilize these methods in actual clinical settings by improving the various problems, including the considerations of patient mouth opening limitations, surgical guide preparation, and fixation.

A RADIOGRAPHIC STUDY OF MANDIBULAR CANAL (하악관에 관한 방사선학적 연구)

  • The author invested the gonial angle, the angle of mandibular and mental canal to the lower border of the mandible, and a relationship of the position of the mandibular canal to the root apex of the posterior teeth and the cortical plate of the lower of the mandible. The materials consisted of 458 pantomograms in male and female aged 11-40 and divided into 5 groups at 5 year intervals and subdivided into 3 groups by gonial angle. The results were as follows; 1. The gonial angle decreased with age, but slight increase occurred over 26-30 years. 2. The average angle of the mandibular canal to the lower border of the mandible was 151.6° and did not correlate with age. 3. The average angle of the mental canal to the lower border of the mandible was 36.9° and didn't correlate with age. 4. The angles of mandibular and mental canal to the lower border of the mandible correlated with gonial angle. 5. The distance from the root apex of the posterior teeth to the upper wall of the mandibular canal was most short at the region of the distal root of the mandibular second molar and increased with age at the region of the mandibular second molar. 6. The distance from the lower wall of the mandibular canal to the cortical plate of the lower border of the mandible was most short at the region of the mesial root of the mandibular first molar and didn't correlate with age.

A STUDY ON THE EFFECT OF STRESS DISTRIBUTION OF MANDIBULAR IMPLANT BICORTICATION UTILIZING FINITE ELEMENT ALALYSIS METHOD (하악 임플란트 Bicortication 의 응력분산효과에 관한 유한요소분석적 연구)

  • Dental implantation is a method restoring missing teeth, especially in the case of severely resorbed edentulous patient. But the direct contact between bone and implant surface, induces stress concentration to the bone and eventually becomes a cause. The purpose of this study was to compare the stress distribution patterns between following two cylindrical implant models. One group has implant apex located in the inferior cortical bone and the other in the cancellous bone. Anterior edentulous mandible was modeled with two dimensional 953,878 nodes, 995,918 elements and compared the deflection and stress distribution under the 70 N,4 load cases for 26 models having variant mandibular height and length. The result were as follows; 1. The stress concentration was more affected by the height of the mandible than implant length. 2. Bicortication mitigates the stress of upper cortical and cancellous bone area at the same height of the mandible 3. Perforation of the inferior mandibular cortex significant stress concentration. 4. Stud type porstheses induced less stress concentration to the cortical and cancellous bone than bar type prostheses. 5. Stress of implant apex for stud type was larger than that of bar type.

FINITE ELEMENT ANALYSIS OF STRESS PATTERNS ON PERIODONTIUM OF SPLINTED ABUTMENTSFOR DISTAL EXTENSION REMOVABLE PARTIAL DENTURE (후방연장 국소의치에서 지대치의 splinting에 따른 치주조직의 응력 변화에 관한 유한요소법적 연구)

  • Splint therapy, the immobilization of teeth, has been done for patient's masticatory comforts and an adjunctive aid in periodontal therapy. Mandibular premolars are frequently splinted in many distal extension removable partial denture cases. But splinting is an extensive restoration that may not be conservative of tooth structure and may prove to be quite costly to the patient. The two dimensional finite element analysis method was used to determine the magnitude and mode of distribution of the stresses of the periodontal ligament and supporting alveolar bone when abutments with different periodontal supports were splinted and distal-extension removable partial denture was subjected to different loading schemes. The results were as follows : 1. When abutments were splinted, stresses moved from apico-distal to apico-mesial of terminal abutment on a vertical force and from disto-alveolar crest to apex on a distally directed force. But stresses were generally diminished on a mesially directed force. 2. As vertical bone loss was proceeding, most of stresses were transmitted to residual ridge and the rest of stresses were concentrated on apex of distal abutment. But these apical stresses were minimized when abutments were splinted. 3. As mesially inclined bone loss was proceeding, it seemed to be dangerous that many stresses were concentrated on the distal alveolar crest, especially in the distally directed load case. Abutments splinting decreased the alveolar crestal stresses but not enough. 4. For all vertical stresses were effectively decreased on splinting, stresses were concentrated as highly on apico-mesial area of distal abutment in distally directed load cases as the distal inclination of bone level was severe. 5. The directions and magnitudes of abutment movements were decreased with teeth splinting.

Accuracy of computer-aided template-guided oral implant placement: a prospective clinical study

  • Purpose: The aim of the present study was to evaluate the in vivo accuracy of flapless, computer-aided implant placement by comparing the three-dimensional (3D) position of planned and placed implants through an analysis of linear and angular deviations. Methods: Implant position was virtually planned using 3D planning software based on the functional and aesthetic requirements of the final restorations. Computer-aided design/computer-assisted manufacture technology was used to transfer the virtual plan to the surgical environment. The 3D position of the planned and placed implants, in terms of the linear deviations of the implant head and apex and the angular deviations of the implant axis, was compared by overlapping the pre- and postoperative computed tomography scans using dedicated software. Results: The comparison of 14 implants showed a mean linear deviation of the implant head of 0.56 mm (standard deviation [SD], 0.23), a mean linear deviation of the implant apex of 0.64 mm (SD, 0.29), and a mean angular deviation of the long axis of $2.42^{\circ}$ (SD, 1.02). Conclusions: In the present study, computer-aided flapless implant surgery seemed to provide several advantages to the clinicians as compared to the standard procedure; however, linear and angular deviations are to be expected. Therefore, accurate presurgical planning taking into account anatomical limitations and prosthetic demands is mandatory to ensure a predictable treatment, without incurring possible intra- and postoperative complications.

MTA APPLICATION TO PATIENTS WITH CELLULITIS CAUSED BY DENS EVAGINATUS (치외치로 인한 봉와직염 환자에서 MTA를 이용한 치험례)

  • Dens evaginatus is a dental anomaly involving an extra cusp or tubercle that protrudes from the occlusal surface of the affected tooth. The prevalence of dens evaginatus is 1 to 4 percent, and dens evaginatus is observed most commonly in premolars. It can be worn or fractured easily, resulting in pulpal exposure, pulpal infection, loss of vitality, facial infection and osteomyelitis. Since the tooth frequently has the immature apex when the tubercle is fractured, there is difficulty in treatment. Although calcium hydroxide is widely used for pulp treatment of an immature permanent tooth, several alternatives have been suggested to reduce patient's appointments. Mineral trioxide aggregate is considered biocom-patible and has excellent marginal sealing ability. In addition, it can minimize patient's visits. In this case report, apexification with MTA was attempted on the immature premolars in patients with cellulitis patient, caused by pulp necrosis due to dens evaginatus. Favorable clinical and radiologic results were achieved. In one case, continued root formation was observed.

Evaluation of Microleakage with Retrograde Filling Materials in Blood Contamination (혈액 오염된 역충전 재료의 미세누출 평가)

  • The purpose of this study was to evaluate the microleakage of root-end filling material filled in blood contaminated root-end cavity and self-etching adhesive placed over blood contaminated resected root apices without root-end preparation. Extracted, human maxillary incisors, canines and mandibular premolar were randomly divided into four groups of 15 teeth each. After canal preparation, resection of the apex and root-end preparation, MTA and IRM were filled in the root-end cavity (A and B group). After canal preparation and resection of the apex, Clearfil SE Bond and Prompt L-Pop were applied over the contaminated root-end surfaces (C and D group). The roots were then subjected to 15cm of water pressure to simulate periapical microleakage stress. Data were analyzed using one-way ANOVA. The results were as follows : 1. All groups showed a tendency of decreasing microleakage in process of time after 2weeks later except IRM group. 2. After 2 weeks and 1 month, MTA group showed less microleakage significantly than other groups(p<0.05). After 2 months, Prompt L-Pop group showed less microleakage significantly than other groups(p<0.05). 3. After 9 months, there were no significant differences among four groups(p>0.05). Thus it is considered that apical sealing using adhesives system without root-end preparation is good method in endodontic surgery.
