  • Forel-Ule Index (FUI) is an index which classifies the colors of inland and seawater exist in nature into 21 gradesranging from indigo blue to cola brown. FUI has been analyzed in connection with the eutrophication, water quality, and light characteristics of water systems in many studies, and the possibility as a new water quality index which simultaneously contains optical information of water quality parameters has been suggested. In thisstudy, Ocean Colour-Climate Change Initiative (OC-CCI) based 4 km FUI was spatially downscaled to the resolution of 500 m using the Geostationary Ocean Color Imager (GOCI) data and Random Forest (RF) machine learning. Then, the RF-derived FUI was examined in terms of its correlation with various water quality parameters measured in coastal areas and its spatial distribution and seasonal characteristics. The results showed that the RF-derived FUI resulted in higher accuracy (Coefficient of Determination (R2)=0.81, Root Mean Square Error (RMSE)=0.7784) than GOCI-derived FUI estimated by Pitarch's OC-CCI FUI algorithm (R2=0.72, RMSE=0.9708). RF-derived FUI showed a high correlation with five water quality parameters including Total Nitrogen, Total Phosphorus, Chlorophyll-a, Total Suspended Solids, Transparency with the correlation coefficients of 0.87, 0.88, 0.97, 0.65, and -0.98, respectively. The temporal pattern of the RF-derived FUI well reflected the physical relationship with various water quality parameters with a strong seasonality. The research findingssuggested the potential of the high resolution FUI in coastal water quality management in the Korean Peninsula.

Clinical Characteristics and Comparison of the Various Methods Used for the Diagnosis of the New Influenza A Pandemic in Korea (한국에서의 2009 신종 인플루엔자 A의 임상양상과 다양한 진단 방법들의 비교)

  • Background: Laboratory diagnosis of new influenza A (H1N1) is crucial for managing patients and establishing control and prevention measures. We compared the diagnostic accuracies of the real time RT-PCR (rRT-PCR) test recommended for the confirmation of the new flu and the viral culture method used conventionally for viral disease with that of the rapid antigen test (RAT). Methods: We performed RAT, R-mix culture, and real-time PCR by using 861 respiratory samples collected from December 2009 to January 2010 and evaluated the abilities of these methods to detect new influenza A. The relationship among the positive rates of RAT, grades of culture, and the cycle threshold (Ct) values of rRT-PCR was also evaluated. Results: Of the 861 patients, 308 (35.8%) were diagnosed with new influenza A. The sensitivities, specificities, positive predictive values, and negative predictive values of the tests were respectively as follows: 59.7%, 99.5%, 98.4%, and 81.6% for RAT; 93.2%, 100%, 100%, and 96.3% for R-mix culture; and 95.8%, 100%, 100%, and 97.7% for rRT-PCR. Samples with weak positive grade in culture and those with Ct values of 30-37 in rRT-PCR showed positivities as low as 25.3% and 2.3% in RAT, respectively. The hospitalization rate and death rate of the confirmed patients were 3.2% and 0.3%, respectively, and gastrointestinal symptoms were observed in 7.2% of the patients. Conclusions: R-mix culture and rRT-PCR tests showed excellent reliability in the diagnosis of new influenza A and could be very useful, especially for samples with low viral load.

Postoperative Radiation Therapy for Chest Wall Invading pT3N0 Non-small Cell Lung Cancer: Elective Lymphatic Irradiation May Not Be Necessary (흉벽을 침범한 pT3N0 비소세포폐암 환자에서 수술 후 방사선치료)

  • Purpose: No general consensus has been reached regarding the necessity of postoperative radiation therapy (PORT) and the optimal techniques of its application for patients with chest wall invasion (pT3cw) and node negative (NO) non-small cell lung cancer (NSCLC). We retrospectively analyzed the PT3cwN0 NSCLC patients who received PORT because of presumed inadequate resection margin on surgical findings. Materials and Methods: From Aug. 1994 till June 2000, 21 pT3cwN0 NSCLC patients received PORT at Samsung Medical Center; all of whom underwent curative on-bloc resection of the primary tumor plus the chest wall and regional lymph node dissection. PORT was typically stalled 3 to 4 weeks after operation using 6 or 10 MV X-rays from a linear accelerator. The radiation target volume was confined to the tumor bed plus the immediate adjacent tissue, and no regional lymphatics were included. The planned radiation dose was 54 Gy by conventional fractionation schedule. The survival rates were calculated and the failure patterns analyzed. Results: Overall survival, disease-free survival, loco-regional recurrence-free survival, and distant metastases-free survival rates at 5 years were 38.8$\%$, 45.5$\%$, 90.2$\%$, and 48.1$\%$, respectively. Eleven patients experienced treatment failure: six with distant metastases, three with intra-thoracic failures, and two with combined distant and intra-thoracic failures. Among the five patients with intra-thoracic failures, two had pleural seeding, two had in-field local failures, and only one had regional lymphatic failure in the mediastinum. No patients suffered from acute and late radiation side effects of RTOG grade 3 or higher. Conclusion: The strategy of adding PORT to surgery to improve the probability, not only of local control but also of survival, was justified, considering that local control was the most important component in the successful treatment of pT3cw NSCLC patients, especially when the resection margin was not adequate. The incidence and the severity of the acute and late side effects of PORT were markedly reduced, which contributed to improving the patients' qualify of life both during and after PORT, without increasing the risk of regional failures by eliminating the regional lymphatics from the radiation target volume.

Prediction of Salvaged Myocardium in Patients with Acute Myocardial Infarction after Primary Percutaneous Coronary Angioplasty using early Thallium-201 Redistribution Myocardial Perfusion Imaging (급성심근경색증의 일차적 관동맥성형술 후 조기 Tl-201 재분포영상을 이용한 구조심근 예측)

  • Purpose: The amount of salvaged myocardium is an important prognostic factor in patients with acute myocardial infarction (MI). We investigated if early Tl-201 SPECT imaging could be used to predict the salvaged myocardium and functional recovery in acute MI after primary PTCA. Materials and Methods: In 36 patients with first acute MI treated with primary PTCA, serial echocardiography and Tl-201 SPECT imaging ($5.8{\pm}2.1$ days after PTDA) were performed. Regional wall motion and perfusion were quantified with on 16-segment myocardial model with 5-point and 4-point scaling system, respectively. Results: Wall motion was improved in 78 of the 212 dyssynergic segments on 1 month follow-up echocardiography and 97 on 7 months follow-up echocardiography, which were proved to be salvaged myocardium. The areas under receiver operating characteristic curves of Tl-201 perfusion score for detecting salvaged myocardial segments were 0.79 for 1 month follow-up and 0.83 for 7 months follow-up. The sensitivity and specificity of Tl-201 redistribution images with optimum cutoff of 40% of peak thallium activity for detecting salvaged myocardium were 84.6% and 55.2% for 1 month follow-up, and 87.6% and 64.3% for 7 months follow-up, respectively. There was a linear relationship between the percentage of peak thallium activity on early redistribution imaging and the likelihood of segmental functional improvement 7 months after reperfusion. Conclusion: Tl-201 myocardial perfusion SPECT imaging performed early within 10 days after reperfusion can be used to predict the salvaged myocardium and functional recovery with high sensitivity during the 7 months following primary PTCA in patients with acute MI.

A Study on Estimating Shear Strength of Continuum Rock Slope (연속체 암반비탈면의 강도정수 산정 연구)

  • Considering the natural phenomenon in which steep slopes ($65^{\circ}{\sim}85^{\circ}$) consisting of rock mass remain stable for decades, slopes steeper than 1:0.5 (the standard of slope angle for blast rock) may be applied in geotechnical conditions which are similar to those above at the design and initial construction stages. In the process of analysing the stability of a good to fair continuum rock slope that can be designed as a steep slope, a general method of estimating rock mass strength properties from design practice perspective was required. Practical and genealized engineering methods of determining the properties of a rock mass are important for a good continuum rock slope that can be designed as a steep slope. The Genealized Hoek-Brown (H-B) failure criterion and GSI (Geological Strength Index), which were revised and supplemented by Hoek et al. (2002), were assessed as rock mass characterization systems fully taking into account the effects of discontinuities, and were widely utilized as a method for calculating equivalent Mohr-Coulomb shear strength (balancing the areas) according to stress changes. The concept of calculating equivalent M-C shear strength according to the change of confining stress range was proposed, and on a slope, the equivalent shear strength changes sensitively with changes in the maximum confining stress (${{\sigma}^{\prime}}_{3max}$ or normal stress), making it difficult to use it in practical design. In this study, the method of estimating the strength properties (an iso-angle division method) that can be applied universally within the maximum confining stress range for a good to fair continuum rock mass slope is proposed by applying the H-B failure criterion. In order to assess the validity and applicability of the proposed method of estimating the shear strength (A), the rock slope, which is a study object, was selected as the type of rock (igneous, metamorphic, sedimentary) on the steep slope near the existing working design site. It is compared and analyzed with the equivalent M-C shear strength (balancing the areas) proposed by Hoek. The equivalent M-C shear strength of the balancing the areas method and iso-angle division method was estimated using the RocLab program (geotechnical properties calculation software based on the H-B failure criterion (2002)) by using the basic data of the laboratory rock triaxial compression test at the existing working design site and the face mapping of discontinuities on the rock slope of study area. The calculated equivalent M-C shear strength of the balancing the areas method was interlinked to show very large or small cohesion and internal friction angles (generally, greater than $45^{\circ}$). The equivalent M-C shear strength of the iso-angle division is in-between the equivalent M-C shear properties of the balancing the areas, and the internal friction angles show a range of $30^{\circ}$ to $42^{\circ}$. We compared and analyzed the shear strength (A) of the iso-angle division method at the study area with the shear strength (B) of the existing working design site with similar or the same grade RMR each other. The application of the proposed iso-angle division method was indirectly evaluated through the results of the stability analysis (limit equilibrium analysis and finite element analysis) applied with these the strength properties. The difference between A and B of the shear strength is about 10%. LEM results (in wet condition) showed that Fs (A) = 14.08~58.22 (average 32.9) and Fs (B) = 18.39~60.04 (average 32.2), which were similar in accordance with the same rock types. As a result of FEM, displacement (A) = 0.13~0.65 mm (average 0.27 mm) and displacement (B) = 0.14~1.07 mm (average 0.37 mm). Using the GSI and Hoek-Brown failure criterion, the significant result could be identified in the application evaluation. Therefore, the strength properties of rock mass estimated by the iso-angle division method could be applied with practical shear strength.

Effects of Concurrent Chemotherapy and Postoperative Prophylactic Paraaortic Irradiation for Cervical Cancer with Common Iliac Node Involvement (자궁경부암의 근치적 절제술 후 총장골동맥림프절 침범 시 동시항암화학치료와 예방적 대동맥주위림프절 방사선조사의 효과)

  • Purpose: To retrospectively assess the advantages and side effects of prophylactic Paraaortic irradiation in cervical cancer patients with common iliac nodal involvement, the results for survival, patterns of failure, and treatment-related toxicity. Materials and Methods: From May 1985 to October 2004, 909 patients with cervical carcinoma received postoperative radiotherapy at the Seoul National University Hospital. Among them, 54 patients with positive common iliac nodes on pathology and negative Paraaortic node were included in the study. In addition, 44 patients received standard pelvic irradiation delivered 50.4 Gy per 28 fractions (standard irradiation group), and chemotherapy was combined in 16 of them. The other 10 patients received pelvic irradiation at a dose of 50.4 Gy per 28 fractions in addition to Paraaortic irradiation at 45 Gy per 25 fractions (extended irradiation group). In addition, all of them received chemotherapy in combination with radiation. Follow-up times for pelvic and Paraaortic irradiation ranged from 6 to 201 months (median follow-up time, 58 months) and 21 to 58 months (median follow-up time, 47 months), respectively. Results: The 4-year overall survival, disease free survival, and distant metastasis free survival in the standard irradiation group and extended irradiation group were 67.2% vs. 90.0% (p=0.291), 59.0% vs. 70.0% (p=0.568) and 67.5% vs. 90.0% (p=0.196), respectively. The most common site of first failure for the standard irradiation group was the paraaortic lymph node, while no paraaortic failure was observed in the extended irradiation group. Relatively, hematologic toxicity grade 3 or greater was common in the extended irradiation group (2/10 extended vs. 2/44 standard), while gastrointestinal toxicity of grade 3 or greater was lower (2/10 extended vs. 6/44 standard), and urologic toxicity of grade 3 or greater was observed in the standard irradition group only (0/10 vs. 3/44). Conclusion: Concurrent chemotherapy and prophylactic Paraaortic irradiation in patients with common iliac nodal involvement showed slightly improved clinical outcomes aside from increased hematologic toxicity, which was statistically insignificant. Considering the relatively small number of patients and short follow-up times, additional studies are needed to obtain more conclusive outcomes.

Surgery Alone and Surgery Plus Postoperative Radiation Therapy for Patients with pT3N0 Non-small Cell Lung Cancer Invading the Chest Wall (흉벽을 침범한 pT3N0 비소세포폐암 환자에서 수술 단독과 수술 후 방사선치료)

We did retrospective analyses on the pT3cwN0 NSCLC patients who received PORT because of presumed inadequate resection margin on surgical findings. And we compared them with the pT3cwN0 NSCLC patients who did not received PORT during the same period. Material and Method: From Aug. of 1994 till June of 2002, 22 pT3cwN0 NSCLC patients received PORT-PORT (+) group- and 16 pT3cwN0 NSCLC patients had no PORT-PORT (-) group. The radiation target volume for PORT (+) group was confined to the tumor bed plus the immediate adjacent tissue only, and no regional lymphatics were included. The prognostic factors for all patients were analyzed and survival rates, failure patterns were compared with two groups. Result: Age, tumor size, depth of chest wall invasion, postoperative mobidities were greater in PORT (-) group than PORT (+) group. In PORT (-) group, four patients who were consulted for PORT did not receive the PORT because of self refusal (3 patients) and delay in the wound repair (1 patient). For all patients, overall survival (OS), disease-free survival (DFS), loco-regional recurrence-free survival (LRFS), and distant metastases-free survival (DMFS) rates at 5 years were 35.3%, 30.3%, 80.9%, 36.3%. In univariate and multivariate analysis, only PORT significantly affect the survival. The 5 year as rates were 43.3% in the PORT (+) group and 25.0% in PORT (-) group (p=0.03). DFS, LRFS, DMFS rates were 36.9%, 84.9%, 43.1 % in PORT (+) group and 18.8%, 79.4%, 21.9% in PORT(-) group respectively. Three patients in PORT (-) group died of intercurrent disease without the evidence of recurrence. Few suffered from acute and late radiation side effects, all of which were RTOG grade 2 or lower. Conclusion: The strategy of adding PORT to surgery to improve the probability not only of local control but also of survival could be justified, considering that local control was the most important component in the successful treatment of pT3cw NSCLC patients, especially when the resection margin was not adequate. Authors were successful in the marked reduction of the incidence as well as the severity of the acute and late side effects of PORT, without taking too high risk of the regional failures by eliminating the regional lymphatics from the radiation target volume.

Vegetation Structure and Population Dynamics of Berchemia racemosa Habitats (청사조(Berchemia racemosa) 자생지의 식생구조 및 개체군 동태 분석)

  • The objectives of this study are to investigate and analyze the vegetation structure and population dynamics of Berchemia racemosa habitats in the Weolmyung park in Gunsan city, and base on that to seek the ecological habitat conservation plan for the Berchemia racemosa. In results, the Berchemia racemosa habitats are located at $81{\sim}93$ meters above the sea level, in steep seaside slope of a mountain. The soil texture are silt loam mainly and soil pH were $4.1{\sim}5$. The vascular plants in the Berchemia racemosa habitats has been analyzed as 61 taxa; 33 families, 51 genera, 54 species, 6 varieties, and 1 forms. Berchemia racemosa as a Specific plant species by floral region was the class V. Berchemia racemosa habitats were classified into 7 vegetation communities of Quercus serrata community(A1), Alnus firm a community(A2), Platycarya strobilacea community(A3), Robinia pseudoacacia community(A4) and 3 Pinus densiflora communities(B1, B2, B3). The importance value of Berchemia racemosa were 30%(A1), 15%(A2), 27%(A3), 65%(A4), 18%(B1), 45%(B2) and 35%(B3) on shrubs layer and 12, 27, 20, 18, 11, 18, 21 % on herb layer. The constant companion species with Berchemia racemosa were Stephanandra incisa and Ligustrum obtusifolium. Total 103 populations appear in the 7 Berchemia racemosa habitats. Their spatial distribution pattern were clumped for the most part. The average height was 133cm, the root color diameter was 4.4cm and the ramification branch number was 9.4. From the results of this study, it is suggested the continued monitoring and the active protection measures for the Berchemia racemosa habitats.

Growth Characteristics and Forage Productivity of New Forage Barley Variety, 'Miho' (청보리 신품종 '미호'의 생육특성과 수량성)

  • The purpose of development new variety 'Miho' (Hordeum vulgare L.) is a favorite with livestock feed and develop varieties resistant to disease and lodging. 'Miho' was carrying out the growth habit of group III with green and mid-wide leaf. Awn that is related to preference of livestock is a semi-smooth awn. This cultivar had 96 cm of culm length, 650 of spikes $per\;m^2$. Heading date of 'Miho' was April 27, and maturing date on May 30, which were later than cultivar 'Youngyang'. It also showed strong winter hardiness, and similar resistance to shattering and bariy yellow mosaic virus (BaYMV) compared with those of check one. The best thing among the traits of a good quality with the plant green at the latter growing period. The average forage dry matter yield in the regional yield trial was about 13.1, 12.1 MT per ha in upland and paddy field, respectively, which were 9%, 2% higher than that of the check cultivar. It's also showed 6.8% crude protein, 27.1% ADF (acid detergent fiber), and 67.5% TDN (total digestible nutrients), including higher silage quality for whole crop barley. This cultivar would be suitable for the area whose average minimum temperature was above $-8^{\circ}C$ January in Korean peninsula.

Marine Environmental Characteristics in Western Coastal Waters of the South Sea of Korea (전남서부해역의 수질환경 특성)

  • This study monitored marine environments in system coastal waters of the South Sea of Korea during the period of 2005-2007 and analyzed the data from environmental parameters and nutrients, which contributed to fluctuate marine environmental characteristics in aquaculture. On the basis of the fluctuation of temperature depending on seasons and sampling sites, too seasons of winter and summer showed a remarkable temperature gap of $5^{\circ}C$ compared with spring and autumn. Salinity in spring and summer approached 9 psu in Morkpo. Most of sampling sites showed COD below 2mg/L, indicating somewhat optimal environmental conditions. T-N and T-P also had desirable horizontal distributions all year round the concentration of SS was higher, but Chi. a was well-distributed horizontally except for Morkpo. Marone environmental parameters of surface water were similar to those of bottome water except for Morkpo. Most of sampling sites showed the ratio of N/P below 16, impling that nitrogen played an important role in growth of phytoplankton as a limiting factor. The relationship between Chi. a and nutrients was positive linear, whereas the relationship between Chi. a and COD were very strong positive. winder, Spring and Summer showed the strong relationship between Chi. a and COD. In the analysis of dendogram based on environmental parameters using SPSS ver. 10.0, the station 7 did not belong to other stations, and kept an independent place. The relationships of waters between Jindo, Wando and Heanam ware close, compared with other waters. Consequently, these regions except for Morkpo into which fresh water is introduced by the dike showed somewhat optimal environmental parameters and nutrients in year, and were assumed to maintain the desirable marine conditions for aquaculture.
