Relieving effect for respiratory inflammation of Sojadodamgangki-tang (소자도담강기탕(蘇子導痰降氣湯)의 호흡기 염증 완화효과)

  • Objectives : Sojadodamgangki-tang and its main components are traditional korean medicinal methods for treatment of cough, sputum and dyspnea. Using a respiratory inflammatory model, we intend to reveal the anti-inflammatory effect and its immune mechanism of Sojadodamgangki-tang. Methods : We used a papain-induced respiratory inflammatory mouse model. 8-week-old female BALB/C mice were divided into 3 groups as follows: the following groups: saline control group, papain treated group (vehicle), papain and Sojadodamgangki-tang(200 mg/kg) treated group (n=4). To evaluate the anti-inflammatory effect of Sojadodamgangki-tang extracts, inflammatory cell infiltration was measured in bronchoalveolar lavage fluid (BALF) and nasal lavage fluid (NALF). In addition, the effects of Sojadodamgangki-tang extracts on Th2 cell population in lung were determined by using flow cytometry. Results : Sojadodamgangki-tang extracts administration reduced inflammatory cell infiltration in BALF and NALF, especially of eosinophils. Furthermore, total immunogloblin (Ig)-E levels was reduced in BALF and serum by drug administration. Interestingly, Sojadodamgangki-tang extracts treatment also decreased the Th2 cell (CD4+GATA3+) population in lung. Conclusions : Our findings indicate Sojadodamgangki-tang extracts have anti-inflammatory effects by mediating Th2 cell and B cell activation.

Case Report on Headache Treated with Korean Medicine in a Patient with Epilepsy (뇌전증과 동반된 두통에 대한 한방치료 치험 1례)

  • Objective: This study reports a clinical case of headache accompanied by epilepsy that showed improvement with Korean medicine treatments. Methods: A 67-year-old woman with headache was treated with Korean medicine therapies, including Cheongshimondam-tang, acupuncture, electroacupuncture, and cupping, for 14 days. The response before and after treatment was evaluated using the Numerical Rating Scale, Headache Impact Test-6 (HIT-6), and Quality of Life in Epilepsy-10 (QOLIE-10). Results: After the treatments, the headache was relieved from NRS 8 to NRS 1, and anxiety decreased from NRS 7 to NRS 2. The HIT-6 changed from 66 to 48, and the QOLIE-10 dropped from 44 to 30. Conclusion: This study suggests that combined Korean medicine treatments might be a therapeutic option for relieving headache with epilepsy.

A Study on the Clinical Characteristics of 4-Constitution Groups in 8-Constitution Medicine (8체질의학에서 목$\cdot$$\cdot$$\cdot$수 4체질군의 임상적 특징에 대한 연구)

  • Backgrounds : In recent years there has teen renewed interest in the importance of 8-constitution medicine as evidenced by growth of research in this field. Although diagnosis by 8-constitution depends on pulse types, it still is subjective and thus could be insignificant. Therefore, diagnosis based on objective characteristics is essential. Objectives : The purpose of this study was to find objective characteristics of the 4-constitution groups {木(Mok; hepatonia, cholecystonia), {土(To; pancreotonia, gastrotonia), {金(Gum; pulmotonia, colonotonia), {水(Soo; renotonia, vesicotonia)} in 8-constitution for further diagnosis and therapy. Also, some significant clinical tendencies of 4-constitution groups were studied. Methods : From Nov. 2001 to Apr. 2004, data were collected through a questionnaire given to 720 patients who were classified into one of 8 constitutions according to their pulse types and their responses to constitution-based acupuncture therapy. The questionnaire was composed of 80 items which were about personality, physical shape, habit, physiological and pathological symptoms, response to drugs, favorites and responses to various foods. The data were statistically analyzed by cross-tabulation and one-way analysis of variance, and SPSS V10.0.7PC was used. Results : The results are summarized as follows: 1) Among participants, proportions of hepatotonia, pancreotonia, and cholecystotonia were higher than those of vesicotonia, pulmotonia, colonotonia, renotonia, grstrotonia. 2) 52 items of 80 variances showed significant differences by gender, 3) 13 items were highly correlated with 4-constitution groups in males, and 26 items in fremales. 4) 22 items showed significant group differences among 4-constitution groups in males, and 41 items in females. Also, for each item 4-constitution groups were sorted according to their means, so that the constitutional tendencies were objectively figured out by personality, shape, habit, physiological and pathological symptoms, response to drugs, favorites and responses to foods. Conclusions : Most clinical characteristics of 4-constitution groups found in this study coincided with those from the previous ones in this fold. Thus, classifying patients into one of the 4-constitution groups based on significant objective characteristics is of great benefit to diagnosis and therapy. further study should be followed to improve proper therapy for each constitution.

  • Disease depends on the three factors, agent, host and environment. According to history of disease, by early 1900s the case of deaths is infectious disease, in late 1900s care of infectious diseases and tremendous scale of chronic disease, i.e., heart disease, diabetes, cancers and etc, makes care of chronic diseases be a most important theme. Now, life-style of diet is being westernized and in high industry-oriented society, obesity makes attack fate remarkably increase and life-expectancy become short, so that it causes severe problem of health. Chronic disease, such as obesity, is not affected by specific agent, but depends of interaction between host and environmental factors. There is the theory of constitutional medicine in Korean Medicine. According to it, all the people have constitutional specificity and disease. Because obesity is a kind of disease, there is the corresponding constituent being apt to be fat. Oriental Medicine utilizes herb-medication, acupuncture, and massage-therapy in treating obesity. Therefore study on relationship between constituent and obesity for OPD patients of Sangji-Oriental Medicine Hospital is carried out. The results are summarized as followings. 1. 70.2% of obesity patients are Taeumin(太陰人), 26.9% of those are Soyangin(少陽人), 2.9% of thoese are Soeumin(少陰人). 2. Most cases, high value of Free Fat Acid and Triglyceride not that of Total Cholesterol and Low Density Lipoprptein is meaningful in obesity patient blood. The corelationship between lipid test and Constitution is meaningful in Triglyceride and Free Fatty Acid. 3. Obesity is not related with gene. 4. Obesity is not related with Boyak(Herb-Med : 補藥). 5. Obesity mostly happens after delivery, contraception and operation. 6. Obese Patients are apt to eat between meals, especially food of wheat flour such as a snack. 7. The aim of treating obesity is not persuit of beauty but of keeping healthy. 8. 2.2Kg of body weight is lost after 4 week-treatment. 9. Common cause of obesity is overeating of carbohydrate and lipid than meat.

  • The definitions of Four Constitution (Tae-Yang-In, So-Yang-In, Tae-Eum-In, So-Eum-In) in Four-Beginning-Theory (四端論) and Distribution-Theory (擴充論) were morphologically diagrammatized. the method of measuring the five parts came from the diagrams. The five parts are the followings; Top-Line is the distance between Rt. & Lt.Shoulders. Bosom-Line is the horizontal distance passing through the Rt. & Lt.nipples. Stomach-Line is the horizontal distance passing through the Rt. & Lt. acupuncture point Bool-Yong (不容穴). Navel-Line is the horizontal distance passing through the navel. Bottom-Line is the distance between the Rt.& Lt. anterior superior iliac spines. According to it, 311 people were measured in the clinic. Through the measured numerical values, to find out the objective propriety of the morphological diagrammings and the discriminating ability of Four Constitutions by the method of measuring the five, the author tried this study and got the following results. 1. On the ground of the Internal Organs-Theory (臟腑論), the definitions of Four Constitutions were classified and diagrammatized step by step and it was possible to draw the morphological graphs satisfying the reasonable condition objectively. 2. It was found that the graphings by Mean, Standard Deviation and 95% Confidence Interval for mean of the numerical values of five measured parts and the morphological diagrammings of the definitions in Four-Beginning Theory and Distribution-Theory drove to the same figures. 3. Through the method of the Analysis of variance and the Discriminant Analysis, it was discovered that the measured numerical values of five parts could distinguish the Four Constitutions. 4. Tae-Yang-In has the longest Top-Line and the shortest Bottom-Line. The Mean head of Confidence Interval from Top to Bottom is downwardly decreased at the rate of 3.7, 1.5, 1.8, 3.4. 5. So-Yang-In has the longest Top-Line and the shortest Bottom-Line. The Mean head of confidence Interval from Top to Bottom is downwardly decreased at the rate of 2.3, 1.5, 1.5, 1.1. 6. Tae-Eum-In has the longest Navel-Line. The Mean head of Confidence Interval from Navel to Top is upwardly decreased at the rate of 0.5, 0.4, 2.5 and downwardly decreased at the rate of 2.7 between Navel and Bottom. 7. So-Eum-In has the longest Bottom-Line. The Mean head of Confidence Interval from Bottom to Top is upwardly decreased at the rate of 1.1, 1.1, 1.8, 2.3. From the above findings, it was possible to draw the morphological graphings satisfying the objectively reasonable conditions in the definitions of Four-Beginning-Theory and Distribution-Theory and it was proved that the results of the Positive Analysis, carried with the measured numerical values of five parts from the diagramming, could show the objective propriety of the morphological graphings and the discerning ability of Four Constitutions by the method of measuring the five parts.

  • Lumbago is the common disease in the human, many people have been sicked the Lumbago. As the traditional methods of Lumbago treatment, acupuncture, moxibustion, herb medicines have been applied to a patient, nowadays, new methods have been applied physical remedy, Yakchim (Korean herb acupuncture) remedy, Chuna remedy to. This report was collected 73 Lumbago patients by name, sex, age, motive, symptoms, X-ray, CT, MRI, lRCT, Sasang constitution type, Sasang herb medicine, Yakchim, Chuna, period of remedy, satisfaction of remedy, at the "WooRee Korean Medical Clinic" during 21 months from Sep. 14, 1994 to May 25, 1996. And this report was studied about the distribution of the Sasang constitution type, the Sasang herb medicine, the effect, the period. The results were as follows: 1. Lumbago patients were distributed like that; Taeum-ln (太陰人) 47 (66.3 %), Soyang-In 16 (21.9 %), Soum-In (13.7 %), Taeyang-In (太陽人) 0. This was different from distribution of Donguisuseibowon (東醫壽世保元), Taeum-In (太陰人) 50%, Soyang-In (少陽人) 30 %, Soum-In (少陰人) 20 %, Taeyang-In (太陽人) little, this report shows that the number of Taeum-In (太陰人) is more than that of Donguisuseibowon and the number of Soum-In is less than that of Donguisuseibowon. 2. The average satisfaction of remedy was 60.3 %, Taeum-In's satisfaction was 66.0 %, Soum-In's satisfaction was 56.3 %, Soyang-In's satisfaction was 60.0 %. 3. The effective herb medicines were as follows, Soyang-In used the Hyong Bang Ji Hwang Tang (荊防地黃湯), Yuk Mi Ji Hwang Tang (六味地黃樓), Soum-In used the Sib Yi Mi Goan Jung Tang (十二味寬中湯), Taeum-In used the Chung Sim Yon Ja Tang (淸心蓮子陽), Chung Pae Sa Gan Tang (淸師爾肝湯), Yeol Da Han So Tang (熱多寒少湯). 4. The period of remedy was about 6 weeks. The period of remedy of each types was as follows, Taeum-In was about 5.7 weeks, Soum-In was about 6.8 weeks, Soyang-In was about 4.2 weeks. 5. The method of Lumbago remedy is divided three types, sprain Lumbago, Pyobyong (表病 : outside Syndromes) Libyong (裡病 : inside symdromes). Soum-In's methods are Pyobyong's ascending the Yang (陽), adding the Gi (氣) [升陽益氣], and Libyong's descending the inside Yim (裡陰) [裡陰降氣], Soyang-In's methods are Pyobyongs's decending the outside-Yim [表陰降氣], and Libyong's ascending the cool Yang (濟陽) [淸陽上升]. Taeum-In's methods are Pyobyong's ascending the Lung's Yang (肺陽升氣), and Libyong's colding the dried hot liver (淸肝燥熱). Taeyang's methods are strong the liver and making Yim. (補r肝生陰) 6. There are two methods for using the YakChim (Korean herb-acupuncture) by Sasang constitution medicine, one is to select the Yakchim, the other is to choice the point for appling the Yakchim. The first, to select the Yakchim, the other is follows; Soum-In can select the bee Venom, Soyang-In can select the H.O. (Hong Whoa 紅花), Taeum-In can select the I (Hodo 胡挑), V, O.K. (Ungdarn, 薦膽), Uwhang 牛黃, Sa-Hyang 麝香, etc., Palgang Yakchim (eight principles Korean herb-acupuncture (八剛藥鐵)) could made by abstracted Sasang herb medicine. The second, to choice the points for applying the Yakchim are used in the TaeGiuk Acupuncture method (太梗針法), Sacho (四焦, four warmer) by Sasang constritutional physiology and pathology.

A Study on Breast milk volume of Women according to Sasang Constitution (산후조리원에 입원한 산모의 사상체질과 1회 유축한 모유량과의 상관성에 관한 연구)

  • Purpose : Although the breast milk is the very important physiological function to women, there is no previous study on the breast milk volume of women according to Sasang Constitution. this study was to analyze the difference of breast milk volume pattern according to Sasang Constitution. Methods : This study investigated 109 breast feeding women who were treated by Spostpartum care center from March 2005 to November 2005. The result of survey was collected from the questionnaires that included postpartum pattern and QSCC H .Results : The results of Sasang Constitution analysis showed that 53 women in childbirth of 109 were classified into Soeumin(48.6%), 27 as Taeumin(24.8% ) and 29 as Soyangin(26.6% ). Mean milk volume according to the Sasang Constitution showed statistically no difference between Soeumin and Taeumin, but mean milk volume according to the Sasang Constitution showed statistically the significant difference between Soyangin and Soeumin, Soyangin and Taeumin. Conclusion These results suggest that milk volume of Soyangin is much larger than that of Soeumin and Taeumin.

A Survey on Korean Medicine Doctors' Recognition and Treatment for Developing Korean Medicine Clinical Practice Guideline of Coldness of Hands and Feet (한의표준임상진료지침 개발을 위한 수족냉증에 대한 한의사의 인식과 치료현황)

  • Objectives: The purpose of this study were to researched a Korean medicine doctors' recognition about coldness of hands and feet, and developing of korean medicine clinical practice guidelines (CPG) for coldness of hands and feet. Methods: We conducted a questionnaire survey targeting 399 Korean medicine doctors belonging to the Association of Korean Medicine by e-mail and analyzed the answers. Results: 1. 86.86% of the respondents agreed about the necessity of CPG for coldness of hands and feet. 2. 84.2% of respondents wanted coding of Korean Standard Classification of Diseases (KCD) on coldness of hands and feet. 3. To diagnosis a coldness of hands and feet, the respondents used a Subjective symptoms (98.5%), Infrared thermographic imaging device (DITI) (26.32%) Heart rate variablity test (HRV) (17.04%), Thermometer (9.77%), Cold stress test (2.76%) 4. Causing of coldness of hands and feet, the respondents considered a constitution or heredity (84.71%), stress (73.66%), lack of exercise (64.91%), irregular eating habits (51.63%), Cold meals (32.83%), depression (31.33%), etc. 5. Treating coldness of hands and feet, the respondents used a herbal medicine (66.85%), acupuncture (70.7%) Pharmacopuncture (23.85%) and moxibustion (60.08%) for $10.91{\pm}8.03week$. Conclusions: We researched a Korean Medicine doctors' recognition of CPG, clinical diagnosis, treatment on a coldness of hands and feet, and policy they required.

Improvement of Symptoms after Excision of Hydrocele of Canal of Nuck with Korean Medicine: A Case Report (누크관 수종 제거 이후 후유증에 대한 한방치료 1례 : 증례보고)

  • Objective: The purpose of this case study is to report the hydrocele of the canal of Nuck and the effectiveness of Korean medicine in post-surgery improvement of symptoms. Method: A patient who was diagnosed with hydrocele of the Nuck canal and had symptoms after Lichtenstein's operation was treated using Korean medicine, including acupuncture, herbal medicine, and moxibustion, every day. The evaluation was performed using the Numeral Rating Scale (NRS), the 5-level EQ-5D version (EQ-5D-5L), the EQ visual analogue scale (EQ-VAS), and the World Health Organization Quality of Life-Brief (WHOQOL-BREF). Results: After treatment, changes were observed in NRS (from 8 to 2-3) and EQ-VAS (from 60 to 85), whereas there was no difference in EQ-5D-5L (both 6). There was an overall improvement in the domains of WHOQOL-BREF and subjective symptoms. Conclusion: This study suggests that Korean medicine may improve symptoms after surgery of hydrocele of the canal of Nuck and can be evaluated using an overall QoL questionnaire.

  • We tried to observe the features of ancient medical practice by analysing the records related to medicine in the book, ${\ulcorner}$the Historical Records of the Three Kingdom${\lrcorner}$ of which content includes the features of medicine in mythology, plague, delivery of twins, drugs, medical system, shamanism, constitutional medicine, psychiatry, forensic medicine, deformity, a spa, medical phrase, health and welfare work, religion, death. physiological anatomy, Taoist medicine, acupuncture, the occult af of transformation and etc. Our initial concern was about where to draw line as of medical field and we defined medicine in more broad meaning. The book ${\ulcorner}$the Historical Records of the Three Kingdoms${\lrcorner}$ describes the world of mythology by way of medicine which is not clearly a conventional one. There appears records of birth of multiple offsprings 7 times in which cases are of triplets or more. Delivering multiple offsprings were rare phenomenon though such fertility was highly admired. This shows one aspect of ancient country having more population meant more power of the nation. Of those medical records conveyed in that book includes stories of childbirth such as giving birth to a son after praying, giving birth to Kim Yoo-shin after 20 months after mother's dream of conception, and a song longing for getting a laudable child. Plagues were prevalent throughout winter to spring season and one can observe various symptoms of plagues in the record. Of these epidemic diseases, cold type might have been more common than the heat one. Appearance of epidemic diseases frequently coincided with that of natural disasters that this suggests a linkage between plague and underlying doctrine on five elements' motion and six kinds of natural factors. There exists only a few names of diseases such as epidemic disease, wind disease, and syndrome characterized by dyspnea. Otherwise there appeared only afflictions that were not specified therefore it remains cluless to keep track of certain diseases of prevalence. Since this ${\ulcorner}$Historical Records of the Three Kingdoms'${\lrcorner}$ wasn't any sort of medical book, words and terms used were not technical kind and most were the ones used generally among lay people. Therefore any mechanisms of the diseases were hardly mentioned. Some of medicinal substances such as Calculus Bovis, Radix Ginseng, Gaboderma Luciderm, magnetitum were also in use in those days. 53 kinds of dietary supplies appears in the records and some of these might have been used as medicinal purpose. Records concerning dicipline of one's body includes activities such as hunting, archery, horseback riding etc. In Shilla dynasty there were positions such as professor of medicine, Naekongbong(內供奉), Kongbong's doctor(供奉醫師), Kongbong's diviner(供奉卜師). As an educational facility, medical school was built at the first year of King Hyoso's reign and it's curricula included various subjects as ${\ulcorner}$Shin Nong's Herbal classic${\lrcorner}$, ${\ulcorner}$Kabeul classic of acupuncture and moxbustion${\lrcorner}$, ${\ulcorner}$The Plain Questions of the Yellow Emperor's Classic of Internal Medicine${\lrcorner}$, ${\ulcorner}$Classic of Acupuncturer${\lrcorner}$, ${\ulcorner}$The Pulse Classic${\lrcorner}$, ${\ulcorner}$Classic of Channels and Acupuncture Points${\lrcorner}$ and ${\ulcorner}$Difficult Classic${\lrcorner}$. There were 2 medical professors who were in charge of education. To establish pharmacopoeia, 2 Shaji(舍知), 6 Sha(史), 2 Jongshaji(從舍知) were appointed. In Baekje dynasty, Department of Herb was maintained. Doing praying for the sake of health, doing phrenology also can be extended to medical arena. Those who survived over 100 years of age appear 3 times in the record, while 98 appears once. The earliest psychiatrist Nokjin differentiated symptoms to apply either therapies using acupuncture and drug or psychotherapy. There appears a case of rape, a case of burying alive with the dead, 8 cases of suicide that can characterize a prototype of forensic medicine. Deformity-related records include phrases as follow: 'there seems protrudent bone behind the head', 'a body which has two heads, two trunks, four arms.', 'a body equipped with two heads' In those times spa can be said to be used as a place for he리ing, convalescence, and relaxation seeing the records describing a person pretended illness and went to spa to enjoy with his friends. Priest doctors and millitary surgeons were in charge of the medical sevice in the period of the Three Kingdoms by the record written by Mookhoja(墨胡子) and Hoonkyeom(訓謙). Poor diet and regimen makes people more vulnerable to diseases. So there existed charity services for those poor people who couldn't live with one's own capacity such as single parents, orphans, the aged people no one to take care and those who are ill. The cause of affliction was frequently coined with human relation. There appeared the phenomenon of releasing prisoners and allowing people to become priests at the time of king's suffering. Besides, as a healing procedure, sutra-chanting was peformed. There appears 10 cases of death related records which varies from death by drowning, or by freezing, death from animals, death from war, death from wightloss and killing oneself at the moment of spouse's death and etc. There also exist certain records which suggest the knowledge of physiology and anatomy in those times.
