류마티스 관절염 환자의 건강관련 삶의 질 영향요인 분석: 통증, 질병활성도, 일상생활 수행능력 장애 및 우울과의 관련성 (Health-related Quality of Life in Korean Patients with Rheumatoid Arthritis: Association with Pain, Disease Activity, Disability in Activities of Daily Living and Depression)

  • Purpose: The purpose of this predictive study was to identify factors affecting health related quality of life (HRQoL) in patients with rheumatoid arthritis (RA). Methods: The participants in this study were 131 patients with RA who were recruited from the outpatient clinic of a university hospital in Seoul. Disease activity in rheumatoid arthritis was evaluated by calculating the Disease Activity Score 28. Disability in activities of daily living (ADL) was assessed with the Korean Health Assessment Questionnaire, and depression with The Center for Epidemiologic Studies Depression Scale. HRQoL was evaluated using The Short Form 36 Health Survey. Data were analyzed using descriptive statistics, correlation, and hierarchical multiple regression. Results: Pain, disability in ADL, disease activity, and depression correlated negatively with physical and mental dimensions of HRQoL. But hierachical multiple regression analysis revealed that disability in ADL and depression were the only variables negatively influencing physical and mental QoL after adjustment for influences of sociodemographic variables. Conclusion: Results of this study suggest that disability in ADL and depression, rather than disease activity and pain have profound effects on HRQoL in patients with RA. Further studies are needed to assess the predictive ability of disease activity and pain on HRQoL in this population.

뇌졸중 환자의 장애 특성에 따른 접촉 감각 자극이 두점 구별, 손 기능 및 일상생활수행력에 미치는 효과 (The Effect of Tactile Stimulation on Two Point Discrimination, Hand Function, and ADL in Impaired Characteristics of Stroke Patient)

  • PURPOSE: This study find out the effect of improved two point discrimination (TPD), hand function and activities of daily living (ADL) performance through tactile stimulus of upper limb (U/L) in impaired characteristics of stroke METHODS: We selected 26 stroke patients in BMH who has problems with neglect, sensory and motor deficits. Patients were divided into 3 group with neglect group (NG), sensorimotor deficits group (SMG) and motor deficit group (MG). To compare each group we used assessment tools such as two point discrimination on affected side (TPDas) and unaffected side (TPDus), Manual functional test on affected side (MFTas) and unaffected side (MFTus) and Korean version modified barthel index (K-MBI). RESULTS: 1) In the NG, tactile stimulus on U/L was statistically important for TPDas (forearm, index finger tip) also SMG and MDG was statistically important for TPDas. 2) In the NG, SMG, there was statistically important for MFTas, MFTus and in the MG. K-MBI also was statistically importance. Among three group, there was an statistically important difference for TPTus (forearm, thenar, hypothenar), MFTas and MFTus. We analyzed the relationship among TPD, MFT and K-MBI and There was negative relationship between TPD, MFT and There was positive relationship between TPD and K-MBI CONCLUSION: In impaired characteristics of stroke patients, tactile stimulus on U/L influenced on two point discrimination, hand function and ADL's. And we also found relationship among somatosensory, hand function, and ADL performance.

Participation and Activities of Daily Living, Cognition, And Motor Function According to Residential Area in Stroke Survivors

  • PURPOSE: Social participation is essential for stroke survivors. Although participation restrictions are affected by several factors, a few studies have focused on the effects of living in a particular residential area (urban versus rural) on the participation restrictions after a stroke. This study examined the factors affecting participation restrictions in stroke survivors according to the residential area. METHODS: One hundred sixty-six stroke patients (including 130 living in urban areas and 36 living in rural areas) were recruited for this study. The Korean Modified Barthel Index (K-MBI) was used to assess the activities of daily living (ADL). The Korean Mini-Mental Examination Status (MMSE-K) was used to assess cognition. The motor function was evaluated using the Fugl-Meyer function assessment (FMA). Path analysis was used to test the hypothesized model of participation restriction. RESULTS: The proposed path model showed good fit indices. In rural and urban areas, the direct effects were significant between the ADL and participation restrictions (β = -.673 and -.457, respectively). For urban areas, the direct effects were significant between cognition and participation restrictions (β = -.252). In both area types, motor function and cognition had a significant direct effect on the ADL. CONCLUSION: For urban community-dwelling stroke survivors, the ADL and cognition had direct effects on participation restrictions. For rural area stroke survivors, the ADL had direct effects on participation restrictions. Activity level exercise programs help reduce the participation restrictions. Moreover, it is essential to address cognition training to improve participation in urban community-dwelling stroke survivors.

Hospitalization Risk According to Geriatric Assessment and Laboratory Parameters in Elderly Hematologic Cancer Patients

  • Background: Utilizing geriatric screening tools for the identification of vulnerable older patients with cancer is important. The aim of this study is to evaluate the hospitalization risk of elderly hematologic cancer patients based on geriatric assessment and laboratory parameters. Materials and Methods: In this cross sectional study 61 patients with hematologic malignancies, age 65 years and older, were assessed at a hematology outpatient clinic. Standard geriatric screening tests; activities of daily living (ADL), instrumental activities of daily living (IADL), Mini Nutritional Assessment (MNA), Mini Mental State Examination (MMSE), timed up and go test (TUG), geriatrics depression scale (GDS) were administered. Demographic and medical data were obtained from patient medical records. The number of hospitalizations in the following six months was then recorded to allow analysis of associations with geriatric assessment tools and laboratory parameters. Results: The median age of the patients, 37 being males, was 66 years. Positive TUG test and declined ADL was found as significant risk factors for hospitalization (p=0.028 and p=0.015 respectively). Correlations of hospitalization with thrombocytopenia, vitamin B12 and folic acid deficiency were statistically significant (p=0.004, p=0.011 and p=0.05 respectively). Conclusions: In this study, geriatric conditions which are usually unrecognized in a regular oncology office visit were identified. Our study indicates TUG and ADL might be use as predictive tests for hospitalization in elderly oncology populations. Also thrombocytopenia, and vitamin B12 and folic acid deficiencies are among the risk factors for hospitalization. The importance of vitamin B12 and folic acid vitamin replacement should not be underestimated in this population.

요양시설 거주 여성노인의 기초신체활동 측정 평가 (Assessment of Basic Physical Activity for Elderly Women in Long-Term Care Facilities)

  • 본 연구의 목적은 시설거주 여성노인의 일상생활동작이 반영된 기초신체활동을 측정하여 대상자들의 기초신체활동의 기본적인 필수 측정요소와 측정항목을 제시하기 위함이다. 대상자는 대전과 충남에 소재한 장기요양시설 4곳의 만 65세 이상 여성노인 105명이며, 일상생활동작이 반영된 8개 항목(악력, 스텝 테스트, 의자에서 앉았다 일어나기, 한발 서기, 기능적 팔 뻗기, 8자 보행, 5m 보행, 일어서고 걷기)을 측정하였다. 자료는 평균과 표준편차, 요인분석, ANOVA, 그리고 피어슨의 상관관계로 분석하였다. 연령집단 간각 종목의 평균을 분석한 결과 연령이 증가함에 따라 각 측정항목의 결과에 유의한 차이를 나타냈다. 8개 측정항목은 요인분석을 통해 근력과 이동능력 두 영역으로 제시하였다. 시설거주 여성노인에게 중요한 체력요소인 이 근력과 이동능력 영역은 연령이 증가할수록 모두 유의하게 감소하였다. 따라서 시설거주 여성노인의 운동을 위한 간호전략에 있어 근력 및 이동능력의 요소가 고려되어야 하며, 이 두 영역에 대한 측정항목 또한 객관적인 지표가 마련되어야 한다.

Effects of High-Intensity Muscle Strength Training and Stretching Exercises on Strength, Spasticity, Postural Alignment, and Participation in an Adolescent with Spastic Diplegic Cerebral Palsy: A Single-Subject Design

  • PURPOSE: This study examined the effects of high-intensity (HI) muscle strength training and stretching exercises on muscle strength, spasticity, postural alignment, and participation in the activities of daily living (ADL) in an adolescent with cerebral palsy (CP). METHODS: The study used a single-subject design with a 16-week follow-up. After a three-week intervention-free period, a participant underwent five data collection sessions for the baseline measurements. Subsequently, stretching and HI strength training occurred three times weekly for 48 sessions, with the outcome measures collected weekly post-treatment. Final measurements were taken the day after the last session, and a follow-up assessment occurred six weeks post-study to assess the learning effects. RESULTS: After 16 weeks of treatment, the participant exhibited improved targeted muscle strength and postural alignment without increased spasticity. On the other hand, no significant change in participation in the ADL was observed. CONCLUSION: The research findings suggest that HI strength training and stretching exercises may contribute to improvements in muscle strength and body alignment without increased spasticity in an adolescent with CP, but it may not have a significant impact on participation in ADL.

Assessment of the Nutritive Value of Whole Corn Stover and Its Morphological Fractions

  • This study investigated the chemical composition and ruminal degradability of corn stover in three maize-planting regions in Qiqihaer, Heilongjiang Province, China. The whole stover was separated into seven morphological fractions, i.e., leaf blade, leaf sheath, stem rind, stem pith, stem node, ear husk, and corn tassel. The assessment of nutritive value of corn stover and its fractions was performed based on laboratory assays of the morphological proportions, chemical composition, and in situ degradability of dry matter (DM), neutral detergent fiber (NDF), and acid detergent fiber (ADF). The chemical composition of corn stover was significantly different from plant top to bottom (p<0.05). Among the whole corn stover and seven morphological fractions, leaf blade had the highest crude protein (CP) content and the lowest NDF and ADF contents (p<0.05), whereas stem rind had the lowest CP content and the highest ADF and acid detergent lignin (ADL) contents (p<0.05). Ear husk had significantly higher NDF content and relatively lower ADL content than other corn stover fractions. Overall, the effective degradability of DM, NDF, and ADF in rumen was the highest in leaf blade and stem pith, followed by ear husk. The results indicate that leaf blade, ear husk, and stem pith potentially have higher nutritive values than the other fractions of corn stover. This study provides reference data for high-efficiency use of corn stover in feeding ruminants.

한국 작업치료사의 평가도구 사용 동향: 성인 및 노인 분야를 중심으로 (Trends in the Use of Assessment Tools by Korean Occupational Therapists: Focus on Adult and Older Adult Populations)

  • 목적 : 성인 및 노인 분야에 종사하는 작업치료사를 대상으로 평가도구 사용 동향을 알아보고자 하였다. 연구방법 : 작업치료사를 대상으로 일반적 특성, 평가도구 사용 대상, 평가 시간 및 주기, 영역별 평가도구 사용 동향, 평가도구 사용 및 교육에 대해 설문조사를 실시하였다. 평가도구 영역은 Occupational Therapy Practice Framework (OTPF)에 따라 작업의 Activities of Daily Living (ADL), Instrumental Activities of Daily Living (IADL), 놀이 및 여가 영역과 클라이언트 요인의 인지 기능, 운동 기능, 심리사회 영역으로 구분하였다. 설문에 응답한 98명의 자료는 SAS 9.4를 사용하여 빈도 분석과 일원배치 분산분석을 하였다. 결과 : 설문에 참여한 작업치료사는 대학병원과 종합병원(40.82%)에 가장 높은 빈도로 근무하고 있었다. 환자 한 명당 소요되는 평가 시간은 30분 미만이 62.25%, 재평가 주기는 1~3개월 미만이 68.38%로 가장 높은 빈도를 보였다. 평가도구의 영역 별 사용 빈도는 작업의 ADL은 Modified Barthel Index (94.90%), IADL은 Canadian Occupational Performance Measure (31.63%), 클라이언트 요인의 인지기능은 Mini-Mental Status Examination (89.80%), 시지각은 Motor-free Visual Perception Test (56.12%), 치매는 Global Deterioration Scale (63.27%), 운동 기능은 Jamar Dynamometer & Pinch Gauge (59.18%), 연하는 Video Fluoroscopic Swallowing Study (28.57%), 이학적 검사법은 Range of Motion (59.18%), 심리사회는 Geriatric Depression Scale (33.67%)을 가장 높은 빈도로 사용하고 있었다. 결론 : 본 연구를 통해 성인 및 노인 분야의 평가도구 사용 동향과 그 근거를 확인하였다. 이는 작업치료학과 교육과정 및 보수교율 등 전문성 강화를 위한 정보로 활용될 것으로 기대된다.

노인의 퇴행성 슬관절염과 일상생활 수행력에 관한 연구 (A Study of Activities do Daily Living Performance on Arthritic Elderly Patients)

  • To know the effect of index of social ability (ISA) and function of social life (FSL) with activities of daily living (ADL) and instrumental activities of daily living (IADL), we carried out a study on the elderly with osteoarthritis daily living in Daegu city for 4 months, from April to July 2002. The followings were observed: 1. The score of functional disability of knee, ADL and ISA were better at lower age (p < 0.01). 2. The younger they are, the better FSL they have. The lower age group has good FSL (p < 0.01). 3. Weight affects more to the under 49Kg group than 50-59Kg group and 60-69Kg group in functional disability of knee, while it doesn't make much difference for the other groups (p > 0.05) and other assessment index doesn't show any relationship with weight. 4. Regarding relat ionship with height , taller group has better knee functions: it's proved by post hock examination that the over 160cm group has much higher knee function than the other groups (p < 0.05). In case of ADL, the over 160cm group and under 149cm group were better than 150$\∼$159cm group. (p < 0.01) . 5. ISA was also different depending on height . It was proved that FSL was the better for the over 160cm group than for the others as a result of post hock examination (p < 0.05). 6. In regard of how many months have passed since the attack of functional disability of knee, knee joint function was better for the 12mon$\∼$36mon group than for the others and the score of ADL was lower for the 12mon$\∼$36mon group than for the others (p < 0.01). ISA was lower for the 12mon$\∼$36mon group and the under 6mon group than for the other groups (p < 0.01). FSL was lower for the 12mon$\∼$36mon group and the under 6mon group than the other groups. (p < 0.01). 7. Comparing left knee with right one, the problem with left knee has lower score of functional disability in all items (p < 0.01). 8. Considering friction noise, knee joint function was slightly better for non-friction noise-group, but it's not statistically significant though. ADL and ISA were better for non-friction noise-group (p < 0.01), while no statistical significance was found in FSL, 9. Knee joint function was better for the group who doesn't have limitation on range of knee flexion motion (p,0.05) but no other difference was found in other items from this group. 10. Limitation on range of knee flexion motion has no statistical difference in ADL and FSL. However, ISA was better for the non-limitation group (p < 0.05) . 11. Knee joint function score has high correlation with ISA (r=0.812, p < 0.01) and FSL (r=0.732, p < 0.01) , which shows the better knee joint function, the higher the score. 12. ISA and FSL were highly correlated: the better ISA, the better FSL (r=0.893, p < 0.01), while ADL has no correlation with either knee joint function or ISA or FSL.
