A Survey of Physical Therapy Clinical Progress Note in Korea (한국 물리치료사의 임상기록에 대한 조사연구)

  • 환자에 대한 기록은 일정한 형식을 가지고 환자 내원 초기부터 환자 및 의료진의 요구와 관리목적에 부합한 효과적인 평가, 치료계획, 치료적 중재를 수립할 수 있게 만드는 필수적인 일련의 상호관련학적 의사소통 도구임과 동시에 치료사가 환자관리에 포함된 사고 과정을 분석하는 데 도움을 주며 문제해결에 대한 사고를 체계화하는데 있어 중요한 역할을 한다. 본 연구의 목적은 물리치료사의 일반적 특성과 임상기록의 실태, 임상기록의 중요성을 인식하고 임상활동의 문제점을 개선하며 기록의 체계화를 이루어 임상기록의 활성화와 나아가 물리치료의 질적 향상과 물리치료 서비스에 대한 효율성을 높이는데 있다. 이 연구는 전국 의료기관이나 보건관련기관에서 근무하는 물리치료사를 선정하였다. 2005년 7월1일부터 8월30일 사이에 구조화된 설문지 320매를 직접 전달해 치료사가 작성한 설문지 314매를 직접 수거하였다. 분석 방법으로는 백분율과 빈도, Kuskal-Wallis Test. Pearson Chi-Square, Mann-Whitney U로 검정하였다. 잘못 표기한 응답자 1명을 제외한 응답자 총 313명 중 남자 142명 (45.2%), 여자 171명 (54.5%) 이었으며, 25세 이상 29세 이하 연령층이 119명 (37.9%)으로 가장 많이 분포하였다. 근무처에 따른 근무 개월, 일일 평균 환자수는 통계학적 검정으로 유의한 차이가 있는 것으로 나타났다(p<.05). 근무처에 따른 임상기록 작성에서도 통계학적 검정이 유의하였다(p<.05) 임상기록 작성이 효율적인 치료. 치료사 만족도에도 통계학적으로 유의한 결과가 나타났다(p<.05).

  • 심실세동은 환자의 생명을 위협하는 가장 치명적인 부정맥으로서, 심실세동의 발견즉시 특별한 조치를 취하지 못할 경우 환자는 급사한다. 심실세동을 정지시키는 유일한 방법은 전기적 제세동이며 이는 환자의 심장 부위에 전기적 에너지를 가해 정상 심장 리듬을 되찾도록 하는 방법이다. 심실세동의 발생은 예측할 수 없고, 이를 진단하기 위한 심전도 교육을 모든 의료인에게 할 수 없으므로 심전도를 자동 분석하여 심실세동을 조기에 검출하는 자동 분석장치의 개발은 심실세동에 의한 급사를 줄이는데 매우 중요하다. 본 연구에서는 교차간섭성분의 제거 능력이 뛰어나며 쉽게 구사할 수 있는 Choi-Williams distriguteion (CWD) 방법을 이용하여 급사를 초래할 수 있는 심실세동을 자동 검출하는 알고리즘을 개발하였다. 그리고 개발된 알고리즘의 성능을 검증하기 위하여 본 논문에서는 두 그룹의 심실세동 데이터를 사용하였다. 검출 알고리즘은 심실세동의 시간 및 주파수 특성을 일곱가지 조건들에 대한 만족 여부를 이용하여 심실세동을 판정하였고, 실험의 민감도와 선택도는 각각 92.1%, 97 .2% 이다.

  • 후두 악성종양의 치료로서 후두전적출술을 시행하는 경우 환자는 의사 전달수단인 언어의 장애를 동반하게 된다. 후두전적출술후 언어장애를 해결하기 위해 여러종류의 음성재활법이 제시되어 만족할 만한 결과를 얻고 있으나 그중 식도발성은 대부분의 학자들이 인정하는 가장 추천할 만한 방법이지만 시간이 걸리고 배우기가 어려워 환자들이 쉽게 단념할 수 있다는 단점이 있다. 저자들은 후두전적출술을 받은 10명의 환자에서 인두식도분절을 fluoroscopy로 관찰하여 다음과 같은 결과를 얻었기에 보고하는 바이다. 1) 식도발성을 잘하는 예에서는 pseudoglottis의 전후길이가 좌우길이보다 길었고 식도발성을 못하는 9례에서는 좌우길이가 전후길이보다 크거나 길었다. 2) 식도발성을 못하는 9례중 7례에서 인두식도 분절을 관찰할 수 있었다. 3) 인두식도분절에서 pseudoglottis의 모양이 식도발성을 잘하는 예에서는 정상성대와 유사하게 나타났다. 4) 식도발성을 잘하는 예에서는 pseudoglottis의 위치가 윤상인두괄약근 상방 2cm부위에서 관찰되었으며 식도발성을 못하는 9례중 6례에서는 pseudoglottis의 위치가 윤상인두괄약근 위치였으며 2례에서는 pseudoglottis가 각각 윤상인두괄약근상방 1cm, 1.5cm에 있었다.

  • 유비쿼터스 시대를 맞이해서 휴대용 진단 진료 기기를 환자가 쉽게 휴대 및 착용해서 환자의 질병 및 건강 상태를 모니터링 하여 위험한 상황을 미리 예측, 통지가 가능한 시스템을 발전시켜야 할 필요성이 대두되고 있다. 이를 위하여 다양한 특성을 가진 휴대용 의료 계측 기기 센서들을 환자의 신체에 부착하고 다양한 센서들이 네트워크를 형성하여 인터넷과 통신하는 것은 물론 이동이 가능해야 한다. 즉, 센서 네트워크 기술과 이동성 기술이 접목된 네트워킹 기술이 필요하지만 아직까지 실질적인 적용에 대한 구체적인 연구는 없는 형편이다. 따라서 본 논문에서는 이러한 요구사항을 만족시키는 U-헬스케어 시스템을 구축하기 위해 각 센서 노드에 IPv6 를 적용하여 항상 인터넷에 연결되어 실시간 모니터링이 가능함과 동시에 네트워크 단위의 이동성을 지원하는 프로토콜과 연동하는 시나리오와 구조를 제시한다.

A prospective study of patient satisfaction after implant supported mandibular overdenture treatment in fully edentulous patients (완전 무치악 환자에서 하악 임플란트 지지 피개의치 사용 후의 환자만족도에 관한 전향적 연구)

  • Purpose: The purpose of this prospective study was to compare the satisfaction levels of the patients worn implant supported mandibular overdentures and conventional complete dentures. Materials and methods: Full edentulous patients (n=40) who were planned to do implant supported mandibular overdentures, aged 51 to 82 years, were enrolled in this study. All subjects rated their satisfaction levels (mastication, pronunciation, esthetics, pain, and general satisfaction) of their original conventional dentures prior to treatment and their new overdentures 1 week and 12 weeks after the delivery on 10 cm visual analogue scales. Repeated-measures ANOVA was performed to compare the satisfaction level of each three period (P<.05). Results: The mean satisfaction levels of the implant supported overdentures after 1 week and 12 weeks of delivery were significantly higher than those of the conventional complete dentures in all scoring parameters (P<.05). After 12 weeks of overdenture delivery, the increase of the satisfaction levels in mastication, pronunciation, and pain and the decrease in esthetics and general satisfaction were observed in comparison with those after 1 week of delivery, however, no significant difference was found between the satisfaction levels of 1 week and 12 weeks after delivery. Conclusion: An implant supported overdenture might be one of the effective ways to give more satisfaction to patients who were not gratified with a conventional denture and eventually to improve their quality of life.

Effect of Images about Medical Organizations and the Quality of Medical Service on Customer Satisfaction in Obstetrics and Gynecology Patients (산부인과 환자의 의료기관에 대한 이미지와 서비스 질이 고객만족에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effects images about medical organizations and the quality of service on customer satisfaction of obs and gyn patients. Method: The subjects of this study were selected conveniently 220 women among obstetrics and gynecology outpatients( who visited H- doctor's office of the first medical organ, P- hospital of the second medical organization, Chospital of the third medical organization). The data were collected from August, 20th 2004 using structured questionaires which included modified form of SERVPERF and customer's satisfaction scale by oliver & swan(1989), and modified form of image scale by kang(1997). Results: Image of hospital(45.3%), visiting frequencies(9.3%), service provider (3.9%) and convenient use(1.2%) of the quality of medical service were significant predictors to explain customer's satisfaction. Conclusion: To increase customer's satisfaction of obs and gyn patients, it is required to developed strategies that improve image of hospital and the quality of service to service provider and convenient use of hospital.

The Relationship of Sexual Satisfaction and Daily Stress in the Breast Cancer Patients (유방암 환자의 성생활 만족과 생활 스트레스와의 관계)

  • Purpose: The purpose of this study was to investigate the correlation of sexual satisfaction and daily stress in breast cancer patients. Methods: Data was collected through self-administered questionnaires and analyzed by descriptive statistics, t-test, ANOVA and Pearson's correlation. Data survey was conducted with 500 conveniently selected breast cancer patients who visited the out patient department in 5 university hospitals in Seoul, Gyung-gi and Gang-won province. Results: The sexual satisfaction of breast cancer patients score was 31.34 and there were significant sexual satisfaction differences by age, education level, menopause and sexual activity frequency. Sexual satisfaction was negatively related with daily stress (r = -.177) especially personal stress (r = -.155), economic stress (r = -.138), stress of self (r = -.181), family stress (r = -.154) and stress about leisure (r = -.139). Conclusion: These findings are expected to make a contribution to creation of ideal sexual rehabilitation nursing interventions for breast cancer patients care nurse. Furthermore continuous and customized education and counseling programs can contribute to promote healthy sexual life for breast cancer patients.

Sexual Behavior and Sexual Satisfaction according to Gender in Korean Patients with Cancer (암 환자의 성별에 따른 성행동과 성만족 변화)

  • Purpose: This study was done to investigate sexual behavior and sexual satisfaction according to gender difference in Korean patients with cancer Methods: In this study, a convenience sample of Korean patients with cancer who visited hospitals in J City was used. Data were collected between September and December 2012using structured questionnaires: Sexual frequency scale and General sexual satisfaction index (GSSI). Results: Over half of the patients reported loss of sexual desire and changes in sexual behaviors. There was a significant decrease in sexual satisfaction among the male patients, compared to the female patients (t=2.34, p=.020). There were significant correlations between sexual satisfaction and sexual behaviors (male patients r=.45, p<.001; female patients r=.59, p<.001). Conclusion: The results of this study demonstrate the need to take gender into account in developing interventions for patients with cancer. These results will also contribute to the development of improved interventions for sexual health care.

  • Purpose: Importance of patient satisfaction related to patient-centeredness has been emphasized, and it is known to have effect on various health outcomes including health resource utilization. However, the effect of patient satisfaction has been discussed mostly in terms of hospital marketing in Korea. This study aims to examine the effect of patient satisfaction in patient-physician communication on healthcare utilization in a nationally representative adult population of South Korea. Method: Patient satisfaction with physician communication is assessed using 4 items in the 2011 Korea Health Panel Survey. Generalized linear regression analysis is conducted using 9,325 adults' healthcare utilization in 2012. Findings: Adjusting for the socio-demographic, economic factors, individual health status, health behaviors and healthcare utilization in 2011, more satisfied individuals, more likely to utilize the outpatient service, especially in clinical setting. Practical Implications: The study findings suggests that in context of South Korea healthcare system such as insufficient medical consultation time and the absence of health delivery system, patient satisfaction as a subjective healthcare quality indicator would have effect on the individual's outpatient visit. This study contributes to stimulate patient satisfaction research and discussion in South Korea to further explore its relationship with potential and various health related outcomes. Further implications of the study are discussed.

Consumer Satisfaction with Medical Services and Hospital Patient Gowns (병.의원의 의료서비스와 환자복에 대한 소비자 만족)

  • This study identifies consumer satisfaction with medical services and the patient gowns of hospitals. Also analyzed are the elements that influence hospital satisfaction and the general satisfaction with patient gowns. A survey was conducted among Korean male and female ex-patients regardless of their age. A total of 513 responses were analyzed using descriptive statistics, paired t-test, correlations, regression, and factor analysis derived from data collected in April and May, 2009. The results are as follows. The general satisfaction with the hospital recently visited was higher than the general satisfaction with all of the hospitals visited. The satisfaction with medical services were high in good services, trustful medical examinations, easy access to utilities, and clean utilities. Hospital satisfaction was determined by good services, trustful medical examinations, easy access to utilities, patient gowns, and fresh indoor air. The satisfaction factors of general patient gowns were determined as functionality, fabric/design, sewing, and the management system factors. The most important factor explaining patient gown satisfaction was fabric/design, and this was the most unsatisfied factor at the same time.
