What is Emergency Medicine and Its Agenda for Future (응급의학은 어떤 의학분야이며 그 미래는?)

  • Emergency medicine(EM) is the specialty of evaluating, stabilizing and initiating treatment for patients with life or limb-threatening illnesses or injuries. Techniques unique to the specialty of EM are the triage systems, quick stabilization methods, and emergency surgery procedures. The field of EM encompasses areas such as emergency department management, disaster planning and management, the management of emergency medical service(EMS) systems, research into such areas as brain and heart resuscitation, trauma and disaster management, survival medicine, and environmental emergencies(cold and heat injuries, poisioning, decompression sickness and barotrauma). Today, in addition to providing emergency care, the emergency specialists have moral and legal obligations to assess and report probable cases of child and spouse abuse, sexual assault, and alcohol and drug abuse. Future, the EM should provide surveillance, identification, intervention, and evaluation of injury and disease, therefore EM will remain as a key component of evolving community health care system.

Thoracolumbar Spine Injury (흉요추부 손상)

  • Method of management of the spine injury should be determined, based on the status of neurological injury as well as on the presence of traumatic instability. At the thoracic and lumbar spine, patterns of neurological injury are different from the cervical spine due to their neuro-anatomical characteristics. Especially, at the thoracolumbar junction, neurological injury patterns with their respective prognosis vary from the complete cord injury or conus medullaris syndrome to the cauda equina syndrome according to the injury level. The concept of Holdsworth's instability based on the posterior ligament complex theory has evolved into the current 3-column theory of Denis. Flexion-rotation injury and fracture-dislocation are well known to be unstable that surgical fixation is frequently needed for these injuries. However, there have been some controversies for the stability of burst fractures and their treatment, such as indirect or direct decompression and anterior or posterior approach. In this article, current concepts and management of traumatic instabilities at the thoracic and lumbar spine have been reviewed and summarized.

  • Fifteen patients was analyzed on effect of the management of postherpetic neuralgia by local anesthesia on the special region at pain clinic in Youngnam University Hospital. The results were on follows : 1) The frequency of occurrence of sex and the lesion side were similar in all patients. 2) The age of incidence was between 50 and 70 years old. 3) The most frequent site of lesions was the neck. 4) There was no relationship between age and treatment time. 5) Whole patients was done average 7-10 time local injection.

  • Majority of the eyes with subfoveal neovascular membrane lose the central vision. We observed two patients who regained significant central vision as the result of the involution of subfoveal neovascularization. On follow-up fundus examination, the subretinal lesions revealed grayish neovascular membranes stained with fluorescein, but did not show the fluid leakage. And subretinal hemorrhage and subretinal fluid were gradually resolved. We assumed that functioning retinal pigment epithelium within the macula and young age were the important factors of the spontaneous improvement of visual outcome.

  • Pyogenic vertebral osteomyelitis is rare. It most commonly occurs at the lumbar area and in the fifth to seventh decades of life. Most individuals present with back pain, abdominal pain, hip pain and meningeal syndrome. We experienced three cases of unusual patterns of pyogenic osteomyelitis. The first case developed after sepsis. The second case developed after open fracture and infection of other site. The third case was misdiagnosed as metastatic cancer. We present these cases with a brief review of literatures.

  • Epicanthoplasty was performed in 98 patients with the epicanthal fold. Epicanthal fold was classified into three categories-minimal degree, moderate degree, severe degree-according to its severity. In accurate anatomic dissection in medial canthal area, the pathologic mechanism of the formation of the epicanthal fold, I think, are not only the redundancy of skin, but also the early downward insertion of the preseptal and pretarsal portion of orbicularis oculi muscle. A new surgical technique is described for the correction of the epicanthal fold. The procedure is simple to perform and uniformly gives good results.

  • In 1962, a case of "generalized skeletal dysplasia with multiple anomalies" was reported by Taybi as a new clinical syndrome. The most characteristic features of the syndrome seemed to be hearing loss, cleft palate and peculiar digital anomalies, so, the syndrome designated as otopalatodigital syndrome by Dudding, et al. Recently, The authors have experienced a case OPD syndrome who visited Yeungnam University Hospital with the chief complaint of hypernasality, and underwent pharyngoplasty for correction of VPI. We present here a new case and review the literature on the subject.

Clinical Experiences of the Ureteroscopic Management for the Lower Ureteral Stone (하부요관결석에 대한 요관경하배석술)

  • Ureteroscopic removal of the stone is now popular for the management of the lower ureteral stone. A clinical study was performed on our 75 patients with lower ureteral stone treated with ureteroscopy. Of the 75 stone manipulations 62(83%) were immediately successful and the final success rate including spontaneous delivery of stone or fragment after the procedure was 87 percents(65 cases). Of 57 smaller calculi than 1Cm(radiographic largest diameter) 50(88%) were removed successfully. Mean duration of postoperative hospitalization was 5.6 days. There were no interrelations between the success rate and anesthetic methods. Significant complications durinf or after procedure were not identified. We conclude that ureteroscopic removal of stones under direct vision can be done safely and be the first choice of procedure for the lower ureteral stones.

  • A method to roll-over the paralyzed body by means of Functional Electrical Stimulation(FES) is considered. It is demonstrated that individual joint motions necessary for the rolling-over are realized by electrical stimulation. EMG measurements are also performed to analyze the cooperative activities of the muscles during rolling over motion in a case where an upper extremity was used. These results of two experiments using normal subjects verifies the fundamental feasibility of body control by FES.

  • Treatment of olecranon fractures by prolonged immobilization often results in limited elbow motion. With the use of tension band wiring, anatomical reduction was obtainable, and only a short period of immobilization was needed. We reviewed the cases of 17 patients who underwent surgical treatment of the olecranon fracture. All patients were treated by tension band wiring. In the over-all series, we were able to obtain 53 per cent excellent, 30 per cent good, and 17 per cent fair results.
