A Clinical Study of the Effects of KamiWooseul-tang on Low Back Pain and Kidney Function (가미우슬탕의 요통치료효과 및 신기능에 미치는 영향에 대한 임상적 고찰)

  • Objectives : The aim of this clinical experiment was to investigate the effect of KamiWooseul-tang on low back pain and kidney function. Methods: We investigated 64 low back pain patients. We treated the patients with acupuncture, bed rest, herbal medicine, physical therapy. The patients were divided into two groups as follows, 32 patients (with normal renal function) were treated with only KamiWooseul-tang. Another 32 patients were treated with a commonly used herbal medicine. Results : The obtained results are summarized as follows. The patients were 43.8% male and 56.2% female. Their age were 20s 15.6%, 30s 25%, 40s 17.2%, 50s 9.3%, 60s 18.8% 70s 12.5% and 80s1.6%. Duration is most common in the most acute stage (< 1 week), next was chronic stage ( >6 months ), acute stage (1 week ~ 1 month) and subacute stage(l months ~ 6 months) in turn. Cure rate increased step by step after herb-medicine administration. Duration of patients had no relation with cure rate(P>0.05). The KamiWooseul-tang was more effective than the commonly used herbal medicine after 3 weeks therapy (P<0.05). Serum BUN and Creatinine level in KamiWooseul-tang group changed from $14.69{\pm}4.0,{\;}0.76{\pm}0.19(mg/dl)$ before adminstration to $13.64{\pm}3.92{\;}0.77{\pm}0.19,{\;}13.48{\pm}3.00{\;}0.82{\pm}0.21,{\;}13.26{\pm}3.73{\;}0.87{\pm}0.21(mg/dl)$ at 7th, 14th, and 21st days of administration respectively. Urinalysis showed no specific change in the KamiWooseul-tang group. During medication serum-electrolyte was within normal range. Conclusion : Therefore KamiWooseul-tang was more effective than the commonly used herbal medicine and there was no clinically remarkable difference in the serum BUN, Creatinine level, urinalysis and serum electrolyte between pre-medication and post-medication in the KamiWooseul-tang group.

  • The study was designated to investigate the gross findings following treatment with diethylnitrosamine (DEN) in rats. Forty four male rats (Sprague Dawley), initially 5 to 6 weeks of age and 120 to 150gm in body weight were continuously given water containing 0.01% DEN for 13 weeks and 3~6 rats per week were randomly sacrified at intervals of a week from 8 weeks to 17 weeks. The numbers of rats died until 17 weeks were 11 rats and first death occurred at 10 weeks. Body, liver and spleen weights were weighed and the relative weight levels of the liver and spleen per body weights were increased from 4.4% and 0.5% at control groups to 15.1% and 1.1% at 17 weeks. The numbers of RBC were not varied but PCV were decreased from 44.5% in control group to 27.5% in 16 weeks. A few of greyish-white foci or nodules of tumors were developed or not on the surfaces of the livers at 8 weeks but were developed on all livers after 9 weeks. The diameter of the largest tumor from all rat livers was 35.8 mm at 12 weeks. The numbers of developed tumors per a rat liver were appeared to be about 20 in 13 weeks and about 50~60 in 17 weeks. The diameter of some larger tumors were found to be 3~9mm in 13 weeks and 7~15mm in 17 weeks and more number of tumors were developed in the visceral surfaces than in the diaphragmatic surfaces of the livers. The parenchymae of livers were fragility.

A Comparative Study of Hataedock versus Probiotics on Immunomodulating Effect in Intestinal Mucosa (황련감초 하태독법과 프로바이오틱스의 대장점막 내 면역조절 효과 비교연구)

  • Objectives Hataedock is a treatment that dispels toxic heat and meconium which has been accumulated to the fetus from a pregnant mother via orally administering herbal extracts to a newborn baby. This study was conducted to compare the efficacy of Hataedock, with using the extract of Coptis japonica & Glycyrrhiza uralensis, to the early administration of probiotics for immunomodulation in the intestinal mucosa. Methods NC/Nga mice were divided into three groups; Control group (no treatment), CGT group (3-week-old mice given the extract of Coptis japonica & Glycyrrhiza uralensis), and MBT group (3-week-old mice given a Bifidobacterium). After 2 weeks, the intestinal mucosa tissues of each group of mice were observed. Immunohistochemical staining for IL-4, IL-13, CD40, $Fc{\varepsilon}RI$, $p-I{\kappa}B$, EGF, and VEGF in the intestinal mucosa was performed. Results CGT group showed 65% decrease in IL-4, 67% decrease in IL-13, 58% decrease in CD40, 72% decrease in $Fc{\varepsilon}RI$, 76% decrease in $p-I{\kappa}B$, 41% increase in EGF and 100% increase in VEGF compared to the control group. MBT group also showed 50% decrease in IL-4, 63% decrease in IL-13, 33% decrease in CD40, 53% decrease in $Fc{\varepsilon}RI$, 46% decrease in $p-I{\kappa}B$, 23% increase in EGF and 151% increase in VEGF compared to the control group. Conclusions These results suggest that both Hataedock, with using the extract of Coptis japonica & Glycyrrhiza uralensis, and early administration of probiotics were effective in regulating Th2, relieving inflammation and developing intestinal mucosal tissues. Hataedock with extract of Coptis japonica & Glycyrrhiza uralensis may be more effective for immunomodulation in intestinal mucosa than probiotics.

Effect of Farfarae Flos Extract Drug on Immune Cells and Cytokines in BALF of OVA-induced Asthmatic Mice (관동화(款冬花)가 천식모델 생쥐의 BALF내 면역세포 및 사이토카인에 미치는 영향)

  • This experiment was designed to investigate the effect of Farfarae Flos(FF) on immune cells and cytokines in murine asthma model. C57BL/6 mice were sensitized and challenged with OVA(ovalbumin) for 9 weeks(3 times a week). The experimental group was treated with Farfarae Flos extract(FF) for the later 6 weeks(5 times a week). We measured IL-4, 1L-5, 1L-13, IgE, IFN-${\gamma}$ in bronchoalveolar lavage fluid of ovalbumin induced asthmatic mouse and observed murine lung tissue. The results were obtained as follows: Total leukocytes and eosinophils in BALF of the mice group treated with FF decreased significantly compared with those of control group. The concentration of IL-4, IL-5, IL-13, IgE in BALF of the mice group treated with FF decreased significantly compared with those of control group. The concentration of IFN-${\gamma}$ in BALF of the mice group treated with FF increased significantly compared with that of control group. According to the above results, it is suggested that FF extract might be useful applied for prevention and treatment of allergic asthma.

A Case of Lyme Disease Complicated with Atrioventricular Block in a 13-year-old Boy

  • Lyme disease is a common vector-borne disease caused by Borrelia burgdorferi. Erythema migrans represents the most common manifestation during the early phase of this disease; however, systemic manifestations involving the nervous system, joints, or heart are known to occur. We report a case of Lyme disease accompanied by cardiac complications in a 13-year-old Korean male adolescent. The patient developed annular erythematous lesions on his lower extremities after a field trip during his visit to Connecticut, USA, for a boarding school camp, and his skin lesions were consistent with erythema migrans. Indirect immunofluorescence assay and Western blot analysis for Lyme immunoglobulin M showed positive results. Electrocardiography revealed a first-degree atrioventricular block, and he was diagnosed with Lyme carditis and received a 4-week course of oral doxycycline. Follow-up electrocardiography performed a week later revealed normal findings, and the patient showed an uneventful recovery. Lyme carditis often presents as an asymptomatic heart block that can progress to an advanced heart block; however, this condition is reversible with appropriate antibiotic treatment.

Exploring the Effects of Factors on Hollywood Movies Box Office Success : Focusing on Top-rated Movies on the First Week of Release (할리우드 영화의 흥행요인에 관한 연구 : 개봉 첫 주 흥행 1위 작품을 중심으로)

  • This study examines the impact of factors that affect Hollywood movies' box office performance, using a sample of 149 movies that were ranked number 1 on the first week of release between 2014 and 2018. Such factors as production budget, the number of opening screens, the valence and volume of both critics and netizens, rating, distributor's power, remake, high season, genre are introduced to analyze box office performance. The result shows that the determinants of the US box office and the total box office are different. Considering the difference in budget by genre, the number of opening screens is not influential in the US box office performance of all genres with production budget of less than $100 million. The result suggests the need to refine more elaborated models for future research on box office performance.

  • Objective: This study aims to explore the relationship between isokinetic strength training and the enhancement of physical function in older adults. It also evaluates the effectiveness of isokinetic strength training equipment. Method: This study randomly divided twenty healthy adults into two groups.The experimental group engaged in isokinetic strength training for 30 minutes, three times a week, over a four-week period. In contrast, the control group did not participate in any exercise regimen. We assessed several outcome measures including physical functions (sit-to-stand test, flexibility, Timed Up and Go test), grip strength, balance, thigh circumference, the Korean version of the Patient Health Questionnaire-9 (PHQ-9), and the Korean version of the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Additionally, the experimental group's self-perceived improvement in lower limb condition was evaluated using the Global Rating of Change (GROC) scale. Results: The experimental group showed significant improvements compared to the control group in various aspects: reduced sit-to-stand test duration (7.00±2.05 seconds), increased flexibility (4.69±3.90 cm), improved grip strength (1.54±1.74 kg), increased thigh circumference (left: 1.29±1.19 cm, right: 1.19±1.27 cm), enhanced Timed Up and Go test performance (-1.47±0.86 seconds), better balance (eyes open stance: -8.08±4.03 cm, eyes closed stance: -0.27±0.13 cm/s), and a decrease in depression severity (-0.15±1.51 points). Furthermore, significant increases were observed in the experimental group's maximal isokinetic strength at 90°/s for both flexion (19.62±7.03 Nm) and extension (19.60±14.65 Nm) over the study period. Conclusion: The findings suggest that isokinetic strength training equipment can significantly enhance physical functions in seniors when incorporated into an exercise regimen.

  • The potential toxicological effects of aristolochic acid (AA), a natural component in Aristolochiaceae family, were investigated. The 13-week toxicity study consisted of groups of 10 SD rat/sex administrated water containing 0, 0.05, 0.5, or 5 mg/kg per day AA (Study 1). The tested groups were terminated on Test Day 90 due to mortality and overt clinical signs of toxicity.(omitted)

Clinical Studies on the Effect of Yogurt Toward the Constipation of Female College Students in Korea (요구르트가 여자대학생의 변비에 미치는 임상적 연구)

  • In order to study the prevalence of constipation among female college students in Korean and the effect of yogurt, 3,243 students in Seoul area were surveyed by questionnaires for the status of stool evacuation and 197 students with possible constipation were sampled and fed apple yogurt II from Pasteur dairy Company for 3-weeks period. Students of constipation amounted to a high ratio of 53.4%, which consisted of 13.4% in heavy constipation (1-2 times evacuation per week) and 40.0% in mild constipation(3-4times evacuation per week) during the past one-year period. To 90 of heavy constipation and 107 of mild constipation students were fed 290ml of yogurt every morning for 3-weeks period and examined for the status of stool evacuation by subsequent after 1 weeks, 57.9% after 2 weeks and 60.4% after 3 weeks(p<0.05). The effect of improvement in stool frequency was greater in heavy group than in mild group through the 3-weeks period. The number of students who experienced the disappearance of inconvenience in evacuation after drinking the yogurt was 52.3% after 1 weeks, 72.1% after 2 weeks and 76.1% after 3 weeks. Discomfort such as stomach disorder and mild diarrhea during the drinking period was observed in 24% students and it appeared that a longer drinking period was needed stool frequency was 38.7% after 1 week and 37.6% after 1 month since discontinuing yogurt drinking. The number of students reverted to the previous habit of evacuation after discontinuing was 51.6% after 1 week and 63.5% after 1 month, which were not significantly different between heavy and mild groups(p>0.05).

Effect of processed foods on serum levels of eosinophil cationic protein among children with atopic dermatitis

  • The prevalence of atopic dermatitis (AD) in school-age children has increased in industrialized countries. As diet is one of the main factors provoking AD, some studies have suggested that food additives in processed foods could function as pseudoallergens, which comprise the non-immunoglobulin E-mediated reaction. Eosinophil cationic protein (ECP) is an eosinophil granule protein released during allergic reactions to food allergens in patients with AD. Thus, serum ECP levels may be a useful indicator of ongoing inflammatory processes in patients with AD. The purpose of this study was to investigate the effect of consuming MSG in processed foods on serum ECP levels among children with AD. This study was performed with 13 patients with AD (age, 7-11 years) who had a normal range of total IgE levels (< 300 IU/ml). All participants ate normal diets during the first week. Then, six patients were allocated to a processed food-restricted group (PRDG) and seven patients were in a general diet group (GDG). During the second week, children in the PRDG and their parents were asked to avoid eating all processed foods. On the third week, children in the PRDG were allowed all foods, as were the children in the GDG throughout the 3-week period. The subjects were asked to complete a dietary record during the trial period. Children with AD who received the dietary restriction showed decreased consumption of MSG and decreased serum ECP levels and an improved SCORing score on the atopic dermatitis index (P < 0.05). No differences in serum ECP levels or MSG consumption were observed in the GDG. Serum total IgE levels were not changed in either group. In conclusion, a reduction in MSG intake by restricting processed food consumption may lead to a decrease in serum ECP levels in children with AD and improve AD symptoms.
