Diagnostic Values of Abdominal Ultrasonography in Patients with Fever and Abdominal Symptoms (발열과 복부 증상을 주소로 하는 환아에서 복부 초음파 검사의 진단적 의의에 관한 연구)

  • Purpose: Acute febrile illness in children frequently accompanies with abdominal symptoms such as vomiting, diarrhea, and abdominal pain, even if its etiology is not occured from the gastrointestinal tract. If the etiology of fever was unknown and the fever was accompanied with abdominal symptoms, we should be concerned about whether the etiology of fever was originated from the gastrointestinal tract or interpretated from the abnormality in the gastrointestinal tract. This study was performed to evaluate the diagnostic value of abdominal ultrasonography in patients with fever and abdominal symptoms. Methods: We reviewed retrospectively the medical records of abdominal ultrasonographic (US) findings of 60 cases of acute febrile illness with abdominal symptoms at department of Pediatrics, Ulsan Dongang General Hospital during the period from January 1994 to June 1995. Results: The abnormal abdominal US findings obtained were as follows. 1) The abnormal US findings were seen in 56 cases (93.3%). 2) The most common abnormal US finding was the enlarged mesenteric lymph nodes in 52 cases (86.7%) 3) The enlarged mesenteric lymph nodes with or without intra-abdominal fluid were seen in 30 cases (50.0%) of a wide variety of illnesses, so their diagnostic values were absent. 4) The enlarged mesenteric lymph nodes and splenomegaly with or without ileocecitis were seen in 7 cases. Among them, 4 cases (6.7%) were confirmed as typhoid fever. 5) The US findings in 6 cases of typhoid fever were the enlarged mesenteric lymph nodes in all cases (100%), splenomegaly in 4 cases (66.7%), ileocecitis in 1 case (16.7%), enlarged mesenteric lymph nodes and splenomegaly in 4 cases (66.7%), enlarged mesenteric lymph nodes, splenomegaly and ileocecitis in 1 cases (16.7%). 6) The enlarged mesenteric lymph nodes and the abnormalities around the appendix were seen in 7 cases (11.7%), which were confirmed as appendicitis all. 7) The thickening of wall in urinary bladder was seen in 2 cases (3.3%) of acute cystitis and acute hemorrhagic cystitis. 8) The subtle thickening of wall in colon was seen in 1 cases (1.7%) of shigellosis. Conclusions: The enlarged mesenteric lymph nodes, splenomegaly and ileocolitis on the abdominal ultrasonography in patients with fever and abdominal symptoms aree suggestive findings of typhoid fever. The enlarged mesenteric lymph nodes and the abnormalities around the appendix on abdominal ultrasonography make the rapid diagnosis of acute appendicitis and its complications, when physical examination is difficult in small children and diagnosis of their illnesses is obscure in patients with fever and abdominal pain.

Automatic ray-code generation for efficient ray tracing (효율적인 파선추적을 위한 파선코드 자동 생성에 관한 연구)

  • In constructing a synthetic seismogram using ray-tracing method a suite of ray-code is required to obtain a realistic seismogram which is similar to the actual seismogram or earthquake record under consideration. An infinite number of rays exist for any arbitrarily located source and receiver. One select only a finite number of such rays in computing a synthetic seismogram so their selection becomes important to the validity of the seismogram being generated. Missing certain important rays or an inappropriate selection of ray-codes in tracing rays may result in wrong interpretation of the earthquake record or seismogram. Automatic ray-code generation will eliminate such problems. In this study we have developed an efficient algorithm, with which one can generate systemastically all the ray-codes connecting source and receiver arbitrarily located. The result of this work will helpful in analysing multiple reflections in seismic data processing as well as simulating Lg wave and multiply reflected or converted phases in earthquake study.

  • Recently seismic tomography has been widely used to visualize subsurface structure for resource explorations and construction site evaluation. We studied a way to include fresnel zone concept in the conventional ray-based traveltime tomography. The algorithm developed uses the same order of computing time as the conventional traveltime to mography but incorporates the rigorous wavepath concept of wave-equation tomography. Some experiments to synthetic and real data show reasonable results compared to conventional ray-based traveltime tomography.

Atypical presentation of Kawasaki disease resembling a retropharyngeal abscess (인두후부의 농양과 유사하게 표현된 비전형적인 가와사끼병)

  • Kawasaki disease is an acute systemic inflammatory disorder, the etiology of which has not yet been established. The clinical manifestations are non-specific and are common to many pediatric infectious and immunologic diseases. In 2 cases presenting fever, cervical lymphadenopathy, and retropharyngeal abscess-like lesions on the neck shown in a computerized tomography (CT) scan, the diagnosis of Kawasaki disease was delayed. The patients were not responsive to antibiotic therapy, and fluid collection was not found during retropharyngeal aspiration. They showed typical Kawasaki manifestations over time and atrioventricular valve regurgitation in the echocardiogram. A diagnosis of Kawasaki disease should be considered in similar cases of fever and cervical lymphadenopathy. A CT scan that shows a retropharyngeal abscess-like lesion on the neck can be a false positive finding, and other laboratory exams, including echocardiography, should be performed as early as possible to avoid unnecessary medical and surgical procedures.

Laboratory experiment of evolution of rip current according to the duration of successive ends of breaking wave crests (연속 쇄파선 끝단 지속시간에 따른 이안류 발달 수리실험 연구)

  • The experiment of rip current at successive ends of breaking wave crests was conducted in a laboratory wave basin, and its time-varying evolution according to incident wave durations was observed by using ortho-rectified images. The experiment utilized the generation of a quasi nodal line of the honeycomb-pattern waves (i.e., intersecting wave trains) formed by out-of-phase motion of two piston-type wave makers arranged in the transverse direction, instead of the original honeycomb pattern waves which are generated when two wave trains propagate with slightly different wave directions. The particle moving distance and velocity caused by the rip current were measured by using the particle tracking technique. As a result, the rip current was survived for a while even without incident waves after its generation due to several successive ends of wave crests, and it moved the particles further out to sea.
