In Vitro Study of Anti-inflammatory and Analgesic Effects of Salvia Miltiorrhiza (SM) Extracts Using Luciferase Reporter Gene Assay (Luciferase reporter gene assay를 이용한 단삼(丹蔘)추출물의 소염, 진통작용에 대한 in vitro 연구)

  • Objectives: In order to identify the anti-inflammatory and analgesic properties of Salvia miltiorrhiza (Dan-Sam), widely used in Korean traditional medicine, an in vitro screening system was designed using pGL3, a luciferase reporter vector, and the tumor necrosis factor (TNF)-${\alpha}$ and cyclooxygenase (COX)-2 as target genes. Methods: The promoter regions of each gene were generated by PCR using the human chromosome as template DNA, and inserted into pGL3 vector with Kpn I and Hind III. The final construct was transfected into human myelomonocytic leukemia cells (U-937) that could be differentiated and activated by phorbol 12-myristate 13-acetate (PMA) or lipopolysaccharide (LPS). Using this system, the anti-inflammatory and analgesic effects of several herbal extracts regarded to have the medicinal effects of diminishing body heat and complementing Qi were tested. The chemicals PD98059 and berberine chloride were used as controls of the transcriptional inhibitors of TNF-${\alpha}$ and COX-2, respectively. Results: Salvia miltiorrhiza (Dan-Sam) demonstrated significant decrease of TNF-${\alpha}$ and COX-2 mRNA in the in vitro assay system. In MTT assay, Salvia miltiorrhiza (Dan-Sam) did not significantly inhibit the survival and proliferation of human myelomonocytic leukemia cells (U-937). Conclusions: Salvia miltiorrhiza (Dan-Sam) was found to exhibit the significant medicinal properties of anti-inflammatory and analgesic effects.

Effect of Lower Abdomen Warmer on Primary Dysmenorrhea Patients Taking Nonsteroidal Anti-Inflammatory Drugs (비스테로이드성 소염진통제를 복용하는 원발성 월경곤란증 환자에 대한 하복부 온열찜질기의 효과)

  • Objectives: This study was performed to investigate the pain relief effect of lower abdomen warmer on primary dysmenorrhea patients taking nonsteroidal anti-inflammatory drugs and whether the using of warmer can supplement or replace the drugs. Methods: 30 women with primary dysmenorrhea were assigned to treatment group (n=15) and control group (n=15). At 1st visit, the treatment group was provided with a wirless multiuse warmer and trained to use at least three times per menstrual cycle. The control group was not provided with the warmer, and both groups were provided with a menstrual diary and instructed to record their pain intensity and dose of analgesic every menstrual period. Visual Analogue Scale (VAS) were used to assess the intensity of overall pain and the most severe pain during the menstrual period. And the total number of analgesic taken during menstruation and the average number of analgesic taken during a single dose were measured. Results: There was significant pain relief in the treatment group compared to before baseline, and there was a significant difference from the control group. In addition, there was no significant difference in the frequency of taking analgesic during the menstrual period between the treatment group and the control group, but the dose of analgesic was significantly lower in the treatment group than in the control group. Conclusions: This clinical trial showed that lower abdomen warmer would helpful in relieving primary dysmenorrhea and could help reduce the use of nonsteroidal anti-inflammatory drugs.

  • Both wild peony and cultivated peony demonstrated a slight effect and tranquilizer of anti-convulsion on the strychinine and picrotoxine but the wild plant was more effective. The methanol extract of cultivated peony root demonstrated a weak effect of muscle loosening but its mixture with water extractable fraction did not have this effect in contrast to the case wild type whose methanol extract mixture with water extractable fraction still retained a strong anti-convulsional muscle loosening effect. Therefore, it is concluded that wild peony root has more analgesic and muscle loosening effect than the cultivated counterpart has.

Anti-mechanical Hyperalgesic Effect of Lonicera Japonica in Neuropathic Pain Rats

  • Lonicera japonica has been widely used for chronic inflammatory diseases in many Asian countries. Its analgesic effect has not been explored yet. This study aimed to test the analgesic potential of methanol extracts from Lonicera japonica (MELJ) in rat neuropathic mctel. Neuropathic pain was pacts ed by partial sciatic nerve injury. Two weeks after surgery, neuropathic rats received oral administration of MELJ at doses of either 0.0 g/kg, 0.2 g/kg or 0.4 g/kg. At dose of 0.0 g, rats were administered with saline only and used as conracl. The behavioral tests for f 0.0 g, raand ccld hs were adma were weformed up to 2 hours after treatment. The MELJ at the dose 0.4 g/kg dmg gfg, ntly alleviated f 0.0 g, rahyperalgesia, but not cold hyperalgesia. These results showed that the MELJ had, although transient, analgesic effect on mechanical hyperalgesia in the rat neuropathic model.

  • In order to investigate experimentally the clinical effects of Choweesungchung-Tang that has been widely used in the cardiovascular and neuropsychogenic disease, experimental studies with experimental animals were carried out. The results of these studies were summarized as follows; Suppressive action was not shown on the convulsion induced by strychnine, but significant effect was noted on the convulsion induced by picrotoxin and caffeine. In acetic acid method, analgesic effect was noted. By the rotor rod and wheel cage method, sedative action was noted. A prolongation of hypnotic time induced by pentobarbital-Na was obtained. Relaxing action was noted remarkably on the ileum of mice, also about mice and guinea-pigs, the same effect was recognized on the smooth muscle of the ileum. The expansion of blood vessels by relaxation of smooth muscle and hypotensive action were noted.

Analgesic Effect of Centipede Venom Iontophoresis for Myalgia (왕지네 Venom을 사용한 이온토포레시스가 근육통에 미치는 영향)

  • This study was to determine whether iontophoresis application of centipede venom would produce analgesic effect for clinical practice. Sixty patients suffered from musculoskeletal diseases who lived in Taejon city were analyzed by double-blind central study. After pre treatment, post treatment, post 20 minutes, post 40 minutes iontophoresis using centipede venom, we assessed the response to pin-prick sensation with Endomed 582 in myalgia patients. The results were as follows, control group showed pain threshold of 1.76, 2.03, 2.01, 2.02mA after treatment, but the pain threshold of 1.76, 2.21, 3.85, 3.87mA after iontophoresis application in study group. The pain threshold of Notermans pain score after 20-minute and 40-minute centipede venom iontophoresis group using centipede venom was higher than that after non-centipede venom iontophoresis group. The results showed that by the increasing the analgesic effect in the centipede venom iontophoresis group, especially in time of post 20 minutes after iontophoresis treatment. So we considered that the iontophoresis using centipede venom could be reduced pain of myalgia in musuloskeletal disease patients.

  • The purpose of this study were to 1) determine the analgesic effect of 632.8 nm of helium-neon (He-Ne) laser stimulation on acupuncture point in rat and 2) determine the reversal of analgesic effect by naloxone injection. Eighteen Sprague-Dawley rats were devided into three groups : control (n=6) : laser (n=6), laser stimulation at $3.58\;J/cm^2$ ; and naloxone (n=6), 1 mg/kg of naloxone chloride inject into peritoneum before laser stimulation at $3.63J/cm^2$. Tail-flick latency were measured pretreat and posttreat with hot plate $(55^{\circ}C)$. Data were analyzed using one-way ANOVA and paired t-teat for tail-flick latency. No significant change was noted in the tail-flick latency in either control or naloxone groups. But significant increased in tail-flick latency in taller group. The results suggest that He-Ne laser induced analgesic effect, and endogenous opioids may be involved in He-Ne laser induced analgesia.

Antispasmodic and Analgesic Effects by Concurrent Administration of Etomidoline and Nefopam$\cdot$HCl (Etomidoline과 Nefopam$\cdot$HCl 병용투여시의 진경 및 진통 효과에 관한 연구)

  • Etomidoline is a new synthetic atropine-like drug. The present investigation aimed to study the combined effects of etomidoline and neofopam hydrochloride which has an analgesic and muscle relaxant activity, compared with the effects of each drugs. Acute toxicities (ID$_{50}$) in mouse were 132mg/Kg (p.o.) and 49mg/Kg (i.p.) when combination ratio of etomidoline and neofopam was 1:5 and 103 mg/Kg (p.o.) and 30mg/Kg (i.p.) with the ratio of 1:10. Etomidoline showed more potent anticholinergic effects than neofopam in the isolated rat intestine. Whereas, antibarium effects were twice as active with neofopam than with etomidoline. When etomidoline and neofopam were added in combination ratio of 1 : 5, papaverine-like avtivity was increased, but no changes of anticholinergic effect were observed. Analgesic effect was measured by the anti-writhing method of Whittle in mice. Both of the concurrent and single administration of etomidoline and nefopam reduced significantly the writhing number and the effect of the concurrent administration was more active than that of single, and 1 : 5 combination was slightly more potent than 1:10. Each drug or the combined drug was administered to mice and observed the change of the pupil size. Pupil sizes were increased with each drug and with combined drug, although there were no significant differences between the each group of drugs. However, those effects were less than that of atropine sulfate.e.

  • Experimental studies were conducted to investigate the effect of Sagan-Tang on analgesic, sedative, antipyretic, isolated ileum and blood vessel and so on. The results of this investigation were summarized as follows; Analgesic action by the acetic acid stimulating method in mice were recognized. Prolonging action against the hypnotic duration induced by thiopental-Na was noted in mice. Antipyretic effect in typhoid vaccine febrile rats was recognized. Spontaneous motility of the isolated ileum of mice was suppressed and contractions of the isolated ileum of mice and guinea-pig induced by accetylcholine chloride, barium chloride and histamine were remarkably inhibited. Vaso-diating and hypotensive actions were recognized in rabbits. GOT and GPT activities in the serum of rats damaged by $CCl_4$ and galactosamine were decreased remarkably.

The analgesic effect of Auto-Manual Acupuncture with Twirling Method (자동 염전침의 진통효과)

  • Background and Objective : Twirling is one of the needling methods, which is frequently used for acupuncture in oriental medicine. Some thesis about needle manipulation has been reported that needle manipulation is more effective than plain acupuncture. So we have developed the auto-controlled twirling needle(ACTN) system which is most simple style of needle manipulation. The present study was conducted to see if ACTN can enhance the antinociceptive effect of acupuncture. Methods : To investigate the analgesic effect of acupuncture we used two pain model. One is acute pain model using tail flick latency(TFL), the other is neuropathic pain model using mechanical allodynia. For TFL test, rats were lightly anesthetized with pentobarbital sodium(40 mg/kg, i.p). To produce mechanical allodynia in the rat tail, the right superior caudal trunk was resected between the S1 and S2 spinal nerves. For plain acupuncture(PA), a needle was inserted into a Zusanli(ST36) for 15min. In combining ACTN with PA, twirling needle was performed once in a second. We measured the difference of analgesic effect of only PA and ACTN on two different kinds of pain. Results and conclusion : ACTN increased TFL more than PA. (15min P<0.001, 25min P<0.01). And ACTN also reduced neuropathic pain (15min P<0.01, 25min<0.05). But PA alone can't reduce the neuropathic pain. These results indicate that ACRN had more analgesic effect than PA. The mechanism that play a key role, and the condition which produce best analgesic effect of ACTN are to be studied further.
