A Study on the Self-Management and Physiological Measurements of Patients on Hemodialysis according to Their Age (혈액투석 환자의 연령에 따른 자가관리, 생리적 지표에 관한 연구)

  • Purpose: The study was conducted to investigate correlations between self-management and physiological measurements of patients on hemodialysis according to their age. Methods: The subjects were 120 patients on hemodialysis at three hospitals in S and G cities. Data were collected using a structured questionnaire and medical records. The collected data were analyzed using descriptive statistics including t-test, ANOVA, and Pearson's correlation analysis with the SPSS 23.0 program. Results: The levels of compliance with self-management were highest in the middle-aged group. There was no significant difference in physiological measurements between the different age groups. Conclusion: The results suggest that nurses should consider characteristics of different age groups to improve the levels of self-management and physiological measurements.

  • 당뇨병은 전 세계적으로 급격하게 증가하면서 심각한 대혈관 및 미세 혈관 합병증들의 원인이 되고 있으며 대다수 선진국에서 당뇨병은 만성 신질환 및 말기 신부전의 가장 중요한 원인으로 알려져 있다. 당뇨병성 신증은 단백뇨를 특징으로 하며 30% 정도의 환자에서는 혈뇨도 관찰된다. 그래서 당뇨병성 신증의 발병 검진은 미세알부민뇨 검사로 하는 것이 가장 이상적이며 제 1형 당뇨병환자는 일반적으로 당뇨병 발병 5년 이후에 하며 위험인자가 있는 환자는 더 조기에 한다. 제 2형 당뇨병환자는 당뇨병 진단 시점부터 매년 하는 것을 원칙으로 한다.

  • Modules of hemodiafilter were manufactured by using MC-Hp200 and RC-HP4DOA hoi low fiber membranes of Enka Co. in a unique design. The performances of she hemodiafillers were evaluated by measuring the molecular weight cut-off, ultrafiltration rate, clearance, and pressure drop across the hemodiafiter. As a whole, the performances of the RC-HP400A module were superior to those of the MC-Hp200 module. The modules prepared in thls study showed the satisfactory performances for hemodiafiltration.

Association of food intake with serum levels of phosphorus and potassium in hemodialysis patients (혈액투석 환자의 일상식품군 섭취량에 따른 혈청 인, 칼륨 농도의 상관성 분석)

  • Purpose: Elevated serum phosphorus and potassium levels are a major problem for hemodialysis (HD) patients. Hyperphosphatemia and hyperkalemia are closely related to intake of dietary phosphorus and potassium. Methods: This study was conducted in order to investigate the effects of food consumed on serum phosphorus and potassium levels in 48 HD patients (20 males and 28 females). We collected anthropometric data, biochemical parameters, and dietary data of the subjects. Dietary data for usual intake were obtained by use of a food-frequency questionnaire (FFQ) consisting of 21 food items. Results: The mean body mass index (BMI) was $22.2{\pm}3.0kg/m^2$, mean serum phosphorus level was $4.50{\pm}1.52mg/dl$, and mean serum potassium level was $4.74{\pm}0.73mEq/l$. Hyperphosphatemia (> 4.5 mg/dl) was found in 45.8% of subjects, and hyperkalemia (> 5.0 mEq/l) in 35.4%. Subjects who took medication only were 56% of total, and those who took medication with dietary therapy were 27%. Patients with medication and dietary therapy showed significantly lower serum phosphorus levels compared to patients with medication only (p < 0.05). Mean duration of HD was $7.9{\pm}7.3$ years and it showed positive correlation with serum potassium levels (p < 0.05). Serum phosphorus levels showed positive correlation with intake of mixed grains and soybean milk (p < 0.05). Serum potassium levels showed positive correlation with intake of mixed grains (p < 0.01), potatoes, fish, and high-potassium vegetables (p < 0.05). On the other hand, intake of white rice showed negative correlation with serum potassium levels (p < 0.05). Conclusion: The results of our study suggest that intake of white rice rather than mixed grains is an important factor in sustaining normal serum phosphorus and potassium levels. In addition, limiting intake of soybean milk, potatoes, and fish to under three serving per week is recommended. Finally, conduct of a strict dietary therapy along with medical treatment is desirable because inappropriate food intake increases serum phosphorus and potassium levels to a higher than normal range.

Effects of the Lower Limb Muscle Strength Exercise Program during Hemodialysis on the Leg Strength and Falls Efficacy of Hemodialysis Patients (혈액투석 중 하지근력운동 프로그램이 혈액투석 환자의 하지근력과 낙상효능감에 미치는 효과)

  • Purpose: The purpose of this study was to explore the effects of the lower limb muscle strength exercise program during hemodialysis on the leg strength and falls efficacy (fall-related self-efficacy) of hemodialysis patients. Methods: The study was designed to be a quasi-experimental, nonequivalent control group pre and post test design. A total of 42 patients participated in the study. We applied lower limb muscle strength exercise program to the experimental group in every hemodialysis three times a week for 8 weeks. Data were analyzed using ${\chi}^2$ test, fisher's exact test, t-test and repeated measures ANOVA. Results: The mean age was 58 years old (t=-1.54, p=.132), the mean hemodialysis period was 67 months (t=1.949, p=.058) and there was no significant difference of dependent variable (t=1.17, p=.251)(t=-.89, p=.381) between the two groups before the experiment. After the experiment, leg strength was significantly improved in the experimental group compared to that of the control group (F=6.63, p=.004). However, falls efficacy was not significantly different between the experimental group and the control group (F=2.33, p=.104). Conclusion: The study results indicated that the lower limb muscle strength exercise program during hemodialysis may improve leg strength and potentially to prevent falls for hemodialysis patients. Further studies are warranted in which larger number of participants and longer duration of intervention are recommended.

The Effects of Characteristics of Nurses on Knowledge and Nursing Performance Evaluation of Evidence Based Hemodialysis Nursing Practice in Hemodialysis Unit Nurses (혈액투석실에서 근무하는 간호사의 특성이 근거기반 혈액투석간호지식과 수행정도에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effects of characteristics of nurses on knowledge and nursing performance of evidence based hemodialysis nursing practice among hemodialysis unit nurses. Methods: The participants were 180 nurses working in hemodialysis unit for more than 6 months in 23 private and general hospitals in Seoul and Gyeonggido. Data were collected from March 30th to April 15t in 2016 and were analyzed using stepwise regression analysis, descriptive statistics, t-test and ANOVA. Results: Nurses' knowledge on evidence based hemodialysis nursing practice was $15.87{\pm}4.52$ out of 23 points. Type of hospitals working in and adherence to evidence based hemodialysis guidelines were significant factors to knowledge and these two factors explained 30.0%. Nurses' nursing performance on evidence-based hemodialysis nursing practice was 4.52 out of 5 points. The performace level was significantly related to total nurisng career and necessity of hemodialysis nursing education and these two factors explained 8.0%. Conclusion: A development of guideline and continuing education is necessary for improving knowledge and performance of evidence based hemodialysis nursing practice.
