Magnetic resonance imaging analysis of screw in-type lateral anchor pull-out in large to massive rotator cuff repair in patients older than 60 years

  • Background: This study was performed to identify the incidence of screw in-type lateral anchor pull-out in patients older than 60 years who underwent rotator cuff repair for large to massive rotator cuff tear (RCT). Methods: We reviewed 25 patients over 60 who were diagnosed with large to massive RCT and underwent arthroscopic rotator cuff repair in our hospital from March 2017 to February 2021. Preoperative tear size (anterior to posterior, medial to lateral) was measured via preoperative magnetic resonance imaging (MRI). All 25 patients underwent MRI scanning on postoperative day 1 and at 3 months after surgery. The change of anchor position was measured in axial views on MRI images postoperative day 1 and 3 months after surgery. And it was statistically compared according to bone mineral density (BMD), sex, and number of lateral anchors. Results: Two MRIs (postoperative day 1 and 3 months) in 25 patients were compared. Anchor pull-out occurred in six patients during 3 months (6.7%), and the mean pull-out length difference was 1.56 mm (range, 0.16-2.58 mm). There was no significant difference in the number of pull-out anchors, degree of pull-out difference by comparing BMD (A, BMD≤-2.5; B, BMD>-2.5), sex, or number of anchors used in each surgery (C, two anchors; D, three anchors) (p>0.05). Conclusions: Pull-out of screw in-type anchors was rarely observed and the mean pull-out length difference was negligibly small in our study. The screw in-type lateral anchor seems to be a decent option without concern of anchor pull-out even in elderly patients.

Development of Jacket Pattern Drafting Methods for Women with Turtle Neck Syndrome Body Shapes through Virtual Fit Assessment (가상착의를 활용한 거북목 증후군 체형의 여성을 위한 재킷패턴 제도법 개발)

  • The purpose of this study was to develop jacket patterns that could fit well for women in their 20s with turtle neck syndrome body shape and to present a pattern drafting method. As a research method, an educational pattern was chosen through a preliminary experiment. Fit problems of the educational pattern were derived by wearing it on a virtual model with a turtle neck syndrome body type for each neck angle. Based on the three types of experimental patterns developed by supplementing these problems, a development pattern drafting method was developed for each neck angle. Three development patterns were compared and evaluated with the educational pattern using 3D virtual fitting. Fit problems of the educational pattern included: 1) insufficient ease amounts for hip, hem, bicep, elbow, and sleeve hem circumference; 2) horizontal items could not be leveled due to insufficient back length; 3) the shoulder line went backwards; and 4) the front center line did not form a vertical line. To solve these problems, an experimental pattern was developed by increasing the insufficient ease amount, increasing the insufficient back length, and correcting the position of the shoulder line. Measurements that differed in the pattern drafting method of the three types of development patterns by neck angle were positions of back center line and back neck point, front length, and front neck width. Through appearance evaluation of the development pattern and educational pattern by neck angle, the development pattern was highly evaluated, indicating that problems of the educational pattern were improved.

  • This paper implements an AR manual for a vehicle that can be used even in the vehicle interior space where it is difficult to apply the augmentation method of AR content, which is mainly used, and applies a deep learning model to improve the augmentation matching between real space and virtual objects. Through deep learning, the logo of the steering wheel is recognized regardless of the position, angle, and inclination, and 3D interior space coordinates are generated based on this, and the virtual button is precisely augmented on the actual vehicle parts. Based on the same learning model, the function to recognize the main warning light symbols of the vehicle is also implemented to increase the functionality and usability as an AR manual for vehicles.

  • Additional functional upgrades to the large-area compton camera (LACC) measurement device that can provide characteristics evaluation information (nuclear species and radioactivity) and two-dimensional or three-dimensional distribution imaging information of radioactive materials existing in surface or internal of concrete structures are required in terms of work stability and efficiency in order to apply to actual decommissioning sites such as nuclear power plants or medical cyclotron facilities by using this measurement device. To this purpose, the technology that allows radiation workers to intuitively and visually check the distribution of radioactive materials in advance by matching the two-dimensional distribution imaging information of radioactive materials obtained through the LACC measurement device and visual imaging of the measurement zone (10 m × 5 m) was developed. In addition, the separate system that can automatically adjust the position (height) in units of the measurement area size (0.7 m × 0.3 m × 0.8 m) of the LACC measurement device was developed and the integrated management system for characteristics evaluation information and two-dimensional or three-dimensional distribution imaging information obtained per unit of measurement for radioactive materials was developed. These functional upgrades related to LACC measurement device can improve work efficiency and safety when measuring radioactivity of concrete structures and enable the establishment of appropriate decommissioning strategies using radioactivity measurement information for decommissioning nuclear power plants or medical cyclotron facilities.

  • In recent decades, the protection and vulnerability of civil structures under explosion loads became a critical issue in terms of security, which may cause loss of lives and structural damage. Concrete retaining walls also restrict soils and slopes from displacements; meanwhile, intensive temporary loading may cause massive damage. In the current study, the modified Johnson-Holmquist (also known as J-H2) material model is implemented for concrete materials to model damages into the ABAQUS through user-subroutines to predict the blasting-induced concrete damages and volume strains. For this purpose, a 3D finite-element model of the concrete retaining wall was conducted in coupled Eulerian-Lagrangian simulation. Subsequently, a blast load equal to 500 kg of TNT was considered in three different positions due to UFC 3-340-02. Influences of the critical parameters in smooth blastings, such as distance from a free face, position, and effective blasting time, on concrete damage rate and destroy patterns, are explored. According to the simulation results, the concrete penetration pattern at the same distance is significantly influenced by the density of the progress environment. The result reveals that the progress of waves and the intensity of damages in free-air blasting is entirely different from those that progress in a dense surrounding atmosphere such as soil. Half-damaged elements in air blasts are more than those of embedded explosions, but dense environments such as soil impose much more pressure in a limited zone and cause more destruction in retaining walls.

Preliminary Study on Factor Technology of Selective Catalytic Reduction System in Marine Diesel Engine (선박용 디젤엔진 SCR 시스템 요소 기술에 관한 기초 연구)

  • From 2016, controls on reduction of NOx and SOx emissions from the vessels that are operated in the emission control area were tightened. The selectivity catalytic reduction system of the denitrification equipment which NOx among the above controlled materials is very effective and used commercially very much. But it has the disadvantage that CSR is activated at high temperatures. Therefore, the SCR and SCR activation instrument that can react even at low temperatures by using micro-nano bubbles so that the above problems can be minimized were developed. And the computational fluid dynamics technique was used by ANSYS-CFX package to prepare the plan that improves the SCR system's efficiency. Simulation for the viscous flow analysis of the SCR system was executed by applying the Navier-Stokes equation to it as a governing equation. For the SCR system's shape, 3D modeling was done by using CATIA V5. SCR jet nozzle's position was checked by changing it to the intervals of 1/3, 1/2, and 2/3 from the inlet of the vent pipe to compare the SCR system's efficiency. And the number of nozzles was compared and analyzed by simulating 4, 6, and 8 holes to check an effect of the number on the SCR system's efficiency. The simulation result has found that the closer nozzles are to the inlet of the vent pipe and the more nozzles are, the more efficiency is improved.

Effect of Nurse's Self-Leadership, and Self-Efficacy on Job Involvement (간호사의 셀프리더십, 자기효능감이 직무몰입에 미치는 영향)

  • The purpose of this study was to analyze the factors influencing the clinical nurse's self-leadership and self-efficacy on job involvement. The survey was conducted from October 20 to December 15, 2017 for 205 nurses working at hospitals located in D city and K province of Korea. Analysis was performed with descriptive statistics, independent t-test, one-way ANOVA analysis, Scheffe's post-hoc test, Pearson correlation analysis, and stepwise multiple regression using the SPSS 21.0 for Windows. The results of the study were as follows. Average score for self-leadership was 3.58, for self-efficacy was 3.56, and for job involvement was 3.36. This study showed positive correlations between self-leadership, self-efficacy and job involvement. Self-leadership, marital status, self-efficacy, and job position were significant influences on job involvement. The explanatory power of influence factors was 43.7%. Based on the results of this study, it is necessary for nursing organizations and hospitals to actively support programs that enhance self-leadership and self-efficacy, which will be used to increase nurses' job involvement and improve productivity and efficiency of nursing work.

Development of an Automatic Seed Marker Registration Algorithm Using CT and kV X-ray Images (CT 영상 및 kV X선 영상을 이용한 자동 표지 맞춤 알고리듬 개발)

  • [ $\underline{Purpose}$ ]: The purpose of this study is to develop a practical method for determining accurate marker positions for prostate cancer radiotherapy using CT images and kV x-ray images obtained from the use of the on- board imager (OBI). $\underline{Materials\;and\;Methods}$: Three gold seed markers were implanted into the reference position inside a prostate gland by a urologist. Multiple digital image processing techniques were used to determine seed marker position and the center-of-mass (COM) technique was employed to determine a representative reference seed marker position. A setup discrepancy can be estimated by comparing a computed $COM_{OBI}$ with the reference $COM_{CT}$. A proposed algorithm was applied to a seed phantom and to four prostate cancer patients with seed implants treated in our clinic. $\underline{Results}$: In the phantom study, the calculated $COM_{CT}$ and $COM_{OBI}$ agreed with $COM_{actual}$ within a millimeter. The algorithm also could localize each seed marker correctly and calculated $COM_{CT}$ and $COM_{OBI}$ for all CT and kV x-ray image sets, respectively. Discrepancies of setup errors between 2D-2D matching results using the OBI application and results using the proposed algorithm were less than one millimeter for each axis. The setup error of each patient was in the range of $0.1{\pm}2.7{\sim}1.8{\pm}6.6\;mm$ in the AP direction, $0.8{\pm}1.6{\sim}2.0{\pm}2.7\;mm$ in the SI direction and $-0.9{\pm}1.5{\sim}2.8{\pm}3.0\;mm$ in the lateral direction, even though the setup error was quite patient dependent. $\underline{Conclusion}$: As it took less than 10 seconds to evaluate a setup discrepancy, it can be helpful to reduce the setup correction time while minimizing subjective factors that may be user dependent. However, the on-line correction process should be integrated into the treatment machine control system for a more reliable procedure.

Understanding the Protox Inhibition Activity of Novel 1-(5-methyl-3-phenylisoxazolin-5-yl)methoxy-2-chloro-4-fluorobenzene Derivatives Using Comparative Molecular Similarity Indices Analysis (CoMSIA) Methodology (비교 분자 유사성 지수분석(CoMSIA) 방법에 따른 1-(5-methyl-3-phenylisoxazolin-5-yl)methoxy-2-chlore-4-fluorobenzene 유도체들의 Protox 저해 활성에 관한 이해)

  • 3D QSAR studies for protox inhibition activities against root and shoot of the rice plant (Orysa sativa L.) and barnyardgrass (Echinochloa crus-galli) by a series of new 1-(5-methyl-3-phenylisoxazolin-5-yl)methoxy-2-chloro-4-fluorobenzene derivatives were conducted based on the results (Sung, N. D. et al.'s, (2004) J. Korean Soc. Appl. Biol. Chem. 47(3), 351-356) using comparative molecular similarity indices analysis (CoMSIA) methodology. Four CoMSIA models, without hydrogen bond donor field for the protox inhibition activities against root and shoot of the two plants, were derived from the combination of several fields using steric field, hydrophobic field, hydrogen bond acceptor field, LUMO molecular orbital field, dipole moment (DM) and molar refractivity (MR) as additional descriptors. The predictabilities and fitness of CoMSIA models for protox inhibition activities against barnyard-grass were higher than that of rice plant. The statistical results of these models showed the best predictability of the protox inhibition activities against barnyard-grass based on the cross-validated value $r^2\;_{cv}\;(q^2=0.635{\sim}0.924)$, non cross-validated, conventional coefficient $r^2\;_{ncv.}$ value $(r^2=0.928{\sim}0.977)$ and PRESS value $(0.255{\sim}0.273)$. The protox inhibition activities exhibited a strong correlation with the steric $(5.4{\sim}15.7%)$ and hydrophobic $(68.0{\sim}84.3%)$ factors of the molecules. Particularly, the CoMSIA models indicated that the groups of increasing steric bulk at ortho-position on the C-phenyl ring will enhance the protox inhibition activities against barnyard-grass and subsequently increase the selectivity.

Localization using Neural Networks and Push-Pull Estimation based on RSS from AP to Mobile Device (통신기지국과 모바일장치간의 수신신호강도를 기반으로 하는 신경망과 푸쉬-풀 평가를 이용한 위치추정)

  • Although the development of Global Positioning System (GPS) are more and more mature, its accuracy is just acceptable for outdoor positioning, not positioning for the indoor of building and the underpass. For the positioning application area for the indoor of building and the underpass, GPS even cannot achieve that accuracy because of the construction materials while the requirement for accurate positioning in the indoor of building and the underpass, because a space, a person is necessary, may be very small space with several square meters in the indoor of building and the underpass. The Received Signal Strength (RSS) based localization is becoming a good choice especially for the indoor of building and the underpass scenarios where the WiFi signals of IEEE 802.11, Wireless LAN, are available in almost every indoor of building and the underpass. The fundamental requirement of such localization system is to estimate location from Access Point (AP) to mobile device using RSS at a specific location. The Multi-path fading effects in this process make RSS to fluctuate unpredictably, causing uncertainty in localization. To deal with this problem, the combination for the method of Neural Networks and Push-Pull Estimation is applied so that the carried along the devices can learn and make the decision of position using mobile device where it is in the indoor of building and the underpass.
